Abstract

Background: The health of the elderly is influenced by various risk factors and changes from within themselves and their environment. These various conditions can affect health problems, functional status, level of independence and quality of life of the elderly. Deterioration of health in the elderly can manifest in a variety of diseases, including hypertension. Efforts to control the incidence of hypertension can be done through the provision of health education about hyper­ten­sion, which aims to raise awareness of hypertensives about the concept of hypertension, compli­cations, and how to overcome. This study aims to examine the application of the Health Belief Model (HBM) as a predictor by taking into account the contextual effects of the Elderly Integrated Healthcare Posts in Kepanjen, Malang, East Java, Indonesia. Subjects and Method: This study was an observational analytical study with a cross-sectional approach using a multilevel analysis model. The population in this study was all elderly people with hypertension at the Elderly Integrated Healthcare Posts in the working area of Kepanjen Commu­nity Health Center, Malang, East Java, Indonesia. The sample in this study was 200 elderly with hypertension at the Elderly Integrated Healthcare Posts in the Kepanjen Community Health Center Working Area, Malang, East Java, Indonesia. The study used the Stratified Sampling technique at the healthcare level (Elderly Integrated Healthcare Posts), and Stratified Random Sampling in the selection of the elderly. Results: Tertiary prevention behavior among elderly with hypertension increased with high self-efficacy (b= 0.57; CI 95%= 0.39 to 0.73; p<0.001), cues to action (b= 0.27; CI 95%= 0.35 to 1.71; p<0.001), female gender (b= 0.76; CI 95%= 0.10 to 0.42;  p<0.001), age 70-90 years (b= 0.37; CI 95%= -0.01 to 0.75; p= 0.060). Integrated health posts for elderly on the tertiary prevention beha­vior of hypertension. The Integrated Healthcare Posts had contextual effect on tertiary prevention behavior (ICC=29.87%; p<0.001). Conclusion: Tertiary prevention behavior of hypertension in the elderly increases with high self-efficacy, cues to action, female gender, age 70-90 years.

Abstract

Background: Clinical epidemiological studies show that smoking and alcohol are often used together as social activities. Smoking and alcohol consumption are related to the risk of developing non-communicable diseases. According to data from the Indonesian Health Survey (SKI), (2023) the percentage of smokers in Surakarta City is 23.6% and the percentage of alcohol consumption is 1.4%. This study aims to analyze the influence of social capital on smoking behavior and alcohol consumption in adults. Subjects and Method: The research design used is a cross-sectional study. The population in this study is adults aged 19-59 years in the city of Surakarta. Sampling in the study was carried out by the snowball sampling method, then as many as 200 adults domiciled in the city of Surakarta were obtained. The method used in collecting data for this study is a questionnaire. The analysis used in this study is the Structural Equation Model (SEM) to analyze the influence of social capital elements on smoking behavior and alcohol consumption in adults. The dependent variables are smoking habits and alcohol consumption. The independent variable is social capital including 3 elements, namely structural elements, cognitive elements and relational elements. Results: Social capital had a negative effect on smoking behavior (b= -0.80; 95% CI= -0.88 to -0.71; p= 0.001), and social capital has a negative effect on alcohol consumption (b= -0.71; 95% CI= -0.80 to -0.63; p= 0.001). Conclusion: The results of this study show that social capital constructs include structural elements, cognitive elements and relational elements that are high have a lower likelihood of smoking behavior and alcohol consumption in adults.

Abstract

The digitalization of healthcare services has significantly improved access to data and information regarding HIV/AIDS patients, thereby enhancing monitoring and evaluation efforts. However, discrepancies in data across various providers can lead to inefficiencies and hinder the formulation of comprehensive HIV/AIDS prevention and control programs. Therefore, integrating sustainability into healthcare service digitalization practices is essential. This research aims to explore the reinforcement of HIV/AIDS prevention and control through a sustainable data management system, using Giddens’ structuration theory as the framework. A qualitative research method with an exploratory approach was employed, including data collection through observation, in-depth interviews, Focus Group Discussions (FGD) with key informants, and document processing. The informants included the person responsible for health promotion at the Health Office of Surakarta City, members of the Regional AIDS Commission (KPAD), staff from hospitals and several public health centers (Puskesmas) in Surakarta, field officers, members of the Peer Support Group (KDS), non-governmental organizations (NGOs) involved in HIV/AIDS prevention and control, as well as HIV/AIDS patients and at-risk individuals. The research findings reveal that both people (as the agency) and institutions responsible for health data collection must collaborate to establish a sustainable data system. This collaboration ensures data accuracy and continuity, which, in turn, strengthens HIV/AIDS prevention and control programs. A sustainable data system also supports medication adherence, timely updates on patient information, and various other aspects of HIV/AIDS prevention and control.

Abstract

Background: Cavities or dental caries is one of the most common diseases as the main cause of pain in the oral cavity which can have a significant impact on quality of life. Cavities have several treatment modalities available such as restoration and tooth extraction. These treatment options not only affect the patient's clinical condition, but can also affect the psychological, social, and functional aspects reflected in the Oral Health Related Quality of Life (OHRQoL). The study aimed to determine the effect of restoration treatment and tooth extraction on Oral Health Related Quality of Life (OHRQoL). Subjects and Method: This research was conducted in December 2024 with a total of 200 samples in Surakarta, This study uses a quantitative design with a cross-sectional approach. Data were collected from patients who had undergone restoration treatment and/or tooth extraction on cavities. Dependent variables are in the form of Oral Health Related Quality of Life (OHRQoL) and independent variables in the form of restorative treatments, tooth extraction, age and gender. Data were analyzed by double linear regression. Results: Multiple linear regression analysis showed a significant negative association between the type of dental treatment and OHRQoL. Patients who underwent tooth extraction had OHRQoL scores that were 26.89 units lower than those who received restorative treatment (b = -26.89; 95% CI = -29.39 to -24.39; p < 0.001). No significant association was found between age or sex and OHRQoL. Conclusion: Dental restoration treatments have a significant influence on improving Oral Health Related Quality of Life (OHRQoL), while tooth extraction shows a positive but insignificant relationship. Age and sex variables had no statistical effect on OHRQoL.

Abstract

Di tingkat nasional dan daerah telah terdapat kebijakan publik, program dan kegiatan untuk menanggulangi HIV/AIDS, namun jumlah kasus HIV/AIDS meningkat setiap tahun. Tujuan penelitian ini untuk mengetahui efektivitas collaborative governance antarpemangku kepentingan dalam pelayanan komprehensif berkesinambungan untuk menanggulangi HIV/AIDS di Kota Surakarta. Penelitian ini merupakan studi kasus jenis exploratory. Informan ditentukan dengan menggunakan purposive sampling. Pengumpulan data dilakukan pada 2 Juli – 2 September 2013 dengan wawancara mendalam, observasi, focus group discussion, dan dokumentasi. Teknik analisis data dengan model analisis interaktif. Hasil penelitian menunjukkan bahwa pemberi dan penerima layanan berperan dalam penanggulangan HIV/AIDS sesuai tugas dan fungsinya masing-masing. Program pencegahan dan penjangkauan, layanan kesehatan, reduksi bahaya, dan pemberdayaan belum efektif karena komitmen terhadap tujuan dan sikap saling percaya antarpemangku kepentingan belum optimal, petugas lapangan kurang profesional, terdapat konflik laten antarpemangku kepentingan, kurang optimalnya koordinasi antaranggota Komisi Penanggulangan AIDS Daerah Kota Surakarta dan rendahnya anggaran untuk penanggulangan HIV/AIDS karena HIV/AIDS belum menjadi isu prioritas dalam pembangunan daerah. Disimpulkan bahwa kolaborasi governance antarpemangku kepentingan belum efektif. Untuk meningkatkan kualitas dan efektivitas kolaborasi governance antarpemangku kepentingan, harus ada norma, struktur, dan proses yang jelas dalam menanggulangi HIV/AIDS. There have been public policies, programs and activities to cope with HIV/AIDS in Indonesia at national and local level, but number of HIV/AIDS cases is increasing every year. This study aimed to determine effectivity of collaborative governance between stakeholders in a sustainable comprehensive service to cope with HIV/AIDS in Surakarta City. This study was an exploratory study. Informants were selected using purposive sampling technique. Data collection was conducted on 2 July – 2 September 2013 using in-depth interview, observation, focus group discussion, and documentation. Technique of data analysis was an interactive analysis model. Results showed that service provider and receiver had taken roles in HIV/AIDS coping based on their own duty and function. Prevention and outreach, healthcare service, harm reduction and empowerment programs had not been yet effective because of less optimal commitment to purpose and mutual trust between stakeholders, less professional fieldworkers, latent conflict occurred between stakeholders, less optimal coordination between AIDS Coping Commission of Surakarta City members, and low budget for HIV/AIDS coping as HIV/AIDS is not yet a priority issue in regional development. In brief, collaborative governance between stakeholders is not yet efffective. To improve the quality and the collaborative governance effectivity between stakeholders, there should be any clear norm, structure and process in coping with HIV/AIDS.
