Abstract

Cognitive decline known as one of long-term effect of chronic disease. Individuals with cognitive dysfunction tend to show low quality of life. Yet limited studies has been conducted to determine the prevalence of this issue among people with chronic disease in Indonesia. Therefore, this study was aimed to identify the prevalence and relationship between chronic disease and cognitive function. This study also evaluated the factors influencing this relationship. This research was a cross-sectional analysis that utilized data from 2,396 participants with chronic conditions, drawn from the 2014 Indonesian Family Life Survey (IFLS) through purposive sampling. This study focus on people with hipertention, stroke, and diabetes mellitus. Cognitive impairment was measured using Telephone Interview for Cognitive Status (TICS). The data was analysed using analysed using bivariate logistic regression and multivariate Hierarchical Logistic Regression. The relationship between variables indicated based on Odds Ratio (OR), exponential value values ꞵ (eꞵ), and significancy level 5%. The overall prevalence of impaired cognitive function was 2171(61.7%) with the highest frequency found in people with diabetes mellitus (64.5%). Logistic regression test found a significant relationship between chronic disease and impaired function (p<0.001). Hierarchical Logistic Regression test on all models found-that age, gender, disease duration, and education had a greater influence, with R2 0.467 (46.7%). This study showed people with chronic diseases with older age, female, low education, longer disease duration, smoker, and obese have higher risk of developing impaired cognitive function.

Abstract

Type 2 diabetes mellitus (DM type 2) is a serious problem worldwide due to the impact of the disease which can cause complications and reduce the quality of life of sufferers. Factors that cause this are due to lack of knowledge, self-management and chronic hyperglycemia. Therefore, it is necessary to carry out continuous educational interventions such as Diabetes Self-Management Education (DSME). The purpose of this implementation is to improve knowledge, self-management and control of random blood sugar levels in patients with type 2 DM. This type of quantitative research uses a one-group pre-test-post-test approach, a sample of 15 respondents and the sampling technique used was purposive sampling. The implementation time is April 21 to May 24, 2025 at Dr. H. Abdul Moeloek Regional General Hospital, Lampung Province. The measurement instruments used are Diabetes Knowledge Questionnaire (DKQ-24), Diabetes Self-Management Instrument (DSMI) and GDS measurement sheets. The analysis used is the paired t-test and the Wilcoxon test. The results of the implementation after being given 4 sessions of DSME showed an increase in knowledge as shown by a p-value of 0.003 (<0.05), an increase in self-management with a p-value of 0.001 (<0.05) and control of random blood glucose values with a p-value of 0.001 (<0.05).0.000 (<0.05). The 4-session ideo-based DSME had a positive impact on knowledge, self-management, and random blood sugar values because DSME is a continuous educational intervention that helps respondents understand their disease better.

Abstract

ABSTRACT Diabetes Mellitus is a degenerative disease, if not treated properly it can have a negative impact on complications in various organ systems in the body (Syahid, 2021). One of the complications of diabetes that often occurs is neuropathy (nerve damage) in the feet which increases the incidence of foot ulcers, infections and even the need for foot amputation (Sianipar, 2023). The aim of this research is to prepare Standard Operating Procedures (SPO) for the implementation of EBNP regarding Diabetic Sensorimotor Polyneuropathy (DSPN) Screening in Type 2 Diabetes Mellitus patients. The number of respondents involved in implementing this EBNP was 14 people. In the implementation of EBNP, Diabetic Sensorimotor Polyneuropathy (DSPN) screening was carried out using the mTSN (Modified Toronto Clinic Score Neuropathy) instrument which aims to assess whether there are symptoms of neuropathy and no symptoms of neuropathy. DSPN screening is carried out based on Standard Operating Procedures (SOP). Based on the results of the two instrument tests between mTCSN and mNSI-E using the undercurve test, it can be concluded that the mTCSN instrument has a higher sensitivity value with a value of 0.909, while the sensitivity value for the mNSI-E instrument is smaller than the mTCSN instrument with a value of 0.571. So, to screen for neuropathy it is more effective to use the mTCSN instrument because the sensitivity score is higher. Keywords: Diabetes Mellitus, Screening, Polyneuropathy, DSPN  ABSTRAK Diabetes Melitus merupakan penyakit degeneratif bila tidak ditangani dengan baik dapat berdampak buruk menjadi komplikasi pada berbagai sistem organ dalam tubuh (Syahid, 2021). Salah satu komlikasi dari diabetes yang sering terjadi adalah Neuropati (kerusakan saraf) dikaki yang meningkatkan kejadian ulkus kaki, infeksi dan bahkan keharusan untuk amputasi kaki (Sianipar, 2023). Tujuan penelitian ini adalah tersusunnya Standar Prosedur Operasional (SPO) pelaksanaan EBNP tentang Skrining Diabetic Sensorimotori Polyneurophaty (DSPN) pada pasien Diabetes Melitus Tipe 2. Jumlah responden yang terlibat pada pelaksanaan EBNP ini adalah 14 orang. Pada pelaksanaan EBNP ini telah dilakukan skirining Diabetic Sensorimotori Polineuropathy (DSPN) dengan menggunakan instrumen mTSN (Modified Toronto Clinic Score Neuropathy) yang bertujuan untuk menilai ada gejala neuropati dan tidak ada gejala neuropati. Skrining DSPN dilakukan berdasarkan Standar Operasional Prosedur (SOP). Berdasarkan hasil kedua uji intrumen antara mTCSN dengan mNSI-E menggunakan uji undercurve, maka dapat disimpulkan, instrumen mTCSN memiliki nilai sensitivitas yang lebih tinggi dengan nilai 0.909, sedangkan nilai sensitivitas pada instrumen mNSI-E lebih kecil dari instrumen mTCSN dengan nilai 0.571. Maka, untuk melakukan skrining neuropathylebih efektif digunakan instrumen mTCSN karena skor sensitivitasnya lebih tinggi. Kata Kunci: Diabetes Mellitus, Skrining, Polyneurophaty, DSPN

Abstract

ABSTRACT Diabetes melitus tipe 2 merupakan penyakit kronis dengan prevalensi yang terus meningkat di seluruh dunia dan sering menimbulkan berbagai komplikasi, salah satunya neuropati perifer yang dapat menyebabkan gangguan keseimbangan tubuh. Gangguan keseimbangan pada pasien diabetisi dapat meningkatkan risiko jatuh dan menurunkan kemampuan aktivitas sehari-hari. Salah satu intervensi non-farmakologis yang dapat dilakukan untuk memperbaiki keseimbangan adalah latihan penguatan otot (muscle strengthening exercise) yang berperan dalam meningkatkan kekuatan otot, memperbaiki sirkulasi perifer, serta meningkatkan propriosepsi pada pasien diabetes. Penerapan Evidence Based Nursing Practice (EBNP) ini bertujuan untuk mengidentifikasi dan menerapkan latihan penguatan otot dalam meningkatkan keseimbangan tubuh pada pasien diabetes melitus tipe 2 serta menyusun Standar Prosedur Operasional (SPO) terkait intervensi tersebut.  Metode yang digunakan adalah penerapan Evidence Based Nursing Practice melalui penelusuran literatur menggunakan pendekatan PICO dengan kata kunci diabetes mellitus, exercise, dan balance capacity. Pencarian dilakukan melalui database PubMed pada rentang tahun 2020–2025. Dari hasil penelusuran diperoleh 85 artikel, kemudian diseleksi berdasarkan kriteria inklusi dan eksklusi hingga diperoleh 4 artikel systematic review dan meta-analysis yang relevan. Artikel yang terpilih kemudian dilakukan telaah kritis menggunakan instrumen AMSTAR 2 untuk menilai kualitas evidence.  Hasil penerapan menunjukkan bahwa latihan penguatan otot yang dilakukan secara rutin dapat meningkatkan kekuatan otot, memperbaiki sirkulasi perifer, serta meningkatkan keseimbangan tubuh pada pasien diabetes melitus tipe 2 yang mengalami neuropati perifer. Implementasi intervensi ini juga mendukung pengembangan SOP latihan fisik sebagai intervensi mandiri perawat dalam meningkatkan kualitas asuhan keperawatan pada pasien diabetes. Latihan penguatan otot merupakan intervensi non-farmakologis berbasis bukti yang efektif dalam meningkatkan keseimbangan tubuh pada pasien diabetes melitus tipe 2. Penerapan latihan ini dapat dijadikan bagian dari intervensi keperawatan untuk mencegah komplikasi lebih lanjut dan meningkatkan kualitas hidup pasien diabetisi.  Kata Kunci: Diabetes Melitus Tipe 2, Latihan Penguatan Otot, Keseimbangan Tubuh, Neuropati Perifer, Evidence Based Nursing Practice.   ABSTRACT Type 2 Diabetes Mellitus (T2DM) is a chronic disease with an increasing global prevalence and is often associated with various complications, one of which is peripheral neuropathy that can lead to impaired body balance. Balance disorders in patients with diabetes increase the risk of falls and reduce functional ability in daily activities. One of the non-pharmacological interventions that can improve balance is muscle strengthening exercise, which plays an important role in increasing muscle strength, improving peripheral circulation, and enhancing proprioception in patients with diabetes.  This Evidence-Based Nursing Practice (EBNP) implementation aimed to identify and apply muscle strengthening exercises to improve body balance in patients with Type 2 Diabetes Mellitus and to develop a Standard Operating Procedure (SOP) for the intervention. The method used was the implementation of Evidence-Based Nursing Practice through literature searching using the PICO approach with the keywords diabetes mellitus, exercise, and balance capacity. The literature search was conducted in the PubMed database for articles published between 2020 and 2025. A total of 85 articles were initially identified and then screened based on inclusion and exclusion criteria, resulting in 4 systematic review and meta-analysis articles. Critical appraisal of the selected articles was conducted using the AMSTAR 2 instrument to assess the quality of the evidence.  The results of the implementation indicated that regular muscle strengthening exercises improved muscle strength, enhanced peripheral circulation, and increased body balance in patients with Type 2 Diabetes Mellitus with peripheral neuropathy. The implementation also supported the development of an exercise SOP as an independent nursing intervention to improve the quality of nursing care for patients with diabetes.  Muscle strengthening exercise is an effective evidence-based non-pharmacological intervention for improving body balance in patients with Type 2 Diabetes Mellitus. This intervention can be integrated into nursing care to prevent further complications and improve the quality of life of diabetic patients.  Keywords: Type 2 Diabetes Mellitus, Muscle Strengthening Exercise, Body Balance, Peripheral Neuropathy, Evidence-Based Nursing Practice.
