Abstract

Obesity becomes a global epidemic nowadays. The high-fat diet is used as an alternative therapy for obesity. The optimal composition of a high-fat diet to reduce body weight is still unknown. This study aimed to determine which components of a high-fat diet can decrease body weight, visceral fat, and PPARG expression of visceral fat. This study was conducted at the Faculty of Veterinary Medicine, Universitas Airlangga, for three months by using a randomized post-test only control group design. Fifty male mice, 2-3 months old, 18-30 grams were adapted for one week given standard diet AIN93-M, then mice were divided into five groups, namely K1 (control group, 12% fat, 20% protein, 62% carbs); K2 (30% fat, 60% proteins, 0% carbs); K3 (45% fat, 45% protein, 0% carbs);  K4 (60% fat, 30% protein, 0% carbs); and K5 (75% fat, 15% protein, 0% carbs). Bodyweight was measured before and after treatment, then the visceral fat and PPARG expressions were evaluated. Statistical comparisons were performed using Statistical Package for the Social Sciences (SPSS) software. After treatment, there were forty-three mice. The body weight and visceral fat weight of the mice with a high-fat diet were decreased. The most significant changes in body weight were in K4 with -9,60 ± 3,806 grams reduction. The bodyweight of mice in K5, slightly increased than K2-K4. This could be caused by the hormesis phenomenon. PPARG expressions decreased in groups with a high-fat diet but increased in K5. The composition of a high-fat diet in group K4 was the most optimal to decrease the body weight, visceral fat, and PPARG expressions in mice

Abstract

Highlight:The differences in intensity physical exercise mechanisms associated with increased irisin secretion in overweight and obese subjects were determined. The secretion of irisin in the right intensity blood on obesity can be reduced because the calories were balanced. Abstract:Physical exercise is a non-pharmacological therapy that can secrete various types of myokines to treat obesity problems. One of the myokines that play a role is irisin. Irisin is a polypeptide hormone with 112 amino acid residues that are synthesized in skeletal muscle after the proteolytic precursor cleavage of fibronectin type III domain-containing protein 5 (FNDC5). The release of irisin in the blood circulation will stimulate the browning process in white fat tissue by inducing the expression of uncoupling protein-1 (UCP-1) through signaling p38 mitogen-activated protein kinase (p38-MAPK) to increase energy expenditure, thermogenesis and reduce fat accumulation. This study described the differences in intensity of physical exercise mechanisms associated with the increased irisin secretion in overweight and obese subjects. This study was designed as a literature review that involved studies from research journals in the last 10 years concerning humans from some databases, such as Science Direct, PubMed, and Google Scholar. This study also discussed the relationship between the intensity of physical exercise and the synthesis, secretion, circulation, and regulation of irisin in preventing obesity.

Abstract

Introduction: Ghrelin has been recognized to have a major influence on energy balance. Hence, the discovery of various regulatory factors that control ghrelin secretion may have major implications for the development of drugs and diet control. This study aims to analyze differences in ghrelin secretion pattern and prevention of hunger in the provision of breakfast in low-calorie breakfast, low energy density and high energy density in obese female adolescents. Methods: Subjects were female students aged 18-22 years with a BMI=25 kg/m², receiving low-calorie breakfast with low (n=8) and high energy density (n=8). Before the breakfast, subjects fasted for 12 hours and were subjected to plasma ghrelin level and hunger measurement using visual analog scale (VAS). Breakfast was done at 08.00, divided into three parts and each was taken for 5 minutes. Measurement of plasma ghrelin was carried out again post prandial (PP) at 2 hours and 4 hours. VAS filling was done 1 hour PP, 2 hours PP, 3 hours PP, and 4 hours PP.Result: There were differences in plasma ghrelin level, but not significant. VAS 1 analysis of hunger (p=0.040) at 4 hours PP and VAS 3 of satiety (p=0.025) was significantly different at 3 hours PP. Conclusion: Food density did not affect the plasma ghrelin levels. Low density foods are more effective to prevent hunger in 4 hours PP and increases satiety in 3 hours PP.

Abstract

An overweight individual has morphological changes in the muscle resulting functional impairment and can cause reduction in body balance system. Balance is an ability to maintain both static and dynamic equilibrium on various positions. An overweight person will experience a decrease in abdominal muscle strength and center of gravity shift. Physical training has positive effects to increase body balance. Stretching exercise maintains and increases the elasticity of muscle, tendon, fascia, ligament and joint motion and improve balance. Resistance exercise increases muscle performance which has benefit effect on balance system dynamically. The proprioceptive, an important part of balance maintenance to feel joint position or body when moving, can be improved by training that performed using balance board. Mechanoreceptor that is stimulated and the adaptation of the body's balance control that is influenced by balance board training results a good proprioceptive.

Abstract

Introduction: Coronary heart diseases continue to be the rising cause of mortality amongst Indonesian population, alongside with the increasing number of diabetic patients. The first line management of ACS is percutaneous coronary intervention (PCI), however previous have shown that diabetic patients have worse outcomes after therapy compared to non-diabetic patients. This study aims to compare the clinical outcomes between acute coronary syndrome (ACS) patients with diabetes and those without diabetes following percutaneus coronary intervention therapy. Methods: This study used cross sectional observational approach collecting records of ACS patients that underwent percutaneous coronary intervention in RSUD Dr Soetomo Surabaya from January 2018 to December 2019. Data regarding a patient’s age, gender, diabetic status, location of lesion, revascularization status, and clinical outcome were collected. Those with missing or incomplete data were excluded from the study. A total of 55 patients were included and analyzed, Results: amongst 55 patients that underwent PCI observed, 23 were diabetic and 32 were non-diabetic. Study has shown that diabetic patients have higher mortality rate compared to those without diabetes (6 patients vs. 1 patient, p=0,072) however based on the result analysis the p value of >0,05 showed no significant relationship between patients’ diabetic status and the clinical outcome following PCI therapy. Study has also shown that diabetic patients are more likely to undergo staged PCI (56,5%), than total revascularization PCI. Conclusion: Study has found that diabetic ACS has a higher mortality rate compared to those without diabetes, however analitycal studies found no significant relationship between the two variables. Further studies should be performed with higher number of patients to accurately investigate the relationship between diabetes melitus and PCI outcomes.
