Abstract

Pregnant women with Chronic Energy Deficiency (CED) can cause health problems for both mother and baby. Among them are babies with low birth weight (LBW), who are at risk of stunting. The Pekalongan City Government issued Mayor Regulation Number 11 of 2023 concerning the Acceleration of Integrated Stunting Reduction, where the group of CED pregnant women is one of the targets of Stunting-prone Families who need specific and sensitive interventions. This study aimed to analyze implementation of the policy, especially sensitive interventions of CED pregnant women in Pekalongan City. This study used a descriptive qualitative method. The research was conducted from August to September 2024. The object of the study was the implementation of sensitive interventions of CED pregnant women which was analyzed with Van Metter-Van Horn policy implementation model. The research was conducted through in-depth interviews, observation and documentation. The determination of informants used a non-probability technique with a purposive method, and 11 informants were obtained. The stages in data analysis were data collection, reduction, presentation, data verification and conclusion drawn. The results of the study found that sensitive interventions in form of access to clean drinking water and sanitation, housing improvement programs, social assistance recipient programs and training facilitation for families of CED pregnant women are still not optimal. Pregnant women in CED had not been considered as a priority in obtaining the benefits of assistance programs. It needed to be improved again in policy socialization, quality and quantity of resources, coordination and communication, and improvement of the environmental situation.

Abstract

Central Java Province is one of the provinces that often experiences disasters. During the period 1 May 2023 – 31 May 2023, 72 incidents of strong winds, 8 incidents of land moving, 107 incidents of flooding, 94 incidents of landslides, 6 volcanic eruptions, and 2 incidents of fire were recorded. In disaster emergencies, the health needs of mothers and children are often overlooked. This study aims to analyze the flow of maternal and child health referrals in health crisis management in the focus location regencies in Central Java Province. This research is descriptive research with a qualitative approach. Data was collected through Focus Group Discussion (FGD) in March 2023. The research locations were Semarang Regency, Brebes Regency, Grobogan Regency, and Klaten Regency. The sample selection technique used was purposive sampling. The integrated referral system is not yet running optimally. The health referral flow carried out is not yet specialized in treating mothers and children. Pentahelix collaboration is carried out in managing health crises. The supporting factors in disaster management are the existence of policies, the availability of infrastructure for evacuating victims, as well as coordination from various sectors. Meanwhile, the inhibiting factors are the lack of trained human resources, limited logistics, and low public awareness regarding disasters. Thus, there is a need to develop a special referral pathway for maternal and child health involving multi-sectors, increase public awareness of disaster management, and strengthen Pentahelix collaboration in health crisis management.

Abstract

Incomplete basic immunization in children aged 11-23 months has potential to increase contracting dangerous diseases risk due to suboptimal immunity. Parents play an important role in ensuring the success of children immunization. This study purpose was to analyze the completeness of CBI based on parents' knowledge, attitude, perception, and satisfaction in immunizing their children. It’s a quantitative analytic study with cross sectional approach. The population were all children with sample of 685 children taken by purposive sampling technique from 31 districts in Central Java province. Data were collected through interviews and analyzed univariate with frequency distribution, bivariate with Chi-Square test, and multivariate with multiple logistic regression. A total of 38.7% children were not fully immunized, especially in booster immunization (73.4%), IPV (25.7%) and MR (15.9%). Partially, it was proven that parent’s knowledge, attitude, perception, and satisfaction were related to completeness of basic immunization. Simultaneously knowledge and attitude together affect the completeness of basic immunization in children with an overall effect of 42%. Parents' knowledge and attitudes are key factors for completeness of basic childhood immunization. Need to improving literacy and strengthening positive attitudes through structured IEC using personal approach, as well as involving key figures in the community in their support for immunization program. Utilizing communication technology and social media is an alternative that should be considered.

Abstract

Introduction: Maternal mortality rate (MMR) is an important indicator of the health and well-being of a society, and remains a serious global problem. Preeclampsia is a health problem that can occur in pregnant women who are more than 20 weeks pregnant. Preeclampsia is one of the leading causes of maternal death in Indonesia. The incidence of preeclampsia in pregnant women occurs due to not doing early detection. The importance of Antenatal Care (ANC) check-ups is recognised as a crucial preventive measure to ensure the health of mothers and babies, and to detect early complications during pregnancy. However, ANC service coverage in Indonesia is still below the national target, indicating the need to improve awareness and access to these services. Utilization of Antenatal Care (ANC) services needs to be done by pregnant women in order to detect early and carry out appropriate treatment of pregnant women's health problems, especially preeclampsia. Objective: To analyze the relationship between the compliance of pregnant women in utilizing antenatal care services and the incidence of preeclampsia. Method: This research is a literature review with a scoping review method using the PRISMA approach. The data used in this study used online database literature from Google Scholar, Science Direct, and Scopus with the last 5 years. Result: Based on several previous studies, low compliance of pregnant women in attending ANC visits can be a risk factor for preeclampsia. Pregnant women who are committed to regular check-ups during pregnancy can detect diseases and complications early, including preeclampsia. Quality and regular ANC services can detect pregnancy risks early and help prevent maternal mortality. In this study revealed that the greatest risk of preeclampsia that might occur in pregnant women who are not compliant in conducting ANC is 67 times, this is certainly very risky for the health of pregnant women and babies. Conclusion: This study cloncludes that regular Antenatal Care (ANC) visits are crucial to detect and manage complications early. The low rate of ANC visits should encourage healthcare providers to innovate in promoting ANC in an engaging and understandable way.

Abstract

Introduction: The COVID-19 pandemic has had a significant impact on public health, including in Pati District. Sociodemographic factors such as age, gender, education, employment, and economic status, as well as perceptions of vaccination side effects play an important role in determining the level of COVID-19 risk. An in-depth understanding of these different risk factors is essential to formulate effective and targeted mitigation strategies. Objective: This study aims to analyze how sociodemographic factors (age, gender, education, employment and marital status) and perceptions of vaccination side effects contribute to differences in COVID-19 risk factors in Pati District. Method: The type of research used in this study is quantitative research with a cross sectional approach. The sample size was 210 respondents. The instrument used was a questionnaire with an interview method. Data were analyzed using the Mann Whitney test for numerical data (knowledge and perception of side effects) and using the Chi-Square test for categorical data (age, gender, education, occupation and marital status). Result: The results of the relationship test show that the p value <0.05 between gender and perception of side effects on COVID-19 risk factors Conclusion: Sociodemographic factors and perceived side effects of vaccination play an important role in determining the level of COVID-19 risk in Pati District. Better education on the safety and benefits of vaccination and approaches tailored to sociodemographic characteristics can help reduce risk and increase vaccination participation in the community.

Abstract

Pregnant women and newborns are particularly vulnerable during public health emergencies in Indonesia, where there is a lack of specific guidance to support these groups when emergent threats (ET) arise. Health workers, crucial in delivering maternal and newborn health (MNH) services, often fail to anticipate and address the needs of these vulnerable populations adequately.A literature review analyzing 24 articles from PubMed, Dimension, and Web of Science published between 2016 and 2024 focused on emerging infectious diseases, maternal and newborn health, quality of care, referral systems, and small vulnerable newborns. The review identified substantial gaps in the skills and competencies needed to provide high-quality MNH services globally and in Indonesia. Key issues include non-compliance with existing guidelines, inefficient referral coordination, insufficient monitoring of sick mothers, and a lack of preparedness for emergent threats, all contributing to suboptimal MNH outcomes.These gaps could be addressed by adopting improved training and referral systems, as proposed in the new SVN conceptual framework. Essential actions to enhance MNH outcomes include improving guideline compliance, enhancing referral systems, and ensuring comprehensive training for healthcare providers. Developing an integrated tool tailored to Indonesia's needs that includes SVN, sick mothers, infrastructure, referral systems, and infectious disease management is vital. Implementing these strategies can significantly reduce adverse outcomes and enhance care quality for women and newborns during public health crises.

Abstract

Latar Belakang: Rumah sakit memiliki peran krusial dalam menyediakan layanan trauma center bagi pasien yang mengalami trauma atau kecelakaan kerja. Untuk meningkatkan kepuasan pasien, loyalitas, dan kualitas layanan, manajemen pemasaran rumah sakit perlu memahami faktor-faktor yang mempengaruhi perilaku konsumen dalam memilih layanan rumah sakit. Tujuan: Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang mempengaruhi pemilihan layanan di rumah sakit berdasarkan tinjauan literatur. Metode: Metode penelitian yang digunakan adalah scoping review dengan mengumpulkan artikel dari database Google Scholar dan portal Garuda. Artikel yang digunakan adalah publikasi dari tahun 2014 hingga 2023. Setelah dilakukan screening, berhasil dikumpulkan 12 artikel yang relevan. Hasil: Faktor-faktor yang mempengaruhi pemilihan layanan kesehatan di rumah sakit mencakup variabel produk, tempat, bukti fisik, tarif, promosi, layanan, dan proses. Rumah sakit perlu meningkatkan pelayanan medis dan penunjang, memperbaharui strategi harga, serta meningkatkan promosi melalui media sosial, papan nama, banner, brosur, dan poster. Kesimpulan: Untuk meningkatkan kepuasan pasien, rumah sakit perlu memperhatikan faktor-faktor tersebut dan membuat perbaikan dalam pelayanan serta promosi agar lebih menarik bagi konsumen

Abstract

Background: Exclusive breastfeeding coverage in Bengkulu City has decreased over the last three years. Mothers' confidence in their abilities and good mental health become an essential part of supporting exclusive breastfeeding.Objective: This study aims to examine the differences between self-efficacy and mental health of mothers based on their breastfeeding status, located in Bengkulu City.Methods: The researchers employed a cross-sectional approach to analyze the study. The sample consisted of 72 mothers with infants aged 0-6 months. The researchers selected the samples using a purposive sampling technique. Data collection was carried out using a digital questionnaire (via Google Forms) through validity and reliability tests. Data were analyzed using an independent t-test. The mean self-efficacy score of partially breastfeeding mothers is lower than that of exclusively breastfeeding mothers. The mean mental health score of partially breastfeeding mothers is higher than that of exclusively breastfeeding mothers.Results: The results of the independent t-test found that the average self-efficacy score of exclusively breastfeeding mothers was 55.58, and the average mental health score of partially breastfeeding mothers was 10.32.Conclusion: There was a difference in the self-efficacy and mental health scores of partially and exclusively breastfeeding mothers. A high self-efficacy score indicates high maternal confidence. Meanwhile, high mental health scores indicate that mothers experience symptoms of psychological disorders. The achievement of successful exclusive breastfeeding is closely related to boosting self-assurance and preserving a mother's mental well-being.

Abstract

Background: Excessive waiting time for prescription services at the pharmacy can negatively impact a hospital's reputation. Exceeding the minimum service standard for prescription service waiting time may indicate bottlenecks in the process, leading to inefficient service delivery.Objectives: This research aimed to determine the average waiting time for the prescription services for social health insurance patients and identify external and internal factors that prolong the prescription process.Methods: The research employed a mixed-method approach using an explanatory sequential design. Quantitative data was collected by measuring prescription waiting times from the submission of the prescription sheet by the patient to dispensing medicine to the patients. Qualitative data was gathered through in-depth interviews.Results: The results of the analysis of 100 prescriptions (38 compound and 62 non-compound) showed that the average waiting time for compound prescriptions was 50.42 minutes and 41.03 minutes for non-compound prescriptions. The longest stage in compound and non-compound prescription processes was prescription review, averaging 19 minutes from all outpatient clinic prescriptions sampels. Results of in-depth interviews revealed that incomplete prescription information by doctors, the total number of medicine items, distraction from patients, inadequate staff skills, and a shortage of prescription pharmacists contributed to delays in prescription services.Conclusion: Duration of prescription services for social health insurance that exceeded the standard time was observed in non-compound prescriptions, with bottlenecks occurring during the prescription review. Therefore, pharmacy management should evaluate and improve prescription services to enhance the efficiency of service time.

Abstract

Introduction: The Community Satisfaction Index at BS Hospital in 2021 and 2022 were low, with an average of 86.92 and 89.73, respectively. This levels was lower than the standard set of at least ≥ 90% by the Minister of Health Decree of number 129/ Menkes/ SK/ II/ 2008. This study aimed to determine the description of patient satisfaction with the quality of outpatient services.Methods: This study is a quantitative study with a descriptive approach and data collection using interview techniques on 134 subjects who were outpatients and non-patients who were the families or members of hospital employees. This study used Donabedian theory with a structure, process, and outcome approach. Data collection were done through interviews using structured questionnaires. Data analysis were conductedResults: The study results showed that the lowest satisfaction was attributed to the structure (hospital facilities) in parking services (76%), the process of doctor services (87%) and nurse services (90%).Conclusion: Overall, patients’ satisfaction to the quality of the structure, process, and the results at the Outpatient Installation were less than the minimum service standards of the Hospital according to the Ministry of Health. Actions should be taken to improve the parking facilities
