Abstract

Nguter is one of the villages in Nguter Subdistrict, Sukoharjo Regency, Central Java. The geographical location of Nguter Subdistrict is at an altitude of 104 meters above sea level. One of the healthcare facilities in Nguter Village is the Nguter Community Health Center (Puskesmas). From January to July 2024, the number of low back pain cases treated at Nguter Community Health Center reached 285 people. This figure is considered quite hi gh, as it continues to show a significant increase each year. The goal of this program is to provide education on William Flexi Exercise for myogenic low back pain in the elderly, helping alleviate the complaints of lower back pain. The method used involves counseling sessions through lectures and leaflets for the Elderly Posyandu Community in RW 07, Nguter Subdistrict, Sukoharjo Regency, Central Java. The result of this community service program is that the elderly understood the material provided and are able to perform the exercises independently at home.

Abstract

Nyeri punggung bawah merupakan salah satu gangguan muskuloskeletal akibat dari ergonomi yang salah. Ergonomi adalah ilmu, seni dan penerapan teknologi untuk menyerasikan atau menyeimbangkan antara segala fasilitas yang digunakan baik dalam beraktifitas maupun istirahat dengan kemampuan dan keterbatasan manusia baik fisik maupun mental sehingga kualitas hidup secara keseluruhan menjadi lebih baik. Dengan tujuan untuk mengedukasi pekerja konveksi tentang penanganan nyeri punggung bawah akibat faktor ergonomic dan pencegahan terjadinya nyeri berulang. metode yang digunakan yaitu mulai pengisian kuisioner, pemberian materi, demo penanganan nyeri punggung bawah menggunakan leaflet dan poster dan demo kembali oleh responden. Dari penyuluhan yang dilakukan terdapat peningkatan pengetahuan tentang nyeri punggung bawah dan ergonomi bahwa pemberian edukasi nyeri punggung bawah akibat faktor ergonomi pada pekerja mampu menjadi salah satu cara dalam peningkatan pengetahuan pekerja pada nyeri punggung bawah, ergonomi yang baik saat bekerja dan bentuk stretching yang dapat dilakukan untuk mengurangi nyeri pada punggung bawah.

Abstract

Frozen shoulder is a disorder of the shoulder joint that causes pain and always causes limitation of joint motion in all directions resulting in physical problems and decreased functional activity. Frozen Shoulder due to rotator cuff is caused by inflammation from prolonged tendinitis, characterized by a marked limitation of the glenohumeral joint range of motion, both active and passive. The prevalence of this disease is about 20% of the general population and 10-20% in people with diabetes (Shichling & Walsh, 2001). And often occurs in people above 40 years of age, especially for women aged 50 years. When trauma occurs, it causes a local inflammatory reaction in the joints and causes adhesions to the joint capsule and cartilage, which is characterized by shoulder pain that occurs gradually, sharper pain, stiffness and limitation of motion. Frozen shoulder can be treated with medication and a physiotherapy program. Physiotherapy programs are useful for reducing pain, both pressure and motion, increasing muscle strength, increasing the range of motion of the joints and restoring functional activities/daily activities. The purpose of this study was to determine the effectiveness of the physiotherapy program with Ultrasound, TENS, and Codman Pendular exercise.

Abstract

Introduction:Bell's palsy is an acute facial nerve disease where the first symptom is pain in the mastoid area and causes facial paralysis1. In one year the incidence of this disorder can occur in 23 people out of 100,000 people2. Problems caused by Bell's palsy include the occurrence of facial asymmetry, a decrease in muscle strength on the side of the injured face, which results in decreased functional ability and interferes with activities. Bell's palsy treatment can be given medical therapy and physiotherapy programs. Physiotherapy plays a role in increasing muscle flexibility, facilitating weak muscles in carrying out movements to improve muscle work. This report follows up on the research of3, conducted the same program on 8 patients with similar complaints, getting results that are in accordance with this study. Case Presentation: A 50 years male, had a profession as civil servants and muslim, residing in Ngawi, East Java. The examination that has been carried out on the patient found that there is asymmetry on one side of the face, there are limitation in the movement of opening and closing the eyes, limitations in smiling, puffing the cheeks and smile, while the movements of frowning and raising the eyebrows have not been able to do. Management and Outcome:Physiotherapy modalities with infrared, electrical stimulation, massage facial to increasing muscle flexibility, facilitating weak muscles in carrying out movements to improve muscle work. Furthermore, to determine the patient's progress, measurement results were used in the form of a Manual Muscle Testing (MMT) to determine the increase in strength in facial muscle and Ugo Fisch Scale to determine the increase in functional ability in facial muscle. Discussion: This study was to measure the effects of electrical stimulation, infrared and facial massage on patients with Bell's palsy. This report will follow up the research of3 about their study on 8 patient with the same complaint whether there are result that are ini sync with this study. Conclusion: It is proven that cases of Bell’s Palsy that receive in the form of infrared, electrical stimulation and massage can increase muscle strength and can increase facial functional activity.

Abstract

Introduction: An ACL injury is a damage to the anterior cruciate ligament (ACL), which is the strong tissue that helps connect the femur and tibia. ACL injuries most often occur during sports activities such as football, basketball, badminton that have a lot of motion during sudden stops, changes of direction, jumps, and landings. The incidence of ACL injuries is estimated to range from 30 to 78 per 100,000 people annually. Generally, ACL injuries are performed by ACL reconstruction procedures to restore joint motion function in the knee. However, some people with ACL injuries choose not to undergo ACL reconstruction due to various factors that underlie their reasons. Some of them managed to return to some level of function with non-operative management. Therefore, the role of physiotherapy in non-operative ACL management is to prevent more severe tissue damage, restore movement function and maximize strength by modifying the patient's daily movements or activities to become more optimal. Case Presentation: A 27 years male. He works as a private employee, residing in Solo, Central Java. examinations that have been carried out on the patient found muscle stiffness and tenderness and pain in motion and positive findings were also found during a specific examination using the anterior drawer test and the Lachman test. Management and Outcome: Physiotherapy modalities with infrared, Kinesio taping myofascial release, and combined with muscle strengthening exercises can reduce pain and muscle spasms in ACL patients with non-operative procedures. Furthermore, to determine the patient's progress, measurement results were used in the form of a Numeric Rating Scale ( NRS ) to measure pain, Manual Muscle Testing ( MMT ) to measure muscle strength and Knee injury, and Osteoarthritis Outcome Score ( KOOS ) to measure knee function. Discussion: Several studies have revealed that the use of infrared modalities, Kinesio taping, myofascial release by adding muscle strengthening exercises have a positive effect in reducing pain and stiffness in muscles in cases of ACL injuries. The author tries to prove it by applying several modalities that have been studied previously Conclusion: The physiotherapy program which was carried out in 3 weeks with the physiotherapy modalities used such as Infrared, Myofacial Release, Kinesio Taping, and Strengthening Exercises was able to reduce pain and restore body functions to carry out daily activities. however, no significant increase in muscle strength was found. It can be concluded that this exercise is effective for the short term and helps restore the patient's functional activity.

Abstract

Introduction: Low back pain is one of the major problems faced by a worker. According to data from a survey by work-related diseases of 43,000 workers in agriculture, 27,000 of them complain of low back pain. Wrong posture done by workers for a long time will trigger the emergence of pain in the lower back. Case Presentation: The method used in this study is a case report from a patient low back pain at Ngawi Hospital with a identity TN.BC with the age of 56 years. The patient felt pain in the lower back in November 2020 because he often lifted heavy loads while doing activities in the fields such as lifting fertilizers, lifting spray tanks. Management and Outcome: the provision of infrared, TENS and Core stability exercise (CSE) is one of the effective methods to reduce complaints of pain in the lower back Discussion: This IR application given to LBP patients will provide a relaxing effect and help blood flow to the area. Giving TENS here is used to block the pain that appears. This type of Core Stability exercise aims to improve neuro-omuscular control , endurance, central muscle strength to maintain dynamic spinal stability. CSE can control during the movement of weight transfer, functional activities such as sitting, standing and walking. We all need a good Core stability program to prevent low back pain from initiating limb movement, for proper utilization of muscle strength and to improve performance. Conclusion: Handling physiotherapy in the form of giving IR, TENS and CSE can reduce complaints of low back pain that farmers complain about.

Abstract

Introduction: Patellar fractures most often occur due to trauma, this condition often results in several problems such as pain, decreased muscle strength and decreased range of motion of the joint. The goal to be obtained is to know the effect of TENS, US and balance fatigue on reducing pain, increasing muscle strength and joint range of motion as well as increasing stability and balance in order to reduce risk factors for falls in patients with patellar fracture conditions . case presentation: A male patient aged 65 years in July 2019 had an accident, the accident occurred while driving a motor vehicle and was hit from behind. The patient fell and his right knee hit the barrier. The patient was immediately referred to the hospital for emergency treatment. Management and outcomes: Giving TENS and US modalities as well as balance and stability exercises its effectives for reduce pain, increase muscle strength, increase joint range of motion and improve stability and balance in patellar fractures. Discussion After fourth physiotherapy and exercise interventions were given, the results obtained were a decrease in motion pain and pressure from T1 to , an increase in muscle strength in the flexor knee from T1 with a muscle strength value of 4/5, an increase in muscle strength to 5/5 at T4, an increase in the range of motion of the joint on flexor knee at T1 the result is 950 and becomes 1150 at T4, and there is a decrease in risk factors for falling. Giving TENS and US modalities as well as balance and stability exercises can reduce pain, increase muscle strength, increase joint range of motion and improve stability and balance in patellar fractures. Conclusion:after given by physiotherapy modalities and exercise in patellar fracture it shows a good progression. The fourth physicaltherapy's treatment showed that there was an decrease in pain, improve the range of motion,increase muscle strength and reducing risk factors of fall.

Abstract

Introduction: Stroke is a disease that causes damage to the brain due to infarction or blood vessels supplying blood to the brain which can cause decreased sensory function, muscle weakness, muscle atrophy, weight loss bearing, posture control, coordination, balance, and lower limb instability. Global Burden Disease data in 2010 showed that 80% of the total population had ischemic stroke while 20% had hemorrhagic stroke. While in Indonesia in 2019 the incidence of stroke per year is 200 out of 100,000 population, around 2.5% and the rest are disabled light or heavy. Purpose: The purpose of this case study is to find outEffectivenessHeel-Raise-Lower Exercise and Dual Task Gait Training for a patient Reversible Ischemic Neurologic Deficit Stroke (RIND) to improve balance. Methode: The method used in this study is Case Report. After doing therapy 3 times a week for 2 weeks The results showed an increase in each examination of muscle strength (MMT), Spasticity Examination (Aswort Scale), and Balance Check (Breg Balance Scale). Conclusin: Based on the results of the evaluation, it was concluded that in this case study, the provision of a physiotherapy intervention program in the form of: Heel-Raise-Lower Exercise and Dual Task Gait Training increase muscle strength, reduce spasticity, improve balance, coordination and walking patterns.

Abstract

Introduction: Diabetic peripheral neuropathy (DPN) is a common complication of long-term diabetes mellitus (DM) and is a progressive and irreversible disease. DPN occurs in 40%-59% of the total DM population, in general the estimated prevalence of DPN is 15.3-72.3/100,000 person-years, and in developing countries the prevalence of DPN is around 25-50%. Symptoms of DPN include paresthesias, hyperesthesia, and dysesthesias, and some patients experience neuropathic symptoms (such as burning and tingling) while others may have no signs or symptoms. Many disorders of the lower extremities and feet such as deformity, decreased range of motion, impaired balance coordination, and muscle weakness can be caused by DM. Physical therapy can improve the overall quality of life of DPN patients, and can relieve symptoms of diabetic neuropathy. It can also improve muscle strength, joint mobility, balance, coordination and physical function, when done regularly can reduce neuropathic pain and can help control blood sugar levels. Case Presentation: A 60-year-old woman with Diabetes Mellitus (DM) since 6 years ago (2015). Having complaints of numbness in both legs, these complaints more or less appeared in the last one year. Complaints get worse, especially when used to sit for a long time and stand for a long time, and complaints decrease when resting and walking at home. Management and Outcome: The physiotherapy program provided is in the form of stretching exercises for the lower extremities, balance (One leg stance, Side Lunges), gait training (Walking Exercise) and strengthening (Ankle theraband). Evaluation was carried out by measuring the value of numbness using NRS, measuring muscle strength in the lower extremities with the Five Time Sit to Stand Test, measuring static balance with MCTSIB and dynamically using the Time Up and Go Test. Discussion: Skeletal muscle stretching can help a person to control or lower blood glucose, this movement effectively has the benefit of exercising without physical stress. Gait and balance exercises with function-oriented strengthening programs are directed to simultaneously improve balance and gait in diabetic patients, exercise can improve macro and microvascular factors in diabetes. Strengthening exercises are important to reduce lower extremity weakness, which is a risk factor for falls in patients with DPN. Conclusion: The physiotherapy program which was conducted three times with stretching, balance, gait training and strengthening exercises was able to reduce the value of numbness in both legs.

Abstract

Latar belakang: Diabetes melitus (DM) tipe 2 adalah jenis DM dengan angka kejadian paling tinggi dari pada jenis lainnya. DPN merupakan komplikasi yang banyak terjadi pada DM tipe2 yang berkontribusi terhadap penurunan kekuatan otot ekstremitas bawah. Kondisi hiperglikemi dan durasi yanng lama diduga menjadi faktor pemicu terjadinya DPN yang akan mengganggu kualitas hidup pasien. Tujuan: Penelitian ini bertujuan untuk mengetahui perbedaan kejadian DPN, kekuatan otot ekstremitas bawah dan kualitas hidup serta untuk mengetahui faktor dominan yang mempengaruhi kualitas hidup pasien antara penderita DM tipe 2 durasi 5-10 tahun dan >10 tahun. Metode: Terdapat 2 kelompok dengan masing-masing terdiri dari 121 responden yang dilakukan di Grha Diabetika Surakarta. DPN diperiksa dengan menggunakan tes MNSI, kekuatan otot ekstremitas bawah dengan tes FTSTS dan kuisioner WHOQoL digunakan untuk mengevaluasi kualitas hidup. Hasil: Pada kelompok DM tipe 2 durasi >10 tahun menunjukkan kejadian DPN lebih banyak dan penurunan kekuatan otot ekstremitas bawah dan kualitas hidup disbanding kelompok DM tipe 2 durasi 5-10 tahun (p<0.05). Faktor kekuatan otot ekstremitas bawah menjadi faktor utama yang mempengaruhi kualitas hidup pasien DMT2 durasi >10 tahun. Kesimpulan: DM tipe 2 dengan durasi lebih dari 10 tahun akan memberikan komplikasi signifikan terhadap kejadian DPN, penurunan kekuatan otot dan kualitas hidup. Kekuatan otot ekstremitas bawah menjadi faktor dominan yang mempengaruhi kualitas hidup pasien DM tipe 2 dengan durasi >10 tahun.
