Abstract

ABSTRACT Pulmonary tuberculosis (TB) frequently causes impaired airway clearance due to increased mucus production and ineffective mucociliary function, leading to respiratory symptoms and reduced oxygenation. Postural Drainage (PD) and the Active Cycle of Breathing Technique (ACBT) are airway-clearance interventions proven to enhance secretion mobilization and improve pulmonary function. To determine the effect of combined PD and ACBT therapy on oxygen saturation, sputum volume, and cough–sputum symptoms in patients with pulmonary tuberculosis. This study used a quasi-experimental design with a pre–post test without control group involving 15 TB pulmonary patients recruited through purposive sampling. The intervention consisted of three sessions of combined PD and ACBT. Outcome measures included oxygen saturation (SpO₂), sputum volume, and CASA-Q scores. Data were analyzed using the paired t-test. The combination therapy significantly increased SpO₂ (96.07% to 98.00%; p = 0.001), sputum volume (2.87 mL to 5.40 mL; p = 0.000), and all CASA-Q domains related to cough and sputum (p = 0.000). Combined PD and ACBT therapy is effective in improving airway clearance, respiratory symptoms, and oxygenation in pulmonary TB patients. This intervention can be recommended as a safe, low-cost, and practical non-pharmacological therapy in respiratory nursing care. Keywords: Active Cycle of Breathing Technique, Airway Clearance, Postural Drainage, Pulmonary Tuberculosis.  ABSTRAK Tuberkulosis paru sering menimbulkan gangguan bersihan jalan napas akibat peningkatan produksi mukus dan disfungsi mukosilier, sehingga memicu gejala respirasi dan penurunan oksigenasi. Postural Drainage (PD) dan Active Cycle of Breathing Technique (ACBT) merupakan intervensi pembersihan jalan napas yang terbukti meningkatkan mobilisasi sekret serta fungsi paru. Mengetahui efek terapi kombinasi PD dan ACBT terhadap saturasi oksigen, jumlah sputum, serta gejala batuk dan sputum pada pasien tuberkulosis paru. Penelitian menggunakan desain quasi-eksperimen pre–post test tanpa kelompok kontrol pada 15 pasien TB paru yang dipilih secara purposive. Intervensi diberikan dalam tiga sesi. Pengukuran mencakup SpO₂, volume sputum, dan skor CASA-Q. Analisis data menggunakan paired t-test. Terapi kombinasi memberikan peningkatan signifikan pada saturasi oksigen (96,07% menjadi 98,00%; p = 0,001), jumlah sputum (2,87 mL menjadi 5,40 mL; p = 0,000), serta seluruh domain batuk dan sputum pada CASA-Q (p = 0,000). Kombinasi PD dan ACBT efektif meningkatkan bersihan jalan napas, memperbaiki gejala respirasi, dan meningkatkan oksigenasi pada pasien TB paru. Intervensi ini dapat direkomendasikan sebagai terapi nonfarmakologis yang aman, mudah diterapkan, dan mendukung praktik keperawatan respirasi. Kata Kunci: Active Cycle of Breathing Technique, Bersihan Jalan Napas, Postural Drainage, Tuberkulosis Paru.

Abstract

ABSTRACT Type 2 Diabetes Mellitus is a chronic disease caused by unstable blood sugar levels that exceed normal limits, or hyperglycemia, and disorders of carbohydrate, protein, and fat metabolism. Hyperglycemia can cause complications in form of macrovascular and microvascular blood vessel disorders. Efforts to controls blood sugar include physical activity, such as passive stretching exercise, which are beneficial for increasing cellular glucose uptake, thereby reduce blood glucose levels. This study aims to provide an overview of the impact of implementing passive stretching exercise in type 2 diabetes mellitus patients for reduce blood glucose levels through Evidence-Based Nursing Practice (EBNP). This study used a systematic approach by reviewing various articles and literature, namely PubMed and Google Scholar. The selected articles focused on passive stretching exercise interventions in patients type 2 diabetes mellitus to reduce blood glucose levels. The review identified one article in the form of systematic review with meta-analysis, which examined 13 studies. The results of the analysis showed that passive stretching exercises were effective in reduce blood glucose levels. The implementation of passive stretching exercise has been shown to reduce blood glucose level in patients with type 2 DM. Keywords: Blood Glucose Levels, Diabetes Mellitus Type 2, Hyperglycemia, Passive Stretching Exercise.  ABSTRAK Diabetes Melitus tipe 2 merupakan penyakit menahun yang disebabkan oleh ketidakstabilan kadar gula darah yang melebihi batas normal atau hiperglikemia dan gangguan metabolisme karbohidrat, protein, dan lemak. Hiperglikemia dapat menimbulkan komplikasi berupa gangguan pembuluh daik makrovaskuler atau mikrovaskuler. Upaya kontrol gula darah yaitu dengan melakukan aktivitas fisik, salah satunya adalah latihan passive stretching exercise. Latihan ini bermanfaat untuk meningkatkan serapan glukosa seluler sehingga menurunkan kadar glukosa darah. Penelitian ini bertujuan untuk memberikan gambaran tentang dampak pada penerapan latihan passive stretching exercise pada pasien DM tipe 2 dalam menurunkan kadar gula darah melalui praktik keperawatan berbasis bukti. Studi ini menggunakan pendekatan sistematis dengan meninjau berbagai artikel dan literatur, yaitu PubMed dan google scholar. Artikel-artikel yang dipilih berfokus pada intervensi latihan passive stretching exercise pada pasien DM tipe 2 untuk menurunkan kadar glukosa darah berbasis bukti dalam keperawatan. Tinjauan ini mengindetifikasi satu artikel dalam bentuk tinjauan sistematis dengan meta-analisis, yang meneliti 13 penelitian. Hasil analisis menunjukkan bahwa latihan passive stretching exercise efektif menurunkan kadar glukosa darah.  Penerapan passive stretching exercisemenunjukkan hasil dapat menurunkan kadar glukosa darah pada pasien DM tipe 2.  Kata Kunci: Diabetes Melitus Tipe 2, Hiperglikemia, Kadar Glukosa Darah, Latihan Peregangan Pasif.

Abstract

HIV infection continues to be a significant public health problem worldwide, including Indonesia, with impacts spanning social, economic, and cultural aspects. People living with HIV (PLHIV) are faced with various pressures and are in dire need of coping strategies. This study aimed to determine the relationship between self-efficacy and spiritual well-being with the use of coping strategies in PLHIV. This study is an analytic study with a cross sectional approach. A sample of 39 ODHIV was selected using purposive sampling method. Data were collected by filling out the GSES questionnaire to assess self-efficacy, the SWBS questionnaire to measure spiritual well-being, and the BRIEF RCOPE questionnaire to determine coping strategies. Logistic regression multinomial inferencial test found no association between self-efficacy and spiritual well-being with the use of coping strategies. Most were male (76.9%), high school education level (64.1%), and married (59%). Descriptive analysis obtained respondents who used problem-oriented coping strategies (28.2%); emotion-oriented (38.5%); and religious-oriented (33.3%). Multinomial logistic regression test showed no significant association between self-efficacy and the use of coping strategies and no association between spiritual well-being and coping strategies (p-value > 0.05). Spiritual aspects are important to consider as they can be a source of coping and motivation to help patients cope with sudden illness and reduce the level of pain due to the perceived condition so as to quickly move towards healing. Nurses need to apply nursing care using coping instruments, comprehensively from the assessment process to the evaluation process.

Abstract

Background: Diabetes Mellitus is a metabolic disease with an increase in blood sugar levels above normal, which can cause complications if not properly managed. One of the complications that can arise is diabetic ulcers, which require nursing interventions after debridement or post-amputation. Methods: This study employed a mixed-methods approach, combining both quantitative data and a case study design. Following STROBE guidelines, the quantitative component included 19 individuals diagnosed with diabetes mellitus. Using purposive sampling, 7 participants were selected based on the inclusion criteria of having diabetic ulcers and being hospitalised for at least 3 days, and 5 nurses who cared for the patients. Data were collected using structured observation sheets and nursing care records. For the qualitative component, guided by COREQ criteria, in-depth interviews were conducted using semi-structured interview guides to explore patients' experiences and perceptions. Data analysis for the qualitative data used grounded theory methodology, while descriptive statistics were applied to the quantitative data. NVivo software was used for qualitative coding, while SPSS was employed for descriptive analysis.  Results: Orem's theory can be used in self-care efforts by carrying out wholly compensatory system nursing interventions, such as wound care, monitoring infections, and carrying out antibiotic treatment, while partially compensatory systems can be carried out by patients independently, such as insulin treatment at home, physical activity, and not smoking, while supportive educative systems by providing foot care education by assessing foot health status. Orem's theory can be used in self-care efforts by performing wholly compensatory system nursing interventions with education, and patients can perform self-care with partially compensatory systems, such as physical activity, not smoking, and insulin use. Conclusion: Self-care nursing interventions for diabetic ulcers include partially compensatory and totally compensatory systems.

Abstract

Diabetes Mellitus is a chronic condition characterized by impaired insulin production, leading to hyperglycemia. Physically and psychologically, Diabetes Mellitus can cause stress and fluctuations in blood sugar levels. This application aims to determine the effect of Progressive Muscle Relaxation (PMR) on stress and blood sugar levels in Diabetes Mellitus patients through the evidenced based nursing practice. This study used a quasi-experimental method with one group pretest posttest design. Data collection was carried out using the Diabetes Distress Scale (DDS-17) and blood sugar level measurements. Determination of the sample using purposive sampling of 25 Diabetes Mellitus patients. This study showed that there was a decrease in stress scores of 0.162 points and a decrease in blood sugar levels of 8.12 mg/dl after the implementation of Progressive Muscle Relaxation for 3 days. Progressive Muscle Relaxation has been proven to be effective in reducing stress and blood sugar levels in patients with Diabetes Mellitus and can be used as an evidence-based intervention in nursing care for patients with Diabetes Mellitus.

Abstract

Diabetes mellitus is a metabolic disorder characterized by increased blood sugar levels (hyperglycemia) caused by lack of insulin, inability of insulin to work or both. Lack of education or information from health workers can have an impact on patient compliance with treatment, which will result in repeated treatment. This innovation program aims to reduce the risk of readmission in Diabetes Mellitus patients by providing integrated education using the DiaTOHC method, patients who are screened using the DERRI application if the score is > 27%, patients are at risk of readmission, then they will be given education using the DiaTOHC method, which is integrated education. When patients are treated, they will be given education on the 5 pillars, after the patient goes home during the control, they will be reviewed whether there are any obstacles or obstacles in the treatment, then in weeks 2, 3 and 4 the patient will be contacted by telephone for re-education and evaluation of treatment. The results of the trial of this program from 17 samples were successful, all of which were readmissions, it was concluded that there was effectiveness in providing education using the DiaTOHC method in Diabetes Mellitus patients to reduce the risk of readmission. Data is collected by interview, observation and filling out questionnaires, after the data is collected, data analysis is carried out by inputting all data found in excel software and then assessing the percentage of patients who are readmitted.

Abstract

Diabetic Foot Care is primary prevention in diabetic foot management to prevent injury to the occurrence of wounds on the feet of people with diabetes mellitus. Objective: To prove that educational interventions can prevent chronic complications, namely diabetic neuropathy (motor, sensory and autonomic) or death of the nerves in the feet, often exacerbated by infection. Method: This study used the Quasi Experiment method with the One Group pretest and posttest design method . Data collection was carried out using NAFF (Nottingham assessment of Functional Footcare) and the Attitude instrument modified by the Knowledge, Attitude and Practice (KAP) questionnaire. Determination of the sample using the accidental sample method of 24 people in Type 2 Diabetes Mellitus patients. Results: This study showed an increase in value of 11.12 points on NAFF and an increase of 1.41 points on attitudes after educational interventions related to diabetic foot care were carried out from May 30, 2024 to June 7, 2024. Conclusion: Diabetic Foot Care Education Intervention has been proven effective in reducing complications in diabetic feet and can be used as an evidence-based intervention in preventive care of diabetic feet in patients with Diabetes Mellitus.

Abstract

Diabetic retinopathy (DR) is a leading microvascular complication of diabetes mellitus (DM), primarily caused by chronic hyperglycemia. It leads to vascular occlusion, increased vascular permeability, retinal abnormalities, and detachment, ultimately resulting in visual impairment and blindness. Implementing Evidence Based Nursing Practice using diagnostic tests to determine the sensitivity and specificity of the assessment instrument for Diabetic Retinopathy Risk Factors in Type II DM Diagnostic tests are descriptive observational studies with a cross-sectional study design. This application is classified as descriptive observational because only observations are made without any intervention (treatment). The test is carried out with ROC where to see the sensitivity and specificity of the application instrument. The results of the analysis showed that the sensitivity of the application of the assessment of diabetic retinopathy risk factors using the ROC Curve Test was 100%. Meanwhile, the specificity of the instrument for the application of the assessment of diabetic retinopathy risk factors was 81.3%. The application of diabetic retinopathy risk factor assessment can be used to assess the risk of diabetic retinopathy because it has good sensitivity (%) and specificity (%) values. However, when applying the diabetic retinopathy risk factor assessment, only the risk value of diabetic retinopathy was obtained where the application patient did not experience complaints and decreased visual acuity.

Abstract

Nausea and vomiting are common side effects experienced by breast cancer patients undergoing chemotherapy. Complementary therapies, such as peppermint and ginger aromatherapy, have been widely studied as alternatives to relieve these symptoms. However, the effectiveness of each type of aromatherapy needs further evaluation. Objective: This study aims to analyze the effectiveness of peppermint and ginger aromatherapy in reducing nausea and vomiting in breast cancer patients undergoing chemotherapy. Methods: A Quasi-Experimental design was used with 28 breast cancer patients at Husada Hospital Jakarta, divided into two groups: peppermint and ginger. Nausea and vomiting levels were measured before, 2 hours, and 6 hours after chemotherapy and analyzed using ANOVA Repeated Measures and Independent T-Test. Results: The results showed that both peppermint and ginger aromatherapy significantly reduced nausea and vomiting after chemotherapy (p < 0.05). However, at 6 hours after chemotherapy, the peppermint group had lower average nausea and vomiting scores compared to the ginger group (6.57 vs. 10.29, p = 0.036). This indicates that peppermint is more effective than ginger in reducing nausea and vomiting after chemotherapy. Conclusion: Both peppermint and ginger aromatherapy are effective in reducing nausea and vomiting in breast cancer patients undergoing chemotherapy. However, peppermint showed higher effectiveness compared to ginger at the 6-hour post-chemotherapy time point. Future research could explore the combination of peppermint and ginger aromatherapy to enhance effectiveness in reducing nausea and vomiting in breast cancer patients.

Abstract

Multifactorial fatigue syndrome is a symptom often felt by people with type 2 diabetes mellitus. Fatigue can prevent someone from doing daily activities that support glucose control. Progressive muscle relaxation and Bensonspiritual careis a combination of relaxation to reduce fatigue. This study was to determine the effectiveness of the combination of progressive muscle relaxation and Bensonspiritual careon fatigue in patients with type 2 diabetes mellitus. Research design using designQuasy Experimental pre test – post test control group designand techniquespurposive sampling. The sample of this study consisted of 2 groups, namely intervention and control, each with 24 respondents. The sampling technique used is purposive sampling. Fatigue was measured using the Indonesian Multidimensional Fatigue Inventory (IMFI-20). There was a decrease in fatigue with the combination of progressive muscle relaxation and Bensonspiritual care with the average fatigue value decreasing significantly (-59.5) compared to the control group (-16.8). The results of the independent t-test obtained ap value <0.001, there was a difference in the average fatigue (difference) between the intervention group and the control group. The study concluded that there was a combined effect of progressive muscle relaxation and Bensonspiritual care on reducing fatigue in patients with type 2 diabetes mellitus. Interventions can be developed as independent nursing interventions in patients with type 2 diabetes mellitus with fatigue.
