Abstract

Human Trafficking (TPPO) is a structured and systematic form of crime that violates human rights as the inherent rights of human beings as creations of God Almighty. This crime not only occurs in Indonesia but has also become a transnational crime, in which the rampant practice of human trafficking is often not accompanied by optimal protection for victims and effective countermeasures against perpetrators. Based on this, this paper aims to examine how national law regulates the crime of human trafficking, as well as how the Indonesian national legal system provides protection and countermeasures, by considering the factors that cause the crime of human trafficking. The research method employed in this paper is a juridical-normative approach, which examines and analyzes the implementation of legal provisions in legislation concerning human trafficking crimes, utilizing secondary data derived from library research. Therefore, this paper will discuss Law No. 21 of 2007 on the Eradication of the Crime of Trafficking in Persons, which not only regulates criminal sanctions for perpetrators and the protection of victims' rights but also examines how countermeasures are taken against the crime of human trafficking. Consequently, the results of this study demonstrate that Law No. 21 of 2007 has comprehensively regulated the scope of TPPO, encompassing both criminal aspects for perpetrators and protection for victims including restitution, rehabilitation, and confidentiality of identity. Thus, this regulation becomes a concrete instrument for strengthening cooperation with various stakeholders and serves as a key factor in the success of preventing, protecting, and eradicating trafficking in persons as a transnational crime, particularly in Indonesia.

Abstract

Vitamin D is an essential nutrient that not only plays a role in calcium homeostasis and bone health, but also has broad functions in the regulation of various biological processes, including cell cycle dynamics. This study aims to examine in depth the role of vitamin D in cell cycle dynamics based on evidence from molecular and clinical studies. The method used was qualitative research with a literature study approach, where data was collected from relevant articles such as PubMed, EuropePMC, and Google Scholar. Analysis was carried out through the process of filtering, presenting data, and drawing conclusions. The results showed that vitamin D in its active form, 1,25-dihydroxyvitamin D3, interacts with the vitamin D receptor (VDR) in cells and modulates the expression of genes involved in cell proliferation. This mechanism involves an increase in the expression of cell cycle inhibitory genes, such as p21 and p27, and a decrease in the expression of cell cycle promoting genes, such as cyclin D1, which contributes to the arrest effect on the cell cycle. Thus, in addition to being important for bone health, vitamin D has a significant role in cell cycle regulation, and its deficiency can increase the risk of various diseases, including cancer. Therefore, maintaining optimal vitamin D levels through sun exposure, consumption of foods rich in vitamin D, or supplementation is essential. Further research is needed to understand the role of vitamin D in genetic regulation, particularly in relation to cancer.

Abstract

This study examines the integrated role of educational institutions, media, and government in promoting media literacy among Palembang City residents using the Environmental Factors concept from the European Commission. The research employs a qualitative case study approach through in-depth interviews with key stakeholders, including educators, lecturers, Regional Indonesian Broadcasting Commission representatives, journalists, and media activists. The study reveals that successful media literacy promotion requires collaborative efforts between government agencies, educational institutions, and media organizations. The findings indicate that media availability in Palembang includes diverse internet providers, widespread smartphone penetration, and multiple radio and television stations. However, the media literacy context shows gaps in formal education integration, limited policy frameworks, and insufficient civil society participation. The research demonstrates that the synergy between government initiatives, educational programs, and media industry participation significantly enhances media literacy skills among citizens. These findings contribute to understanding multi-stakeholder approaches in media literacy education and provide practical insights for developing comprehensive media literacy strategies in urban Indonesian contexts.

Abstract

Vitamin D plays a role in the healing process of diabetic foot ulcers (DFU); it can improve the immune system, regulate inflammation function, and regulate growth factors in diabetes mellitus (DM). The immunomodulatory effects of vitamin D include reduced production of IL-2, IL-6, and TNF-alpha and increased production of IL-10. It also increases the activity of antimicrobial peptides (AMP) such as cathelicidin, and it promotes the differentiation of monocytes into macrophages, phagocytosis, and the production of lysosomal enzymes. Furthermore, vitamin D stimulates growth factors including vascular endothelial growth factor (VEGF) and transforming growth factor beta (TGF-β). In this study, the incidence of DM and DFU in DM patients at RSUP Dr. Mohammad Hoesin Palembang will be compared to serum vitamin D levels. This research uses quantitative methods with an analytical observational design. The Mean vitamin D serum level was 11,727 ng/mL, and there was a significant association between serum vitamin D deficiency and DFU (p-value; 0.036 < 0.05). There was a significant association between serum vitamin D deficiency and DFU occurrence (p-value; 0.036 < 0.05) in Mohammad Hoesin General Hospital Palembang. Based on the results of this study, we proposed that patients with diabetic foot ulcers should have their serum vitamin D levels checked, and that vitamin D should be given to them as part of their therapy.

Abstract

Introduction: Primary bone lymphoma (PBL) is an uncommon lymphoid cell neoplasm that appears as one or more bone lesions and does not appear in any lymph nodes or other places outside the lymph nodes. This type accounts for approximately 1% of all lymphomas and 7% of all cancerous primary bone tumors.Case Presentation: We collected two respective cases of primary bone lymphoma in the Department of Anatomic Pathology, Faculty of Medicine, Sriwijaya University, Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Both cases were diffuse large B-cell lymphoma, non-germinal center subtype (DLBCL non-GCB), in individuals 48 and 18 years old, located in the right femur. The prognosis for both patients was poor due to the advanced stage, as well as the second patient's refusal of chemotherapy. Conclusion: These cases demonstrate the diagnostic and therapeutic challenges of primary bone diffuse large B-cell lymphoma, particularly the aggressive non-GCB subtype. Early recognition, accurate histopathological diagnosis, and prompt, multidisciplinary management are essential to improve prognosis, especially in advanced-stage presentations. Delay in treatment or refusal of therapy significantly worsens patient outcomes.

Abstract

The structure of the building will be stable if the response of the structure is able to ignore the static and dynamic loads. The behavioral stability of the building structure is also influenced by geometry, configuration of horizontal and vertical elements, rigidity, lateral ability, and daktility of the structure. (Khanal Chaulagain, 2020). When the structure configuration of the building is irregular, there will be eccentricity which means that the center of the mass is incorrectly at the centre of rigidity (Faisal Darsono, 2019). If such conditions are not anticipated, the structure will be tortured until it eventually collapses. (Naveen dkk., 2019). In Surabaya there are office buildings that have irregular structures. This research will model the structure of the building into five variations. Each variation has a reduction in the value of structural irregularity. Next, we will examine the impact of structural irregularities on the reaction of structures of buildings. The results of the analysis showed that the irregularity of the inner angle had a significant impact on the shift of the roof, with the R-square of 0.9025 in the X axis and 0.9872 in the Y axis. Furthermore, based on the value of the soft level rigidity to the earthquake style at each level, R-quares of 0.8433 were found at the X-axis, and 0.825 at the Y-axe. Furthermore, changes in the inner angle of variations 2, 3, 4, and 5 resulted in successive increases in the mass of the structure by 0.18%, 4.09%, 7.86%, and 0.43%. These increases result in an increase in the basic sliding style in both directions. The variation 5 building designed without an inner angle showed an increase of 0.46% in the base sling style in either direction. Movement targets based on ASCE 41-13 NSP are 321.903 mm and 308.497 mm for the X and Y directions, respectively, which are in the Life Safety and Immediate Occupancy categories. While the total maximum drift values based on ATC-40 for the x and y directions are 0.0148 and 0.0150. The performance of the existing building structure is between B to C on the level of damage control (Immediate Occupancy to Life Safety), which means the building has suffered minor non-structural damage and may suffer structural damage but can still be repaired.

Abstract

Background A frozen section or vries coupe (VC) is an examination procedure performed by a pathologist during a patient operation on the surgical table. VC provides immediate information to surgeons regarding the benign or malignant status of a tissue, as well as determines the incision margin free from tumor mass. Objective This study aimed to evaluate diagnostic accuracy using the slide preparation archives of VC and histopathology at the Anatomic Pathology Department of Dr. Mohammad Hoesin Central General Hospital (RSUP), Palembang. Methods: Samples of patients were collected from 1 January 2021 to 1 October 2023, while sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for VC were assessed compared to the definitive diagnosis of histopathology. A reassessment was conducted by two pathologists on slide preparations with unmatched results between VC and histopathological diagnosis. Result VC examination showed sensitivity, specificity, PPV, and NPV of 99.7%, 99.7%, 98.3%, and 98.3% for benign tumors as well as 98%, 99.6%, 98.3%, and 98.3% for malignant tumors, respectively. The reassessment of the inappropriate slide preparations obtained a weighted kappa value of 0.586, signifying moderate agreement. Diagnostic discordance is caused by the experience of pathologists, freezing artifacts, and challenges in obtaining precise samples under a limited timeframe. Conclusion This study provided valuable insights into the effectiveness of VC procedure in intraoperative tissue diagnosis, showing the need for collaborative efforts and experienced pathologists to achieve improved accuracy.

Abstract

Sepsis is an emergency condition as a result of host dysregulation systemic immune response to infection that is related to the end stage of organ dysfunction. Both sepsis and septic shock conditions are the main problems in ICU especially, those that affect millions of people in the whole world every year. Studies in line with immune-neuroendocrine related to sepsis get high attention about factors that play roles in sepsis pathogenesis and prognosis, like correlation to gender, hormones, and other factors. In this case, lots of experimental studies and clinical studies showed that sepsis has significant sexual dysmorphic. The female gender has proven protective against sepsis, meanwhile, males could have worse sepsis because of decreasing immunologic response that mediates cell and cardiovascular function. Estrogen is a hormone in women that plays important roles in not only reproductive function but also non-reproductive function. Physiological estrogen in women is divided into three forms: estrone (E1), estradiol (E2 or 17β- estradiol), and estriol (E3). Several experimental studies in animals showed that estradiol has a protective response when infection occurs. Estrogen generally stimulates cytokine release, induction of HO-1, and restoration of organ function due to sepsis. Potential pathogenesis for this condition is a specific expression of cytokine pro and anti-inflammation. This pathological inflammatory condition is related to gender that is found in surgical patients at the molecular level.
