Abstract

Background: Early mobilization after general anesthesia is a key component of Enhanced Recovery After Surgery (ERAS) programs that improves postoperative recovery. However, evidence on its effectiveness, limitations, and implementation remains inconsistent across surgical settings. Therefore, this scoping review aimed to map and synthesize the available evidence on the effectiveness, benefits, limitations, and factors influencing the implementation of early mobilization after general anesthesia. Methods: This scoping review followed the Arksey and O'Malley methodological framework and was reported in accordance with the PRISMA-ScR guidelines. Literature searches were conducted in Scopus, PubMed, ScienceDirect, and SpringerLink databases for studies published between 2022 and 2026. A total of 312 records were initially identified. Following duplicate removal, studies underwent title, abstract, and full-text screening based on predefined inclusion and exclusion criteria. Eligible studies included primary research articles involving adult patients undergoing general anesthesia and evaluating early mobilization in perioperative or postoperative settings, whereas review articles, editorials, case reports, and studies unrelated to early mobilization or general anesthesia were excluded. Data were systematically charted using a standardized extraction form, and findings were synthesized using thematic analysis to identify key themes related to effectiveness, benefits, determinants, and limitations of early mobilization. Results: From 312 records initially identified, 250 studies remained after duplicate removal and underwent title and abstract screening. Following full-text assessment, 12 studies met the inclusion criteria and were included in the final synthesis. Thematic synthesis generated four major themes: (1) clinical effectiveness of early mobilization, including improvements in physical function, pain reduction, shortened length of stay, and decreased postoperative complications; (2) physiological and pharmacological determinants influencing mobilization readiness; (3) neurocognitive determinants affecting patients’ ability to participate safely in mobilization; and (4) organizational and healthcare system factors influencing implementation. The review also identified important literature gaps, including the lack of a standardized operational definition of early mobilization, substantial heterogeneity in intervention timing and outcome measures, and limited evidence regarding long-term recovery outcomes. Conclusion: This scoping review suggests that early mobilization after general anesthesia improves postoperative outcomes, including ADL, pain, LOS, and complications. However, evidence remains limited by indirect findings and variability across studies. Future research should evaluate standardized interventions and long-term outcomes in diverse healthcare settings.

Abstract

The transition of care from hospital to home is a critical period for patients, especially those with chronic conditions such as heart failure. During the transition period, patients often experience difficulties managing their disease, which can lead to an increased risk of rehospitalization. This literature review aims to analyze scientific evidence about the effectiveness of transitional care interventions in improving the quality of health care in hospitals, especially for patients with chronic conditions. The method of this study is a literature review. This study used electronic databases such as PubMed, ScienceDirect and Google Scholar. The selected articles are quantitative research articles published within the last 5 years available in full text and not the result of a review. The results of the review show that transitional care interventions have a significant positive impact on the quality of health care. This intervention can increase self-management, treatment adherence and patient satisfaction with heart failure. Some components of effective interventions include patient education, care coordination, outpatient follow up and social support. Transitional care interventions are an effective strategy to improve the quality of health care, especially in patients with chronic conditions. The implementation of a comprehensive transitional care program in hospitals needs to continue to be improved to achieve optimal results.

Abstract

The implantation of a permanent pacemaker has become a primary strategy for managing cardiac arrhythmias in elderly patients to enhance quality of life, reduce cardiovascular symptoms, and improve physiological function. While pacemakers have been proven effective in enhancing mobility, emotional well-being, and physical endurance, their impact varies among patients depending on health conditions, social support, and psychological readiness. Therefore, further exploration of the clinical and psychosocial implications of pacemaker implantation in elderly individuals is required. This scoping review aims to identify and analyze the impact of permanent pacemaker implantation on the quality of life of elderly patients, with a focus on patient safety, psychosocial adaptation, and implementation challenges. Methods: A systematic search was conducted in ScienceDirect, ProQuest, and PubMed to identify relevant articles published between January 2015 and December 2024. Out of 1,321 articles initially identified, 33 articles were selected for full-text review after applying inclusion and exclusion criteria. A critical appraisal using the Joanna Briggs Institute (JBI) framework determined that 9 articles met the final criteria for analysis. The studies included in this review involved elderly patients with pacemakers across various healthcare settings, including hospitals, outpatient clinics, and community healthcare services, to assess the impact of pacemakers on physical and mental well-being as well as patient satisfaction. Results: Pacemaker implantation generally improves the quality of life of elderly patients, particularly in terms of cardiovascular function, mobility, and independence. Additionally, pacemakers help reduce the risk of arrhythmia-related complications, enhance patient confidence, and improve adherence to medical therapy. However, several challenges persist, including post-implantation infection risks, psychosocial disturbances, and limited access to pacemaker technology in certain regions. The studies also identified research gaps, particularly in longitudinal evaluations of the impact of pacemakers on elderly patients with comorbidities and cognitive impairment. Conclusion: Pacemaker implantation is an effective intervention for improving the quality of life of elderly patients with cardiac arrhythmias. However, a more comprehensive patient-centered approach, including psychosocial support, pre-implantation education, and optimized post-procedural monitoring, is crucial. Further research is recommended to explore the long-term effects of pacemaker implantation and develop more adaptive care strategies for vulnerable elderly populations.

Abstract

Background: The adult Early Warning Score (EWS) is an essential instrument for early detection of a patient's deteriorating clinical condition. However, the effectiveness of EWS depends on nurses' compliance with its implementation. Non-compliance with EWS implementation can hinder early detection of patients' conditions and affect patient survival. This study aims to analyze the relationship between adult EWS implementation compliance and inpatient survival status. However, the evidence on the effect of compliance with EWS implementation on patient survival in real inpatient settings remains limited. This study aimed to examine the association between nurses’ compliance with adult EWS implementation and inpatient survival status. Methods: This study used an observational, cross-sectional design and was reported according to the STROBE guidelines. The research sample comprised 188 patients selected using a phased sampling technique (cluster sampling followed by simple random sampling). Data were obtained from the medical records of inpatients who met the inclusion and exclusion criteria. The independent variable in this study was compliance with the Early Warning System (EWS), while the dependent variable was the patient's survival status. Statistical analysis was carried out using Fisher's Exact test. Results: A total of 188 patients were analyzed, with 173 patients (92%) surviving and 15 patients (8%) dying. In the group that did not comply with the Early Warning System (EWS), there were 6 deaths (16.7%) and 30 patients survived (83.3%), while in the group that did comply with the Early Warning System (EWS), there were 9 deaths (5.9%) and 143 patients survived (94.1%). There was a significant association between EWS compliance and inpatient survival status (p = 0.044). Patients with EWS adherence had a higher chance of survival than non-compliant patients (OR = 3.18; 95% CI: 1.06–9.54). Conclusion: The implementation of EWS that is compliant plays an essential role in detecting early deterioration of patient conditions and improving patient survival and safety.

Abstract

Non-communicable diseases (NCDs) are a leading cause of mortality and morbidity worldwide. Strengthening the capacity Community Health Workers (CHWs) is a key strategy for improving NCD prevention and control. However, its implementation remains suboptimal, particularly in rural settings. This study aims to co-design, implement, and refine a CHW capacity-building intervention for NCD prevention and control in rural settings using a Participatory Action Research (PAR) approach. This protocol describes a qualitative PAR study with process evaluation from August 2024 to June 2026 at Kaliwates Public Health Center (PHC), Jember, Indonesia. The completed pre-PAR phase involved 20 participants (CHWs and healthcare providers) to identify baseline needs and intervention development, while the subsequent PAR phase will involve 15 purposively selected CHWs. The post-PAR phase will explore participants' experiences, perceived benefits, feasibility, and sustainability of the co-developed intervention through observations, in-depth interviews, the Delphi method, and focus group discussions (FGDs). Data from the completed pre-PAR phase were analyzed using thematic analysis and Multiple-Perspective Interviews (MPI), supported by ATLAS.ti software, whereas data from the PAR and post-PAR phase will be analyzed upon completion. Ethical approval was obtained before the commencement of the pre-PAR phase. Preliminary findings from the pre-PAR needs assessment identified three overarching themes: systemic barriers to CHW role fulfilment; capacity gaps in training and skill development; and the need for participatory, culturally embedded training approaches. These findings provide an empirical foundation for the co-design, implementation, and evaluation of a contextually relevant CHW capacity-building intervention for NCD prevention and control in rural settings.

Abstract

Happiness at work (HAW) plays a critical role in nurses’ performance and their ability to deliver compassionate care, as it affects nurses’ motivation, empathy, and caring behavior. However, culturally grounded interventions to improve nurses’ happiness, particularly within Islamic spiritual frameworks, remain underexplored. Therefore, this study examined the effect of an Islamic-based spiritual program based on local religious and cultural values on nurses’ HAW. This research employed a quasi-experimental design with a non-equivalent control group. The intervention included a structured religious talk on the meaning of life and daily listening to Quranic recitations (murottal) during work for four weeks. The study involved 64 inpatient nurses from hospitals. The participants were assigned to an intervention group (n = 32) and a control group (n = 32). Data were collected using the Shortened HAW questionnaire and analyzed using repeated-measures ANOVA and the Kruskal–Wallis test. The results showed that happiness scores increased significantly in the intervention group (mean difference = 1.53, p < 0.001; η² = 0.354). In contrast, the control group showed minimal change. These findings indicate that Islamic-based spiritual programs can effectively enhance nurses’ happiness at work and may contribute to nurse retention, strengthen caring values, and improve healthcare quality.

Abstract

ABSTRACT Ventilator-Associated Pneumonia (VAP) is a common nosocomial infection in ICUs, contributing significantly to morbidity, mortality, and healthcare costs. Implementing VAP bundles has shown effectiveness in reducing VAP incidence, but its success depends on various internal and external factors. This study employed a literature review with a quantitative approach. Data were gathered from 27 relevant articles published between 2013 and 2023, selected from an initial 231 articles obtained through structured searches in Google Scholar, PubMed, Scopus, and ScienceDirect using Boolean keywords such as “Ventilator-Associated Pneumonia” AND “VAP bundle” AND “Compliance.” Analysis revealed that internal factors, including medical staff knowledge and competency, significantly affect compliance with VAP bundles. Challenges such as insufficient continuous training and limited resources were highlighted. External factors, such as regular audits, managerial support, and a conducive work environment, were identified as critical in supporting successful VAP bundle implementation. Addressing internal challenges through structured training programs and resource allocation is essential. Simultaneously, external support mechanisms like frequent audits and strong managerial backing are necessary to ensure sustained compliance with VAP bundles. An integrative approach that encompasses intensive training, routine audits, and reinforced institutional policies is vital for the effective implementation of VAP bundles, thereby improving ICU patient care quality. Keywords: Ventilator-Associated Pneumonia, VAP Bundle, Internal Factors, External Factors, Protocol Compliance, ICU. AND “VAP bundle” AND “Compliance.”

Abstract

Background: Stroke is a neurological disorder frequently associated with reduced cerebral perfusion and impaired consciousness. While conventional interventions are widely implemented, their effectiveness may be enhanced by evidence-based approaches such as Familiar Auditory Sensory Training (FAST), a non-pharmacological intervention that utilizes meaningful auditory stimuli to activate neural pathways and support consciousness recovery. Objective: This study aims to evaluate the effectiveness of FAST in addressing the nursing problem of risk for ineffective cerebral tissue perfusion in an ischemic stroke patient. Methods: A case study was conducted involving a 73-year-old male patient in a soporous state. FAST was administered over three consecutive days, three times daily, with each session lasting 10 minutes. Data were collected through direct observation and Glasgow Coma Scale (GCS) measurements before and after each session. Patient outcomes were evaluated using the Nursing Outcomes Classification (NOC), focusing on indicators including consciousness level, blood pressure, and agitation. Results: The patient’s level of consciousness improved from sopor (6) to somnolence (9). Improvements were also noted in blood pressure, heart rate, and respiratory rate, although an increase in restlessness was observed. FAST supported cerebral responsiveness and cognitive function recovery, contributing to the resolution of the nursing problem: risk for ineffective cerebral tissue perfusion. Conclusion: FAST can be utilized as a therapeutic nursing intervention to enhance hemodynamic stability in patients with decreased consciousness

Abstract

Non-communicable diseases (NCDs), particularly hypertension and diabetes mellitus, remain major contributors to morbidity and mortality in Indonesia. Suboptimal patient engagement and poor adherence to therapy continue to hinder effective NCD management at the primary care level. This community-based intervention aimed to enhance patients’ knowledge, treatment adherence, and self-management through the use of a print-based educational tool, the SEHATI book. The program was implemented over a one-week period at Tanjung Karang Primary Health Center, Mataram City, involving 17 participants, including healthcare providers, community health cadres, and patients with hypertension and diabetes. The intervention consisted of group education sessions, simulation activities, cadre-led home visits, and pre- and post-intervention assessments. The results demonstrated a substantial improvement in participants’ knowledge, increasing from 52% to 89%, and medication adherence, rising from 61% to 90%. Qualitative findings indicated that the SEHATI book facilitated self-monitoring practices and enhanced patient motivation. Additionally, health cadres reported that the tool strengthened their capacity as community educators. This community-based approach, particularly through the active involvement of health cadres, proved effective in promoting patient engagement, especially in settings with limited access to digital health resources. The intervention is consistent with the Health Belief Model, as it enhances perceived benefits and self-efficacy among patients. Overall, the SEHATI book represents a practical, scalable, and contextually appropriate educational strategy for sustainable NCD management in primary healthcare settings.

Abstract

Suboptimal discharge planning in hospitals remains a critical issue, contributing to high readmission rates and inadequate patient and family readiness for post-hospital care. This community service activity aimed to improve the effectiveness of discharge planning through the implementation of the MANDIRI-D innovation program, a digital-based approach utilizing QR Codes. The program provided accessible discharge information, including care guidelines, flowcharts, and patient education materials tailored to patient needs. The activity involved 20 patients and family members using a quasi-experimental pre–post design without a control group. Interventions included structured education delivered at least one day prior to discharge, access to QR Code-based educational media, and reinforcement through the teach-back method. The results demonstrated an improvement in patient knowledge and readiness for discharge (Δ total score 1.05), increased nurse compliance with discharge planning standard operating procedures (≥85%), and more efficient delivery of patient education. The MANDIRI-D program effectively enhanced patient and family preparedness, improved the quality of nursing services, and has the potential to reduce readmission rates. This innovation is recommended for broader implementation and integration into hospital electronic medical record systems to support sustainable and standardized discharge planning practices.
