Abstract

The percentage of pregnant women with Chronic Energy Deficiency (CED) in Indonesia is still quite high. This will give a negative impact on the mother’s and baby’s health. Indonesian government overcomes the problem of CED pregnant women through the provision of supplementary food. The identified obstacles in implementing supplementary food program for pregnant women can be used as consideration in planning, implementation, monitoring and reporting at the Community Health Center. The method used in this study is scoping review. Article identification uses two databases, namely Google Scholar and Garuda. The article searched in the database was 919 articles. After screening with inclusion and exclusion criteria, there were 7 eligible articles. In the input aspect, deficiencies were found in the quantity and quality of health human resources, funds and facilities. In the process aspect, the distribution of PMT from the center was uncertain and there was no monitoring of the use of PMT by the target. Some CED pregnant women were also less compliant in taking PMT and consuming PMT. In the output aspect, the system of program’s monitoring and evaluation at the Community Health Center was not carried out well.

Abstract

Environmental factors play a crucial role in the incidence of tuberculosis (TB) and stunting in children. This study highlights the significant impact of environmental conditions, such as poor ventilation and inadequate sanitation, on the increased risk of both stunting and TB. Poor ventilation, uncontrolled humidity, and high occupancy density are directly linked to higher TB incidence, while inadequate sanitation has been shown to negatively affect child growth and increase the risk of stunting. Recent data indicates that children living in poor sanitation conditions are twice as likely to experience stunting, while those in poorly ventilated environments are at higher risk of contracting TB. To reduce the burden of both health issues, it is recommended to improve household ventilation, enhance sanitation conditions, and provide community-based health education. These interventions can help lower the prevalence of TB and stunting in children, particularly in resource-limited areas.

Abstract

Tuberculosis (TB) and stunting are major health problems affecting children, particularly in developing countries. This study aims to analyze the factors associated with the incidence of TB and stunting among children in Pekalongan City. A cross-sectional study was conducted involving children diagnosed with TB and those experiencing stunting. The population in this study includes all toddlers in Pekalongan City in the year 2024, totaling 18,992 individuals. The sample consists of toddlers residing in Pekalongan City who meet specific inclusion and exclusion criteria. The sample size was determined using the Slovin formula with a 10% margin of error, resulting in a total of 100 respondents. Inclusion criteria include toddlers aged 6–59 months, those living in Pekalongan City, and those who possess a Maternal and Child Health (KIA) book. Exclusion criteria include toddlers with chronic illnesses other than tuberculosis (TB) and those whose parents could not be reached at home. The sampling technique used was proportional random sampling, which ensures each unit has a chance to be selected proportionally based on the population distribution. This method increases representativeness and minimizes variability within the sample. Data were collected through structured interviews, medical records, and anthropometric measurements. Statistical analysis was performed using the chi-square test and logistic regression to identify significant factors. The results indicate that nutritional status, household economic conditions, history of TB contact, and immunization status are significantly associated with TB incidence in children. Additionally, maternal education level, dietary intake, and recurrent infections are key determinants of stunting. Children with a history of TB exposure and poor nutritional intake had a higher risk of developing stunting. The findings highlight the need for integrated health interventions focusing on nutrition improvement, early TB detection, and enhanced public health education to mitigate both TB and stunting in children.

Abstract

Pregnant women with Chronic Energy Deficiency (CED) can cause health problems for both mother and baby. Among them are babies with low birth weight (LBW), who are at risk of stunting. The Pekalongan City Government issued Mayor Regulation Number 11 of 2023 concerning the Acceleration of Integrated Stunting Reduction, where the group of CED pregnant women is one of the targets of Stunting-prone Families who need specific and sensitive interventions. This study aimed to analyze implementation of the policy, especially sensitive interventions of CED pregnant women in Pekalongan City. This study used a descriptive qualitative method. The research was conducted from August to September 2024. The object of the study was the implementation of sensitive interventions of CED pregnant women which was analyzed with Van Metter-Van Horn policy implementation model. The research was conducted through in-depth interviews, observation and documentation. The determination of informants used a non-probability technique with a purposive method, and 11 informants were obtained. The stages in data analysis were data collection, reduction, presentation, data verification and conclusion drawn. The results of the study found that sensitive interventions in form of access to clean drinking water and sanitation, housing improvement programs, social assistance recipient programs and training facilitation for families of CED pregnant women are still not optimal. Pregnant women in CED had not been considered as a priority in obtaining the benefits of assistance programs. It needed to be improved again in policy socialization, quality and quantity of resources, coordination and communication, and improvement of the environmental situation.

Abstract

This study examines the implementation and challenges of performance-based capitation (KBK) at Community Health Centers (Puskesmas) in Indonesia, as a part of the National Health Insurance Program, aiming to improve the efficiency and quality of primary health services. Despite its potential, the implementation of KBK faces various challenges including inadequate human resources, limited infrastructure, and insufficient monitoring systems. The study uses a scoping review framework based on the Public Health Care Monitoring Conceptual Framework by WHO, analyzing 11 selected studies from 2021 to 2024. Findings indicate that internal factors such as management capacity, health worker competence, and organizational culture significantly impact the effectiveness of KBK. External factors like local government support, funding policies, and infrastructure availability also play crucial roles. Technological barriers, particularly in remote areas, hinder optimal policy implementation. The study emphasizes the need for clear regulations, consistent policy dissemination, and effective monitoring to ensure successful KBK implementation. Recommendations include strengthening leadership, training health workers, improving infrastructure, and enhancing digital health technology utilization. These strategies aim to optimize resource allocation, enhance service access, and improve patient satisfaction in Indonesia’s primary health care system.

Abstract

One of the most important hospital management functions is the management of pharmaceutical drug logistics. Drug planning is the process of selecting the type and quantity of drugs required for procurement. Various managerial theories can be used to analyze drug control in pharmaceutical installations, and three research journals on pharmaceutical drug logistics control in hospitals are used for this literature review. The results of the analysis show that there are four methods in drug control: EOQ (economic purchase amount), ABC (better control at all times), ROP (point of repurchase), and VEN (vital, essential, and non-essential). One way to help pharmaceutical logistics stock inventory be more efficient, efficient, and responsive to patient needs and service dynamics is to apply one or a combination of these four methods. Good drug stock management can ensure optimal inventory control, improve service quality, reduce costs, and better manage risks.

Abstract

Traditional health care remains the community's choice in the midst of the modern health care system. However, their utilization is not optimal due to various factors. This study aims to analyze the influence of these three factors on the utilization of traditional health services in Pekalongan City. This study used a cross-sectional design with a quantitative approach. A sample of 393 respondents was selected using proportional random sampling technique. Data were collected through a structured questionnaire and analyzed using the Chi-square test and multiple logistic regression. The results of the analysis showed that cost and perception variables were related to utilization, but the information exposure factor was not statistically proven to be related. Cost was the largest influencing factor, with respondents who rated the cost of services as affordable being 3.5 times more likely to utilize traditional services. Cost and perception contribute to the utilization of traditional health services. Policies that focus on improving community perceptions and affordability are needed.

Abstract

The Visual Inspection with Acetic Acid (IVA) test aims to diagnose early so that it can speed up treatment, reduce morbidity and mortality in women due to cervical cancer. IVA test coverage in Magelang district has not reached the target, especially in the at-risk group. The study aims to see the difference in utilization of IVA services in groups of mothers who are divided into at-risk and non-risk groups. The method used was quantitative method that uses a comparative design. The study population was all mothers aged 30-50 years in Magelang district. Respondents of each group were 68 people determined by proportional sampling technique. Univariate analysis with frequency distribution table and bivariate analysis with Mann-Whitney test because the data was not normally distributed. There was no difference in the utilization of IVA services between the at-risk and non-risk groups (p=0.205). However, it was statistically proven that the at-risk group had a better level of knowledge, attitude, and lifestyle, and felt more vulnerable and needed an IVA test than the non-risk group. Meanwhile, the non-risk group had better perceptions of accessibility and health worker support than the risk group. Risk factor mapping at the Health Center level needs to be done to develop needs-based interventions to increase IVA coverage.
