Abstract

ABSTRACT Non-facility based childbirth remains a maternal health problem, particularly in rural areas. Delivery outside health facilities increases the risk of maternal and neonatal morbidity and mortality due to limited access to skilled health personnel and adequate obstetric services. This study aimed to analyze the determinants influencing non-facility based childbirth among mothers in the working area of UPT Salbait Primary Health Center, Timor Tengah Selatan Regency. This study employed a quantitative case-control design. The study population consisted of all mothers who delivered in the working area of UPT Salbait Primary Health Center, with a total sample of 80 respondents, including 40 cases (non-facility based delivery) and 40 controls (facility based delivery). The variables studied included maternal knowledge, distance to health facilities, economic status, antenatal care (ANC) visits, participation in maternity classes, and parity. Data were analyzed using the Chi-Square test. The results showed significant associations between maternal knowledge and non-facility-based childbirth (p = 0.000), distance to health facilities (p = 0.000), economic status (p = 0.011), ANC visits (p = 0.000), participation in maternity classes (p = 0.000), and parity (p = 0.000). Mothers with low knowledge, long distance to health facilities, low economic status, incomplete ANC visits, non-participation in maternity classes, and high parity were more likely to choose non-facility based delivery. In conclusion, non-facility based childbirth is influenced by predisposing, enabling, and need factors. Strengthening maternal health education, improving antenatal care services, and optimizing maternity class programs are necessary to promote facility based delivery. Keywords: Antenatal Care, Health Facilities, Maternal Knowledge, Non-Facility-Based Childbirth.  ABSTRAK Persalinan yang dilakukan di luar fasilitas kesehatan masih menjadi permasalahan kesehatan ibu, khususnya di wilayah pedesaan. Persalinan non fasilitas kesehatan berisiko meningkatkan angka kesakitan dan kematian ibu serta bayi akibat keterbatasan akses terhadap tenaga kesehatan terlatih dan pelayanan obstetri yang memadai. Penelitian ini bertujuan untuk menganalisis determinan yang memengaruhi persalinan non fasilitas kesehatan pada ibu di wilayah kerja UPT Puskesmas Salbait Kabupaten Timor Tengah Selatan. Penelitian ini menggunakan desain kuantitatif dengan pendekatan case control. Populasi penelitian adalah seluruh ibu yang melahirkan di wilayah kerja UPT Puskesmas Salbait dengan jumlah sampel sebanyak 80 responden, terdiri dari 40 responden kelompok kasus (persalinan non fasilitas kesehatan) dan 40 responden kelompok kontrol (persalinan di fasilitas kesehatan). Variabel yang diteliti meliputi tingkat pengetahuan, jarak ke fasilitas kesehatan, kemampuan ekonomi, kunjungan antenatal care (ANC), keikutsertaan kelas ibu hamil, dan jumlah paritas. Analisis data dilakukan menggunakan uji Chi-Square. Hasil penelitian menunjukkan bahwa terdapat hubungan yang signifikan antara tingkat pengetahuan dengan persalinan non fasilitas kesehatan (p = 0,000), jarak ke fasilitas kesehatan (p = 0,000), kemampuan ekonomi (p = 0,011), kunjungan ANC (p = 0,000), keikutsertaan kelas ibu hamil (p = 0,000), serta jumlah paritas (p = 0,000). Ibu dengan pengetahuan rendah, jarak tempat tinggal jauh, kemampuan ekonomi rendah, kunjungan ANC tidak lengkap, tidak mengikuti kelas ibu hamil, dan paritas tinggi cenderung memilih persalinan di non fasilitas kesehatan. Kesimpulan penelitian ini menunjukkan bahwa persalinan non fasilitas kesehatan dipengaruhi oleh faktor predisposisi, faktor pemungkin, dan faktor kebutuhan. Diperlukan upaya peningkatan edukasi kesehatan ibu, penguatan layanan antenatal care, serta optimalisasi program kelas ibu hamil untuk mendorong persalinan di fasilitas kesehatan. Kata Kunci: Antenatal Care, Fasilitas Kesehatan, Persalinan Non Fasilitas Kesehatan, Pengetahuan Ibu.

Abstract

Unwanted teenage pregnancy refers to a pregnancy that occurs when a teenager (typically aged 10–19 or 15–19, depending on the study’s definition) did not intend or plan to become pregnant at the time of conception. This phenomenon is not merely a medical issue—it is a social, economic, and educational problem because its consequences are often long-term for adolescents, the child born, families, and society. Many adolescent pregnancies occur in contexts where access to reproductive health education, contraceptive services, and social protection remains limited; social factors such as poverty, gender norms, early marriage, and peer pressure also play a significant role. Individual awareness of the risks and consequences of unintended pregnancy plays a crucial role in shaping preventive behavior. Motivation can be defined as an internal drive within a person to act and behave. Motivation can also be defined as a need—a potential within a person that requires a response. Intention describes an individual’s readiness to act. This study employs a qualitative approach with a phenomenological design. This approach was chosen because the research focuses on a deep understanding of adolescents’ subjective experiences regarding their motivation, intention, and perceptions of unintended pregnancy. Research data were collected from 10 female respondents at State High School 1 Nekamese, Kupang District. Ratings provided by respondents to assess adolescents’ awareness, motivation, and intentions regarding unintended pregnancy. This study involved primary respondents, peers, and guidance counselors. The study was conducted to assess the influence of parents, peers, and teachers on adolescents’ motivation and intentions to prevent unintended pregnancy at State High School 1 Nekamese, Kupang Regency.

Abstract

This quasi-experimental study with a non-equivalent control group aimed to analyze the effects of omega-3 supplementation on early childhood development, focusing on gross motor skills, fine motor skills, language development, and social skills among stunted children aged 1–3 years in the Bakunase Health Center area. Malnutrition remains a serious issue in Indonesia, with 20.6% underweight, 18.4% stunted, and 9.4% wasting toddlers reported in Kupang City in 2024. A total of 20 participants were divided into treatment and control groups (10 each), and data were collected using questionnaires and analyzed with the Wilcoxon Signed Rank and Mann-Whitney tests. Results showed that omega-3 supplementation significantly improved language (p = 0.034) and social skills (p = 0.046) in the treatment group, with no significant changes observed in gross (p = 1.000) or fine motor skills (p = 0.317). No improvements were found in any domain in the control group, while the Mann-Whitney test confirmed significant differences between the two groups in language and social development (both p = 0.012).

Abstract

Occupational safety is a crucial aspect of loading and unloading activities at ports, which inherently carry high levels of risk. This study aimed to analyze the relationship between individual characteristics and safety climate with safety behavior among loading and unloading workers at Tenau Port, Kupang City. A quantitative study with a cross-sectional design was conducted among loading and unloading workers at Tenau Port. The study population consisted of all active workers registered in the TKBM Cooperative, and a sample of 73 respondents was selected using a random sampling technique. Data were collected using structured questionnaires and analyzed using Chi-Square and logistic regression tests. The results showed that most respondents had good individual characteristics (82.2%) and perceived a strong safety climate (74.0%). Bivariate analysis indicated a significant relationship between individual characteristics and safety behavior (p = 0.029) as well as between safety climate and safety behavior (p = 0.008). Multivariate analysis showed that safety climate was the most dominant factor associated with safety behavior (p = 0.038; Exp(B) = 5.312; 95% CI = 1.101–25.634). Workers who perceived a stronger safety climate were more likely to demonstrate compliant safety behavior than those who perceived a weaker safety climate. In conclusion, individual characteristics and safety climate were significantly associated with safety behavior among loading and unloading workers at Tenau Port, Kupang City. Strengthening workplace safety culture and improving workers’ competencies are recommended to enhance safety compliance.

Abstract

ABSTRACTSmoking behavior remains a persistent issue even within healthcare environments, such as the Vera Cruz Inpatient Community Health Center in Dili, Timor–Leste. While healthcare workers are expected to act as role models for healthy behavior, smoking practices still occur within health service areas.This study aimed to analyze the factors influencing smoking behavior among healthcare workers and visitors using the Precede-Proceed model.A quantitative, descriptive-correlational, and cross–sectional study was conducted involving 92 respondents selected randomly from a population of 118. Data were collected using a validated questionnaire and analyzed using univariate, bivariate (Chi-square), and multivariate logistic regression tests.The results revealed that 46.7% of respondents were current smokers, while 42.4% were non–smokers. Significant factors associated with smoking behavior included knowledge of smoking hazards (p = 0.017), health promotion (p = 0.042), anti–smoking policy enforcement (p = 0.031), and stress level (p = 0.022). Multivariate analysis showed that respondents with low knowledge (OR = 3.45), poor health promotion exposure (OR = 2.75), absence of anti–smoking policies (OR = 2.56), and moderate to severe stress levels (OR = 3.21) were significantly more likely to engage in smoking behavior. Observational findings indicated weak policy enforcement, ineffective health promotion, and stress as primary drivers of continued smoking. The study concludes that smoking behavior in healthcare settings is a complex issue influenced not only by individual knowledge but also by institutional policy strength, health communication effectiveness, and mental health conditions. It recommends improving policy enforcement, designing contextual health promotions, and providing psychosocial support to create a truly smoke-free healthcare environment. Keywords: Health Promotion, Healthcare Workers, Policy Enforcement, Smoking Behavior, Stress.  ABSTRAK Perilaku merokok masih menjadi masalah yang persisten bahkan di lingkungan pelayanan kesehatan, seperti di Puskesmas Rawat Inap Vera Cruz di Dili, Timor–Leste. Meskipun tenaga kesehatan diharapkan menjadi teladan dalam perilaku hidup sehat, praktik merokok masih ditemukan di area pelayanan kesehatan.Penelitian ini bertujuan untuk menganalisis faktor-faktor yang mempengaruhi perilaku merokok pada tenaga kesehatan dan pengunjung dengan menggunakan model Precede-Proceed. Penelitian ini menggunakan pendekatan kuantitatif dengan desain deskriptif korelasional dan pendekatan cross-sectional. Penelitian melibatkan 92 responden yang dipilih secara acak dari populasi sebanyak 118 orang. Data dikumpulkan menggunakan kuesioner yang telah divalidasi dan dianalisis menggunakan analisis univariat, bivariat (uji Chi-square), serta regresi logistik multivariat.Hasil penelitian menunjukkan bahwa 46,7% responden merupakan perokok aktif, sedangkan 42,4% merupakan non-perokok. Faktor yang berhubungan secara signifikan dengan perilaku merokok meliputi pengetahuan tentang bahaya merokok (p = 0,017), promosi kesehatan (p = 0,042), penegakan kebijakan anti-merokok (p = 0,031), dan tingkat stres (p = 0,022). Analisis multivariat menunjukkan bahwa responden dengan pengetahuan rendah (OR = 3,45), paparan promosi kesehatan yang kurang (OR = 2,75), tidak adanya kebijakan anti-merokok (OR = 2,56), serta tingkat stres sedang hingga berat (OR = 3,21) memiliki kemungkinan lebih besar untuk melakukan perilaku merokok. Hasil observasi juga menunjukkan bahwa lemahnya penegakan kebijakan, promosi kesehatan yang tidak efektif, serta stres menjadi pendorong utama berlanjutnya perilaku merokok.Penelitian ini menyimpulkan bahwa perilaku merokok di lingkungan pelayanan kesehatan merupakan masalah yang kompleks yang dipengaruhi tidak hanya oleh pengetahuan individu, tetapi juga oleh kekuatan kebijakan institusi, efektivitas komunikasi kesehatan, serta kondisi kesehatan mental. Oleh karena itu, diperlukan peningkatan penegakan kebijakan, perancangan promosi kesehatan yang kontekstual, serta penyediaan dukungan psikososial untuk menciptakan lingkungan pelayanan kesehatan yang benar-benar bebas asap rokok. Kata Kunci: Perilaku Merokok, Stres, Promosi Kesehatan, Penegakan Kebijakan, Tenaga Kesehatan, Pengunjung, Timor-Leste.

Abstract

Background: The informal sector, characterized by its lack of structure, formality, and legal recognition, often receives inadequate attention regarding occupational health. In Indonesia, over 60% of the population resides in coastal areas, with the majority working as fishermen. Due to the inherently high-risk nature of fishing at sea, fishermen are frequently exposed to unpredictable ocean conditions, making them susceptible to various work-related health issues, including musculoskeletal disorders, dermatitis, keratopathy, and barotrauma. Purpose: To investigate the factors linked to the occurrence of occupational diseases among fishermen. Method: A descriptive correlational, specially focusing on cross-sectional method. The study took place in December 2024 in Kupang City. The independent variable was knowledge, duration of work, unsafe behaviors, and unsafe working conditions. The dependent variable included the incidence of occupational diseases. Using random sampling and Lameshow formula 89 respondents were selected who meet specific inclusion and exclusion criteria. The analysis proceeded with a univariate test to determine the frequency distribution of respondent’s characteristics, followed by bivariate analysis using chi-square test. Results: The study found significant associations between length of work and occupational diseases (p = 0.011), and between behavior and occupational diseases (p = 0.001), while knowledge and environmental conditions showed no significant effects. Conclusion: Occupational diseases result from multifactorial determinants, including exposure duration, behavior, and environmental conditions. For informal workers such as fishermen, government involvement in risk management and safety governance is essential. Preventive measures should focus on skill development, occupational safety culture, proper use of protective equipment, health education, and access to health insurance.
