Abstract

Cervical cancer screening program by visual inspection with acetic acid (VIA) was breakthrough effort to accelearte the decrease of morbidity and mortality rate for cervical cancer. However, the implementation of this program in Semarang still has not optimum yet. The objective of this study was analyze the implementation of cervical cancer screening program by VIA at primary healthcare in Semarang. This research was descriptive-qualitative study. The data collection was done using indepth interview based on purposive sampling criteria of the primary healthcare center that had increase and decrease of extreme in VIA. The research subjects were 5 midwife executor of VIA as the main informant and for the triangulations informants were 1 head of infectious diseases unit in Semarang District Health Office, 5 head of primary healthcare center, and 5 women of reproductive age. Content analysis method was applied in the data analysis. The variables researched were implementation, communication, human resources, infrastructure, information, mind and character, motivation, division of works, and supervision. The result of the study show that implementation of program by VIA still had some constraint in promotion and education through media and socialization of program. The implementation was influenced by communication, human resources, materials, information, mind and character, motivation, division of works, and supervision. In communication , midwife and women of reproductive age still had not have good understanding of program information. Although infrastructures were fine but there was no avaibility of midwife and information. Mind, character, and motivation of midwife showed  that need to be improved. Regular supervision and clear division of works need to between midwife and manager of infectious diseases unit.

Abstract

The Minimum Service Standards (MSS) are a national strategic program and the Puskesmas are the leading unit in efforts to achieve the MSS targets. Pati Regency occupies the top 6 highest infant mortality cases in the province of Central Java in 2018 and the highest number of cases is in the Dukuhseti Primary Health Center (PHC). Dukuhseti PHC is a BLUD PHC that must apply MSS including newborn health services, but the implementation is not optimal as indicated by the coverage of neonatal visits that have not reached the target. The study aims to analyze the implementation of health MSS in newborn health services in Dukuhseti PHC. This research is a qualitative research with in-depth interviews and observations using content description analysis. The results showed that the implementation of SPM in newborn health services was constrained in the implementation of KN2 and KN3, neonatal care counseling, health examination with MTBM, and handling neonatal referral cases. That was because most of the midwives had not received MTBM training, there was no budgeting for health training, double jobs because midwife HR at the PHC was lacking, the targets used a lot of real data, recording and reporting were not in order, supervision from the leadership was not optimal, difficulties in finding health facilities referral, and there is no strict sanction from the District Health Office (DHO). PHC are expected to be able to budget funds for training for health workers, provide guidance for better coordination, fix information systems and referral applications by implementing an online-based Integrated Referral System. The DHO is expected to be able to improve supervision by implementing more systematic evaluations and reinforcing sanctions so that the implementation of MSS in newborn health services runs optimally.

Abstract

Since 2015 source of village income increased, it was because getting allocations from the central government called (the village fund). However, the budgetary funds provided for Posyandu as one of the basic services in the village center was still low, while there are many health problems that occur. This is possible because of the lack of concern for the government in improving the utilization of the village fund for Posyandu. This study aims to determined the government’s role in the preparation of the budget for the development of health programs in the Posyandu. This study used a qualitative method by which data were obtained from 4 (four)- depth interviews with the village chief. The results showed that interpersonal’s role  in participation and evaluation budgetary had not been good. This is indicated by the exclusion of Posyandu cadres in the preparation of the budget in one of the villages and the lack of coordination in evaluating the budget in several villages. The village chief related to informational’s role in the preparation of the budget was also not optimal, it was indicated by the complaints of cadres in Posyandu who felt the village chief did not find out about the clarity of targets, difficulty of targets, and evaluation of the existing budget in Posyandu. In a decisional’s role was also not going optimally, as indicated by the complains of the secretary of the village who said in decision-making related to difficulty of targets and evaluation of budgets submitted to him. Where the village chief should also be required to had decision making skills in solving problems that exist within the organization he leads. This is expected village chief can improve coordination and good communication both with the village secretary and cadres in order to avoid misunderstandings and differences of perception related to the budget given to Posyandu.

Abstract

ABSTRACTSense-Community Health Center becomes one of the attention of local governments in reducing the incidence of high sensory health problems in Central Java. Job satisfaction becomes organization need to improve the service quality. The increasing number of patient visits and the absence of additional human resources makes the high workload led by inadequate employee benefits and high absence also indicates that employee has low job satisfaction. This research type is observasional with survey method and cross sectional approach. The research instrument uses a structured questionnaire for all employees of BKIM Central Java Province, which are 57 people. The research data was processed quantitatively with univariate and bivariate method using chi square test. The univariate analysis shows that high percentage of compensation (50,9%), good working condition (57,9%), good organization policy and administration (70,2%), good co-worker (63,2%), high achievement (68,4%), the work itself (61,4%), good responsibility (63,2%), good opportunity (68.4%), and high job satisfaction (52.5%). Meanwhile, the result of bivariate analysis shows that there is a significant correlation between compensation, working condition, policy and organization administration, co-worker, supervision, achievement, job itself, developing opportunity with employees’ job satisfaction (p<0,05) while responsibility has no relationship with employees’ job satisfaction (p>0,05). The recommendations that can be used in managing employees’ job satisfaction is supervision can give feedback for the works that all employees have done.

Abstract

Implementation of informed consent is one of the important aspect in assessment elements hospital accreditation. Permenkes RI no. 290 of 2008 set need for informed consent to patients of medical procedure that will be received.Based on preliminary studies on installation central surgery of district hospitalSemarang, many informed consent forms were incomplete and in shortprovision of medical procedures information that given by doctor to patient.This study aimed to analyze implementation of approval medical action (informed consent) in preparation for accreditation at installation central surgery of regional public hospital Semarang as seen from attitude, knowledge and organizational policies which are factors that affect doctor’scompliance to implement informed consent.This type of research is a descriptive qualitative.Data collected through depth interviews with key informants. Then, processing and analysis data. Result showed that doctors know about informed consent and its purpose and function, but lacking knowledge of legal consequences of informed consent. Whereas for attitude, the doctors agreed to obligation of perform informed consent and agree to both rule of law and consequences of implementation of informed consent.But until now, the exists organization's policies, operating procedures, monitoring and sanctions not ensure the doctor’s compliance in applying informed consent.Relation to the preparation of accreditation, there are regulations and implementation of informed consent in hospitals haven’t appropriate with the standards of accreditation. Therefor, it needs a firm and clear arragement with socialization of the informed consent procedure. Also, its necessary for management to develop monitoring and sanctions.

Abstract

SEC adalah layanan unggulan dari RSISA yang memberikan pelayanan pada kesehatan mata yang menjadi pusat rujukan pengobatan mata bagi masyarakat Jawa Tengah. Dalam tiga tahun terakhir jumlah kunjungan rawat jalan mengalami peningkatan meskipun terdapat kecenderungan menurun. Kepuasan pasien menjadi salah satu indikator yang digunakan sebagai pencitraan awal rumah sakit apakah rumah sakit tersebut memberikan pelayanan yang baik bagi pelanggannya. Pihak rumah sakit perlu memahami karakteristik pasien untuk pengambilan keputusan terkait pelayanan rumah sakit sehingga pasien dapat merasa puas. Namun dalam pelaksanaan masih ada kecenderungan kurang puas pada pasien yaitu terhadap fasilitas pelayanan, waktu menunggu, pelayanan dokter, tarif, dan pelayanan administrasi pendaftaran pasien ASKES serta pelayanan administrasi pembayaran. Tujuan penelitian dilakukan untuk mengetahui hubungan antara karakteristik pasien dengan kepuasan pasien pada pelayanan poli mata SEC RSISA. Penelitian dilaksanakan di SEC RSISA menggunakan penelitian kuantitatif dengan survei kepuasan Cross Sectional dengan jumlah responden 100 orang pasien. Hasil penelitian menunjukkan bahwa karakteristik responden termasuk dalam kelompok umur dewasa (68%), jenis kelamin wanita (58%), asal suku jawa (86%), pendidikan lanjut (61%), bekerja (54%), pendapatan tinggi (75%), diagnosa penyakit kasus dengan operasi (72%), dan tipe pembiayaan umum (55%). Selain itu sesuai dimensi kepuasan Linder-Pelz bahwa pasien merasa puas pada dimensi accessibility (43%), availibility of resources (35%), countinuity of care (29%), efficacy (54%), finances (47%), hummaneness (37%), information of gathering (44%), information of giving (30%), pleassantness of surrounding (35%), dan quality (10%). Hasil uji ChiSquare, uji korelasi Pearson Product Moment dan uji korelasi Rank Spearman menunjukkan tidak ada hubungan antara karakteristik dengan kepuasan pasien.

Abstract

Job satisfaction is a reflection of the workers’ feeling in their work. It can be shown from employee's attitude to the work and their working environment. The nurse's job satisfaction needs to get serious attention from hospital management because nurse holds the most important service role in the hospital. The objective of the research was to determine the related factors with nurse job satisfaction in inpatient Tugurejo Hospital Semarang. The research used quantitative method with cross sectional approach. The sample used in the research was 80 inpatient nurses of Tugurejo Hospital Semarang with proportional random sampling method. The research used univariate analysis, bivariate analysis by Rank Spearman test. The result showed that the respondents had good recognition (67,5%), responsibility (55%), potential development (60%), promotion opportunities (51,3%), income (66,3%), working conditions (60%), hospital administration and policy (82,5%), interaction between individuals (90%), supervision (66,3%). Variables related to inpatient nurses’ job satisfication of Tugurejo Hospital Semarang were recognition (p = 0,000), potential development (p = 0,000), promotion opportunities (p = 0,000), income (p = 0,000), working conditions = 0.000), hospital administration and policy, interaction between individuals (p = 0,000) and supervision (p = 0,000). Meanwhile, unrelated variable to inpatient nurses’ job satisfication of Tugurejo Hospital Semarang was responsibility (p = 0,365). In order to manage nurse job satisfaction, the hospital management should be able to create a pleasant working environment, provide positive feedback, and able to improve the career path system.

Abstract

Voluntary Counseling and Testing (VCT) is one of the early way to detect HIV for people. The services include pre-test counseling, HIV testing, and post-test counseling. The implementation of VCT service in Salatiga is’t optimal yet, it’s provide by the decrease of VCT service visit in the whole public helath center in Salatiga. This research intends to analyze the implementation of VCT service by community health clinic. It’s a qualitative research which got by doing deep interview to six main informants and fourteen triangulation informants. The research variable includes analysis of the implementation of VCT service by the health staff, based on aspects of communication, resources, disposition, characteristic of the executive body and the aspects of economy, social, and politic environment support. The outcome of the research shows that the service implementation of VCT by public health center’s staff is still unsatisfactory. The Matter is caused by the VCT teams of the whole public health center have no case management, so tasks haven’t been shared by the case management. Counselor in public health center hasn’t implemented the procedures completely and even hasn’t SOP yet. The atitude of the health staff is felts less friendly and make the patients reluctant to return again, even sometimes they’re expelled from the public health center. The VCT team hasn’t been consistent in service scheduling. Examination room is narrow and mixed with another infectious disease that make the patients feel uncomfortable. Related to stigma of the society that still considers percussion with the VCT service.

Abstract

Maternal mortality rate in Semarang in 2015 is 128,5/100.000 live births by the largest percentage in puerperium (74.29%). Most deaths occurred in the KF2 and KF3 where the mother's position is already at home.  The coverage of KF3 is still not reached in accordance with the government’s target as many as 86,91%. This is possible because of the lack of motivation BPM to monitor the condition of the mother during puerperium. The purpose of this research was to analysis how far motivation of BPM when providing postpartum maternal health sevices. This research uses a qualitative method with indepth interviews to 6 BPM. Results showed that hopes can forced BPM to providing services because BPM will get satisfaction, pleasure, tranquility, appreciated, respected, helpful to others, trusted also can increase financial income. Its also can make the personal needs fulfilled, such as felt protected and cherished by people around, more confident, and even get the chance to become a facilitator of Bidan Delima or head branch in the organization. But when facing obstacles, BPM will change their attitude into be annoyed and tend not perform her duty well. It also affects the persistence of BPM in providing services. So far, there is output of service that not achieved, such as have not been able to change mindset about the myths and the lack of number of visits. It is estimated due to the lack of sacrification in providing services. IBI Semarang should improve supervision and coordination to motivate BPM in providing postpartum meternal health care services.

Abstract

BPJS Kesehatan have to maintain quality control and cost control. As an effort to realize the management of BPJS patient claims that quality are closely related to the quantity and quality of personnel in the BPJS Center Unit. This study analyzes the workload and manpower needs in the BPJS Center Krakatau Medika Hospital Cilegon City. Work sampling method used for measure the workload, while the method Workload Indicator Staffing Needs (WISN) is used for measure the power requirements based on actual workload. Work sampling conducted on May 15 – June 2 2017. The results of this study indicate that the Administrator personel who have been using 80,31 % of the work time with productive activities. Of productive activity is, as much as 60,90% is for direct productive activities, 19,41% is for not directly productive activities. meanwhile 8,82% used for personal acitivities and 10,86 for unproductive activities. Work sampling results to Coder are 81,05% of the work time with productive activities, 61% is for direct productive activities and 20,05% is for not directly productive activities, for personal activities is 6,20% and 12,76% for unproductive activities. Based on the results of the work sampling, with WISN turns out that the current force is smaller than the power requirements to complete the tasks and functions of the BPJS Center Krakatau Medika Hospital Cilegon City.
