Abstract

Hepatitis B remains a major global health challenge, with mother-to-child transmission contributing significantly to chronic infection and severe complications. In Indonesia, despite national targets of 95% coverage, screening rates among pregnant women in Semarang City remain suboptimal, with limited evidence on the determinants of service utilization. This study aimed to identify factors associated with the utilization of hepatitis B early detection services among pregnant women in Semarang City. A quantitative cross-sectional design was employed, using primary data collected through structured interviews with 200 respondents selected purposively. Data analysis was conducted using chi-square tests to examine associations between independent variables and screening utilization. The findings revealed that maternal attitude (p=0.006), health worker support (p=0.001), perception of service procedures (p=0.023), access to information (p=0.045), healthcare access (p=0.007), and perceived severity (p=0.008) were significantly associated with service utilization. In contrast, maternal education, employment status, knowledge, family support, and perceived susceptibility showed no significant association.  These findings highlight the importance of behavioral and system-level factors in improving screening coverage through strengthened counseling, better service delivery, expanded information access, and equitable healthcare. Moreover, early detection aligns with Islamic values of Hifz an-nasl by protecting maternal and child health, where faith-based and community-centered strategies can enhance family health and accelerate hepatitis B elimination.

Abstract

Introduction: The results of the community satisfaction survey at K Referral Center Hospital in 2019-2023 showed a decrease in patient satisfaction. In inpatient installation, satisfaction levels reached only 83.53% in May 2024, below the minimum standard of 90% set by the Ministry of Health (Kepmenkes Number 129 / Menkes / SK / II / 2008) for inpatient care. This study aimed to identify the level of satisfaction regarding the quality of inpatient services among non-national health insurance patients.Methods: This was a cross-sectional quantitative study. Sample calculation using the proportional sampling technique with accidental data collection for 97 non-national health-insured inpatients in January-February 2025. Data were processed using the chi-squared relationship test with SERVQUAL variables.Results: Most patients were satisfied (60.8%) with the services they received, whereas the others were dissatisfied (39.2%). A significant relationship was found between the tangible dimensions, responsiveness, reliability, and patient satisfaction. No significant relationship was found between the dimensions of empathy, assurance, and patient satisfaction.Conclusion: Inpatients satisfaction in K referral Hospital is related to the facilities owned by the hospital, attitudes of medical personnel, speed of response to complaints, and reliability of officers in providing services that can increase trust in health services

Abstract

Bone marrow transplantation (BMT) is a life-saving procedure for haematological malignancies and other severe blood disorders. It is need resource-intensive treatment with significant financial implications. The cost of BMT varies across different countries and healthcare systems, influenced by factors such as treatment protocols, hospital infrastructure, medication expenses, and post-transplant care. Understanding the financial aspects of BMT is crucial for improving accessibility and ensuring sustainable healthcare service. Objective to analyse the cost of BMT across different regions, identify sources of financing, and explore financial challenges and potential solutions for patients undergoing BMT. A comprehensive literature search was conducted using PubMed, Scopus, and Web of Science databases from 2001 - 2025. MeSH Terms using ((haematology malignancy OR leukaemia OR acute leukaemia OR acute myeloid leukaemia OR acute lymphoblastic leukaemia OR myeloma OR lymphoma OR thalassemia) AND (bone marrow transplantation OR stem cell transplantation OR haematopoetic stem cell transplantation OR hematopoietic stem cell transplantation OR allogeneic transplantation ) AND (OR costing OR payer)). Data were extracted on total BMT costs, cost components, sources of financing, and financial challenges associated with the procedure. The analysis followed the Cochrane Handbook for Systematic Reviews of Interventions. Out of 528 initially screened studies, 19 articles met the inclusion criteria. The cost of BMT varied significantly by country, with developed nations reporting higher costs than developing countries. The primary cost components included hospital and treatment expenses, medications, and follow-up care. Financing sources varied across regions, including private health insurance, government-funded healthcare programs, non-profit organisations, fundraising efforts, and out-of-pocket payments. Many patients in low- and middle-income countries face financial barriers due to limited insurance coverage and high out-of-pocket expenses. BMT remains a costly procedure with significant financial challenges, particularly in regions with limited healthcare funding.

Abstract

The KIA Handbook is a manual that contains information sheets and records of health services for mothers and children. However, the use of the KIA handbook has not achieved maximum results because the documentation system is still done conventionally. This study aims to analyze and describe the input, planning, and organizing factors that affect the completeness of filling out the KIA handbook. This study uses a literature review method. Journal searches were obtained from the Google Scholar database using keywords and inclusion criteria. The steps of data analysis are: data collection, data reduction, data presentation, and conclusions. The article search results obtained 34 articles published in national online journals using the Google Scholar search engine with keywords and inclusion criteria, namely: time period, language, subject, type of article, and theme of article content. The conclusions in this study are: (1) The input factors that affect the completeness of filling out the KIA book from the midwife's side are knowledge, attitudes, skills, while from the mother's side, namely in terms of knowledge, (2) Planning factors that affect the completeness of filling out the KIA book are service SOPs. Antenatal care at the Puskesmas is made by the Puskesmas based on the integrated antenatal service guidelines set by the District Health Office, and (3) Organizing factors that affect the completeness of filling out the KIA handbook, namely cooperation and joint commitment by each part involved such as: Health Office, Head of Puskesmas, Implementing Midwives, Local Government (Districts), Pregnant Women's Families, and Employers

Abstract

Kesehatan merupakan hak dasar manusia yang harus dijamin negara melalui sistem pelayanan yang adil, merata, dan berkualitas. Penelitian ini bertujuan untuk menganalisis perbandingan transformasi kebijakan sistem kesehatan antara Thailand dan Indonesia dalam upaya mencapai Universal Health Coverage (UHC). Metode yang digunakan adalah kualitatif dengan pendekatan tinjauan pustaka, yang bersumber dari jurnal ilmiah, laporan resmi pemerintah, serta publikasi dari lembaga internasional terkait. Hasil penelitian menunjukkan bahwa Thailand menerapkan reformasi besar-besaran (big bang reform) pada tahun 2002 melalui Universal Coverage Scheme (UCS) dengan pendekatan terpusat, sistem kapitasi, dan dukungan kuat relawan kesehatan desa. Reformasi tersebut berhasil meningkatkan cakupan layanan kesehatan dari 76% menjadi 99,5% dalam satu dekade dan menurunkan beban biaya kesehatan masyarakat secara signifikan. Sebaliknya, Indonesia menempuh pendekatan transformasi bertahap melalui enam pilar transformasi sistem kesehatan nasional yang menekankan penguatan layanan primer, digitalisasi melalui SATUSEHAT, serta peningkatan kompetensi sumber daya manusia kesehatan. Meskipun Jaminan Kesehatan Nasional (JKN) telah mencakup lebih dari 96% penduduk, tantangan masih terdapat pada kesenjangan infrastruktur, ketimpangan distribusi tenaga kesehatan, dan koordinasi lintas sektor. Penelitian ini menyimpulkan bahwa keberhasilan transformasi sistem kesehatan sangat dipengaruhi oleh tata kelola yang efektif, keterlibatan komunitas, serta kesiapan layanan primer dan digitalisasi sebagai fondasi reformasi berkelanjutan menuju pencapaian UHC.

Abstract

Purpose: This study aims to examine the relationship between response time and the length of stay (LOS) in obstetric emergency management at the public hospital of Central Kalimantan. Methods: A cross-sectional observational study was conducted using 205 medical records of pregnant women treated for obstetric emergencies between July and September 2024. Data on three types of response time—initial emergency action, obstetrician consultation, and patient disposition—were analyzed using univariate and Pearson correlation tests to assess their association with the LOS in the emergency department. Results: All patients received an initial emergency response within five minutes, indicating good early management. However, over 60% of patients experienced delays in obstetrician consultation. Most patients received timely disposition decisions, which showed the most decisive influence on patient LOS. Statistical analysis revealed a weak positive correlation between initial response time and LOS (r=0.33), a very weak negative correlation with obstetrician consultation time (r= -0.098), and a perfect positive correlation with disposition time (r=1.000). These results suggest that the speed of disposition decisions plays a critical role in determining how long patients remain in the emergency room. Conclusion: Disposition efficiency plays the most pivotal role in determining LOS. Improving administrative processes, increasing human resources, and optimizing infrastructure are essential. These findings provide practical insights for strengthening emergency service protocols and can guide health policy interventions to enhance maternal care quality in referral hospitals across Indonesia.

Abstract

Introduction: The prevalence of HIV, Syphilis and Hepatitis B infections among pregnant women in Indonesia has reached 0.3%, 1.7% and 2.5%, respectively. The risk of transmission from mother to child respectively 20%-45%, 69-80% and more than 90%. Transmission of these three diseases from pregnant women to babies during pregnancy, childbirth and breastfeeding cause disabilities in children and even death. The government stipulates that all pregnant women are required to HIV/AIDS, Syphilis and Hepatitis B screening (Triple elimination examination) at integrated antenatal services. The target set is that early detection of HIV, syphilis and hepatitis B in pregnant women must reach 100% through examinations at First Level Health Facilities and Referral Health Facilities by 2022. However, in 2021 the achievements were only 50.8%, 16.4% and 60.3% for HIV, syphilis and hepatitis B. Objective: This study aims to determine the factors associated with the utilization of triple elimination examinations by pregnant women. Method: This study uses a scoping review method. Article collected from online databases including scopus, science direct, proquest, EBSCO, PupMed and Google Scholar with keywords "Related" AND "Pregnant Women" OR "Pregnancy" AND "Triple Elimination Examination". Article selection conducted based on PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Result: The results showed 8 articles that met the inclusion criteria. The research showed factors that associated with the utilization of the triple elimination examination are sources of information, the role of health workers, use of classes for pregnant women, family support, and age of pregnant women. Conclusion: This study concluded that the utilization of triple elimination examinations by pregnant women associated with external factors around pregnant women.

Abstract

Introduction: The Maternal Mortality Rate (MMR) of Batang Regency in 2023 is still high. Many efforts have been made to reduce the mortality of pregnant women and babies born with integrated ANC services, but have not received optimal results. Objective: The purpose of this study is to analyze aspects of the process including planning, organizing, implementing and assessing in the triple elimination program. Methods: The study used qualitative methods with process variables consisting of planning, organizing, implementing and assessing in a triple elimination program. Data collection through in-depth interviews with 8 main informants and 10 triangulation informants. The main informants are Midwives, Coordinators, and Analysts of the triple elimination program in 4 selected Puskesmas. Triangulated informants are selected based on management level. District level to the Head of Kesga and P2P Dinkes Batang, Puskesmas level to 4 selected Puskesmas Heads and grass-root level to 4 mothers who receive integrated ANC service benefits at Puskesmas. The instrument uses an interview guide. Analysis using qualitative descriptive methods. Results: Planning is carried out in stages from the implementation of activities, namely the integrated ANC team of the Puskesmas by calculating needs. Furthermore, it was discussed with the team and submitted to the Head of the Puskesmas which was then submitted to the District through the Batang Health Office. Organizing is done from the Coordinator Midwife who manages his team. Team management is discussed together and approved by the Head of Puskesmas. Implementation is carried out in accordance with applicable SOPs. The triple elimination assessment is good but there needs to be improvement. Especially in the refusal of triple elimination examination from pregnant women during the implementation of the program. Conclusion: The triple elimination program process in Batang Regency has been good. There needs to be a strategy to invite all pregnant women so that there is no rejection of the triple elimination program.

Abstract

Introduction: Community perceptions of COVID-19 vaccines are responses or acceptance based on the information obtained regarding beliefs about the halal status of vaccines, willingness to be vaccinated, and the capacity of healthcare workers providing the COVID-19 vaccine. This is due to numerous issues affecting information about the COVID-19 vaccine. These facts have encouraged the government to enhance the COVID-19 vaccination program within the community. Objective: This study aims to explore and analyze the public perception of the COVID-19 vaccine. It seeks to identify key factors influencing attitudes towards the vaccine, including trust in health authorities, misinformation, demographic variables, and socio-political influences. The objective is to provide a comprehensive understanding of the drivers of vaccine acceptance and hesitancy to inform public health interventions. Method: This writing uses a literature review method. It involves searching for and filtering articles that meet predetermined criteria through inclusion criteria. In this writing, scientific articles were found using Google Scholar with search keywords "community perceptions," "knowledge," and "COVID-19 vaccine." Result: The study revealed a complex landscape of public perception regarding the COVID-19 vaccine. Trust in health authorities and exposure to misinformation emerged as significant factors impacting perceptions. There is a relationship between knowledge and community behavior, education level and vaccination history with knowledge, religion with community attitudes, and COVID-19 vaccination history. Conclusion: Knowledge influences community perceptions of the COVID-19 vaccine. Therefore, it is necessary to provide comprehensive and equitable information to all communities about the usefulness, safety of the COVID-19 vaccine, and all updated information regarding the COVID-19 vaccine.

Abstract

The development of human resources (HR) is strongly influenced by nutritional factors. The impact of malnutrition on pregnant women leads to low birth weight babies (LBW), malnutrition in toddlers and even stunting. This study aims to determine whether there are differences in the perceptions of pregnant women with chronic energy deficiency on nutritional fulfilment in stunting prevention based on stunting cases in Semarang City. This type of research is quantitative research with analytical observational research using a comparative study approach. The data collection method in this study was interview using a questionnaire that has been prepared by the researcher based on the Health Belief Model theory. The sample in this study were pregnant women with Chronic Energy Deficiency (CED) who were in the health centre with the highest stunting cases and the health centre with the lowest stunting cases, a sample of 50 respondents was taken for each health centre with stunting cases using total sampling. Data were collected by meeting pregnant women with Chronic Energy Deficiency through pregnant women's classes and door to door accompanied by health workers. Based on the results of the analysis, the data were not normally distributed so the data analysis used the Mann-Whitney u test and the significance level (p = 0.05) The results showed that there were differences in the perceived barrier to action of pregnant women with Chronic Energy Deficiency (CED) on the fulfilment of nutrition in preventing stunting based on differences in stunting cases namely (p value 0.033), then other variables did not show differences.
