Abstract

Highlight: CPAP therapy is a medical treatment for individuals suffering from OSA, a condition that is frequently linked to cardiovascular disease and resistant hypertension. Five studies clearly showed that combining CPAP therapy with standard medications had a big effect on the blood pressure of patients with both obstructive sleep apnea (OSA) and resistant hypertension. Administering CPAP in conjunction with standard treatment, as per guidelines, has demonstrated the capability to lower both daytime and nighttime blood pressure in individuals affected by OSA and resistant hypertension.   ABSTRACT Introduction: Obstructive sleep apnea (OSA) is an upper respiratory tract disorder that is often associated with cardiovascular diseases, one of which is resistant hypertension (RH). On the other side, Continuous Positive Airway Pressure (CPAP) is a medical treatment that is often used for patients with OSA and RH. Objective: This meta-analysis aimed to determine the effectiveness of CPAP in patients with OSA and resistant hypertension by measuring systolic and diastolic blood pressure at diurnal, nocturnal, and 24 hours. Methods: We conducted a systematic review using a PRISMA flowchart, utilizing sources such as PubMed, Scopus, Science Direct, and ClinicalTrials.gov with MeSH. We then reviewed these sources for quality studies using RoB2, and analyzed the data using the Revman website version. 5.4.Results: The five studies included in the analysis found that CPAP, while maintaining conventional drugs, significantly affected the blood pressure of patients with OSA and RH. The significant results were made clearer by obtaining data for nocturnal SBP pressure, mean -3.89 mmHg (95% CI: -7.03 to -0.76) with a p-value < 0.02, and then nocturnal DBP obtained a mean of -2.34 mmHg (95% CI: -4.70 to 0.02) with a p-value < 0.05. Meanwhile, the 24-hour results for SBP obtained a mean of -2.97 mmHg (95% CI: -5.88 to -0.06) with a p-value < 0.05, and the 24-hour results for DBP obtained a mean of -2.39 mmHg (95% CI: -4.62 to -0.16) with a p-value < 0.04. Conclusion: CPAP, while maintaining conventional treatment according to indications, can reduce 24-hour and nocturnal blood pressure in patients with OSA and resistant hypertension.

Abstract

Currently, infectious disease is increase in world wide. The African leaf (Vernonia amygdalina) – VA is used to antimicrobial treatment. It may protect the host against microbial attack in several ways. This plant has attracted the interest of researchers in recent decades because of the constituents have important roles in modulating immune system in bacteria infection. The aim of study is to analyze the prophylactic activity of VA's ethanol extract in modulating the levels of IL-6 and IL-10 as well as the number of bacteria in male Wistar rats that were (Staphylococcus aureus) – SA – infected. There were as many as 30 rats were divided into 5 treatment groups: negative control (NC) was treated by CMC Na 2% (w/v); positive control (PC) was treated by 9mg/200g body weight (BW) of cephadroxil; T1; T2; and T3 were respectively treated with ethanol extract of VA of doses 20mg/200g BW; 40mg/200g BW and 80mg/200g BW. After the oral treatment was administered, all the rats were infected with 0.25mL (3x108cfu) SA via intra peritoneal route. Their blood was drawn in order to identify the IL-6 and IL-10 levels by ELISA. Furthermore, their peritoneal fluid was also taken to count the number of survived bacteria by pour plate method. The results are showed median of IL-6 and IL-10 levels as well as bacterial number respectively in NC 370.530pg/mL; 67.044pg/mL; 7.4x103cfu/mL; in PC 234.556pg/mL; 42.839pg/mL; 6.8x103cfu/mL,; in  T1 164.019pg/mL; 17.240pg/mL; 1.1x104cfu/mL; in T2 49.291pg/mL; 2.961 pg/mL; 6.3x103cfu/mL and in T3 43.342pg/mL; 13.235pg/mL; 7.1x103cfu/mL. These results are implied that VA's ethanol extract is effective as a prophylactic agent to suppress the bacterial invasion at dose of 40mg/200g BW in Wistar rat particularly shown by the decrease level of IL-6 and the number of bacteria.

Abstract

Presbycusis is a progressive hearing loss which occurs at the age of 65. Communication difficulties caused by these problems are usually not understood or understood by people with normal hearing functions. Therefore, the understanding of these problems must be improved so that there is no gap between people with normal hearing and people with low hearing. The research method used is a quantitative descriptive research design carried out in a number of elderly care foundations. The data we share is in the form of pre-test and post-test questionnaires to elderly caregivers with hearing loss in order to evaluate the caregiver's understanding of the educational video. There was an increase in the understanding of the caregivers about how to communicate effectively based on the comparison of the scores from the pre-test and post-test final scores carried out before and after the education was given. Providing education to caregivers with educational video media provides effective results in increasing caregiver knowledge in this field. The formal caregiver group has better results than the informal caregiver group at 77.7% on the pre-test by formal caregiver and an assessment of 65% on the pre-test by informal caregiver. In the post-test assessment, the formal caregiver got a score of 87.7% and for the informal caregiver it was 90%. There is an increase in the post-test scores of the two caregiver groups after providing education through video media.

Abstract

Early detection of syphilis, especially during pregnancy, is important to prevent comorbidities for the mother and the baby. This study aims to determine whether VDRL is more sensitive and specific than RPR as a diagnostic screening test for syphilis infection in pregnant women. Literature searches for relevant articles were conducted in PubMed, Cochrane, and Proquest using the keywords "VDRL AND RPR AND pregnancy AND syphilis". From the search results, we found 7 articles in Pubmed, 2.290 articles in Proquest, and no relevant articles were found in Cochrane. Title and abstracts were screened for their conformity with the case and clinical questions that had been made. Selected articles were then critically appraised. The results of the study in selected articles indicated that VDRL and RPR showed a false positives rate of 10.5% and 9.6%, respectively. The sensitivity and specificity of VDRL were 71.6% and 89.5%, and those of RPR were 73.5% and 90.5%. VDRL and RPR have a moderate agreement with the TPHA (kappa = 0.6). From these studies it can be concluded that VDRL is not more sensitive and specific than RPR, implying that RPR is a better diagnostic screening test for syphilis infection in pregnant women than VDRL.

Abstract

Cholelithiasis affects 10-15% of the adult population. There are four main factors commonly associated with cholelithiasis abbreviated as "4F''namely female, forty, fertile and fat. However, the relationship between dietary habits as a risk factor of cholelithiasis has not been explained. The aim of the study was to determine the association between dietary habits and cholelithiasis. This study used a systematic review design with a comprehensive search of PubMed, Scopus, ScienceDirect, SpringerLink, and SAGE published from 2017 to 2021. Five studies (three prospective cohorts and two case-control studies) were included. The overall analysis of studies reported a significant risk factor of cholelithiasis in high intake of animal fats, meat, and fried foods and significant protection against cholelithiasis in high intake of fruits, nuts, fish, MUFA/SFA, n-3 FA/omega-3 fatty acids, and vegetables. Healthy dietary habits characterized by high intake in vegetables, fruits, fish, MUFA/SFA, n-3 FA/omega-3 fatty acids, and nuts will lower the risk of cholelithiasis.
