Abstract

The Donggala District Government has made efforts to control Dengue Hemorrhagic Fever (DHF), however based on the number of cases, and free larvae rates, DHF is still a public health threat in this area. The study aimed to evaluate the implementation of DHF control program in Donggala Regency. This was an evaluative descriptive qualitative research with a case study design. Research location in Donggala Regency, Central Sulawesi Province. Data was collected from September to October 2014 using a purposive sampling technique. Data validity carried out by source triangulation while data analysis was conducted by manually using content analysis. The informants were 10 persons from DHF control program staffs. Data was collected using in-depth interviews and document studies. The results showed that the budjet allocation on implementation of the DHF control program was lower than other infectious diseases. Cross-program collaboration has been carried out properly, however It is required more cross-sector collaboration for mosquito control program activities. From the output of program, the incidence rate (IR) of Donggala Regency was lower than IR of Central Sulawesi Province. Both Primary Health Care with low and high cases turn to have free larva index below 95% and those values tend to decrease in 2011-2012. Donggala District Health Office should increase the health promotion of DHF control through cross-sector collaboration and community to seek more awareness to the risks of transmission and improve dengue prevention.

Abstract

Background: The main causes of neonatal deaths in Semarang city were low birth weight (LBW) approximately 38%, birth asphyxia (23%), congenital defect (8%), infections such as sepsis (3%), jaundice (3%), and others (25%). The most common causes of mortality were considered preventable and treatable at an affordable, easily-to-implement, and efficient through integrated management of  young  infants (IMYI) performed during neonatal visits. It has previously been observed that IMYI has contributed to reducing the neonatal mortality rate by 30-60%. This study aims to investigate the factors that determine the integrated management of young infants carried by maternal and perinatal death surveillance and response (MPDSR) workers in Semarang city.Methods: Quantitative research designs were adopted to provide the correlation between independent variables and dependent variables using statistical hypothesis testing. A cross-sectional approach was utilized to collect the data, both independent dan dependent variables simultaneously. The data were analyzed using chi-square, Fisher exact test, and logistic regression.Results: The result suggests that knowledge, work period, attitude, workload, recording system, supervision had an association toward the practice of IMYI conducted by MPDSR workers in Semarang City. No significant correlation was found on the availability of tools and appraisal toward the practice of IMYI. Work periods was the most dominant factor affecting IMYI practice.Conclusion: these results indicate that knowledge, work period, attitude, workload, recording system, supervision has an important role to increase the practice of IMYI performed by maternal and perinatal death surveillance and response (MPDSR) workers in Semarang city.

Abstract

Background : Immunization is an effort that deliberately provides immunity to infants/children to avoid certain diseases. Immunization is important because it affects the growth of infants and children, when administered within the stipulated time period. The facts show that many parents do not immunize on schedule and tend to postpone it. The objectives of the study analyzed the punctuality of complete basic immunization in infants aged 12 to 13 months (“Baduta”) in Semarang.Methods: This research is an observational study with cross sectional design. The population means all Baduta in Semarang, with sample of 689 Baduta from 37 Puskesmas (Society Health Center) taken randomly. Baduta immunization status is obtained from the KIA Book.Results: The results show 98.25% of Baduta have received basic immunization although not all yet complete. Immunization of measles is the most immunization which have not been received yet by Baduta (13.35%),followed by Polio-4 (8.27%) and DPT / HB / HiB-3 (7.4%). Sources of information on immunization are generally obtained from health workers and cadres.Conclusion : Immunization HB-0 is given at the most punctual (96.28%), followed by DPT / HB / HiB-3 (90.13%) and Polio-4 (85.44%), while immunization of measle s has the lowest punctuality (41.04%), followed by BCG immunization (61.5%).

Abstract

Background: Treatment adherence among patients with severe mental disorders is needed to avoid drop-outs, relapses, and shackling. Kedungmundu Primary Health Center in Semarang City is known to have a low treatment adherence to patients with severe mental disorders. This study aimed to determine the factors associated with treatment adherence among patients with severe mental disorders.Methods: A quantitative study with a cross-sectional approach. The samples were 73 patients with severe mental disorders at The Kedungmundu Primary Health Center. Data were collected using questionnaires and analyzed using frequency distribution analysis and Spearman Rank Correlation Test.Results: The results indicated that knowledge (p = 0,000; r = 0,715), attitudes (p = 0,000; r = 0,839), family support (p = 0,000; r = 0,427), assessment of health workers (p = 0,000; r = 0,456), and the need for health services (p = 0,000; r = 0,716) have a positive correlation with treatment adherence among severe mental disorders patients.Conclusion: Kedungmundu Primary Health Center is suggested to provide psychoeducation to patients and families, improve the implementation of mental health programs, and empower mental health cadres.

Abstract

Background: Planning becomes the most important management function because it arranges all management activities. Non-compliance with the standards of the planning process has an impact on the quality of planning that is unable to bridge needs which ultimately lead to dissatisfaction. PHC as public organizations must meet public service standards. Organizational factors are determinants of organizational performance as they relate to the provision and arrangement of vaious resourcesAims: The purpose of study was to determine affect of organizational factors on compliance with the standards of PHC planning process.Methods: An observational study with cross sectional design. All 56 PHC in the 2 districts in Central Java province (Semarang and Kudus) became the samples. The respondents are officer in charge of the PHC planning programme.. Data collection by interviews using questionnaires and observations. Data were analyzed descriptively and statistically.Results: Only 55.4% of PHCs comply with standard planning process. Weaknesses and inconsistencies still occur at every stage, especially with regard to inaccuracy of time, incomplete and inappropriate data, and low understanding of members of the team. Study shows that the aspect of manpower, infrastructure, organizing and monitoring-evaluation function are positively correlated with adherence to standard planning processes, where monitoring-evaluation factors are the dominant factor.Conclusion: All the gaps identified as the cause of puskesmas non-compliance to the standard planning process can be overcome through increasing staff commitment and competency by providing structured managerial training and routine socialization. Strengthening the monitoring-evaluation function needs to be pursued consistently to ensure the quality of the puskesmas planning process and the implementation of the work plan produced

Abstract

Background: Occupational Health Post (Pos UKK) is a program aims to improve occupational health and productivity of informal sector workers in Indonesia through promotive, preventive, simple curative and rehabilitative care with the principle of Community Based Health Efforts (UKBM). The number of Pos UKK registered in Semarang City Health Office is 30 units. The results of the preliminary study illustrate different situations because there were only 6 Pos UKK. Among those, only 2 were active (6.7%) from the registered Pos UKK. It’s alleged that inactivity of Pos UKK due to low support from various parties and impacted on low sustainability of the program. This study aims to analyze the reinforcing factor with the involvement of informal sector workers in Pos UKK in Semarang City.Methods : Quantitative study with cross sectional approach. Population were 146 workers and selected sample were 70 workers. Independent variables include co-worker support, business owner support and Community Health Centre (Puskesmas) support and dependent variable is involvement of informal sector workers in Pos UKK.Results : Majority of informal sector workers involvement in Pos UKK were low (51,43%). Co-worker and business owners support was good but Puskesmas support as guide was not. Statistically, co-worker support (p = 0,000, OR = 30.8), business owner support (p <0.001, OR = 10.6) and Puskesmas support (p <0.001, OR = 13.5) proved to have a significant relationship. Conclusion:This study showed that reinforcing factors have significant relationship in increasing involvement of informal sector workers in Pos UKK to ensure program sustainability. Puskesmas is expected to provide continuing assistance and socialization about the benefits of Pos UKK to informal sector workers.

Abstract

Background: Indonesia applies decentralization policy in health sector. However, there are Health Minimum Service Standards (HMSS) that must be met at provincial and district level. In district level, there are 12 indicators that must be achieved include of maternal and child health with the coverage must be 100%. Nevertheless, there are various problems in achieving HMSS both quantity and quality aspect. This study aimed to determine the problems that were happened and determine alternative solutions in the implementation of HMSS especially in maternal and child sector.Methods: This research was done in three steps. The first step was conducted by Focus Group Discussion (FGD) with participant from many stakeholders in Central Java and Yogyakarta. This step aims to identify problems in the implementation of HMSS and develop the alternative solution. Second stage was developing the conceptual framework.Results: Based on the research, there are many problems in quantitative and qualitative aspects related to healthcare services. Program planning and budgeting is the main key to achievement HMSS. In second and third steps, an integrated planning and budgeting framework between multisector was obtained including problem identification, problem cause analysis, multisector identification, setting solutions and programs, planning and budgeting, and monitoring and evaluation. So that, there were no overlapping programs, less optimal, and lack of supervision and guidance.Conclusions: Integrated planning and budgeting was alternative solution to solve the implementation problem in HMSS especially in maternal and child aspect. However, strong commitment and monitoring was needed between stakeholder.

Abstract

WHO memperkirakan kekurangan profesi perawat tahun 2030 di negara-negara berkembang. Kepuasan kerja tenaga kesehatan sangat bervariasi dari waktu ke waktu dan dari tempat ke tempat. Dari berbagai tenaga di puskesmas Kabupaten Kotawaringin Barat ditemukan kepuasan kerja 45% dan kurang puas 55%. Profesi yang paling banyak kurang puas adalah perawat. Penelitian bertujuan untuk mengetahui faktor determinan kepuasan kerja perawat puskesmas di Kabupaten Kotawaringin Barat. Desain penelitian cross sectional dengan pendekatan kuantitatif. Populasinya perawat di puskesmas induk, dengan teknik non probability purposive sampling sehingga diperoleh n=95. Instrumen menggunakan e-kuisioner yang dibagikan melalui link dan ditunggui peneliti. Data diolah dengan IBM SPSS versi 20.0 Pengukuran kepuasan kerja menggunakan MSQ. Hasil menunjukan kepuasan kerja 41,1% dan kurang puas 58,9%. Terdapat hubungan yang signifikan antara lingkungan kerja fisik (p=0,00), pengembangan karir (p=0,00), insentif (p=0,00), K3 (p=0,004) dengan kepuasan kerja. Tidak terdapat hubungan signifikan dengan beban kerja (p=0,755). Variabel yang masuk pemodelan multivariat yakni lingkungan kerja fisik (p=0,022), pengembangan karir (p=0,060, insentif (p=0,00), K3 (p=0,341). Kesimpulan bahwa faktor determinan kepuasan kerja perawat puskesmas yakni lingkungan kerja fisik, pengembangan karir, insentif, K3. Diharapkan adanya perbaikan ASPAK, penyediaan anggaran pendidikan dan pelatihan, utilisasi insentif berdasarkan capaian kinerja, dan medical general chech-up secara berkala guna meningkatkan kepuasan kerja perawat.

Abstract

The number of hypertensive patients in Purwoyoso Public Health Center has increased from 2016 to 2018. However, the coverage of hypertensive patients who seek treatment regularly is still low and far from the 100% target coverage. The purpose of this study is to describe the management of hypertension in PIS-PK in Purwoyoso Public Health Center, Semarang City. This research is a qualitative research with a descriptive analytic approach. Data collection using in-depth interviews were selected based on purposive sampling techniques. The research subjects were PTM Program Holders, Doctors, Nurses, PIS-PK Coordinators, Head of Puskesmas and four patients with hypertension. The aspects studied were the diagnosis of hypertension, lifestyle interventions, pharmacological efforts and medication adherence, and the referral system. The results showed that the patient only did a one-time examination so that it affected the process of providing education, administering medication and taking medication compliance, as well as the patient's unwillingness to be referred. There are several obstacles in the management of hypertension, including patient busyness, distance to the puskesmas, the level of patient awareness in conducting routine checks, low participation in educational activities, not taking drugs again after the drugs run out, and do not want to be referred. The availability of Posbindu to conduct early detection of hypertension only takes place in three hamlet in Kalipancur. In this study, suggest officers provide drug counseling to improve medication adherence and maximize the role of families in supervision of hypertension sufferers.

Abstract

Hospital has to design a process,  monitor and evaluate performance, analyze incident intensively in improving its patient safety performance. ‘Seven Steps to Patient Safety’ is a reference for hospitals to implement patient safety program. The objective of this research is to analyze the implementation of Seven Steps to Patient Safety in Sultan Agung Islamic Hospital Semarang. This research is qualitative descriptive research. Data were collected by indepth interview and observation method. The key informants of this research are four Quality Link Safety Champions of four different departments. The result showed that not all steps in Seven Steps to Patient Safety are implemented. There is still blaming culture in departments; the implementation of patient safety is still about collecting data only; mindset is still about only doing the program , not improving the program; lack of management commitment for demonstrating patient safety culture; there are still delays in reporting incident; there is no open communication policy about telling incident to patient and family; and there is no active learning about incident which involving all departments in hospital. Suggestion in this research is hospital need to do a routine meeting and excecutive walkabout which involve excecutive leader, Quality and Safety Committee, and departmens that focuses only about patient safety for demonstrating, enhance patient safety commitment, also improve open and fair patient safety culture.
