Abstract

Latar belakang Proses penyembuhan luka terjadi melalui beberapa tahapan dan dipengaruhi berbagai faktor lokal dan sistemik seperti benda asing, nutrisi, konsumsi obat-obatan. Kortikosteroid banyak digunakan secara bebas oleh masyarakat untuk berbagai tujuan. Penggunaan kortikosteroid dapat mempengaruhi proses penyembuhan luka, dengan cara menurunkan proses pembentukan fibroblas, menurunkan jumlah gerakan dan fungsi leukosit, mengurangi pergerakan polimorfonuklear (PMN) keluar dari kompartemen vaskular, dan mengurangi jumlah sirkulasi limfosit, monosit, dan eosinofil, terutama dengan meningkatkan gerakan sel radang keluar dari sirkulasi. Tujuan penelitian ini untuk melihat perbedaan proses penyembuhan luka secara histopatologi pada mencit (Mus musculus) yang diberi kortikosteroid dengan jangka pemberian yang berbeda. Metode Penelitian ini adalah penelitian ekserimental dengan desain post test only controlled group yang menggunakan 18 ekor mencit putih (Mus musculus) yang dibagi menjadi tiga kelompok, yaitu kelompok kontrol, perlakuan 1 dan perlakuan 2. Kelompok perlakuan 1 diberikan kortikosteroid dalam bentuk preparat deksametason dengan dosis 16 mg/kg/bb selama 10 hari, sedangkan kelompok perlakuan 2 diberikan deksametason dosis yang sama, selama 30 hari. Pada hari terakhir pemberian deksametason, semua kelompok, termasuk kontrol dilakukan luka insisi di punggung sepanjang 2 cm. Setelah hari ke-9 pembuatan luka, mencit diterminasi untuk diambil jaringan luka, dan dilakukan pemeriksaan histopatologik untuk menilai pembentukan jaringan granulasi dengan menghitung jumlah pembuluh darah baru, fibroblast dan sel radang menggunakan mikroskop cahaya pembesaran 400 kali pada 3 lapangan pandang. Hasil Neovaskularisasi yang terbentuk pada kelompok kontrol lebih sedikit dibandingkan kelompok perlakukan, sebaliknya jumlah fibroblast lebih banyak ditemukan pada kelompok kontrol. Jumlah netrofil pada kempok kontrol lebih sedikit dibandingkan kelompok perlakuan dan sebaliknya jumlah limfosit lebih banyak ditemukan pada kelompok kontrol dibanding perlakuan. Setelah dilakukan uji statistik menggunakan uji One Way ANOVA didapatkan perbedaan bermakna antar kelompok pada jumlah neovaskuler, fibroblas, neutrofil, limfosit, berturut-turut 0,007; 0,025; 0,009;

Abstract

Post-acne scars are a condition of skin layer damage due to the healing process of acne vulgaris, causing the skin to appear depressed. This condition disrupts facial aesthetics and may become permanent, thereby affecting the patient’s psychological condition and quality of life. This study aimed to determine the relationship between the severity of post-acne scars and the quality of life among medical students of FK UNAND, Padang, Indonesia. This study was an analytic observational study with a cross-sectional design conducted from June to December 2023. The population consisted of medical students of FK UNAND batch 2020–2022 who had acne scars. The total sample was 88 respondents selected using consecutive sampling. Data were collected using the Facial Acne Scar Quality of Life (FASQOL) questionnaire and Dysmorphic Concern Questionnaire (DCQ). Statistical analysis was performed using One-Way ANOVA and Fisher’s Exact Test. The results showed that most respondents were female and aged 21 years. The majority experienced mild acne scars (46.6%). The mean FASQOL score was 14.11. Most respondents felt disturbed by the presence of acne scars on their face. The DCQ results showed that 88.6% of respondents had no dysmorphic disorder features. There was no significant relationship between acne scar severity and quality of life (p=0.938) nor dysmorphic disorder (p=0.241). The study concluded that there was no relationship between the severity of post-acne scars and the quality of life among medical students of FK UNAND, Padang, Indonesia

Abstract

Background: Papillary thyroid carcinoma (PTC) is generally indolent, yet specific histological subtypes defined by the World Health Organization (WHO) are linked to aggressive behavior and poor prognosis. The loss of the cell-adhesion protein E-cadherin is a hallmark of the epithelial-to-mesenchymal transition (EMT), a process implicated in tumor aggression. However, its role in stratifying PTC subtypes versus its correlation with tumor stage remains a significant controversy in the literature. This study aimed to disentangle these two parameters by clarifying the relationship between E-cadherin expression and both histological phenotype and tumor stage. Methods: This was an observational, cross-sectional pilot study on 40 randomly selected, formalin-fixed, paraffin-embedded (FFPE) PTC cases from a 2024 cohort (N=74) at a tertiary hospital in Indonesia. All cases were re-evaluated and classified according to the WHO 5th Edition (2022) criteria as non-aggressive (n=34) or aggressive (n=6). E-cadherin expression was assessed by immunohistochemistry (IHC) using a standardized semi-quantitative scoring system (product of intensity and proportion) adapted from previous studies, with inter-rater reliability assessed (Cohen’s Kappa = 0.88). Scores were dichotomized as 'High' (n=25) or 'Low' (n=15). The association between E-cadherin expression and both histological subtype and AJCC 8th Edition tumor stage (Early: I/II [n=32] vs. Advanced: III/IV [n=8]) was analyzed using Fisher's Exact Test, with Odds Ratios (OR) and 95% Confidence Intervals (CI) calculated. Results: High E-cadherin expression was observed in 62.5% of cases. A statistically significant and strong association was found between E-cadherin expression and histological subtype (p=0.021; OR 12.0; 95% CI 1.2–118.9). Low E-cadherin expression was present in 83.3% (5 of 6) of aggressive-subtype tumors, versus only 29.4% (10 of 34) of non-aggressive subtypes. In contrast, no significant correlation was found between E-cadherin expression and advanced tumor stage (p=0.126; OR 3.67; 95% CI 0.7–18.6). Conclusion: Loss of E-cadherin expression is a significant biomarker associated with high-risk, aggressive histological phenotypes in PTC. Its lack of correlation with tumor stage, confirmed by an uncertain OR, suggests E-cadherin's role is indicative of an inherent tumor biological phenotype (aggressiveness) rather than a linear marker of tumor progression (stage). This dichotomy, likely reflecting EMT/MET plasticity, positions E-cadherin IHC as a powerful ancillary tool for pathological risk stratification.

Abstract

Background: Uterine smooth muscle tumor of uncertain malignant potential (STUMP) presents a diagnostic dilemma, especially in Southeast Asian populations, where delayed presentation often leads to advanced tumor burden. This study aimed to characterize the clinicopathological profile of STUMP in Indonesia using the 2021 WHO Classification and modified Stanford parameters. Methods: A descriptive study analyzed 37 STUMP cases diagnosed between 2023 and 2025 at a tertiary referral center in West Sumatra. A panel of pathologists re-evaluated archival slides for atypia, mitotic activity, necrosis, and growth patterns, establishing inter-observer reliability. Fisher’s Exact Test was employed to correlate tumor size with morphological markers of aggression. Results: The cohort demonstrated a distinct large-mass phenotype, with 54.1% of tumors exceeding 10 cm and a median diameter of 14.5 cm. Pathological review classified 56.8% of cases into WHO Group 4 (atypia with ambiguous mitosis). A statistically significant correlation was identified between tumor size >10 cm and the presence of infiltrative growth margins (81.1%; p=0.034). Conclusion: Indonesian STUMP cases are distinctively characterized by massive size and high rates of infiltrative growth, likely driven by prolonged natural history. The prevalence of ambiguous mitotic figures underscores the utility of the WHO Group 4 category in resource-limited settings. These findings advocate for aggressive surgical management and wider resection margins in large-mass variants to mitigate local recurrence risk.

Abstract

Background: Hepatocellular carcinoma (HCC) is the sixth most common cancer globally and shows significant regional variations in clinicopathological characteristics. Limited data exist on HCC patterns in Indonesia, particularly in West Sumatra. This study aimed to characterize the clinicopathological features of HCC at Dr. M. Djamil Hospital, Padang, from 2021to2024.Methods: A descriptive retrospective study was conducted using medical records and histopathological re-evaluation of 46 HCC cases. Specimens were examined for demographic data, tumor characteristics, histopathological subtypes according to the WHO 2019 classification, differentiation grade, lymphovascular invasion, and background liver pathology.Results: The median age was 56 years, with male predominance (60.9%). Most specimens were resection specimens (60.9%) with tumor size ≥4 cm in 47.8% of cases and single nodules in 52.1%. Clear cell (39.1%) and macrotrabecular massive (37.0%) were the predominant subtypes, contrasting with global data showing NOS predominance. Most tumors were moderately differentiated (63.0%), with lymphovascular invasion present in 65.2% and cirrhotic background in 58.7% of casesConclusion: HCC in West Sumatra demonstrates distinct histopathological patterns, with a predominance of clear cell and macrotrabecular massive subtypes and high rates of lymphovascular invasion. These finding suggest histopathological features associated with aggressive tumor behavior (particularly macrotrabecular massive subtype and lymphovascular invasion), although no clinical outcome or survival analysis was performed.

Abstract

Bitter melon (Momordica charantia) has been empirically proven for antimicrobial properties that can be used as an alternative medicine, while the changes that occur in Escherichia coli-infected mouse intestine after administering bitter melon have not been widely studied. The aim of this study was to determine the effect of Momordica charantia administration on histopathology of intestine of E. coli infected mice. This research was post-test control randomized group study involving 20 mice’s small intestine paraffin blocks which were divided into 4 groups consisted of control and different dosage of bitter melon juice, observed subsequently under a microscope to measure the of the small intestine lining and assess inflammation using modified Barthel Mandja scoring. This study found significant changes the height of mucosa and villi of mice small intestine after administration of 100 grams of bitter melon juice and the muscle layer and crypts after administration of 200 grams of bitter melon juice. The Barthel Manja score showed decrease of inflammation degree among P2 group, while increased inflammation degree found in P3 group. It can be concluded that administration of bitter melon juice has the potential to improve the intestinal histopathology of mice infected with Escherichia coli.
