Abstract

Background: Gold weight implantation is a well-established surgical intervention for lagophthalmos, a condition characterized by incomplete eyelid closure. While generally safe and effective, gold implants can elicit immune responses, potentially leading to complications. This systematic review aims to comprehensively evaluate the immunological implications of gold weight implantation in lagophthalmos treatment. Methods: A systematic search of PubMed and ScienceDirect databases was conducted, encompassing studies published from 2000 to June 2024. Keywords included "gold weight implant" and "gold implant AND eyelid." Studies reporting quantitative data on immune responses to gold implants were included. Data extracted included study design, patient demographics, implant characteristics, follow-up duration, complications, and long-term outcomes. Results: Thirteen studies met the inclusion criteria, encompassing a total of 340 patients (370 eyelids). Reported complications included hypersensitivity reactions (11.9%), lymphoma (0.6%), infection (1.2%), extrusion (8.1%), and nonspecific inflammatory reactions (4.3%). Hypersensitivity reactions were mediated by T-cell and B-cell responses, leading to chronic inflammation. Lymphoma cases highlighted the potential for chronic inflammation to trigger lymphoproliferative disorders. Infections were infrequent but could necessitate implant removal. Extrusion rates varied, influenced by implant size and placement. Nonspecific inflammatory reactions were observed, often requiring implant removal or steroid treatment. Conclusion: Gold weight implantation can trigger diverse immune responses, ranging from mild inflammation to severe hypersensitivity and lymphoma. Careful patient selection, meticulous surgical technique, and vigilant postoperative monitoring are crucial to minimize complications. Further research is warranted to elucidate the precise mechanisms underlying these immune responses and develop strategies for their prevention and management.

Abstract

Cervical cancer is still the second biggest cause of mortality in women in Indonesia. Pap Smear cytology can detect precancerous lesions as precursor lesions early. Every married woman should have a Pap Smear examination done on a regular basis. If precancerous lesions are discovered, they must be treated immediately so that they do not proceed to cervical cancer. Counseling and Pap Smear Examination will be used to carry out the activity, which is aimed at all married women. The goal of this activity is to raise public awareness about the necessity of early cervical cancer screening and Pap Smear examination for early diagnosis of cervical precancerous lesions. Pre-test and post-test assessments were used to establish the degree of knowledge of participants. Counseling and examination activities was place four times in September-October 2021, at the Gandus Health Center and the practice of midwife Hj. Nani. This community service has attracted 130 counseling participants. Participants' understanding of the importance of early identification of cervical cancer increased from a score of 75.57 to 89.28 as a result of the pretest and post-test counseling outcomes. According to the Pap Smear diagnostic results, the majority of patients had non-specific chronic cervicitis, which was classified as Negative for Intraepithelial Lesion or Malignancy (NILM). Cervical cancer can be prevented with counseling and early detection.

Abstract

The procedure for making preparations in the Anatomical Pathology Laboratory consists of three stages, including the pre-analytical process. The impact of pre-analytical process errors can lead to misdiagnosis and treatment. This community service is located at AR Bunda Hospital Prabumulih City, on Saturday, October 15, 2022 in the form of educational seminars, discussions and hands-on workshops to practice the correct tissue fixation and lamellation procedures. The activity was held in conjunction with the 60th Anniversary of the Faculty of Medicine, Sriwijaya University and the Prabumulih City Government. The seminar participants were active enough to ask questions and discuss. There were 51 participants, coming from 4 hospitals, namely Prabumulih Hospital (31.4%), AR Bunda Hospital (31.4%), Pertamina Prabumulih Hospital (19.6%), Fadillah Prabumulih Hospital (9.8%), and from other institutions around 7.8%, consisting of room nurses, general practitioners, dental nurses, operating room nurses, pharmacy, and laboratory workers. An increase in participants' knowledge by 15% was obtained based on pre-test and post-test results, so it is expected that the tissue processing process in the Anatomic Pathology Laboratory can produce paraffin blocks of good quality, immunohistochemical examinations to be accurate, and patient tissue diagnosis to be more precise.

Abstract

Hemostasis is an emergency medical treatment to reduce pain and patient mortality, therefore research is being developed to find effective hemostasis. The utilization of natural materials for hemostasis and wound healing is rapidly expanding, including chitosan found in shell extracts. Chitosan is obtained from chitin found in the soft shells of marine animals such as squid, shrimp, and crabs, or from hard shells such as clams, crabs, and lobsters. Chitosan offers advantages such as good biodegradability, biocompatibility, and non-toxicity, and has been widely used in biomedical, chemical, food, and cosmetic industries. This literature review aims to investigate the potential of shell extracts, particularly the characteristics of chitosan, in wound healing across hemostasis, inflammation, proliferation, and remodeling stages. The study results indicate that extracts from shells containing chitosan exhibit varying characteristics in terms of molecular weight and degree of deacetylation. Chitosan with higher molecular weight and degree of deacetylation tends to yield better outcomes in hemostasis and wound healing. The material is effective in reducing antithrombin, enhancing blood clotting processes, and aiding clot formation. Increased molecular weight contributes to stimulating various cytokines, such as TNF-α, TGF-ß1, and FGF2, which play a key role in the wound healing process. Additionally, higher degree of deacetylation chitosan is effective in stimulating fibroblast proliferation. Chitosan also influences VEGF in inducing angiogenesis and enhancing neovascularization in bone healing. Chitosan from shell extracts with certain molecular weight characteristics and degree of deacetylation has the potential to be the material of choice for accelerating hemostasis and wound healing.

Abstract

On July 14, 2016, the Government enacted the Government Regulation Number 29 of 2016 regarding Amendments in Authorized Capital of a Limited Liability Company (LLC). Article 1 paragraph (3) of the regulation showed that the amount of authorized capital was submitted to the agreement of the LLC founders. This regulation was issued in order to increase Indonesia’s ‘ease of doing business’ rank, especially in ‘starting a business.’ This article aims to examine the legal protection for the third party over the amount of authorized capital based on the agreement of the LLC founders using the study of dogmatic law. Regulations referenced are Law Number 40 of 2007 regarding Limited Liability Companies and Government Regulation Number 29 of 2016 regarding Amendments in Authorized Capital of LLC. Based on the results of this study, it was found that the determination of authorized capital based on the agreement of LLC founders has neglected the protection of the third parties. This manifested particularly in protecting minority investors and resolving insolvency. Through the enactment of authorized capital based on the agreement of the founders, the mechanism of preventive and repressive legal protection to the third parties are assumed to be eliminated. Abstract

Introduction: Cases of neuroendocrine tumors (NET) can occur in various organs, generally in the form of gastroenteropancreatic (GEP-NET) and pulmonary tract (PNET). The incidence of thymic neuroendocrine tumors (TNET) is rare, accounting for less than 5% of thymic and mediastinal malignancies, and 0.4% of all neuroendocrine tumors. Case presentation: A 57-year-old man came to RSUP Dr. Mohammad Hoesin Palembang with complaints of coughing, bluish and curved blood vessels in the chest, and shortness of breath during activities that have been getting worse for six months. CT scan showed a tumor measuring 18.6 x 11.7 x 11 cm in the anterior mediastinum and pressing on the superior vena cava and abdominal aorta. TTNA and core biopsy with CT guidance showed NET Grade 2, mitoses 6/2 mm2, positive tumor cells with immunohistochemistry for synaptophysin, chromogranin, CD 56, AE1/AE3, and Ki67 9%. The patient received paclitaxel and carboplatin chemotherapy for 6 cycles. Conclusion: The prognosis for atypical carcinoid patients is 20-80% for a 5-year survival rate. Appropriate patient management is needed to improve the outcome of patients with primary thymic atypical carcinoid (NET, grade 2).

Abstract

The histopathology preparations procedure in the Anatomic Pathology Laboratory consists of three stages: pre-analytic, analytical and post-analytic. Errors in the pre-analytic process can affect the quality of histopathology preparations, thereby affecting diagnosis and patient management. The length of fixation and the large size of tissue lamellae are variables in the pre-analytic process that influence the quality of histopathological preparations. The aim of this study is to determine the relationship between the length of fixation and the size of large tissue lamellae on the quality of histopathological preparation. A cross-sectional study was conducted on medical record data of histopathological examination of breast tissue examined at the Barokah Palembang Anatomical Pathology Laboratory between January - December 2022. There were 37 cases of histopathological specimens that met the inclusion criteria, of which 3 cases were underfixed, 13 cases were overfixed and the rest were normal. One case from the underfixation, 20 cases from the normal fixation and 3 from the overfixation group were included in the good specimen quality category. The Spearman test showed that there was a significant relationship between normal fixation and good specimen quality (p=0.00217). Of the 37 research subjects, there were 18 subjects who were categorized as good lamellation. When related to specimen quality, good lamellation is associated with good specimen quality (p=0.02161). The correct fixation and lamellation during the pre-analytic phase are associated with good specimen quality.

Abstract

Indonesia is among the nations with the highest number of natural disasters. This syndrome is aggravated by unbalanced information provided by the media. The media use framing to enforce information distribution rules, particularly in regards to forest and land fire reporting. Using a constructivist research methodology, this study argues that framing is the key to influencing public opinion towards actors who might be perceived positively or negatively. The results showed that mass media framing was divided into several themes. This framing explains how the mass media has an important role in disseminating disaster-related information to the public. Some evidence suggests that political competition among the local political elite contributes to the occurrence of forest fires, it’s also ensued in mass media in Indonesia. If the government is unable to enforce strong regulations on irresponsible parties, forest fires will persist. The mass media must exist as a tool to regulate its population so that they do not burn forests and land that is detrimental to the nation.

Abstract

Background: esophageal stricture is loss of esophageal lumen patency and may involve injuries after ingesting caustic substances. This systematic review aims to review clinical presentations, diagnostic modalities involved, treatments and outcomes, and complications of esophageal strictures following caustic injuries.Methods: this systematic review involves original researches conducted between 2002-2022 indexed in PubMed and EuropePMC searched using following MeSH terms: “((caustic injury) OR (caustic burn)) AND (ingestion) AND (inflammation)” following PRISMA algorithm. Biases were assessed using STROBE checklist and presented as narratives.Results: searches yielded 326 results with 16 studies included in this review. Majority of caustic ingestion and strictures involved pediatric population due to accidental ingestion. Alkali ingestion has more severe presentation. Radiological imaging, including CT and MRI, may help in determining damages involved. Degrees of damages are related with mortalities or morbidities, and dilatation may improve quality-of-life in some cases.Conclusion: pediatric patients are at risk for accidental ingestions and alkali ingestion creates more severe damage. Radiological imaging may assist in determining involvement of underlying tissues.

Abstract

Investigation into the mechanism underlying excessive trophoblast invasion yields further strategies and insights for the diagnosis and treatment of placenta accreta spectrum disorder (PASD). We conducted a comprehensive literature review to analyze the relationship between CXCR2 expression and PASD, as well as the possibility of CXCR2 being used as a therapeutic and diagnostic biomarker for PASD. Chemokines are well-known mediators in the immune system, particularly for cell recruitment, angiogenesis, and tumor infiltration. CXCR2 is an important component of the immune system, particularly in neutrophils. One of the CXCR2 ligands, IL-8, has also been found to be expressed in the decidua and trophoblasts of humans and to promote autocrine or paracrine trophoblast migration and invasion. Kami melakukan tinjauan literatur yang komprehensif untuk menganalisis hubungan antara ekspresi CXCR2 dan SPA serta potensi CXCR2 sebagai penanda terapeutik dan diagnostik untuk SPA. Kemokin berperan sebagai mediator dalam sistem imun karena perannya dalam perekrutan sel, angiogenesis, dan infiltrasi tumor. CXCR2 mempengaruhi sistem imun, terutama pada neutrofil. Salah satu ligan CXCR2, IL-8, juga telah ditemukan diekspresikan dalam desidua dan trofoblas manusia serta untuk mempromosikan migrasi dan invasi trofoblas autokrin atau parakrin. Peran CXCR2 dalam invasi trofoblas pada SPA memungkinkan para peneliti untuk melihat sekilas patogenesis molekuler SPA.Kata kunci: penanda; CXCR2; plasenta akreta
