Abstract

Background: Leptospirosis is zoonotic disease caused by Leptospira bacteria and transmitted to human through contact with animal urine or environment is contaminated with this bacteria. Since 2010 Leptospirosis have increased, in Pati Regency, Central Java. In 2014 from January until Febuary the number of cases increased significantly compared to the previous year as 30 cases with 6 death. The purpose of the research is to identify risk faktors of phisical and biological  environment that influence the incidence of leptospirosis at Pati regency. Methods: This research was an explanatory research with observational method using case control approach in 30 cases and 30 controls taken with inclusion criteria. The data of cases and controls obtained from medical records Pati Health District Agency. Diagnosis of control was taken based on clinical diagnosis and examination of blood supply using rapid test the type of Leptotek Lateral Flow with negative results.Results: The results of analysis showed there was  relationship between the stagnant water of the ditch pools ((OR = 5,8; CI 1,03 – 32,84) and bad sewer condition with leptospirosis (OR = ( 7,1; 95% CI 2,01 – 25,11)  . Test of water sample had been done at 8 location ans soil at 6 location. No one of them showed positif contain Leptospira sp.Conclusion: Stagnant water of ditch pools and bad sewer condition are influence of incidence of leptospirosis in Pati regency. There is one positive serum of mice that contained the bacteria Leptospira serovar bataviae with bacteria leptospira serovar bataviae.leptospirosis patients tend to spread in flood prone areas.Keywords : Leptospirosis, environmental risk factors

Abstract

Background : Gold mining in Jendi Village Selogiri Subdistrict Wonogiri District is artisanal gold mining without a permit using amalgamation process (process of extracting gold ore by mixing gold with mercury). Mercury is a neurotoxic of the central nervous system including cerebellum. The purpose of this study was to analyze the association between the history of mercury exposure and body imbalance on traditional gold miner in Jendi Village, Selogiri Subdistrict, Wonogiri District. Methods : The kind of this research is observational analytic study and the research design used was cross sectional. The population in this study are all workers in traditional gold mining region, then the sample are taken is determined based on the formula and the obtained sample size of 42 people. The research were taken through the interview, laboratory test (mercury level in the blood), and clinical examination (Romberg test). Data woukd be analyzed using Chi Square and Logistic Regression. Results : The results showed that from the four independent variables, there are two independent variables that had p values > 0,05 are the type of occupation (p value = 1,000) and duration of work per day (p value = 0,119), whereas the other two independent variables had p values ​​< 0,05 are period of work (p value = 0,004) and mercury level in the blood (p value = 0,005) so that Ho is rejected for two independent variables that had p values ​​< 0,05, which means that there is a significant correlation between period of work and mercury level in the blood with body imbalance on traditional gold miner in the Jendi Village Selogiri Subdistrict Wonogiri District. Conclusions : From this research can be concluded that as many as 88,1% of the respondents had a positive result of Romberg test and as many as 97,62% of the respondents had blood mercury level exceeding the EPA threshold value. Suggested should pay more attention to health and safety aspects of working in the gold mining process as an effort to reduce the health effects due to mercury exposure.   Keywords: mercury, traditional gold miner, body imbalance

Abstract

ABSTRACT Background : Indonesia is a tropical country which is appropriate of the mosquito vector, particularly  Anopheles. In  Java and Bali Islands malaria is still an endemic disease. Malaria in Central Bangka Regency in 2007 was reported with an Annual Malaria Incidence (AMI) of  38,07 ‰. Especially in Koba (Sub-District) , the cases were still high, and Annual Malaria Incidence (AMI) was measured at  48,1 ‰. and Annual Parasite Incidence (API) was 30,52 ‰. Hence, Central Bangka Regency was still above the National Annual Malaria Incidence (AMI) which was measured less than  30 ‰. Method : An observational retrospective research by Case Control Study Design method was done to measure the risk factor of independent variable of indoor, out door and its effect  effect on malaria incidence. The case group was people with blood specimen positive malaria while the control group was negative blood specimen. The number of patients and control groups were both 76. Data were  analyzed by using Chi-square analysis for bivariate analysis and logistic regression analysis for multivariate analysis. Result : Anopheles identification showed, that malaria vector was consisted of   Anopheles sundaicus, Anopheles letifer and Anopheles nigerrimus. The dominant species was Anopheles letifer because the population vector was more than the others. The analysis result of bivariate was demonstrated that sex was a  risk factor for malaria incidence. Six  factors were analyzed by multivariate house wall-density (OR=2,357;95% CI=1,019 – 5,452), the gauze wiring on ventilation (OR = 5,063; 95% CI = 1,925 – 13,312), the existence of water pond (OR = 4,407; 95% CI=1,542-12,591), the brush wood existence(OR = 2,693; 95% CI = 1,466 – 5,985), the usage of mosquito potion (lubricated, roasted, or sprayed) (OR = 7,169; 95% CI = 2,912 – 17,650). Conclusion : Variable that is most dominant to malaria case is the   usage of mosquito net has value p = 0,0001 with Confidence Interval ( CI) 95 % = 6,835-91,281  in tin mine region in sub district Koba Central Bangka Regency. Habit applies mosquito net at group of lower case that is 57,9 %, while at group of konrol habit applies higher mosquito net that is 90,8 %. Abstract

The Soeprapto Special Psychiatric Hospital (RSKJ) Bengkulu Province is a hospital that has not yet conducted a PROPER assessment. Management of environmental quality in hospitals is not yet fully in accordance with regulations.  The PROPER of assessment is still not well understood, licensing is incomplete, lack of human resources and limited budget for environmental quality management. This study aims to identify the quality of the hospital environment in supporting PROPER at Soeprapto RSKJ Bengkulu Province. This type of research design is descriptive. The population in this study were all sections that intersect with environmental management in hospitals using purposive sampling techniques. The results showed that the assessment of the proper index at the Soeprapto RSKJ Bengkulu Province included 4 criteria and 1 assessment was more than obedience. The value of the environmental document criteria / environmental permit 100%. The water pollution control criteria 83.3%. Criteria for controlling air pollution 40%. Criteria for controlling pollution of hazardous and toxic waste 62.5% and for criteria more than 0% compliance. From the results of the assessment, it can be concluded that Soeprapto RSKJ Bengkulu Province received is in the red category, where environmental management efforts only partially achieve results consistent with the requirements of regulations.

Abstract

Latar belakang: Pestisida dapat menjadi neurotoksik yang kuat, keracunan jangka pendek, pengurangan koordinasi dan kemampuan berpikir. Keracunan jangka panjang dapat menurunkan kecerdasan dan ketidakmampuan belajar, dihubungkan dengan kerusakan otak. Dampak jangka panjang terhadap anak mengakibatkan penurunan IQ. Sebanyak 95 % orang tua siswa adalah petani pengguna pestisida sejak tahun 1970, dan ditemukan 15 siswa berkebutuhan khusus lamban belajar. Penelitian bertujuan menganalisis hubungan riwayat paparan pestisida dengan IQ siswa SD di area pertanian hortikultura Kecamatan Ngablak Magelang. Metode: Penelitian ini merupakan analitik observasional dengan pendekatan desain cross sectional. Populasi penelitian adalah siswa sekolah dasar kelas 4-5 di KecamatanNgablak, sampel penelitian siswa kelas 4 - 5 SDN Kanigoro 1 dan 2 sebanyak 44 siswa berdasarkan Cluster Sampling. Data dikumpulkan dengan wawancara menggunakan instrumen kuesioner dan test IQ metode CFIT Skala 2A. Analisis data menggunakan uji statistik Chi-Square dan Fisher's Exact. Hasil: Sebanyak 50% siswa memiliki kebiasaan bermain di area pertanian dengan kategori buruk, 52,3 % memiliki kebiasaan kebersihan diri dengan kategori buruk, 56,8% diketahui masa paparan dialami sejak dalam kandungan, dan 38,6% siswa memiliki kategori IQ rendah. Hasil uji Chi-Square yang berhubungan dengan IQ siswa yaitu kebiasaan bermain di area pertanian (p-value=0,002); RP(4,667), kebiasaan kebersihan diri (p-value=0,001); RP(6,484), dan masa paparan (p-value=0,016); RP(3,547). Secara multivariat terdapat hubungan kebiasaan bermain di area pertanian OR=5,476 dan kebersihan diri OR=10, 190 dengan IQ siswa besar probabilitas 83 %. Simpulan: Perilaku siswa dalam hal ini kebiasaan bermain di area pertanian, kebiasaan kebersihan diri serta masa paparan pestisida berhubungan dengan IQ siswa.

Abstract

Latar belakang: Pasar Tradisional Kedunggalar dikategorikan sebagai pasar tipe A. Hasil pengamatan menunjukkan bahwa pengelolaan sampah masih belum sesuai dengan Peraturan Menteri Pekerjaan Umum Republik Indonesia Nomor 03/PRT/M/2013 diantaranya tidak adanya proses pemilahan sampah dan pengolahan sampah serta banyak dijumpai sampah yang berserakan di lingkungan pasar. Penelitian ini bertujuan  untuk mengevaluasi pelaksanaan pengelolaan sampah di Pasar Tradisional Kedunggalar.Metode: Penelitian ini merupakan observasional bersifat deskriptif. Metode yang digunakan adalah kuantitatif dan kualitatif dengan  pendekatan cross sectional. Subjek penelitian ini adalah pegawai Dinas Perdagangan dan Pengelolaan Pasar, Pengelola Pasar, Petugas kebersihan pasar, dan Pedagang Pasar. Pengambilan sampel menggunakan teknik purposive sampling . Objek penelitian ini adalah karakteristik sampah dan aspek pengelolaan sampah.Hasil: Berdasarkan hasil penelitian, sumber penghasil sampah adalah los, kios, dasaran, kamar mandi, kantor pengelola pasar, gudang, halaman, parkiran, dan mushola dengan jenis sampah berupa sampah organik, anorganik dan B3 (Bahan Berbahaya dan Beracun). Jumlah sampah rata-rata yang dihasilkan mencapai 64,576 kg/hari. Hasil penelitian menunjukkan bahwa pengelolaan sampah dilakukan dari operasional kelembagaan, hukum, keuangan, dan peran serta masyarakat. Penerapan pengelolaan sampah meliputi pemilahan, pengumpulan, pengangkutan, pengolahan, dan pembuangan akhir sampah yang tidak sesuai dengan peraturan yang berlaku.Simpulan: Hasil penilaian pelaksanaan aspek pengelolaan sampah mendapatkan presentase 72,60% untuk kategori sesuai dan 27,39% untuk kategori belum sesuai. Presentase kesesuaian tidak sesuai dan belum memenuhi syarat berdasarkan Peraturan Menteri Pekerjaan Umum Republik Indonesia Nomor 03/PRT/M/2013, yang merupakan minimal 80%.Kata kunci: Pengelolaan; Sampah; Pasar; Evaluasi ABSTRACT Title: Evaluation Of Aspect Of Waste Management In Kedunggalar Traditional Market, Kedunggalar District, Ngawi Regency, East JavaBackground: The Kedunggalar Traditional Market is categorized as a type A market. The results of the observations show that waste management is still not in accordance with the Regulation of the Minister of Public Works of the Republic of Indonesia Number 03/PRT/M/2013 including the absence of waste sorting and waste processing and there is a lot of garbage scattered around the environment. market. This study aims to evaluate the implementation of waste management in Kedunggalar Traditional Market.Method: This research is descriptive observational. The method used is quantitative and qualitative with a cross sectional approach. The subjects of this study were employees of the Department of Trade and Market Management, Market Managers, Market Cleaning Officers, and Market Traders. Sampling using purposive sampling technique. The object of this research is the characteristics of waste and aspects of waste management.Result: Based on the results of the research, the sources of waste generation are booths, kiosks, foundations, bathrooms, market management offices, warehouses, yards, parking lots, and prayer rooms with types of waste in the form of organic, inorganic and B3 (Hazardous and Toxic Materials) waste. The average amount of waste produced reaches 64.576 kg/day. The results showed that waste management was carried out from institutional operations, law, finance, and community participation. The implementation of waste management includes sorting, collecting, transporting, processing, and final disposal of waste that is not in accordance with applicable regulations.Conclusion: The results of the assessment of the implementation of the waste management aspect obtained a percentage of 72.60% for the appropriate category and 27.39% for the unsuitable category. The percentage of conformity does not match and does not meet the requirements based on the Regulation of the Minister of Public Works of the Republic of Indonesia Number 03/PRT/M/2013, which is a minimum of 80%.Keywords: Management; Waste; Market; Evaluation

Abstract

Dyslipidemia is a risk factor for cardiovascular disease is the leading cause of death in the world and in Indonesia. Treatment in some patients experience side effects include nausea, vomiting, headache, muscle pain and flatulence necessitating alternative or complementary medicine. Red gedi leaf is one of the medicinal plants which contain tannins, polyphenols and flavonoids were allegedly able to affect the lipid profile (cholesterol, triglycerides, LDL and HDL). This study aims to determine the effectiveness of red-leaf gedi (Abelmoschus manihot L.) in the form of pasta and tea with the content measured against changes in lipid profile rat (Rattus novergicus) hyperlipidemia, so it can be used as an alternative or complementary medicine in cases of dyslipidemia. The study was quantitative with true experimental method. Design or study design is randomized two groups and one positive control pretest-posttest design. Subject of research that 18 rats (Rattus novergicus) male wistar strain were divided into three groups: those fed atherogenic simvastatin and simvastatin 0.10 mg, the group given feed atherogenic pasta and red gedi leaves pasta 128 mg containing 100 mg of flavonoids , as well as the group given feed atherogenic tea and red gedi leaves tea 4.5 g containing 100 mg of flavonoids for 7 days. Lipid profile checked by CHOD-PAP method. Data were analyzed using paired t test, ANOVA and post hoc repeated. Decreased levels of cholesterol with simvastatin (41.81%) were significantly (p = 0.000) and better than the pasta (11.60%; p = 0.011) and tea (7.37%; p = 0.005). Decreased levels of triglycerides and simvastatin (41.82%) were significantly (p = 0.023) and equally well with pasta (38.09%; p = 0.006) but better than tea (14.22%; p = 0.358). Decrease in LDL cholesterol with simvastatin (80.99%) were significantly (p = 0.000) and at better than pasta (56.79%; p = 0.001) but better than tea (0.41%; p = 0.945). Increased levels of HDL using simvastatin (17.67%) but not significant (p = 0.138) and less well than pasta (45.66%; p = 0.004) but better than the tea is decreasing (25,86%; p = 0.030). Conclusion: Formula pasta red gedi leaf can be classified standardized herbs that can be used as a complementary alternative medicine cases of dyslipidemia. Formula red gedi leaf tea which is below the standard herbal medicine, research needs with increasing dose and duration of administration.

Abstract

The biggest case on urinary clinic is urolithiasis. Based on the medical record data of Kebumen Hospital, there were 91 patients, consist of 65 men (71.43 %) and 26 women (28.57 %) who treated as urolithiasis patiens in 2013. Urinary tract stone formation is influenced by intrinsic and extrinsic factors. Purpose : To analyze the risk factors of urolithiasis in Redisari village, Rowokele,  Kebumen district.This was an observational study with cross sectional design. The sample were 80 people taken by purposive sampling technique. The incidence of urolithiasis as the dependent variable, while the amount of intake of drinking, Ca content of drinking water, habit of holding urine, history of hypertension, history of urinary tract infections, consumption of supplements as independent variables. Data were collected through interviews and laboratory tests. Analyzed using univariate and bivariate analysis.The laboratory examination of urine respondents, positive Ca oxalate urine 34 (42.5%) and negative 46 (57.5%). Respondents with a positive urinary Ca oxalate, 51% its drinking water intake is less than 2 liter dilay whereas 24%  drinking water intake enough. The bivariate analysis of people who drink less than 2 liters daily at risk of suffering from urolithiasis, p value = 0.035 and PR = 2.112 (95% CI = 1.050 - 4.249). Those who drink less than 2 liters daily at risk for urinary tract stones (urolithiasis). It is recommended for adults to drink at least 2 liters daily to avoid urolithiasis. Keywords : Risk factors, Urolithiasis, Redisari

Abstract

Background : Semarang city as the province and industry city have high mobility, which the amount of vehicle increases at rate of 5 to 9 % per year. Vehicles increases in Semarang city have potency for air pollution so that it would have effect to our health. Air pollution occurred caused by transportation would affect lung function suffered by worker around the street, such as cloister merchant. This research aimed to identify difference of lung function capacity on cloister merchant based on totalambient dust level in Semarang City National Street.Methods: This research was an observational research using a cross sectional approach. Analysis unit of this research were 3 National Street in Semarang City (Kaligawe Street, Siliwangi Street, and Perintis Kemerdekaan Street) also 63 cloisters merchant were taken in purposive sampling.Result : Dust concentration data obtained by direct measurement to three Semarang City National Streets (Kaligawe Street= 624,7 μg/m3, Siliwangi Street= 533 μg/m3, Perintis Kemerdekaan Street= 377,2 μg/m3). While, lung function data obtained with examine the lung function of cloister merchant using spirometri (μ %KVP=92,75, SD %KVP=16,861 dan μ %VEP1=99,27, SD %VEP1=19,345 ) and other data obtained by interview. Data analysis performed using one-way Annova (=0,05).The statistic result by one-way Anova performed following result: there was no significant difference of prediction value of %KVP (df=62; p=0,110) and prediction value of %VEP1 (df=62; p=0,829) on cloisters merchant based on total ambient dust level in three Semarang City National Street.Conclusions: Nutrient status, examination habit, homogeneity of respondent, also respondent disease track record may become potential factor in reinforcing pulmonary function disorders. Conclusion there are no statistical difference of respondent’s lung capacity, even though total ambient dust concentration in three Semarang city national street are different.Keywords : Lung function capacity,dust, cloister merchant

Abstract

Abstract: Long Bean Leaves, Haemoglobin, Pregnancy Second Trimester. Anemia in pregnancy is called the 'potencial danger of mother and child' because anemia is a serious problem and need attention from all parties involved in maternal and child health services. WHO reported that the prevalence of anemia in pregnancy in the world amounted to 55% and tended to increase with increasing gestational age. To determine the effect of bean leaf consumption to increased hemoglobin levels of second trimester- pregnant women with anemia in Puskesmas Polanharjo working area. This research was Quasy experiment with a pretest posttest control group design. This research used a treatment group is the group of second trimester-pregnant women with anemia has given long bean leaves consumption as many as 15 respondents and the control group is the group of pregnant women with anemia second trimester without additional Fe tablet intake as many as 15 respondents. There is a difference between the average of hemoglobin levels before the intervention group consumed long beans leaves with an average of hemoglobin levels in the intervention group after consuming long beans leaves. By eating 350 grams of long beans leaves for one month (28 days) may help prevent anemia in pregnant women.
