Abstract

Lymphoma is divided into Hodgkin lymphoma (HL) and non-Hodgkin lymphoma (NHL). Among NHL T-cell lymphoma is a rare malignancy of T cells, accounts for 12% of all NHL cases, it is further classified into 53 subtypes by WHO. T-cell lymphoma generally have worse prognosis than those B cells. In 2020 there were 16.125 new cases in Indonesia with 9.024 deaths for all ages and genders. Anemia occurs in some cases of lymphoma, which will make the prognosis of lymphoma worse. This study aims to determine the relationship between T-cell lymphoma subtype and anemia. This observational analytic study utilized a cross-sectional design conducted at the Department of Anatomic Pathology Faculty of Medicine Sriwiaya University/Dr. Mohammad Hoesin Palembang Hospital (RSMH) and the central laboratory of RSMH. The samples from this study were all patients who were diagnosed histopathologically and immunohistochemically as T cell lymphoma at RSMH Palembang from 2018 to 2022. Among 38 samples, T-cell lymphoma predominated in the age group <55 years (73,7%), male gender (73,7%), extranodal location (73,7%), and ENKTCL, nasal subtype (55,3%). Anemia occurred in 60,5% cases and most often it is mild anemia (60,9%). There was no significant correlation between T-cell lymphoma subtype and anemia status (*p = 0.178) and T-cell lymphoma subtype and anemia degree (*p = 0.342). It can be concluded that there is no significant correlation between the T-cell lymphoma subtype and anemia.

Abstract

Beberapa penelitian telah dilakukan untuk mengevaluasi kadar 25(OH)D pada pasien penyakit Parkinson. Namun, hasil penelitian ini tidak konsisten. Penelitian ini melakukan meta-analisis terhadap kadar 25(OH)D pada pasien dengan penyakit Parkinson. Database PubMed digunakan untuk melakukan pencarian literatur secara sistematis. Meta-analisis dilakukan dengan menggunakan model efek acak untuk menghitung perkiraan efek gabungan. Tujuh belas artikel dimasukkan dalam penelitian ini. Meta-analisis mengungkapkan bahwa pasien dengan penyakit Parkinson memiliki kadar 25(OH)D yang lebih rendah dibandingkan dengan kontrol. Temuan dari artikel prospektif dan retrospektif sebagian besar konsisten. Besarnya respon nocebo dalam uji coba terkontrol acak yang dirancang paralel pada penyakit Parkinson sangat penting dan harus dipertimbangkan dalam interpretasi hasil keamanan dan dalam desain dan interpretasi uji klinis masa depan.

Abstract

Introduction: Myeloid sarcoma (MS) is a tumor mass that arises outside of the bone marrow area and is composed of either mature or not myeloid blasts of white blood cells. Acute myeloid leukemia (AML), chronic myeloid leukemia (CML) with an imminent blast crisis, and other underlying undiagnosed myeloproliferative disorders are frequently linked to MS, an uncommon disease. Without a history of leukemia, MS is difficult to diagnose and might be misdiagnosed for lymphoma or undifferentiated carcinoma. For patients, correct tissue diagnosis based on immunohistochemistry and histomorphology is important.Case Presentation: Here, we present a 27-year-old woman who had a surgical procedure in The Department of Otorhinolaryngology-Head and Neck Surgery with the chief complaint of right nasal congestion and growing right eye protrusion. A solid mass in the right sinonasal region with infiltration into the retrobulbar area was confirmed by a head computed tomography (CT) scan. Cells with small to medium-sized cells, finely distributed chromatin, tiny nucleoli, and sparse cytoplasm were examined under a microscope. These cells were grouped in diffuse, linear, or Indian file patterns. Microscopic examination revealed morphology of myeloid tumor. Immunohistochemistry analysis emphasized the diagnosis of myeloid sarcoma. The patient then had chemotherapy, comprising of six cycles of docetaxel and carboplatin. After completing chemotherapy, a reduction in the size of the tumor mass was observed.Conclusions: MS was diagnosed based on histological, radiographic, and immunohistochemical studies. Additional immunohistochemistry analysis testing is necessary to rule out a number of differential diagnoses when diagnosing MS in patients without a history of leukemia or hematological malignancy. The positive expression of myeloid markers such as CD68, CD15, CD33, and CD34 supports a diagnosis of MS.

Abstract

Rumput merupakan salah satu permasalahan yang serius di Indonesia Saat ini sebagian besar rumput dihasilkan akibat aktivitas manusia. Adanya perubahan pola hidup dalam mengkonsumsi makanan serta kurangnya kesadaran masyarakat terhadap kebersihan lingkungan menyebabkan peningkatan jumlah dan jenis rumput. Masalah yang akan dibahas adalah bagaimana merancang mesin pencacah rumput organik yang menghancurkan dengan hasil butiran atau serpihan dengan luas 2 mm sampai 5 mm agar dalam proses fermentasi dapat berjalan dengan baik untuk hasil kompos lebih baik. Tujuan dari proyek akhir ini ialah merancang bagian dinamis mesin pencacah rumput organik sebagai bahan dasar kompos. Metode pelaksann yang dilakukan berupa pencarian data untuk pengrakitan alat yang kemudian dirakit dan dicoba terlebih dahulu. Kemudian terdapat penyempurnaan alat agar fungsinya lebih efektif lagi. Berdasarkan hasil perancangan dan pengujian mesin pencacah rumput, maka dapat disimpulkan: Daya yang diperlukan untuk mencacah rumput adalah 162 W, diameter pulley penggerak 70 mm dan diameter pulley yang digerakan adalah 100 mm., bahan poros yang digunakan adalah S30C dengan kekuatan tarik (σB) = 48 kg/mm2. Diameter poros yang digunakan 25 mm dan panjang poros 380 mm, Bantalan yang digunakan untuk menumpu poros adalah bantalan radial bola sudut dalam dengan nomor bantalan UCP205, Berdasarkan hasil pengujian, prosentase rumput yang keluar 90,69% sisanya 9,31% atau sebesar 0,6 kg tidak keluar, hal ini terjadi dalam setiap sekali proses dan Kapasitas mesin pencacah rumput sebesar 103 kg/jam. Dari hasil data tersebut mesin pencacah berfungsi dengan baik dan dapat menampung smapah dengan efektif.

Abstract

Breast cancer is a global and national health concern that affects women worldwide and it is responsible for causing the highest number of cancer-related deaths annually. Additionally, the high incidence of breast cancer imposes a financial burden on the nation. Early breast cancer detection in reproductive age women using breast self-examination (SADARI) was chosen due to simple, inexpensive, and non-invasive procedure. SADARI is very necessary to find out any breast abnormalities so that they can be treated early. This community service project involved providing counseling to female workers in a rubber factory at Palembang city. The counseling was delivered through a lecture and demonstration of SADARI using mannequin. A pre-and post-test questionnaire containing 30 questions was utilized as the evaluation instrument. This activity aimed to measure the increase in knowledge about breast cancer early detection with SADARI among female workers in a Palembang city rubber factory. After counseling, the mean score was 1.42 points higher than before counseling, with p-value of 0.155. The results showed non-significant increase in knowledge before and after the counseling session.

Abstract

BRAF V600E mutation in thyroid cancer is of major concern because it is associated with increased tumor aggressiveness through the MAPK pathway activation that affects phenotypic characteristics such as morphology and tissue invasion patterns. The results of studies on the effects of this mutation often vary and are influenced by differences in study design and detection methods. This systematic review aims to clarify the relationship between BRAF V600E and phenotypic characteristics of PTC, as well as identify its clinical implications. This study method was a systematic literature review using the PRISMA method, and there were 8418 journals. After exclusion, 16 articles were found that met the research objectives. Based on the results, it was found that there was a relationship between BRAF V600E and thyroid cancer phenotypes such as female gender, older age at diagnosis, larger tumor size, histological subtypes, especially tall cell, higher nuclear score, capsule, extrathyroid, vascular, nerve invasion, lymph node metastasis, higher clinical stage, recurrence, and lower survival. However, there are other inconsistent studies regarding the association of BRAF V600E with these phenotype characteristics. BRAF V600E mutation significantly affects the phenotypic characteristics of thyroid cancer and is associated with several aggressive characteristics.

Abstract

BackgroundAngiogenesis is the process of new blood vessel growth that has an important role in tumor development. Microvessel density (MVD) assessment is one of the methods to assess angiogenesis. A high MVD is associated with a poor prognosis in various types of cancer including Hodgkin lymphoma (HL). Although, HL incidence is lower in developing countries, the death rate is higher in these regions. Moreover, this type of lymphoma is higher among young and reproductive people. The aim of this study was to determine the association between microvessel density and HL clinicopathological characteristics.MethodsThis was an observational analytic study with a cross-sectional method, conducted at the Anatomical Pathology Department/Dr. Mohammad Hoesin Palembang. The samples were paraffin blocks diagnosed as HL for period 2016-2021 and had complete clinicopathological data according to the criteria of this study. The slides were immunostained with antibody anti-CD34. One microvessel was determined by observing the endothelial morphology and CD34 expression in endothelial cytoplasm and/or cell membrane, then was evaluated and calculated by Image J. Data was statistically analyzed by STATA version 15.ResultsMajority HLs in this study were discovered in the age group of <45 years (71.00%), male (61.30%), nodal location (80.60%) and lymphocyte rich classical HL (LRCHL) subtype (51.60%). Although there was no significant association between MVD and the clinicopathological characteristics of HL, high MVD tend to occur in the age <45 years, male, nodal location, and LRCHL subtype.ConclusionThere was no significant association between microvessel density and HL clinicopathological characteristics.

Abstract

Introduction: In reconstructive surgery, free flaps are a superior method for resurfacing defects. While free flap viability is typically monitored by subjective clinical examination, lactate and glucose levels in free flaps, which can affect tissue metabolism during ischaemia and reperfusion, can help predict viability. This study aims to review previous research and provide a theoretical basis for using artificial intelligence in lactate and glucose measurement as a means of assessing flap viability.Method: The primary databases used to retrieve the key medical literature presented in this study were book references and Google Scholar, PubMed and Science Direct, using search terms related to the topic. Only articles written in English and published less than ten years ago were included.Results: Lactate levels detect perfusion impairment earlier than clinical signs or other biochemical markers while glucose monitoring can indicate underlying metabolic dysregulation or physiological stress, helps early detection of complications. Combining lactate and glucose measurements enhances diagnostic accuracy and allows for timely interventions for flap viability. Studies confirm this dual monitoring is a practical, unbiased, and has the potential to be developed into an artificial intelligence tool to improve patient outcomes.Conclusion: Lactate and glucose measurements in free flap monitoring have distinct benefits. Lactate detects ischaemia and reflects tissue metabolism, while glucose monitors energy metabolism and systemic health. Combining these leading to improved flap survival rates. With accessible tools, this approach improves patient care and outcomes in reconstructive surgery.

Abstract

Vitamin D plays a role in the healing process of diabetic foot ulcers (DFU); it can improve the immune system, regulate inflammation function, and regulate growth factors in diabetes mellitus (DM). The immunomodulatory effects of vitamin D include reduced production of IL-2, IL-6, and TNF-alpha and increased production of IL-10. It also increases the activity of antimicrobial peptides (AMP) such as cathelicidin, and it promotes the differentiation of monocytes into macrophages, phagocytosis, and the production of lysosomal enzymes. Furthermore, vitamin D stimulates growth factors including vascular endothelial growth factor (VEGF) and transforming growth factor beta (TGF-β). In this study, the incidence of DM and DFU in DM patients at RSUP Dr. Mohammad Hoesin Palembang will be compared to serum vitamin D levels. This research uses quantitative methods with an analytical observational design. The Mean vitamin D serum level was 11,727 ng/mL, and there was a significant association between serum vitamin D deficiency and DFU (p-value; 0.036 < 0.05). There was a significant association between serum vitamin D deficiency and DFU occurrence (p-value; 0.036 < 0.05) in Mohammad Hoesin General Hospital Palembang. Based on the results of this study, we proposed that patients with diabetic foot ulcers should have their serum vitamin D levels checked, and that vitamin D should be given to them as part of their therapy.

Abstract

This study examines the effects of digital fatigue, work-family conflict (WFC), and perceived role overload on career regression among banking employees, with burnout as a mediating factor. Using a cross-sectional design, data were collected from 384 banking employees in Palembang through structured questionnaires. The findings reveal that digital fatigue, WFC, and role overload significantly contribute to burnout, which mediates their impact on career regression. Grounded in the Job Demands-Resources (JD-R) Theory, the results demonstrate that excessive job demands heighten the risk of burnout when unbalanced by sufficient resources, ultimately hindering career development. The study underscores the importance of organisational efforts to minimise digital demands, promote work-life balance, and manage workloads to mitigate burnout. Limitations include reliance on self-reported data and a focus on the banking sector. Future research should explore broader populations and examine additional factors such as social support and coping strategies.
