Abstract

Non communicable diseases such as diabetes mellitus and hypertension constitute a major health burden in Indonesia and place growing pressure on the national health insurance system. BPJS Kesehatan launched the Chronic Disease Management Program Prolanis to strengthen monitoring, medication adherence, and patient education through primary care services, yet national reports indicate that only a small fraction of eligible patients are registered and even fewer achieve clinical control. This study diagnoses the gap between the target and actual coverage of Prolanis at X Primary Health Center. A qualitative descriptive case study was conducted from January to September 2025 with purposively selected participants, including one program coordinator, six healthcare workers, four community health volunteers, and four program participants. Data were collected through in depth interviews, observations of program activities, and analysis of monthly reports and participant registers, then thematically analyzed at organizational, team, and individual levels. With 298 diabetes and 1,250 hypertension patients, the target of five percent controlled participants was exceeded for diabetes only in February and March and was never met for hypertension. Enabling factors included leadership commitment, supportive policies, team cohesiveness, and staff motivation, while barriers involved the absence of integrated standard operating procedures, limited information tools, inefficient workflows, staff and training shortages, and low health literacy. Prolanis coverage at X Primary Health Center remains far below the target, indicating that organizational redesign and stronger community engagement are required to improve coverage and clinical control.

Abstract

This study aims to comprehensively examine the theory of motivational leadership and its relevance within the context of Islamic educational leadership. The discussion focuses on fundamental leadership concepts, modern and traditional leadership theories, motivation in leadership, and the application of Path Goal Theory in Islamic educational institutions. The method employed in this study is a literature review, drawing on scholarly books and national and international journal articles related to leadership, motivation, and Islamic educational management. Data were analyzed using a descriptive-analytical approach to obtain an in-depth understanding of the relationship between motivational leadership, Islamic values, and educational performance. The findings indicate that motivational leadership in Islamic education is not solely oriented toward academic achievement but also emphasizes character building as well as moral and spiritual development of learners. Educational leaders who are able to integrate Islamic values such as trustworthiness (amanah), justice, consultation (shura), and exemplary conduct are proven to enhance teachers’ and students’ motivation, foster a positive organizational culture, and improve performance and learning quality. Furthermore, the application of Path Goal Theory provides a strategic framework for Islamic educational leaders to adapt their leadership styles to the needs and characteristics of their followers. This study confirms that motivational leadership grounded in Islamic values is a relevant and strategic approach to improving the quality and effectiveness of Islamic educational institutions.

Abstract

Dengan penerapan strategi tersebut, diharapkan partisipasi peserta Prolanis di Klinik SMC Banyusari dapat meningkat secara berkelanjutan Kata Kunci: prolanis, analisis SWOT, Strategi Manajerial     ABSTRACT Low participation in the Chronic Disease Management Program (Prolanis) remains a major challenge for primary healthcare facilities, including SMC Banyusari Clinic in Denpasar. Clinic records show that the average monthly attendance rate of Prolanis participants is approximately 21.2%, which is below the minimum target of 50% set by the National Health Insurance Agency (BPJS Kesehatan). This study aims to analyze internal and external factors using a SWOT analysis to determine appropriate managerial strategies for improving the implementation of the Prolanis program at SMC Banyusari Clinic. This study employed a qualitative approach with a case study design. Data were collected through in-depth interviews, observations, and document reviews involving six informants, consisting of the clinic head, Prolanis officer, and Prolanis participants. The data were analyzed using the Internal Factors Analysis Summary (IFAS) and External Factors Analysis Summary (EFAS) matrices. The findings indicate that the clinic’s internal strengths outweigh its weaknesses, reflected by an IFAS score of 1.31, while external opportunities exceed existing threats, with an EFAS score of 1.12. These results place the clinic in a stable growth strategic position, enabling program improvement through the optimization of internal resources and the utilization of external opportunities. Recommended managerial strategies include strengthening participant-centered health education, enhancing the role of Prolanis officers, utilizing information technology, and developing collaboration with local communities and village authorities. Implementing these strategies is expected to sustainably increase Prolanis participant attendance at SMC Banyusari Clinic. Keywords: Prolanis, SWOT Analysis, Managerial Strategy

Abstract

Background: The Double Burden of Nutrition (DBGH) is a public health issue in urban areas of Indonesia. Hospitals are in a unique position to assess the clinical effects of DBGH. However, there are strong suspicions that medical records in hospitals significantly underreport this information. This situation hides the full extent of the problem, which then hinders the proper allocation of resourcesObjectives: T(1) to describe the epidemiological features of pediatric patients with DBGH (Malnutrition/E40-E46 and Obesity/E66) at Muhammadiyah General Hospital Malang (RSUMM) from 2019 to 2025, including diagnosed prevalence, age distribution, and sex; (2) to critically evaluate the diagnosed prevalence (N=1,092) by comparing it to community prevalence, considering the possibility of under-reporting; and (3) to analyze the patterns of comorbidities in the cases that were successfully recorded. Methods: This study used a quantitative, descriptive epidemiological design, specifically a cross-sectional approach. Data were gathered from the electronic medical records of pediatric patients, aged 0-18 years, who had been diagnosed with malnutrition (ICD-10 codes E40-E46) or obesity (E66) at RSUMM between January 1, 2019, and December 31, 2025. This process yielded a total sample size (N) of 1,092 cases. Results During the study, we found 1,092 cases of BGG in the medical records. Of the total, 671 cases (61.4%) were classified as Malnutrition (E40-E46), while 421 cases (38.6%) were classified as Obesity (E66). The highest rate of diagnosed cases was found in the 0-5 age group, which had a rate of 65.9%. Although the 1,092 cases seem substantial and are increasing, a critical examination reveals that this number is considerably lower than estimates of the community-level burden of BGG (e.g., national stunting prevalence in 2023 SKI >20%), suggesting considerable under-reporting. Analysis of the data shows a significant increase in BGG cases, going from 100 cases in 2019 to 250 cases by 2024. Acute Respiratory Infections (ARI/Pneumonia) and Diarrhea (Gastroenteritis) are the most common health problems seen with BGG.
