Abstract

Tuberculosis (TB) remains a significant public health problem in Indonesia, including in Pekanbaru City, which has shown a relatively high trend of cases in recent years. TB cases in Pekanbaru remain high, with 1,685 cases in 2024 and 1,616 cases in 2025. In the working area of Karya Wanita Public Health Center, there were 58 cases, with slum areas accounting for 8.57%. This study aimed to analyze the relationship between home environmental conditions, socioeconomic factors, behavior, and settlement conditions with the incidence of tuberculosis in the working area of the Karya Wanita Public Health Center, Pekanbaru City. This study employed a quantitative analytical approach with a case-control study design. The research was conducted in the working area of the Karya Wanita Public Health Center, Pekanbaru City, from February to March 2026. The total sample consisted of 174 respondents, including 58 TB cases and 116 non-TB controls. Data were collected through interviews using questionnaires and observations of housing conditions. Data analysis was performed using univariate analysis, bivariate analysis with the Chi-square test, and multivariate analysis using logistic regression. The results showed that, in the multivariate analysis, variables significantly associated with TB incidence were house ventilation (p=0.040; OR=5.929), residential density (p=0.047; OR=4.754), type of floor (p=0.044; OR=9.707), income (p<0.001; OR=41.207), and settlement conditions (p=0.048; OR=0.164). Income was identified as the most dominant factor associated with TB incidence. Meanwhile, lighting, humidity, mask-wearing habits, education, and occupation were not statistically significant but were retained in the model as confounding variables. The logistic regression model produced a Nagelkerke R Square value of 0.773, indicating that the variables included in the model explained 77.3% of the variation in TB incidence. The study concludes that home environmental conditions and socioeconomic factors, particularly income, play an important role in the occurrence of tuberculosis. TB control efforts should focus on improving housing environmental

Abstract

This study aimed to analyze strategies for improving outpatient pharmacy service waiting time at RS Mesra using a SWOT analysis approach. A qualitative research design was applied by identifying internal and external factors that influence pharmacy service performance. Data were collected through in-depth interviews, observations, and SWOT questionnaires involving the head of the pharmacy unit, pharmacists, pharmacy technicians, and hospital management selected through purposive sampling. Internal and external factors were analyzed using the Internal Factors Analysis Summary (IFAS) and External Factors Analysis Summary (EFAS) matrices. The IFAS results showed that weaknesses outweighed strengths, indicating internal challenges such as limited human resources, long compounding processes, and high patient queues. Meanwhile, the EFAS results revealed that opportunities were greater than threats, highlighting favorable external conditions including national waiting time standards, technological advancements, and managerial support. The SWOT matrix positioned the outpatient pharmacy unit in a condition that requires both improvement and development strategies. Therefore, optimizing internal resources, strengthening information systems, and utilizing external opportunities are essential to reduce waiting time and enhance the quality of pharmacy services in a patient-centered hospital setting.

Abstract

The management of hazardous and toxic solid medical waste (B3) in hospitals remains a critical issue due to its potential to cause infection risks and environmental pollution if not properly managed. In Riau Province, the compliance level of health facilities with medical waste management standards is still low, while the volume of waste continues to increase. This study aims to analyze the overview of management practices, identify the types of medical waste, and examine policies related to the management of B3 solid medical waste in hospitals in Riau Province. The study employed a Systematic Literature Review (SLR) method by searching scientific articles through the Google Scholar database using keywords related to hospital medical waste management in Riau. From 169 identified articles, 10 met the inclusion and exclusion criteria and were analyzed narratively. The results indicate that infectious waste and sharps are the most dominant types, mainly generated from inpatient wards, emergency departments, laboratories, and operating rooms, with an average volume of about 100–150 kg per day. Several issues were identified, including inconsistent waste segregation, substandard temporary storage facilities, limited transportation infrastructure, and high dependence on third-party services for waste treatment. In conclusion, the management of B3 solid medical waste in hospitals in Riau Province is not yet optimal and requires improved regulatory compliance, better infrastructure, and strengthened monitoring and evaluation systems.

Abstract

Household waste management is a crucial component of environmental health, particulary in rural riverside areas where limited facilities and poor community practices contribute to environmental degraduation. This study aims to identify the conditions and factors contributing to suboptimal household waste management in Sungai Tonang Village. A qualitative descriptive method was uses, involving field observations and semi- structured interviews with village officials, health workers, and residents. Data were analyzed thematically and verified through triangulation of interviews, observations, and supporting documents. The findings indicate that household waste management in the village remains inadequate, as residents commonly dispose of waste into the river, burn it, or bury it due to the lack of waste sorting facilities, temporary storage, and transportation systems. Community knowledge of proper waste management and the 3R concept is low, and the village lacks regulations or institutional structures to support waste management. This situation creaters environmental pollution and increases the potential for vector-borne diseases. The study concludes that ineffective waste management practices in Sungai Tonang Village pose a significant environmental health risk, highlighting the need for regulatory development, facility provision, and ongoing community education to support sustainable waste management practices.

Abstract

Stunting remains a significant public health problem in Kuantan Singingi Regency, Riau Province, with a relatively high prevalence influenced by nutritional, environmental, and behavioral factors. This study aimed to analyze strategies to accelerate the reduction of stunting prevalence in Kuantan Singingi Regency using a SWOT approach and the IFAS–EFAS matrix. This was a descriptive-analytic study with a cross-sectional design employing SWOT analysis. Primary data were collected through in-depth interviews and observations with key informants from public health units, nutrition, environmental health, and health promotion. Secondary data were obtained from health profiles, stunting reports, and sanitation coverage data. Strategic analysis was conducted using SWOT analysis supported by the Internal Factor Analysis Summary (IFAS) and External Factor Analysis Summary (EFAS) matrices. The IFAS calculation produced a score of −9.4, the EFAS calculation yielded −6.9; this negative value indicates that the external opportunities supporting WASH–stunting in Kuantan Singingi Regency are also weak, because external threats are far greater than external opportunities. Furthermore, based on the IFAS and EFAS results, strategic positioning for accelerating stunting reduction was mapped onto the SWOT quadrant diagram. The SWOT quadrant shows that the coordinate point (−9.4; −6.9) falls in Quadrant IV (Weakness–Threat), indicating the dominance of internal weaknesses and external threats in accelerating stunting reduction. This position suggests that the most appropriate approach is a defensive strategy: addressing mothers’ low knowledge through education delivered by health centers (puskesmas) and community health volunteers (cadres); establishing a routine schedule for counseling, home visits, and cadre-based education activities led by environmental health cadres; strengthening monitoring and evaluation of STBM and SKAMRT through a simple reporting system supported by cadres; and addressing limited education budgets through collaboration with village funds to reinforce support for STBM and SKAMRT programs and to support the construction of improved, adequate latrine.

Abstract

Fire incidents in hospitals pose a serious safety threat that requires systematic prevention and control measures. This study aimed to identify factors influencing fire prevention and control in a hospital in Dumai City using a quantitative approach with a cross-sectional design, involving hospital staff selected through simple random sampling. Data were collected using a structured questionnaire that had been tested for validity and reliability, and were analyzed using association tests and multivariable analysis to examine the influence of variables. The findings indicated that most respondents demonstrated good levels of fire prevention and control practices, and there were significant associations between attitude, emergency response simulation, and supervision with fire prevention and control efforts. Attitude emerged as the most dominant factor, indicating that respondents with negative attitudes were more likely to have poor fire prevention and control practices compared to those with positive attitudes, with this relationship controlled by knowledge. It can be concluded that work capacity influencing fire prevention and control includes attitude as the primary factor, supported by knowledge, emergency response simulation, and supervision. Therefore, it is recommended that the Hospital Occupational Health and Safety Committee strengthen positive attitudes among staff, enhance the quality of emergency response simulations, and implement consistent supervision to support effective fire prevention and control.
