Abstract

Komunikasi terapeutik medik adalah proses interaksi tenaga medik dan pasien secara individual dengan tujuan membantu pasien merasa nyaman dan tidak takut untuk mengungkapkan perasaan dan pikiran mereka. Hanya saja, ada indikasi komunikasi terapeutik medik di RSUD Dr. Pirngadi Kota Medan belum efektif. Salah satu penyebabnya adalah tingkat pengetahuan dan keterampilan yang rendah dari dokter mengenai cara menyamankan dan meyakinkan pasien. Agar dokter dapat meningkatkan keterampilannya mengenai bagaimana memprioritaskan kenyamanan dan keyakinan pasien saat berkomunikasi dengan pasien, maka diperlukan pelatihan mendengar aktif, mendengar dengan empati, komunikasi persuasif, dan komunikasi asertif. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan dan keterampilan dokter mengenai cara menyamankan dan meyakinkan pasien berobat di rumah sakit. Pelatihan ini menggunakan metode ceramah, games, dan role play. Peserta pelatihan yang mengikuti sampai akhir hanya 12 orang. Berdasarkan evaluasi pelatihan dengan pre-test dan post-test, pengetahuan peserta tentang cara menyamankan dan meyakinkan pasien berobat di rumah sakit RSUD Dr. Pirngadi Kota Medan meningkat dari 0% menjadi 83,3%. Berdasarkan hasil evaluasi pelatihan dengan simulasi, seluruh peserta mendapatkan nilai 70 ke atas. Disimpulkan bahwa pelatihan komunikasi terapeutik medik yang telah dilakukan dapat meningkatkan pengetahuan dan keterampilan dokter mengenai cara menyamankan dan meyakinkan pasien berobat di rumah sakit RSUD Dr. Pirngadi Kota Medan. Diharapkan pelatihan ini dapat dilanjutkan secara mandiri oleh mitra dan didukung oleh pihak terkait, serta ke depan pelatihan serupa dapat diperluas ke rumah sakit lain dengan pemantauan berkelanjutan.

Abstract

Introduction: Problems that dominate adolescence include sexuality, HIV/AIDS, and drugs.  sexual behavior of adolescents can hurt their health, both physically, psychologically, and socially, which often ends in unwanted pregnancies, early marriage, abortion, sexually transmitted infections, and other manifestations in the form of increased maternal mortality rates (MMR). Objectives: to examine one of the factors related to adolescents' exposure in shaping their sexual behavior, namely by analyzing the length of internet and social media access of adolescents in a day with adolescent sexual behavior. Methods: This study used a cross-sectional design, using a sample in the study of 170 adolescents in Terjun Village, Medan Marelan Subdistrict which was taken by simple random sampling. the data obtained were analyzed using the chi-square test. Results: show that there is a significant relationship between the length of internet access and adolescent social media in a day with adolescent sexual behavior with a p value = 0.0005 (p ≤ 0.05). Conclusions: To prevent risky sexual behavior in adolescents related to access to the internet and social media, better monitoring is needed from parents and also providing education to adolescents about positive internet access so that all adolescents become wiser in using the internet and social media they have.

Abstract

This community engagement program focused on empowering coastal women members of the Indonesian Coastal Women’s Association (KPPI) in Bagan Deli, Medan Belawan District, by addressing gender issues and reproductive health rights. The activity was motivated by high maternal and infant mortality rates, limited understanding of family planning (FP), restricted access to women-friendly health services, and the strong influence of patriarchal culture that hinders women’s autonomy in reproductive decision-making. The methods included interactive health education, focus group discussions (FGDs), and the distribution of leaflets as educational media to extend knowledge within families and communities. The results indicated a significant improvement in participants’ knowledge after the intervention, as reflected in the differences between pre-test and post-test results. FGDs revealed women’s real experiences of limited contraceptive choices, lack of partner support, and socio-economic barriers influencing their reproductive health. The leaflet-based education strengthened comprehension and raised critical awareness of gender equality, enabling women to become more empowered in making family health decisions. Overall, this program contributed to enhancing health literacy, empowering coastal women, and supporting the achievement of the Sustainable Development Goals on health (SDG 3), gender equality (SDG 5), and reducing inequalities (SDG 10).Program pengabdian masyarakat ini berfokus pada pemberdayaan perempuan pesisir yang tergabung dalam Kesatuan Perempuan Pesisir Indonesia (KPPI) Bagan Deli, Kecamatan Medan Belawan, melalui isu gender dan hak kesehatan reproduksi. Kegiatan dilatarbelakangi oleh tingginya angka kematian ibu dan bayi, rendahnya pemahaman tentang keluarga berencana (KB), keterbatasan akses layanan kesehatan ramah perempuan, serta kuatnya pengaruh budaya patriarki yang membatasi otonomi perempuan dalam pengambilan keputusan reproduksi. Metode yang digunakan adalah penyuluhan interaktif, diskusi kelompok terarah (FGD), serta pendistribusian leaflet sebagai media edukasi yang dapat diperluas ke lingkungan keluarga dan komunitas. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan peserta secara signifikan setelah intervensi, ditandai dengan perbedaan hasil pre-test dan post-test. FGD berhasil menggali pengalaman nyata perempuan mengenai keterbatasan dalam memilih kontrasepsi, minimnya dukungan pasangan, serta kendala sosial-ekonomi yang memengaruhi kesehatan reproduksi mereka. Penyuluhan berbasis leaflet turut memperkuat pemahaman dan membuka ruang kesadaran kritis tentang kesetaraan gender, sehingga perempuan lebih berdaya dalam menentukan keputusan kesehatan keluarga. Keseluruhan kegiatan ini berkontribusi pada peningkatan literasi kesehatan, pemberdayaan perempuan pesisir, serta mendukung pencapaian tujuan pembangunan berkelanjutan terkait kesehatan (SDGs 3), kesetaraan gender (SDGs 5), dan pengurangan ketimpangan (SDGs 10).

Abstract

Adolescent sexual behavior is a complex issue that has a major impact on the health and well-being of young people worldwide. In today's internet era, it is shaped by many factors, including peer influence, family support, access to pornography, and reproductive health services. This study set out to examine the variables that influence adolescent sexual behavior. Using a cross-sectional design, researchers surveyed 170 adolescents in Terjun Village, selected through random sampling. Data were analyzed with the chi-square test to identify relationships between variables. The results showed that knowledge (p = 0.006), family support (p = 0.0005), and access to pornographic content (p = 0.0005) were significantly related to adolescent sexual behavior. In contrast, reproductive health services (p = 0.196) and peer influence (p = 0.440) did not show a significant relationship. These findings suggest that to help adolescents live healthier and more responsible sexual lives, efforts should focus on comprehensive sex education, strong family support, reproductive health promotion, and policy advocacy that reinforce these strategies Keywords: Adolescent, Sexual Behavior, Health, Risk

Abstract

Men who have sex with men (MSM) remain a key population with a heightened risk of HIV infection, underscoring the importance of HIV care, support and treatment (CST) services in improving health outcomes. However, the utilization of CST services among MSM is often hindered by stigma, discrimination, and fear of receiving a positive HIV diagnosis. These barriers highlight the need to understand the role of social support in encouraging service uptake. This cross-sectional study involved 83 MSM in Medan City, selected through purposive accidental sampling. Data were collected using structured questionnaires and analyzed through univariate, chi-square bivariate test, and logistic regression test. The results showed that all assessed forms of social support—emotional, appraisal, informational, and instrumental— were significantly associated with the use of HIV CST Services. Among these, informational support was identified as the most influential factor in increasing service utilization (p=0,007, OR=4,3). In conclusion, informational support plays a dominant role in enhancing the utilization of HIV CST services among MSM. Strengthening the provision of accurate information and guidance is therefore essential in improving service engagement. Recommendations include expanding outreach efforts, enhancing social support mechanisms for MSM, and promoting safe and supportive environment to reduce stigma and encourage access to CST services.

Abstract

Background: Female workers in waste processing industries are highly vulnerable to occupational hazards, including biological, chemical, ergonomic, and reproductive risks. Despite the implementation of Occupational Health and Safety (OHS) programs, evidence regarding gender-responsive OHS practices for female workers remains limited, particularly in low- and middle-income countries (LMICs). This systematic review aimed to synthesise evidence on the implementation and effectiveness of gender-responsive Occupational Health and Safety (OHS) programs among female workers in waste processing industries Methods: A systematic review was conducted following the PRISMA 2020 guidelines. Literature searches were performed in Scopus, PubMed, ScienceDirect, Web of Science, and CINAHL for studies published between 2014 and 2024. Two independent reviewers conducted study screening through title, abstract, and full-text assessment. Methodological quality was evaluated using the Joanna Briggs Institute (JBI) critical appraisal tool. Eighteen studies met the eligibility criteria. Due to heterogeneity in study designs, outcomes, and measurement indicators, meta-analysis was not feasible; therefore, the data were synthesised using a thematic narrative analysis. Results: The PRISMA process identified 1,246 records, of which 18 studies were included in the final synthesis. Studies were predominantly conducted in LMIC settings and involved cross-sectional, qualitative, and mixed-method designs. Three major themes emerged: (1) occupational hazard exposure among female waste workers, (2) implementation of OHS interventions and personal protective equipment (PPE), and (3) barriers to gender-responsive occupational health protection. Most studies reported inadequate PPE utilisation, limited reproductive health protection, and weak institutional OHS policies. The quality of evidence ranged from low to moderate due to methodological heterogeneity and limited longitudinal studies. Conclusion: Gender-responsive OHS implementation in waste processing industries remains suboptimal, particularly in LMICs. Strengthening workplace policies, reproductive health protection, PPE compliance, and gender-sensitive occupational programs is essential to improve worker safety. Future studies should employ longitudinal and intervention-based designs to evaluate the effectiveness of OHS programs among female workers.

Abstract

Background: HIV and AIDS is a global public health problem. Until the end of 2016, there were more than 36.7 million people living with HIV. HIV & AIDS counseling is a confidential communication between clients and counselors aimed at increasing the ability to deal with stress and make decisions regarding HIV & AIDS. The purpose of this study was to determine the effect of the ability to pay and the environmental factor on consultation health service utilization at Budi Kemuliaan hospital, Batam.Subjects and Method: This was a cross-sectional study was conducted at Budi Kemuliaan Hospital, Batam, from May 8 to June 18, 2018. A sample of 76 HIV patients was selected by systematic random sampling. The dependent variable was the use of consulting service. The independent variables were risk of illness and environment. The data were collected using questionnaires and analyzed by a multiple logistic regression.Results: The use of consultation health service was influenced by the risk of illness (OR = 6.82; p = 0.011) and supportive environment (OR= 4.59; p= 0.028).Conclusions: The use of consultation services is influenced by risk factors for illness and the environment.Keywords: consultation service, the risk of illness, environment, HIV/AIDS patientCorrespondence:

Abstract

Background: Strategic purchasing plays an important role in the cost structure and competitiveness of small and large businesses. Managing Board of Social Insu-rance (BPJS) has implemented strategic purchasing of health services in the National Health Insurance (JKN) scheme. Strategic purchasing is important to provide quality products and services at low costs. Strategic purchasing of health service has five critical criteria: (1) what is to purchase; (2) to whom it is for; (3) whom it is from; (4) what is the price; (5) how to purchase it. This study aimed to assess BPJS barrier to strategic purchasing of primary care service at public and private health facilities in Subulussalam, Aceh.Subjects and Method: This was a qualitative study conducted in Subulussalam, Aceh, from February to July 2015. A total of 12 key informants for this study, including BPJS administrators and hospital managers. The theme was strategic purchasing of health service. The data were collected by in-depth interview, direct observation, and document review. The data was analyzed by thematic analysis.Results: BPJS administered the same standards of strategic purchasing health service from the public and private health providers. However, there were some unresolved problems: (1) BPJS faced difficulty to select primary care providers independently; (2) Illegal use of other members JKN card to get health service; (3) Redundant claims from the primary care providers; (4) Claim of inpatient days exceeding the maximum allowable limit; (5) Delay in non-capitations reimbur-sement payment to primary care provider; (6) Inequitable distribution of health manpower across health facilities.Conclusion: BPJS has implemented strategic purchasing of primary care services with the same standards applied to public and private primary care providers. However, there remain some problems of purchasing health services that call for immediate corrective actions, including routine monitoring, explanation of delay, advocacy, equitable distribution of health manpower.Keywords: strategic purchasing, primary care service, health facility, BPJS.Correspondence: Satria Darma.
