Abstract

Introduction: Stunting is a chronic condition characterized by impaired linear growth, cognitive deficits and long‑term economic consequences. South Sumatra reports one of the highest stunting burdens in Indonesia. This activity aimed to improve participants’ knowledge of stunting prevention and to monitor changes after educational and supplementary feeding activities. Methods: This community service was conducted during the 63rd Student Study Service (Kuliah Kerja Nyata/KKN) of Universitas Muhammadiyah Palembang in July 2025 in four villages of Payaraman District, Ogan Ilir Regency, South Sumatra. Pregnant women and mothers of infants and toddlers were recruited via local health posts and community leaders. Activities included didactic sessions explaining the definition, causes and prevention of stunting; distribution of high‑protein mung bean porridge fortified with moringa leaf; vitamin A supplementation; and pre‑ and post‑intervention questionnaires to assess knowledge. Results: A total of 98 participants attended the programme. Pre‑ and post‑tests were conducted in Payaraman Timur and Payaraman Barat. In Payaraman Timur, correct responses increased from 8 of 16 (50 %) at baseline to 13 of 16 (81.25 %) after the intervention, representing a 31.25 % improvement in knowledge. In Payaraman Barat, the post‑test showed that 24 of 29 participants (82.8 %) answered correctly. Qualitative feedback indicated that participants better understood the importance of adequate nutrition during the first thousand days of life and the role of hygiene and infection prevention in reducing stunting. Conclusion: The community education and supplementary feeding programme enhanced participants’ understanding of stunting prevention, as reflected by the marked increase in correct responses in Payaraman Timur. Future programmes should include pre‑ and post‑tests in all locations and monitor long‑term anthropometric outcomes to assess the impact on child growth. Abstract

Solid pseudopapillary neoplasm (SPN) of the pancreas is a rare epithelial tumor that predominantly affects young women and is generally regarded as a low-grade malignant neoplasm with favorable prognosis after complete surgical resection. However, a small subset of cases may exhibit aggressive behavior, including local infiltration and distant metastases. This case is noteworthy because it demonstrates that SPN may present with indolent and nonspecific symptoms despite already showing aggressive intraoperative findings. A 25-year-old woman presented with a 3-month history of intermittent dull epigastric pain, progressive abdominal enlargement, early satiety, abdominal discomfort, and occasional nausea. Physical examination revealed abdominal distension with a palpable mass in the epigastric to left hypochondriac region. Abdominal ultrasonography demonstrated a pancreatic mass measuring approximately 6–8 cm with solid and cystic components, while computed tomography showed a well-defined encapsulated solid-cystic pancreatic mass suggestive of SPN. The patient underwent tumor resection. Intraoperatively, infiltration to the bowel, ovary, and pancreas, as well as hepatic and intestinal synchronous metastaseses, were identified, without major vascular involvement. Histopathologic examination revealed pseudopapillary structures composed of monomorphic tumor cells with cystic degeneration and hemorrhage, consistent with SPN. SPN should be considered in young women presenting with slowly progressive, nonspecific abdominal symptoms and a large solid-cystic pancreatic mass. The main contribution of this case is to emphasize that SPN, although usually indolent, may already demonstrate aggressive behavior at presentation. Integration of clinical presentation, imaging, intraoperative findings, and histopathology remains essential for diagnosis and surgical decision-making, particularly in resource-limited settings where tumor markers and immunohistochemistry are not available.

Abstract

Soil-transmitted helminthiasis masih menjadi masalah kesehatan masyarakat, terutama pada anak sekolah dasar di wilayah dengan sanitasi yang kurang memadai. Pemeriksaan mikroskopis feses masih banyak digunakan karena sederhana dan murah, namun memiliki keterbatasan pada infeksi dengan intensitas rendah. Metode molekuler seperti Polymerase Chain Reaction (PCR) dikembangkan untuk meningkatkan deteksi, tetapi kinerjanya pada setting lokal masih perlu dievaluasi. Penelitian ini merupakan uji diagnostik dengan desain cross-sectional yang melibatkan 30 sampel tinja siswa sekolah dasar. Pemeriksaan dilakukan menggunakan pewarnaan langsung dengan eosin (PLDE) sebagai metode rujukan dan PCR. Analisis dilakukan menggunakan tabel 2×2 untuk menentukan sensitivitas, spesifisitas, nilai prediktif positif (PPV), nilai prediktif negatif (NPV), dan akurasi. DSE menunjukkan 8 dari 30 sampel (26,7%) positif, terdiri dari Ascaris lumbricoides (7 sampel) dan Trichuris trichiura (1 sampel). PCR mendeteksi DNA Ascaris lumbricoides pada 11 sampel (36,7%), tanpa deteksi Trichuris trichiura maupun hookworm. Dibandingkan metode rujukan, PCR memiliki sensitivitas 62,5%, spesifisitas 72,72%, PPV 45,45%, NPV 84,21%, dan akurasi 70%. PCR menunjukkan kinerja diagnostik sedang dan berpotensi sebagai metode pelengkap, terutama untuk menyingkirkan kemungkinan infeksi. Namun, PCR belum mampu mendeteksi seluruh spesies STH yang teridentifikasi secara mikroskopis. Penggunaannya perlu mempertimbangkan kesiapan laboratorium, kualitas sampel, dan kemampuan deteksi spesies. Penelitian selanjutnya disarankan menggunakan sampel lebih besar, target molekuler lebih luas, multiplex PCR, dan metode rujukan yang lebih komprehensif.

Abstract

Obesity is a growing global health problem. Obesity is more common among patients with bipolar disorder than in the general population and has been associated with greater clinical burden, including depressive symptoms, impaired functioning, poorer quality of life, and metabolic complications. Case Presentation: A 38-year-old woman with a five-year history of bipolar III disorder presented with a 15 kg weight gain over two years. She had a history of manic episodes followed by depressive episodes. During depressive phases, she reported fatigue, low motivation, emotional eating, increased appetite, and preference for high-carbohydrate foods. She had been treated with valproate 750 mg/day and olanzapine 10 mg/day. Management included pharmacotherapy review, switching olanzapine to lurasidone due to lower metabolic risk, nutritional counselling, structured physical activity, cognitive behavioral strategies for emotional eating, sleep routine education, and family support. Conclusion: Behavioral intervention combined with regular physical activity may be a relevant adjunctive approach for patients with bipolar disorder, obesity, and emotional eating. This case highlights the need to assess eating behavior, sleep routine, physical activity, metabolic risk, and adaptive coping strategies in patients with bipolar disorder and obesity.

Abstract

Papilledema is swelling of the optic disc due to increased intracranial pressure (ICP) which can cause neurological symptoms such as headache, nausea, vomiting, and progressive visual impairment and can lead to permanent blindness if not treated appropriately. This condition must be differentiated from disc edema due to other etiologies because it has a different pathophysiology and management. This case report discusses a 32-year-old woman who presented with complaints of blurred vision such as shadows in the left eye for five days, accompanied by frontal headache radiating to the retrobulbar, nausea, intermittent fever, and impaired eye movement. Ophthalmological examination showed decreased vision, limited mediolateral movement of both eyes indicating paresis of the sixth nerve, and fundoscopic images of bilateral blurred papilla borders with more severe swelling in the left eye accompanied by hard exudate. The patient was diagnosed with papilledema with diplopia ODS ec. paresis of the sixth nerve and was given supportive therapy in the form of cendo lyteers and mecobalamin as well as regular check-up education. This case emphasizes the importance of early detection of papilledema through history taking, clinical examination, and fundoscopy to prevent permanent visual complications and determine comprehensive and timely management.

Abstract

Soil-transmitted helminth (STH) infection remains a public health concern in tropical urban settings. Understanding its spatial distribution and relationship with environmental determinants is important for targeted control. This cross-sectional spatial study included 294 primary school children (grades III and IV) from 18 districts of Palembang, Indonesia. Schools and participants were selected using two-stage cluster sampling. Stool specimens were examined using the direct smear method with eosin staining. District-level population density, latrine coverage, and piped water access were obtained from official statistics. Spatial autocorrelation was assessed using Global Moran's I, Local Indicators of Spatial Association (LISA), and bivariate LISA. The overall STH prevalence was 16.7% (49/294 children). Most districts had low prevalence (<20%), while Ilir Barat Dua (41.2%), Ilir Barat Satu (35.3%), and Kertapati (29.4%) showed moderate prevalence (20-50%). Global Moran's I indicated significant positive spatial autocorrelation for latrine coverage (I = 0.190; p = 0.043), but not for population density (I = -0.135; p = 0.078) or piped water access (I = -0.049; p = 0.449). LISA mapping identified localized clusters of higher STH prevalence in areas with inadequate latrine coverage. STH endemicity in Palembang is low to moderate, but localized clusters remain. Sanitation, particularly latrine availability, was the main environmental factor associated with spatial clustering, whereas population density and piped water access showed no significant spatial association. Targeted sanitation improvement, hygiene education, and spatially guided surveillance are recommended.
