Abstract

Loneliness affected sleep disorders among the elderly. A poor sleep pattern will be increasing blood pressure among elderly. The aimed his study was to analyzed the correlation between loneliness and sleep disorder among the elderly with hypertension. A case-control design was conducted among 51 elderlies (33 with hypertension, 18 with non-hypertension). A self-administered questionnaire was used to measure the sociodemographic of elderly while UCLA Loneliness Scale was used to measure loneliness, then the PSQI questionnaire was performed to measure sleep disorder. The correlation between loneliness and sleep disorder was analyzed using Chi-square test. The result showed among 51 elderly were 39,4% of mild loneliness of elderly with systole hypertension, and 75,8% of sleep disorder was mostly experienced by the elderly with systole hypertension. There were difference loneliness and sleep disorder among elderly with hypertension and non-hypertension in systolic and diastoilc (p value<0,005). However, the level of loneliness (systole: x2= 10,143,p value=0,017; diastole: x2= 5,137,p value=0,162 ) and level of sleep disorder  (systole: x2= 7,104,p value=0,008; diastole: x2= 0,391,p value=0,531) for hypertension of elderly were more higher compared with non hypertension of elderly. The conclusion of this study, there is a correlation between loneliness and sleep disorder among the elderly. Therefore, the elderly should more take part in activities in home nursing to express their feeling and reduce their loneliness.

Abstract

Background: Acute respiratory infections (ARI) in developing countries at the age of under five have a fairly high prevalence, which relates to air pollution at home, such smoke as firewood, cigarette and mosquito coils.Methods: The purpose of this study was to examine the relationship between smoke exposure of ARI in age child 0-5 year in agricultural area Panti District, Jember Regency. A case control study design was conducted among grup control 120 and case grup 120 of respondent using stratified random sampling. A self administered questionnaire was used to interview indoor air quality. Chi Square test was perform to analyze the relationship between smoke exposure of ARI.Result: The result identified as exposure to firewood smoke 63.3%, exposure to cigarette smoke 75.8% and exposure to smoke from mosquito coils 68.3%. There were significantly relationship between firewood (χ2 = 5.813; p-value = 0.016), exposure to cigarette smoke (χ2 = 9.094; p-value = 0.004), can immediately bring mosquito coils (χ2 = 4.929; p-value = 0.037). Smoke exposure is relationship with ARI in age child 0-5 year.Conclusion : Therefore, efforts to ARI can be prevented by family who maintain air quality at home.

Abstract

ABSTRAK Pasung adalah tindakan pengekangan atau pembatasan aktivitas secara fisik untuk mengendalikan Orang dengan Gangguan Jiwa (ODGJ) yang tidak terkontrol oleh masyarakat biasa atau non profesional. Masalah yang dijumpai pada ODGJ yang dipasung adalah stigma dari masyarakat. Stigma merupakan salah satu faktor penghambat dalam penyembuhan ODGJ. Tujuan dari penelitian ini untuk mengetahui gambaran stigma masyarakat terhadap ODGJ yang dipasung. Desain penelitian menggunakan deskriptif kuantitatif. Sampel 150 responden dengan menggunakan teknik purposive sampling. Instrumen penelitian menggunakan kuesioner Community Attitudes toward the Mentally Ill. Analisis data yang digunakan adalah distribusi frekuensi. Hasil penelitian menunjukan 50.7 % masyarakat pro stigma sedangkan 49.3 % kontra stigma. Hasil penelitian menunjukkan masih terdapat stigma tentang ODGJ yang dipasung. Stigma terhadap ODGJ yang dipasung memiliki dampak negatif pada seluruh aspek kehidupan. ODGJ sering menjadi korban pada tindakan – tindakan kriminal, didiskriminasikan, dan dikucilkan. Pemasungan pada ODGJ dapat dicegah dan diatasi dengan melibatkan peran aktif semua pihak secara komprehensif. Kata kunci : Gangguan Jiwa, Pasung, Stigma. ABSTRACT Pasung is an act of restraint or physical limitation of activities to control people with mental health disorders (ODGJ) that are not controlled by ordinary people or non-professionals. The problem encountered in the installed ODGJ is the stigma from the community. Stigma is one of the inhibiting factors in healing ODGJ. The purpose of this study is to find out the description of the community's stigma of ODGJ being put in a parachute. The study design uses quantitative descriptive. Sample of 150 respondents using purposive sampling technique. The research instrument used the Community Attitudes questionnaire toward the Mentally Ill. Analysis of the data used is the frequency distribution. The results showed that 50.7% of the people were pro-stigmatized while 49.3% were counter-stigmatized. The results showed that there was still a stigma about the ODGJ being installed. The stigma of the ODGJ installed has a negative impact on all aspects of life. ODGJ are often victims of criminal acts, discriminated against and excluded. Inclusion in ODGJ can be prevented and overcome by involving the active role of all parties comprehensively. Abstract

The problems of people with mental health disorders (PWMHD) include physical problems, psychological problems, and social problems. Jember Regency in 2018 had 514 new cases, and Panti village was ranked first with 114 clients (22.1%) of the total cases in the Jember Regency. Community views accompanied by cultural background often prevent families with PWMHD from coming to the health center to seek care and management of health services. Negative public views on PWMHD will reduce PWMHD visits to seek treatment so that cases of drug withdrawal will increase. Nurses have an essential role in providing nursing care to clients with mental disorders. Without knowledge of the cultural viewpoints of the community, nurses will find it difficult to find strategies to reduce the occurrence of stigma and discrimination in PWMHD This study uses an ethnography study, qualitative approach, and descriptive design to explore and understand about the Jember community view in the treatment of mental disorders with a perspective of transcultural nursing. The technique of collecting data uses in-depth interviews with a semi-structured interview guide involving seven participants, and the data were analyzed using interpretive phenomenology analysis. This research has passed the ethical test with ethical approval No.595 / UN25.8 / KEPK / DL / 2019. This research conducted five themes. First is the family efforts to seek healing; the second theme is received trials from God; the third theme is community stigma to people with mental disorders; the fourth theme is community expectations for people with mental disorders, and the fifth theme is community acceptance of people with mental disorders. Considering the impact that will arise from this condition if it does not get proper treatment, it is needed the role of health workers in providing services to the family to be able to increase visits to health services in order to improve the recovery of people with mental disorders.

Abstract

Health care providers (HCPs) in Public Health Center (PHCs) have essential role to fulfil the various complexities of physical, psychological, economic, and social needs among people affected by leprosy (PALs) during treatment in communities. The complexity of these needs is important to prevent the stigmatization and discrimination of PALs for transmission and disability during their rehabilitation. Objective of this study to explore the experience of HCPs for fulfilling the PALs needs during attending community based rehabilitation in PHCs of Jember regency, Indonesia. A descriptive phenomenology was performed to HCPs during July to August 2020. In depth interview was saturated to six of HCPs who were responsibility to care PALs in community. Data was analyzed using systematic coding to all of verbatim of participants. Regarding our in depth interview for sixt of HCPs, we were identified of three of themes which explained into seven of subthemes and sixteen of categories. The main themes were adherence to treatment efforts, treatment to solve social limitation, and caring for preventing disability. The experiences of HCPs in caring for PALs during community based rehabilitation were identified in three main themes, namely: efforts to maintain adherence to treatment, efforts to prevent social barriers, and basic care to prevent disability. Therefore, regular visits through home visits can be carried out on PALs so that MDT compliance is good and reduces side effects. The formation of self-care groups is indispensable in teaching basic care skills in an effort to prevent future disability.

Abstract

Older people (OP) have high risk for hypertension related with changing during older process in particularly system of cardiovascular. The purpose of this study is to identify prevalence of hypertension and related factors among OP in nursing home of Jember, East Java, Indonesia. A cross-sectional study was conducted among 42 of OP in nursing home. A self-administered questionnaire was used to identify characteristic of participants. Data of activity daily living, cognitive function, emotional status, depression status, and spiritual activity was measured using Katz Index of Independence in Activities of Daily Living (ADL), Short Portable Mental Status Questionnaire (SPMSQ), The Mini Mental State Examination (MMSE), Depression Anxiety Stress Scale 14 (DASS-14), and Daily Spiritual Experience Scale (DSES), respectively. Blood pressure was measured using a standard protocol employing a stethoscope and sphygmomanometer. A logistic regression analysis was used to determine factors related to hypertension among OP. Blood pressure of systolic and diastolic among OP were 131.90 ± 16.56 mmHg and 82.02 ± 10.18 mmHg, respectively. The prevalence of systolic and diastolic hypertension among OP in nursing home were 38.1% and 21.4%, respectively. The factors of systolic hypertension were age (p= 0.038), activity daily living (p= 0.043), depression status (p= 0.012), and spiritual activity (p= 0.015). Meanwhile, factors of diastolic hypertension were age (p= 0.041) and education (p= 0.049). Increasing age of OP related a risk factor of hypertension by supported by there are psychological problems like depression, although could be reduced by existence of spiritual activity of OP. Provision of health education on changes in the cardiovascular system and adaptation of lifestyle changes in the OP will be able to reduce hypertension factors in the OP.

Abstract

The family with special need children experience children’s dependency behavior. This situations related to parenting stress which affected parental quality of life. The objective of this study was to examine the correlation characteristic of parents and parenting stress and parental quality of life among special need children’s parents. A cross sectional study design conducted among 52 parents with special need children in School of Disabled Children in Badean Bondowoso. A self-administered questionnaire including the Alabama Parenting Questionnaire and the World Health Organization Quality of Life-BREF Indonesian version were used to assess parenting stress; and parental quality of life, respectively. Pearson Product Moment and Spearman Rank was performed to answer the objective of this study. The results showed that, there were no correlation between parenting stress and parental quality of life of special need children’s parents (p >0.05). However, length of disability correlated with parenting stress (r = 0.339; p = 0.014). Meanwhile; parent’s gender (Z = -2.089; p = 0.037), special need children’s gender (Z = -2.102; p = 0.036), and number of childrens (r = -0.297; p = 0.033) were correlated with parental quality of life. This study concluded that characteristic of parents and children with special needs correlated with parental quality of life. Therefore, adaptive parenting environment should implement to reduce parenting stress.

Abstract

Fungsi keluarga merupakan salah satu faktor yang mempengaruhi status gizi balita pada keluarga tiri di Kecamatan panti Kabupaten Jember. Dimensi fungsi keluarga berupa penyelesaian masalah, komunikasi , peran, Responsivitas Afektif, Keterlibatan afeketif, kontrol perilaku , fungsi umum.  Penelitian ini bertujuan untuk menganalisis hubungan antara fungsi keluarga dan status gizi. Penelitian ini menggunakan desain analitik observasional dengan pendekatan cross-sectional. Responden dalam penelitian ini adalah 45 dengan menggunakan teknik total sampling. Kuisioner yang dikelola sendiri digunakan untuk mengukur sosiodemografi keluarga, sementara alat Family Assessment Device (FAD) untuk mengukur fungsi keluarga, dan timbangan digital berat badan digunakan untuk mengukur status gizi. Analisis data dilakukan dengan uji korelasi Spearman-Rank dengan tingkat signifikansi 0,05. Hasil penelitiandidapatkan dari 45 keluarga tiri yang menunjukkan fungsi keluarga nilai mediannya adalah 139,00 dengan nilai minimum 47 dan nilai maksimum 188. Status gizi balita pada keluarga tiri di kecamatan panti terbanyak mengalami gizi baik yaitu 60% .Hasil uji statistik menunjukkan bahwa nilai P value 0,040< 0,05. Koefisiensi  korelasi 0,308 yang artinya terdapat hubungan fungsi keluarga dengan status gizi balita di Kecmatan Panti Kabupaten Jember adalah cukup kuat. Ada hubungan fungsi keluarga dengan status gizi balita pada keluarga tiri di Kecamatan Panti, sehingga jika fungsi keluarga lebih ditingkatkan maka status gizi anak balita juga akan meningkat menjadi optimal.

Abstract

Sikap pilih-pilih makanan pada balita adalah masalah bagi setiap orang tua karena dapat mempengaruhi pola makan balita. Peran keluarga sangat dibutuhkan dalam hal strategi gizi melalui komunikasi keluarga fungsional. Tujuan penelitian untuk mengetahui hubungan komunikasi keluarga terkait kebutuhan nutrisi dengan status gizi anak. Penelitian ini menggunakan desain cross sectional dilakukan pada 236 keluarga yang memiliki balita berusia 2-5 tahun dengan pengambilan sampel secara berurutan. Kuesioner menggunakan Parental Nutritional Communication Quistionaire (PNCQ). Uji Kruskal Wallis digunakan dalam penelitian ini untuk menjawab tujuan penelitian ini. Hasil perhitungan gizi menurut BB / U menunjukkan bahwa balita kurang gizi di Kecamatan Panti adalah 22 (9,3%), gizi rendah 63 (26,7%), gizi normal 148 (62,7%) dan obesitas 3 (1,3) %) dan ada hubungan komunikasi keluarga terkait kebutuhan gizi dengan status gizi balita di Kabupaten Panti, Kabupaten Jember dengan nilai p 0,000. Komunikasi keluarga berkaitan dengan kebutuhan gizi dengan status gizi anak, Perlunya pengoptimalan komunikasi keluarga untuk meningkatkan status gizi balita

Abstract

F Petani dengan hipertensi perlu membatasi asupan natrium mereka untuk kondisi yang lebih baik. Tujuan dari penelitian ini adalah mendeskripsikan pembatasan asupan sodium pada petani hipertensi di Wilayah Kerja Puskesmas Panti, Kabupaten Jember. Desain penelitian Cross-Sectional dilakukan di antara 131 petani hipertensi menggunakan consecutive sampling. Kuesioner digunakan untuk mengidentifikasi sosiodemografi. Dietary Sodium Restriction Questionnaire (DSRQ-I) digunakan untuk mengukur pembatasan asupan sodium pada petani dengan hipertensi di Wilayah Kerja Puskesmas Panti Kabupaten Jember. Sphygmomanometer dan stethoscope digunakan untuk mengukur tekanan darah. One sample t test digunakan untuk menjawab tujuan penelitian (p >0,05). 75,6% mengalami hipertensi sistolik tahap I dan 74,8% hipertensi diastolik tahap I. Sementara itu, skor sikap subyektif dan norma yakni 27,29 ± 4,602 di antara petani dengan hipertensi, menunjukkan bahwa petani dengan hipertensi memiliki sikap buruk untuk membatasi asupan natrium. Selanjutnya, skor kontrol perilaku yang dirasakan adalah 17,12 ± 4,237 di antara petani dengan hipertensi, menunjukkan bahwa petani dengan hipertensi memiliki banyak kendala untuk membatasi asupan natrium mereka. Oleh karena itu perlu dilakukan monitoring tekanan darah secara berkala pada petani hipertensi di Wilayah Kerja Puskesmas Panti Kabupaten Jember dan pengaturan pembatasan asupan sodium pada petani hipertensi melalui kegiatan Posbindu Penyakit Tidak Menular (PTM) secara berkala setiap bulannya.
