Abstract

Background: Obesity is a worldwide epidemic problem that is increasing continuously. Obesity negatively affects several functions in the body, including reproductive function (fertility). The prevalence of infertile couples in Indonesia in 2013 was 15-25% with male infertility accounting for 25-30% of total infertility cases. Objectives: This study aimed to determine the association between obesity and infertility in productive-aged men. Methods: The research results from eight observational studies gathered in accordance with inclusion criteria, credible literature on the topic of obesity and male infertility, were examined in this study using the literature review method. Additionally, the exclusion criteria encompassed references that were not accessible in full text and related to animal studies. Discussion: Obesity can have a direct or indirect effect on male fertility through several mechanisms of hormonal profile changes. Moreover, the excessive conversion of androgens to estrogens caused by obesity can lead to an increase in aromatase activity and subsequent reproduction; this imbalance ultimately results in lower-quality sperm. Additionally, obesity causes high plasma leptin levels. Furthermore, sperm plasma membrane fluidity, impairing sperm motility, and increasing sperm deoxyribonucleic acid (DNA) damage are all negatively affected by leptin-induced increases in oxidative stress and the uncontrolled production of reactive oxygen species (ROS). Conclusions: An increased BMI increases the risk of decreased sperm parameters, which indicates a higher level of infertility in men. Obese men may increase the risk of changes in sperm parameters both quantitatively (volume, number, and concentration) and qualitatively (motility, morphology, and sperm DNA).

Abstract

Background: One of the basic capitals for the formation of quality human beings is the consumption of breast milk. The amount of breast milk produced is highly dependent on the mother's diet. This study aimed to identify the relationship between the nutritional status of breastfeeding mothers and breast milk adequacy in Indonesia. Subjects and Method: This was a systematic review using PICO model to formulate the research question. Population= breastfeeding mothers. Intervention= adequate BMI and MUAC. Comparation= poor BMI and MUAC. Outcome= breast milk adequacy. The database used is Google Scholar with keywords “Anthropometry” OR “BMI” OR “Breastfeeding mothers” OR “breast milk adequacy” OR “Indonesia”. There were 10 cross-sectional studies and 2 cohort studies published in 2011-2023 that met the inclusion criteria. Results: There is mostly a relationship between the nutritional status of breastfeeding mothers as measured by BMI and MUAC with the adequacy of breast milk. Conclusion: The adequacy of breast milk can be influenced by the nutritional status of the mother while breastfeeding. The nutritional status of breastfeeding mothers is no less important than the nutritional status of pregnant women so that risks such as stunting do not occur.

Abstract

The food insecurity problem is closely related to poverty. One of the communities belonging to the lower middle class in Indonesia is Fishermans. Stunting toddler is one of the chronic nutritional problems caused by low access to and affordability of food. Food and nutrition security is a unity, where nutrition is a very important element in improving the life quality of the population. The purpose of this study was to analyze the relationship between family characteristics, toddler characteristics, and food security with the incidence of stunting in toddlers. This research is included in the cross-sectional study design. The research sample was 87 fisherman households in the Ngemplakrejo Village, Pasuruan City, which were randomly selected using simple random sampling. Data were analyzed using chi-square correlation tests. The results showed that the stunting prevalence was still high at 43.7%. In addition, families in the category of severe food insecurity were also found, namely 16.1%. Based on the study results, there was a relationship between household food expenditure (p=0.027), birth body length (p=0.027), exclusive breastfeeding history (p=0.034) and household food security (p=0.000) with stunting in toddlers. The conclusion of this study is stunting in toddlers can occur due to factors such as family characteristics, toddler characteristics, and food security. This study recommends increasing in food security by paying attention to the adequacy of family food needs through coping strategy, household expenditure by prioritizing toddler nutrition and increasing household food security by providing training and skills in diversification of catch products and productive businesses.
