Abstract

The lack of an organized school dental health policy in Indonesia requires an affordable, easily accessible, and sustainable strategy. Provision of dental and oral health knowledge delivers by a little doctor at the school level can help the implementation of the program that school-age is the time of peer groups. The school-age is in which children begin to learn basic oral hygiene practices and are most vulnerable to dental caries. This study was conducted to assess the effect of little doctor training on the knowledge, attitudes and skills of little doctors, as well as on the skills and patterns of brushing teeth in Bandung Regency, Indonesia. This research used a quasi-experimental design with pretest and posttest. A total of 12 little doctors and 291 school children in grades 4-6 elementary school from four government schools were included in this study. The instrument consisted of a knowledge and attitude questionnaire, an observation sheet, and a checklist sheet. Evaluation of tooth brushing patterns at school was done every month for 3 months. Data analysis using paired t-test. Data before and after training were compared and it was found that there was a significant improvement in the knowledge, attitudes, practices of the little doctors and students (p = 0.005). Skills and practice patterns of brushing teeth of school children increased significantly after the program (p = 0.005). The results of this study indicate that school-based oral health education methods that involve little doctors are effective for improving oral hygiene in elementary school children.Abstrak: Kurangnya kebijakan kesehatan gigi sekolah yang terorganisir di Indonesia membutuhkan strategi yang terjangkau, mudah diakses, dan berkelanjutan. Pembekalan pengetahuan kesehatan gigi dan mulut yang disampaikan oleh dokter kecil di tingkat sekolah dapat membantu pelaksanaan program bahwa usia sekolah adalah masa peer group. Usia sekolah adalah masa dimana anak-anak mulai mempelajari praktik dasar kebersihan mulut dan paling rentan terhadap karies gigi. Penelitian ini dilakukan untuk menilai pengaruh pelatihan dokter cilik terhadap pengetahuan, sikap dan keterampilan dokter cilik, serta terhadap keterampilan dan pola menggosok gigi di Kabupaten Bandung, Indonesia. Penelitian ini menggunakan desain eksperimen semu dengan pretest dan posttest. Sebanyak 12 dokter kecil dan 291 anak sekolah kelas 4-6 sekolah dasar dari empat sekolah negeri diikutsertakan dalam penelitian ini. Instrumen terdiri dari angket pengetahuan dan sikap, lembar observasi, dan lembar checklist. Evaluasi pola menggosok gigi di sekolah dilakukan setiap bulan selama 3 bulan. Analisis data menggunakan uji-t berpasangan. Data sebelum dan sesudah pelatihan dibandingkan dan ditemukan adanya peningkatan yang signifikan pada pengetahuan, sikap, praktik dokter kecil dan siswa (p= 0,005). Keterampilan dan pola praktik menggosok gigi anak sekolah meningkat secara signifikan setelah program (p= 0,005). Hasil penelitian ini menunjukkan bahwa metode pendidikan kesehatan gigi dan mulut berbasis sekolah yang melibatkan dokter kecil efektif untuk meningkatkan kebersihan gigi dan mulut pada anak sekolah dasar.

Abstract

Background: The prone position is one of the highly recommended interventions to treat hypoxaemia and lung injury in ARDS patients. However, its efficacy in non-intubated patients remains inconclusive due to predominantly being reviewed through observational and case studies. This review aims to evaluate the efficacy of prone position on the oxygenation status and mortality rate of ARDS patients through experimental studies. Methode: This review systematically searched three international databases: PubMed, Web of Science, and CINAHL from November to December 2022, without restricting the publication year. The focus was solely on experimental studies examining the effectiveness of prone positioning in adult, non-intubated ARDS patients. The quality of studies was assessed using the JBI critical appraisal tool. Results: Of the 12 included studies, most RCTs were identified as poor quality. All patients appeared to be infected with Covid-19, and the majority were on conventional oxygen therapy. Most studies showed that the prone position was effective in improving the oxygenation status for up to two hours post-session, with a mortality rate not substantially different from that of control group. Further good-quality experimental studies are needed to strengthen these results. Conclusion: The prone position is quite effective in improving the oxygenation status of patients.

Abstract

Introduction:Pain is a problem experienced by patients with burns. Pain reduction is the main focus in the management of burn patients. Non-pharmacological treatment options with music therapy and spiritual care approaches are part of nursing care. Objective: The study was conducted to identify the effectiveness of music therapy and spiritual care in reducing pain during dressing changes for burn patients. Materials and methods:The study used a quasi experiment pre-post test method on 30 burn patients with the music therapy and other with spiritual care intervention. Samples were taken using accidental sampling. Patients were measured for pain level using Numeric rating scale (NRS). Data were analysed using univariate analysis, dependent t-test and an independent t-test to compare between music therapy and spiritual care. Result: Music therapy and spiritual care have a significant effect on the measurement results before and after the intervention resulting in a value of (P=0.000) p a (0.005). While the mean difference between music therapy and spiritual care after after dressing change (P=0.000) and (t=5.875). Conclusion: There is an effect of music therapy intervention and spiritual care on reducing pain in burn patients undergoing dressing change procedures. Spiritual care had a higher average reduction than music therapy.

Abstract

Postoperative pneumonia (POP) is the most common complication in patients after CABG surgery. It increases morbidity, mortality, length of stay, and hospitalization costs. This review aims to identify the factors that influence pneumonia and the microorganisms that cause it in CABG postoperative patients. This scoping review used the framework developed by Arksey and O'Malley (2005). Primary sources of information were searched through five databases: PubMed, Scopus, Science Direct, Oxford, and EBSCO, and 2 search engines: Sage and Google Scholar. Article quality was assessed using the Joanna Briggs Institute critical appraisal checklist. A total of 8 articles with a total of 35235 participants were included in this review. The prevalence of post-CABG POP ranged from 2.4% to 18%. Dominant factors influencing postoperative pneumonia included duration of surgery, CPB time, blood transfusion, mechanical ventilation, LVAD/Tx, smoking history, chronic lung disease, and preoperative malnutrition. Meanwhile, the microorganisms that cause postoperative pneumonia are gram-positive and negative bacteria, fungi, and other bacteria. Infection prevention can be an important point in the CABG postoperative care continuum. By understanding the factors that influence POP, healthcare professionals can optimize patient outcomes, reduce complications, and improve the efficiency of postoperative care.

Abstract

Indonesia as an archipelagic country has various natural and non-natural disasters, the preparedness of health workers, especially nurses, influences disaster management, where nurses have the largest number during disaster management. Objective to identify nurses' perceptions of nurse preparedness in the pre-disaster, disaster, and post-disaster phases in the Bandung City area. The type of research used is descriptive quantitative survey research. The population is all nurses in the West Java region who are adults. The sampling technique used in this study uses convenience sampling. Respondents were nurses in Bandung, a total of 287 people. Data was collected by filling out a questionnaire using Google Forms. The data obtained was then analyzed univariately using descriptive analysis through frequencies and percentages. The results showed that most (80.5%) of disaster education and training was lacking, and a small proportion (19.5%) were good. More than half (54.7%) of knowledge and information about disasters needs to be improved, and almost half (45.3%) is good. Handling of bioterrorism and emergency response Most (61.3%) are poor, and almost half (38.7%) are good. Nurses' response during a disaster was more than half (51.2%) poor, and almost half (48.8%) were good. During the disaster evaluation, more than half (51.2%) were poor, and almost half (48.8%) were good. Overall, the perception of disaster preparedness is more than half (50.4%) poor, and almost half (49.6%) is good. Nurses' perceptions of disaster preparedness in some aspects are still lacking, and some are good.

Abstract

Anemia is a significant concern during pregnancy, requiring proper treatment to prevent ongoing issues. This study aims to explore interventions for preventing anemia in pregnant women. Using databases like Ebsco, Pubmed, Proquest, and ScienceDirect with keywords 'intervention,' 'prevention of anemia,' and 'pregnancy,' articles meeting inclusion criteria—focusing on anemia prevention interventions, RCT/Quasi experiment research designs, full text, and publication between 2011–2020—were sought. Initially, 1,052 articles were retrieved, and after screening and validity assessment, the study considered the last 7 eligible articles. The findings addressed four main intervention themes: new iron supplements, nutritional provision, handbook utilization, and health promotion for pregnant women. This research sheds light on various interventions available for preventing anemia during pregnancy.

Abstract

Shock is a life-threatening condition. It is essential to perform hemodynamic support on shocked patients to restore adequate circulation. Passive leg raising (PLR) is a critical act to assess fluid responsiveness which can provide significant information about fluid needs of shocked patients. The present study aimed to identify the implementation of passive leg raising in shocked patients. The scoping review was the method used in the present study. Relevant literatures were obtained from PubMed, CINAHL and Google Scholar Search engines from 2013 to 2023. The articles were manually extracted through tabulation and the data were thematically analyzed with an exploratory descriptive approach. Nine articles were found relevant and hence were included in the present study. The findings of the 9 articles revealed that the implementation of passive leg raising in shocked patients was an early strategy in assessing fluid responsiveness and fluid administration could be decreased after 48 hours of ICU admission. The findings of the current study suggested that the implementation of PLR significantly assessed fluid responsiveness which in the end guided in conducting fluid resuscitation in shocked patients and could reduce fluid administration in shocked patients for the first 48 hours of ICU admission. Therefore, health professionals working in critical care units including critical care nurses should consider performing PLR to shocked patients.

Abstract

This study investigates the impact of using warmed irrigation fluid over room-temperature fluid on patient body temperature during surgery, while also exploring the variables that influence its effectiveness. Systematic review following PRISMA guidelines. Comprehensive searches were conducted in PubMed/MEDLINE, Embase, Cochrane Central, and Web of Science (inception to March 2024) using PICO framework-derived search terms. Inclusion criteria: randomized controlled trials (RCTs) and systematic reviews comparing warmed irrigation fluids (≥36°C) versus room temperature in adult surgical patients. Quality assessment used Joanna Briggs Institute (JBI) critical appraisal tools. Data synthesis included descriptive analysis and meta-analysis where appropriate. Ten studies (eight randomized controlled trials and two systematic reviews/meta-analyses) met inclusion criteria, comprising 2,459 participants across various surgical procedures. Overall methodological quality was high (mean JBI score 10.2/11, 93%). Effectiveness analysis revealed that 67% of individual studies (6/9) demonstrated significant benefits, with clear procedure-specific patterns. Arthroscopic shoulder surgery showed the highest success rate (75%, 3/4 studies) with effect sizes of 67–80% reduction in hypothermia incidence when using 37°C irrigation. TURP procedures demonstrated conditional effectiveness (50% success rate) depending on anesthesia type, while laparoscopic surgery showed no benefit (0% success rate) due to competing heat loss mechanisms. Warmed irrigation fluids at 37°C demonstrate procedure-specific effectiveness, with arthroscopic shoulder surgery showing highest success rates (75%) and laparoscopic surgery showing no benefit.

Abstract

This study aims to determine the effectiveness of diabetic foot exercises on DPN values ​​in DMT-2 patients. The method used is a quasi-experiment with Pretest-Postest with a control group design. The study results showed that in the intervention group using Wilcoxon, 0.000 was obtained, and 0.001 in the control group. Still, if you look at the clinical results of a difference of less than 10, there is no significant difference in the degree of DPN before and after the intervention. The results of the study with the Mann-Whitney test showed that diabetic foot exercises are effective in reducing the degree of DPN (p = <0.05), where the intervention group had a lower degree of DPN (12.24) compared to the control group (30.76). There is a difference in the degree of DPN between the intervention and control groups, with a p = 0.001. In conclusion, foot exercises effectively reduce the degree of diabetic peripheral neuropathy in patients with type 2 diabetes mellitus at Dustira Hospital Cimahi. Abstract

Background: Ventilator-associated pneumonia (VAP) remains a leading cause of morbidity and mortality among patients in intensive care units (ICUs). Nurses play a central role in preventing VAP through consistent implementation of evidence-based prevention bundles. Although VAP bundles are widely recommended, suboptimal adherence among nurses continues to contribute to high VAP incidence rates, highlighting the need for strategies that improve compliance. Objective: This scoping review aimed to identify and synthesize interventions designed to enhance nurse compliance with VAP bundle implementation in ICU settings. Methods: A scoping review was conducted using the Population–Concept–Context (PCC) framework, focusing on nurses, interventions to improve VAP bundle compliance, and ICU settings. Electronic databases—including PubMed, Cochrane Library, ProQuest, EBSCOhost, ScienceDirect, ClinicalKey, SAGE Journals, and Google Scholar—were searched for full-text articles published in English between 2016 and 2021. Study selection followed the PRISMA framework. Data were extracted, analyzed, and synthesized narratively. Results: From 945 identified records, eight studies met the inclusion criteria. The reviewed literature identified educational and training programs, experiential learning strategies, interprofessional collaboration, and real-time compliance monitoring dashboards as effective interventions to improve nurse adherence to VAP bundles. Conclusion: Multifaceted interventions, particularly continuous education supported by supervision and monitoring, are effective in enhancing nurse compliance with VAP bundle implementation. Sustained organizational support is essential to improve adherence and reduce VAP incidence in ICU settings.
