Abstract

Kesehatan merupakan hak dasar manusia yang harus dijamin negara melalui sistem pelayanan yang adil, merata, dan berkualitas. Penelitian ini bertujuan untuk menganalisis perbandingan transformasi kebijakan sistem kesehatan antara Thailand dan Indonesia dalam upaya mencapai Universal Health Coverage (UHC). Metode yang digunakan adalah kualitatif dengan pendekatan tinjauan pustaka, yang bersumber dari jurnal ilmiah, laporan resmi pemerintah, serta publikasi dari lembaga internasional terkait. Hasil penelitian menunjukkan bahwa Thailand menerapkan reformasi besar-besaran (big bang reform) pada tahun 2002 melalui Universal Coverage Scheme (UCS) dengan pendekatan terpusat, sistem kapitasi, dan dukungan kuat relawan kesehatan desa. Reformasi tersebut berhasil meningkatkan cakupan layanan kesehatan dari 76% menjadi 99,5% dalam satu dekade dan menurunkan beban biaya kesehatan masyarakat secara signifikan. Sebaliknya, Indonesia menempuh pendekatan transformasi bertahap melalui enam pilar transformasi sistem kesehatan nasional yang menekankan penguatan layanan primer, digitalisasi melalui SATUSEHAT, serta peningkatan kompetensi sumber daya manusia kesehatan. Meskipun Jaminan Kesehatan Nasional (JKN) telah mencakup lebih dari 96% penduduk, tantangan masih terdapat pada kesenjangan infrastruktur, ketimpangan distribusi tenaga kesehatan, dan koordinasi lintas sektor. Penelitian ini menyimpulkan bahwa keberhasilan transformasi sistem kesehatan sangat dipengaruhi oleh tata kelola yang efektif, keterlibatan komunitas, serta kesiapan layanan primer dan digitalisasi sebagai fondasi reformasi berkelanjutan menuju pencapaian UHC.

Abstract

ABSTRACT Outpatient clinic is a vital hospital service that demands focus on both quality and patient satisfaction. With increasing competition among hospitals in Semarang, understanding consumer behavior is crucial. Market segmentation can assist in identifying patient characteristics and their perceptions of healthcare services, which are shaped by factors such as medical communication, the comfort of facilities, and treatment outcomes. These perceptions significantly influence patients' decisions to seek care again in the future. The pediatric outpatient clinic at XYZ Hospital has experienced inconsistent and relatively low visit rates. Therefore, it is essential to analyze consumer perceptions through segmentation to formulate strategies aimed at increasing patient visits.This research adopts a descriptive qualitative study design, employing a case study approach at XYZ Private Hospital in Semarang. The participants in this study are the parents of pediatric patients. We conducted interviews using questionnaires administered to thirty respondents to gain an overview of their characteristics. We also conducted in-depth interviews with five primary informants to validate the findings. The demographic and geographic data reveal that the average age of parents is 36.03 years (SD 5.25), with 80% of respondents being female and 93.1% residing in Semarang. Additionally, 77.7% hold a bachelor's or master's degree (S1/S2), and 40.74% are employed in the private or entrepreneurial sector. In terms of psychographic and behavioral aspects, 60% of respondents discovered the pediatric clinic through friends or family, while 80% initiated their visits independently. The decision to visit was primarily influenced by the strong reputation of the doctors (60%), and 93.34% utilized personal funds or insurance for payment. Regarding costs, 90% rated outpatient fees as "moderate," with 83.33% agreeing that the costs were commensurate with the quality of service. Furthermore, 53.33% rated all staff services as positive or very positive. Furthermore, 60% found the location to be accessible, 63.33% assessed the facilities as clean, and 80% considered them comfortable.The study indicates that patients hold a positive perception of the pediatric clinic services at XYZ Hospital. They value the reputation of the doctors and the quality of facilities more than cost or location. Positive assessments of service quality, comfort, and cost fairness reflect high levels of satisfaction. Keywords: Segmentation, Patient Perception, Pediatric Outpatient.

Abstract

ABSTRACT Rehabilitation of drug abusers is an effort to recover drug abusers from addictive substance dependence, which includes initial reception, medical rehabilitation, and/or social rehabilitation, and post-rehabilitation. Environmental support factors that influence the success of rehabilitation include the economic, social, and political environment. The purpose of this study was to determine the economic, social, and political support in the successful rehabilitation of drug abusers at the National Narcotics Agency Clinic in Central Java Province. This research method is qualitative research with a case study research type. The research uses Van Horn Van Metter analysis with an environmental variable approach. The informants in this study included 8 main informants and 6 triangulation informants. The results of the study stated that economic, social, and political environmental support influenced the successful implementation of sustainable rehabilitation. Client economic support influences the client's willingness to access rehabilitation services. Social support that affects rehabilitation can be from family and the community. Meanwhile, political support both in terms of budget, regulations, and regulations that facilitate the implementation of drug rehabilitation services. The results of this research can be the basis for policy improvement and strengthening environmental support from all stakeholders. Keywords: Policy, Economic, Social, Political, Narcotic.  ABSTRAK Rehabilitasi penyalah guna narkotika merupakan upaya pemulihan penyalah guna narkotika dari ketergantungan zat adiktif yang mencakup penerimaan awal, rehabilitasi medis, dan/ atau rehabilitasi sosial, serta pasca rehabilitasi. Dukungan faktor lingkungan yang mempengaruhi keberhasilan rehabilitasi meliputi lingkungan ekonomi, sosial, dan politik. Tujuan dari penelitian ini adalah untuk mengetahui dukungan ekonomi, sosial, dan politik dalam keberhasilan rehabilitasi penyalah guna narkotika di Klinik Badan Narkotika Nasional Wilayah Provinsi Jawa Tengah. Metode penelitian ini merupakan penelitian kualitatif dengan jenis penelitian studi kasus. Penelitian menggunakan analisis Van Horn Van Metter dengan pendekatan variabel lingkungan. Informan dalam penelitian ini meliputi 8 informan utama dan 6 informan triangulasi. Hasil penelitian menyebutkan bahwa dukungan lingkungan ekonomi, sosial, dan politik mempengaruhi keberhasilan implementasi rehabilitasi berkelanjutan. Dukungan ekonomi klien mempengaruhi kemauan klien untuk mengakses layanan rehabilitasi. Dukungan sosial yang mempengaruhi rehabilitasi dapat dari keluarga dan masyarakat. Sedangkan dukungan politik baik secara anggaran, peraturan, maupun regulasi yang memfasilitasi penyelenggaraan layanan rehabilitasi narkotika. Hasil penelitian ini dapat menjadi dasar perbaikan kebijakan serta penguatan dukungan lingkungan dari seluruh stakelholer. Kata Kunci: Kebijakan, Ekonomi, Sosial, Politik, Narkotika.

Abstract

The COVID-19 pandemic has caused an increase in the death rate of pregnant women. The government is taking preventive action by providing COVID-19 vaccination to pregnant women in accordance with the Circular Letter of the Ministry of Health of the Republic of Indonesia HK.02.01/I/2007/2021. Even though it has been carried out for 2 months, vaccination coverage is still low, especially in Temanggung Regency. This study aims to determine behavioral determinants in the use of COVID-19 vaccination services for pregnant women. This research uses an analytical survey method with a quantitative approach through a cross-sectional study. Taking 200 samples using the Accidental Sampling technique based on inclusion and exclusion criteria. Data was collected using a research questionnaire and then analyzed using the Spearman Rank test. The research results show that there is a weak relationship between employment status and perceived vulnerability and utilization of COVID-19 vaccination services among pregnant women. There is a fairly strong relationship between education level, perception of waiting time, and accessibility with the utilization of vaccination services for pregnant women. There is a strong relationship between attitudes, knowledge, availability of health facilities, availability of health workers, and ease of information with the use of COVID-19 vaccination services for pregnant women.

Abstract

This study aimed to analyze factors associated with the performance of childhood immunization recording and reporting using the Sehat Indonesia-Ku (ASIK) application among village midwives in Magetan District, Indonesia. The ASIK application plays an important role in strengthening Indonesia’s digital immunization reporting system by improving data accuracy, timeliness, and monitoring at primary healthcare facilities. A quantitative cross-sectional study was conducted among 228 village midwives from 22 primary health centers using a total sampling technique. Data were collected using a structured questionnaire and analyzed using univariate, bivariate (Chi-square), and multivariate analyses with binary logistic regression. The results showed that perception (p = 0.047), attitude (p = 0.002), and workload (p = 0.014) were significantly associated with performance. Midwives with positive perceptions were more likely to demonstrate good performance (OR = 1.749), while attitude emerged as the strongest predictor (OR = 2.400). In addition, a lighter workload significantly increased the likelihood of good performance (OR = 1.998). Conversely, length of service, knowledge, and motivation were not significantly associated with performance (p > 0.05). These findings suggest that immunization program managers and primary healthcare facilities should strengthen training programs, provide continuous technical support, and optimize workload distribution to enhance ASIK utilization and improve immunization data quality.

Abstract

Background: Malnutrition especially during the critical period of the first 1000 days of life, will affect the growth and development of children, especially brain development, thus increasing the likelihood of suffering from chronic diseases in adulthood. The World Health Organization has set targets for nutritional status development, one of which aims to reduce the number of stunted children under the age of five by 40% by 2025.Purpose : To provide an overview of stunting prevention interventions in fulfilling nutrition in toddlers.Methods: This research design uses Scoping Review and article search and selection is based on the PRISMA method.Results: Search results on the ProQuest, Science Direct, and Scopus. Obtained as many as 238 articles that match the keywords searched. After screening according to the inclusion and exclusion criteria, 7 articles were selected to be included in this study, which are stunting prevention interventions in fulfilling nutrition in toddlers with lipid-based nutrient supplements (LNS), animal protein source complementary foods, complementary feeding of breast milk, combination of fish oil supplementation, provision of nutritious biscuits, and feeding containing hibiscus sabrida.Conclusion: Intervention of Stunting Prevention in the Fulfillment of Nutrition in toddlers can improve stunting prevention and increase linear growth in toddlers.

Abstract

The healthcare sector faces global challenges in controlling service costs, particularly in operative procedures that require substantial resources, including highly skilled medical personnel, advanced medical technology, and adequate supporting facilities. These circumstances encourages hospitals to implement various strategies to improve cost efficiency while maintaining the quality of care. This study aims to review cost-efficiency strategies implemented by hospitals in managing operative procedures across different countries. A scoping review was conducted using the PRISMA approach. Literature searches were conducted in the PubMed, Scopus, Google Scholar, Web of Science, and Cochrane Library databases for articles published between January 2020 and May 2025. A total of 14 articles were analyzed. Effective strategies include the implementation of standardized clinical protocols such as Enhanced Recovery After Surgery (ERAS) and fast-track surgery, optimization of medical technologies, unit cost analysis, application of lean management principles, and more flexible human resource management. Cost-efficiency strategies in operative care vary considerably, and no single universal approach can be applied across all healthcare settings. The effectiveness of these strategies depends on the specific context of each hospital, including the availability of resources, the types of surgical procedures performed, and the characteristics of the healthcare system.

Abstract

The Maternal Mortality Rate (MMR) is an indicator used to measure a country's health development and is one of the targets in the Sustainable Development Goals (SDGs). In Indonesia, the MMR is still relatively high. Various factors are considered to contribute to the high MMR, including inadequate access and quality of antenatal care (ANC) services, delays in seeking and receiving appropriate maternal services, limited availability of birth attendants, socioeconomic tensions, cultural barriers, and inadequate handling of emergency obstetrics. One potential intervention to reduce MMR is by improving the quality of ANC services through various strategies. One such strategy is the provision of Ultrasonography (USG) services in integrated antenatal care at the primary level. However, unfortunately, the availability of USG at primary level health facilities in developing countries like Indonesia is still very limited. This scoping review aims to determine how the availability of USG services can improve the quality of integrated antenatal care at Puskesmas or Primary Level Health Facilities and provide strategic insights for its implementation. The methodology employed in this scoping review involves selecting topics and determining keywords for journal searches using both English and Indonesian languages through two scientific article databases, namely PubMed and Google Scholar. After applying the PRISMA model (Preferred Reporting Items for Systematic Reviews and Meta- analyses), six articles were identified that met the inclusion criteria. The provision of ultrasound services has the potential to enhance the quality of ANC services and should be implemented comprehensively and sustainably.Keywords: Ultrasonography, Primary Health Care, Quality of ANC

Abstract

Background Informal workers represent a highly vulnerable group due to limited access to preventive health services. Various individual, social, economic, policy, and environmental factors contribute to their low participation in preventive health programs. Objective This study aims to identify the determinants of preventive health service utilization among informal workers based on a review of 13 national and international journals published between 2021 and 2025. Method A systematic review approach was applied by examining relevant articles from international and national databases. The analysis included socio-demographic variables, employment status, health insurance policies, and environmental factors influencing access to preventive services. Results Education, health knowledge, attitudes, and family support were found to enhance preventive health service utilization. Economic constraints such as unstable income and lack of social security emerged as major barriers. Furthermore, distance to health facilities, service quality, and limited availability of health professionals exacerbated access disparities, making informal workers more vulnerable to chronic diseases. Conclusion Preventive health service utilization among informal workers is influenced by a combination of individual, social, economic, policy, and environmental factors. Inclusive policies, community-based interventions, and improved health infrastructure are required to increase participation and ensure equitable access to preventive health services. Abstract

Background Oral health is an essential aspect of quality of life, particularly among older adults who are vulnerable to various oral health problems. However, their level of knowledge and behavior in maintaining oral hygiene remains relatively low. Objective This study aimed to examine the effectiveness of audiovisual media in promoting oral health among older adults. Method Participants were elderly individuals who received oral health education through audiovisual materials, including short videos demonstrating proper oral hygiene practices, the importance of regular dental check-ups, and the impact of oral health on overall well-being. Results The results indicated a significant improvement in oral health knowledge among older adults after receiving audiovisual education. Participants demonstrated better understanding of proper tooth brushing techniques, the importance of dental visits, and the use of oral hygiene aids. Conclusion Preventive health service utilization among informal workers is influenced by a combination of individual, audiovisual media is an effective educational tool to enhance knowledge, attitudes, and oral health behaviors among older adults. By engaging multiple senses in the learning process, audiovisual education improves information retention, motivation, and consistency in healthy practices.
