Abstract

Stroke is a disease that is a health problem throughout the world. Stroke can cause long-term damage to nerve function or brain damage, long-term disability, and even death. Stroke can cause sufferers to experience weakness due to damage to nerve function. If a stroke attacks the left brain and hits the speech center, it is likely that the patient will experience speech disorders or verbal communication disorders. One form of rehabilitation therapy to correct verbal communication disorders in someone who suffers from verbal communication disorders is "AIUEO" vocal therapy. The aim of this research is to obtain an overview of AIUEO vocal therapy for verbal communication disorders in non-hemorrhagic stroke patients. The method used is to collect the results of the analysis of the patient's verbal communication disorders on the first, second and third days after being given AIUEO vocal therapy using SOP and the Frenchay Aphasia Screening Test (FAST) instrument. The results obtained during the three days of implementation were that patients experienced an increase in verbal communication skills from an initial FAST score of 12 (severe category) to 19 (moderate category). The conclusion of this research is that giving AIUEO vocal therapy can reduce the score of verbal communication disorders in patients, can overcome the weakness of speech muscles in stroke patients, and can stimulate improvement in language skills in stroke patients.

Abstract

Controlled hypertension is a health problem that is closely related to mortality. The prevalence of this disease increases significantly every year throughout the world. Failure in self-awareness results in decreased self-control and decreased self-efficacy, resulting in patients not having regular control and not regularly taking hypertension medication. This study aims to increase the knowledge of female cadres about controlled hypertension. Using pre-test and post-test results of elderly cadres in blood pressure measurement training for hypertensive patients with the T-Test. Inclusion criteria of elderly cadres who have a blood pressure measurement training certificate at the UPTD Tegalampel Public Health Center. The 20 elderly cadres who took part in blood pressure measurement training with using T-Test to see the effect of training for elderly cadres toward blood pressure measurement level of knowledge. Based on the results of the T-Test, there was no significant improvement after health education regarding the prevention and treatment of hypertension (p=0.115). Given to the level of knowledge of elderly cadres regarding blood pressure measurement training. This is caused by increased self-awareness, self-control, and self-efficacy. Efforts to increase the level of knowledge of elderly cadres about controlled hypertension can increase self-awareness by forming a healthy heart group (the hypertension group is supervised regularly and intensively by officers and cadres) by forming a WA group, training elderly cadres with blood pressure measurement training and therapy counseling traditional (guava leaves, garlic, cat's whiskers leaves, ginger, bay leaves, noni, soursop leaves, soak feet in warm water)to increase awareness of controlled hypertension patients in improving healthy lifestyles, namely regular health checks, taking medication regularly.

Abstract

Peptic ulcer disease frequently precipitates acute pain and gastrointestinal hemorrhage, yet conventional analgesic use remains constrained by bleeding risks and mucosal irritation. This study evaluated the clinical effectiveness of integrating handgrip exercise and lavender aromatherapy into nursing care for pain reduction in a patient presenting with melena secondary to peptic ulcer disease. Employing a prospective single-patient case design, the intervention was administered over three consecutive days using standardized nursing protocols. Pain intensity was quantified using the Numerical Rating Scale (NRS) immediately before and after each 30-minute session, with continuous hemodynamic and clinical monitoring. Results demonstrated a progressive attenuation of pain intensity, with NRS scores declining from 7 to 4 across the intervention period, concurrent with improved physiological stability and enhanced patient-reported comfort. These outcomes suggest that the combined modalities operate through complementary neurophysiological pathways, leveraging diffuse noxious inhibitory controls and limbic-mediated autonomic regulation to modulate both sensory and affective pain dimensions. The findings underscore the clinical feasibility, safety, and analgesic potential of structured, nurse-delivered nonpharmacological interventions in acute gastrointestinal settings. Systematic integration of such multimodal protocols into routine nursing practice offers a viable strategy for optimizing pain management while minimizing pharmacological adverse effects. Further randomized controlled investigations are recommended to establish dose-response parameters, validate long-term efficacy, and develop standardized implementation guidelines for broader clinical adoption.

Abstract

Chronic Kidney Disease (CKD) is a significant health problem with increasing prevalence, characterized by a progressive decline in kidney function resulting in fluid accumulation in the body. One common clinical manifestation is edema in the lower extremities. Ankle pump exercises and foot massage are non-pharmacological interventions that can help reduce edema by improving venous and lymphatic circulation. This study aims to analyze the implementation of nursing care in relation to the effect of ankle pump exercise and foot massage on reducing leg edema in patients with chronic kidney disease (CKD). This study used a case study design with a nursing care approach in a CKD patient with edema in the Anturium Ward of Dr. Soebandi Regional General Hospital, Jember. Ankle pump exercises were performed for 10 minutes, followed by a 15-minute foot massage, which was repeated for three consecutive days of treatment. The degree of edema was evaluated using the edema pitting scale before and after the intervention. The results showed a decrease in edema from +2 to +1 (1 mm in the left leg) and from +1 to 0 (2 mm in the right leg). There was also a decrease in ankle circumference of 20 cm and instep of 25.5 cm on the right leg, and 19 cm at the ankle and 23.7 cm at the left leg. Furthermore, muscle strength increased from 2 cm to 5 cm in the left leg and from 1 cm to 4 cm in the right leg. Ankle pump exercises and foot massage effectively reduce edema in CKD patients. This therapy can be used as an independent nursing intervention to enhance the quality of life for patients with CKD who experience edema.

Abstract

Diabetes Mellitus (DM) is a chronic disease that requires long-term and consistent monitoring to prevent complications. However, low adherence to follow-up visits remains a significant challenge in primary healthcare settings, including at Cakranegara Public Health Center in Mataram City. Based on data from January to May 2025, only 62.6% of the total 957 DM patients returned for scheduled follow-up visits. This community service initiative aimed to implement and analyze the effectiveness of a digital innovation, SI-KEPPO (Educational Control Information System), in improving follow-up adherence among DM patients. The methods included needs assessment and the development of an Android-based SI-KEPPO application. Evaluation was conducted through observations and interviews with program officers and patients. The results demonstrated improved follow-up compliance during the 4-day trial period. Patients reported that the application made it easier to remember visit schedules and made them feel more cared for through regularly delivered educational messages. The SI-KEPPO application also enhanced family involvement in patient support and facilitated monitoring of non-compliant patients by program officers. In conclusion, the implementation of the SI-KEPPO innovation effectively increased follow-up visit adherence among DM patients, strengthened digital approaches in community health services, and is recommended for integration into the Prolanis management system at the primary healthcare level.

Abstract

Low adherence of patients with Non-Communicable Diseases (NCDs) and the elderly to scheduled follow-up visits remains a major challenge in primary healthcare services. This issue contributes to delayed detection of complications and reduced effectiveness in managing chronic conditions. This community service program aimed to evaluate the effectiveness of the TERKONTROL application—an automated WhatsApp-based reminder system designed to improve the regularity of patient follow-up visits. The program was implemented at Dasan Agung Public Health Center using a Participatory Action Research (PAR) approach, actively involving program officers, academic facilitators, and patients. The desktop-based application features patient data input, visit scheduling, a monitoring dashboard, and an automated reminder system. Implementation results showed that out of 16 patients who received reminder messages, 13 attended their scheduled visits, achieving a compliance rate of 81.3%. Evaluation revealed positive perceptions from both patients and healthcare staff regarding the application's usability and usefulness. These findings suggest that simple digital innovations such as TERKONTROL (Tertib Kontrol dan Respon Lanjut Pasien PTM & Lansia) can serve as effective tools for real-time patient monitoring, program reporting enhancement, and improved communication in primary healthcare settings.

Abstract

Background: Improper diet arrangements in type 2 diabetes (T2D) patients can cause eating patterns to be disturbed in the form of binge eating behavior. The family can play a helping role in improving diabetes management. One of the interventions that can be given to empower families is the Family Caregiver Empowerment Model (FCEM). This study aimed to analyze the effect of FCEM on binge eating in T2D patients. Methods: This study was a randomized control group pre-test and post-test design with a total sample of 85 respondents taken by cluster random sampling, divided randomly (treatment = 41; control = 44). The independent variable is FCEM, while the dependent variable is binge eating behavior. Data was collected using the Indonesian version of the Binge Eating Scale questionnaire and analyzed used the Wilcoxon sign rank test and independent T-test. Results: There were differences in binge eating behavior in the treatment group before and after intervention (Z= -5.347; p=0.001), whereas in the control group, there was no difference in pre-test and post-test scores (Z= -0.599; p= 0.549). The study result showed that there was a significant difference in the average value between the treatment and control groups (t =5.017; p=0.001). Conclusion: FCEM intervention can reduce binge eating behavior to increase adherence to the diabetes diet program. Nurses can use FCEM intervention to empower family caregiver to help T2D patients improve and maintain eating habits to improve diabetes self-care behaviors.

Abstract

ABSTRACTPhlebitis is a common complication of intravenous therapy that increases patient morbidity, length of hospital stay, and healthcare costs. Nurses’ preventive behaviors play a crucial role in reducing phlebitis incidence; however, field practice shows persistent non-adherence to standard procedures. This study aimed to develop a phlebitis prevention behavior model among nurses based on the Theory of Planned Behavior (TPB). An analytic observational design with a cross-sectional approach was employed. The sample consisted of 98 inpatient and emergency department nurses at Blambangan General Hospital, Banyuwangi, recruited using a total sampling method. Data were analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM). Results revealed that perceived behavioral control (PBC) significantly influenced both the intention and actual behavior of phlebitis prevention. Social and informational factors affected attitudes, subjective norms, and PBC, which in turn shaped nurses’ intentions to perform preventive actions. PBC emerged as the key determinant of phlebitis prevention behavior, directly and indirectly through intention. These findings highlight that enhancing nurses’ behavioral control including self-efficacy, availability of resources, and procedural convenience constitutes a pivotal strategy in phlebitis prevention interventions. Keywords: Phlebitis, Nurses, Theory 0f Planned Behavior, Perceived Behavioral Control, Intention.  ABSTRAK Flebitis merupakan komplikasi umum pada pemasangan terapi intravena yang dapat meningkatkan morbiditas, lama rawat, serta biaya perawatan pasien. Perilaku pencegahan flebitis oleh perawat memiliki peran krusial dalam menurunkan angka kejadian, namun praktik di lapangan menunjukkan masih adanya ketidakpatuhan terhadap standar prosedur. Penelitian ini bertujuan untuk menyusun model perilaku pencegahan flebitis pada perawat berbasis Theory of Planned Behavior (TPB). Desain penelitian menggunakan pendekatan analitik observasional dengan rancangan cross sectional. Sampel penelitian adalah 98 perawat ruang rawat inap dan IGD RSUD Blambangan Banyuwangi yang dipilih dengan metode total sampling. Analisis data dilakukan menggunakan Partial Least Square–Structural Equation Modeling (PLS-SEM). Hasil penelitian menunjukkan bahwa perceived behavioral control (PBC) berpengaruh signifikan terhadap intensi maupun perilaku aktual pencegahan flebitis. Faktor sosial dan faktor informasi berpengaruh terhadap pembentukan sikap, norma subjektif, dan PBC, yang selanjutnya memengaruhi intensi perawat dalam melakukan tindakan pencegahan flebitis. Determinan utama perilaku pencegahan flebitis adalah PBC, baik secara langsung maupun melalui intensi. Temuan ini menegaskan bahwa peningkatan kemampuan perawat dalam mengendalikan perilaku (self-efficacy, ketersediaan fasilitas, dan kemudahan prosedur) menjadi strategi penting dalam intervensi pencegahan flebitis. Kata Kunci: Flebitis, Perawat, Theory 0f Planned Behavior, PBC, Intensi.

Abstract

Patients with pregnancy complications, especially placenta increta, which results in antepartum hemorrhage, will experience hemodynamic instability. Patients who are at risk of long-term hypovolemia are at risk of hemodynamic instability. Hemodynamic instability, or unstable blood flow, is common in critically ill patients requiring intensive care. One additional non-pharmacological therapy that can support the hemodynamic status of ICU patients is foot massage. The purpose of this study was to test the effectiveness of the Foot Massage Therapy intervention in post-CS patients in the ICU. The research method used was a case study, with a 4-day research period. Data collection began with medical records, assessments, and observations. Based on the evaluation results before and after the foot massage procedure, significant improvements in MAP, blood pressure, RR, and pulse rate were observed, with SPO2 values remaining unchanged. Foot massage therapy is effective for patients with APB in the intensive care unit, with significant changes in hemodynamic status. Therefore, this foot massage technique can be applied in all hospital rooms and is quite effective as a complementary therapy to stabilize patient hemodynamics, especially in the ICU.

Abstract

Background: Preoperative education improves patients’ understanding and reduces anxiety before surgery. However, evidence regarding its effectiveness and contextual factors remains inconsistent and fragmented. Therefore, this scoping review aims to map the effectiveness and limitations of preoperative education on surgical patient outcomes. Methods: This study used a scoping review design based on PRISMA-ScR, the PCC (Population, Concept, Context) framework, and the Arksey & O’Malley method. A systematic search was conducted in four databases (Scopus, SpringerLink, PubMed, ScienceDirect) for articles published between 2022 and 2026. The study selection process included duplicate removal, title and abstract screening, and full-text review based on predefined eligibility criteria. Data were extracted using a standardised charting form by independent reviewers, with discrepancies resolved through discussion. The extracted data were analysed using thematic synthesis to identify key themes related to effectiveness and implementation barriers. Results: A total of 514 records were identified from four databases. After excluding articles that were not full-text, not open access, or not written in English (n=207), 307 articles remained for screening. During the screening stage, 263 records were excluded due to duplicate removal, irrelevant titles and abstracts, inappropriate population, intervention, or study type. A total of 44 full-text articles were assessed for eligibility, of which 24 were excluded for not explicitly addressing preoperative education as the primary focus. No studies were excluded based on quality assessment. Finally, 20 studies were included in the review. Three main themes emerged from the synthesis: (1) Cognitive and psychological effectiveness, where most studies reported improvements in patient knowledge and reduced anxiety, although some studies found limited or non-significant effects; (2) Clinical and health system outcomes, including pain reduction, reduced opioid use, postoperative recovery metrics, patient satisfaction, and healthcare service efficiency, although findings varied across contexts and populations; and (3) Implementation barriers and limitations, including variability in educational delivery, timing, digital technology effectiveness, patient characteristics, and methodological constraints. Overall, substantial heterogeneity in effectiveness was observed, along with gaps in integrating multidimensional outcomes and standardizing educational approaches. Conclusion: Preoperative education is a beneficial intervention that can improve clinical outcomes and healthcare efficiency, although its effects vary across populations and settings. These findings support the need for standardized, context-adaptive educational protocols and further high-quality studies to evaluate their effectiveness
