Abstract

One key aspect of elderly care is personal hygiene management, which directly influences older adults' physical health, psychological well-being, and dignity. Our observation indicates that 6% of older people experience difficulties in maintaining personal hygiene independently. These older people depend on caregivers to maintain their hygiene. However, many caregivers lack adequate knowledge and skills, highlighting the need for practical training and mentoring interventions. This community service activity aimed to enhance knowledge related to personal hygiene management in elderly care by conducting a structured training and mentoring program for caregivers. This activity, carried out in March 2025, included interactive training sessions and one-on-one mentoring involving 41 caregivers from Pedungan and Melinggih villages. Both villages were chosen because our previous studies were carried out in these places. They were representatives of rural and urban areas. To measure the impact of our program, we conducted a pre-post test experimental design without control groups. Knowledge was measured with a self-developed, validated questionnaire. A Wilcoxon Signed Rank Test revealed a statistically significant increase in the knowledge after training, z = -3.829, P < 0.001, with a large effect size (r = 0.6). The median score on the knowledge increased from pre-program (Md = 13) to post-program (Md = 14). Training and mentoring empower caregivers by enhancing their knowledge to better manage the personal hygiene of older people. This approach not only improves the quality of care provided but also contributes to the dignity, comfort, and overall health outcomes of the elderly population. Future programs need to be expanded to include a broader range of caregivers from different areas.

Abstract

Introduction: Antiretroviral therapy is the main treatment for HIV/AIDS, requiring lifelong medication to suppress the virus, prevent complications, and improve the patient's quality of life. However, adherence to ARV medication remains low, influenced by factors such as self-efficacy, social support, spirituality, depression, and various clinical and psychosocial conditions. This study aimed to review studies on ARV medication adherence and its factors among HIV patients. Methods: This literature review explored studies on ARV medication adherence among HIV patients. Searches were conducted in Google Scholar, ScienceDirect, PubMed, Scopus, Web of Science, and CINAHL using keywords related to HIV, ARV adherence, self-efficacy, non-family caregivers, spirituality, and depression. The inclusion criteria consisted of original research articles published between 2020 and 2025 in English or Indonesian that examined factors influencing ARV adherence. Exclusion criteria included abstract-only papers, blogs, news articles, and review papers. Study quality was assessed using the JBI Critical Appraisal Checklist, and risk of bias was evaluated based on sampling methods, measurement tools, confounding factors, and data completeness. Findings were synthesized narratively. Results: A total of 1,653 publications were identified, and 15 studies met all criteria for inclusion. Overall, ARV adherence among HIV patients remained below the national 95-95-95 target. Key influencing factors were self-efficacy, social support (including non-family caregivers), spirituality, and depression, indicating that adherence is shaped by multiple interacting psychosocial and clinical dimensions. Conclusion: Strengthening self-efficacy, enhancing spiritual well-being, and reducing depressive symptoms may improve ARV medication adherence. Future research should explore integrated psychosocial–spiritual interventions and employ longitudinal or mixed-methods designs to understand better the causal pathways affecting ARV adherence.

Abstract

Introduction: Despite the availability of pharmacological treatments, poor adherence and low patient awareness remain barriers to effective long-term management. Non-pharmacological interventions such as acupressure and progressive muscle relaxation (PMR) have demonstrated consistent benefits, yet broader implementation is limited by inadequate education, practical guidance, and patient engagement. Mobile health (mHealth) technologies offer opportunities for real-time monitoring and self-management support, but integrating complementary therapies into digital platforms remains underexplored. It aims to synthesize current evidence on the effectiveness of acupressure, progressive muscle relaxation (PMR), and mobile health (mHealth) interventions in enhancing self-management among individuals with hypertension.Methods: A narrative review with narrative synthesis was conducted in accordance with PRISMA 2020 and the PICOS framework. Searches were performed in PubMed, ProQuest, ScienceDirect, and Google Scholar using predefined keywords. No artificial intelligence (AI)–based tools were employed. Fifteen eligible studies were identified and thematically analyzed. The review included 15 eligible studies that examined non-pharmacological and digital interventions for hypertension management. Results: Specifically, three studies on acupressure explicitly reported significant reductions in systolic and diastolic blood pressure, while eight studies on progressive muscle relaxation (PMR), including multimodal formats combined with breathing exercises, music, or audiovisual support, consistently demonstrated improvements in blood pressure, stress, and sleep quality. In addition, three studies on mobile health (mHealth) interventions reported enhanced adherence, patient engagement, and self-monitoring behaviors. One additional study investigated acupuncture (needle-based stimulation rather than manual acupressure); this study was retained for completeness but not synthesized with the primary findings due to differences in intervention characteristics.Conclusion: Overall, although strong evidence supports the effectiveness of acupressure, PMR, and mHealth individually, none of the included studies integrated acupressure and PMR within a single mHealth platform, underscoring a critical gap in digital hypertension management research.

Abstract

Musculoskeletal health problems are among the most common health issues affecting communities, particularly in rural areas, due to heavy physical labor, non-ergonomic working postures, and limited access to health education. These conditions can reduce productivity, impair quality of life, and increase the risk of disability if preventive measures are not implemented early. Community-based health promotion programs that combine educational approaches with hands-on practice represent an effective strategy to enhance community knowledge and skills in preventing musculoskeletal problems. Furthermore, cross-national collaboration in community service initiatives can enrich learning methods and broaden perspectives in delivering health education. This community service program aimed to provide health promotion to prevent musculoskeletal health problems in rural areas by involving students from Indonesia and Thailand. The program employed a pre- and post-intervention design to evaluate improvements in knowledge and skills among 20 participants in a rural community. The results demonstrated increased competency following student-led simulations, with correct responses rising to 95% in injury prevention, 90% in stretching and muscle strengthening, and 90% in the ability to identify populations vulnerable to musculoskeletal problems. Specifically, participants’ ability to perform back-strengthening exercises improved to a mean score of 4.10±1.02, indicating that direct demonstration of techniques had a significant impact. Participants also showed the highest level of confidence when performing upper-arm stretching exercises, with a mean score of 4.65±0.87, reflecting strong understanding among the participants. Another key finding revealed that video was the most preferred learning method, chosen by 55% of participants. The use of this learning medium has considerable potential to support musculoskeletal health promotion in rural communities. Overall, the program achieved its intended objectives and provides a replicable model for similar health promotion initiatives in other rural areas. These findings may inform future health promotion strategies by emphasizing the use of multimedia and experiential learning approaches in delivering education and enhancing skills within musculoskeletal health programs.

Abstract

Hypertension is one of the non-communicable diseases whose prevalence continues to increase and remains a major risk factor for cardiovascular disease. The management of hypertension does not rely solely on pharmacological therapy but also requires non-pharmacological interventions, such as adopting a healthy diet rich in fruits and vegetables and low in salt. However, public knowledge regarding types of foods that effectively help lower blood pressure remains limited, particularly in rural areas. This community service program (PKM) aimed to provide education on healthy foods that can reduce blood pressure as a form of non-pharmacological therapy for patients with hypertension in the community. The activity employed the Preparation, Action, Reflection, and Evaluation (PARE) approach and was conducted in Melinggih Village, Gianyar Regency, Bali Province, involving 29 participants with hypertension. The program began with a pre-test, followed by health education sessions supported by visual educational media, consultations, discussions, and question-and-answer sessions, and concluded with a post-test evaluation. The results showed that the proportion of participants with good knowledge increased from 34.5% before the intervention to 100% after the implementation of the program. The Wilcoxon test results indicated Z = -4.300; p = 0.001, demonstrating a statistically significant difference in participants’ knowledge levels before and after the activity. The success of the program was supported by lecture methods, consultations, and interactive discussions conducted bilingually (Indonesian and Balinese), as well as the use of colorful leaflets illustrating vegetables and fruits beneficial in lowering blood pressure. Thus, this community-based educational intervention proved effective in improving community knowledge and awareness regarding healthy food choices for hypertension management.

Abstract

Gagal ginjal kronik (GGK) stadium akhir merupakan kondisi kronis progresif yang memerlukan terapi pengganti ginjal jangka panjang dan menimbulkan beban multidimensional bagi pasien serta keluarga. Selain keluhan fisik, pasien sering mengalami tekanan psikologis, keterbatasan sosial, dan penurunan kualitas hidup. Namun, edukasi terstruktur mengenai perawatan paliatif dan perawatan jangka panjang masih terbatas, sementara pelayanan kesehatan cenderung berfokus pada aspek teknis dan kuratif hemodialisis. Kegiatan pengabdian kepada masyarakat ini bertujuan meningkatkan pengetahuan pasien dan keluarga mengenai GGK stadium akhir, perawatan paliatif, perawatan jangka panjang, serta strategi pengelolaan kecemasan melalui program edukasi terintegrasi. Kegiatan diikuti oleh 30 peserta yang terdiri dari pasien GGK stadium akhir dan anggota keluarga pendamping dengan latar belakang sosial dan pendidikan yang beragam. Sebagian besar peserta belum pernah memperoleh edukasi terstruktur mengenai asuhan holistik sebelumnya. Intervensi dilakukan melalui penyuluhan kesehatan, diskusi interaktif, serta pemberian media edukasi berupa leaflet dan materi presentasi. Pelaksanaan program menggunakan pendekatan manajemen POAC (Planning, Organizing, Actuating, dan Controlling). Evaluasi dilakukan menggunakan kuesioner pretest dan posttest yang mencakup empat aspek pengetahuan. Terjadi peningkatan pada seluruh aspek yang dinilai. Pemahaman tentang GGK stadium akhir meningkat dari 80% menjadi 92%. Pemahaman perawatan paliatif meningkat dari 82% menjadi 94%, perawatan jangka panjang dari 84% menjadi 93%, dan strategi pengelolaan kecemasan dari 85% menjadi 95%. Rata-rata skor keseluruhan meningkat dari 83% pada pretest menjadi 93% pada posttest. Program edukasi integrasi perawatan paliatif dan perawatan jangka panjang efektif dalam meningkatkan pengetahuan pasien dan keluarga mengenai asuhan holistik GGK stadium akhir. Program ini berpotensi memperkuat kesiapan pasien dan keluarga dalam menghadapi kondisi kronis secara berkelanjutan.

Abstract

Introduction: Maintaining the functional capacity of older adults is essential to support independence, particularly in rural areas. Understanding the factors contributing to the reduced ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) is essential for developing effective gerontological nursing strategies. This study identified the predictors of ADL and IADL impairment among older adults residing in rural communities. Methods: This cross-sectional study involved 318 older adults selected through simple random sampling. The Barthel Index and Lawton IADL Scale were used to measure ADL and IADL abilities. The Geriatric Depression Scale was used to assess depressive symptoms, whereas difficulties in vision, hearing, and communication were evaluated using the Washington Group Short Set. Chi-square tests and logistic regression were used to examine the associations and predictors. Results: ADL and IADL disabilities were identified in 4.4% and 10.1% of the respondents, respectively. Communication difficulties were a strong predictor of both ADL and IADL disabilities (AOR = 13.72; 95% CI = 3.26–57.76, p < 0.001 and AOR = 7.07; 95% CI = 2.34–21.34, p < 0.001). Depressive symptoms increased the risk of ADL disability (AOR = 4.17; 95% CI = 1.24–14.05, p = 0.021), and adults aged ≥ 75 years showed higher odds of IADL impairment (AOR = 4.39; 95% CI = 1.89–10.16, p < 0.001). Conclusions: Challenges in communication and depressive affect functional capacity among rural older adults. Maintaining independence and improving daily functioning by implementing early identification and intervention targeting communication and mental health are essential.

Abstract

Introduction: Learning in the community presents different challenges than learning in a clinical setting. However, studies on student nurses' experience in the community have not been discussed comprehensively. This study aimed to explored nursing students’ experiences in a community setting using the five stages of the nursing process   Method: This study used a qualitative design with the Photovoice approach. A total of 14 students were involved in the study. Data was collected by taking photos and small group discussions for the narrative photo with the SHOWED approach. Photos and narration are the primary data and are analyzed thematically.   Result: A total of approximately eighteen photos have been included. At the assessment phase, the student requires an adaptation process and strategic anticipation, as the practice was conducted directly in the participants’ community. Formulating nursing diagnosis is done through teamwork and group discussion with an approach based on evidence and theory, and asking for guidance from the lecturer as a facilitator for learning. The development of nursing intervention was done with active participation from society, presenting the intervention plan to the community, community meetings, and ensuring the legality of the plan's activities. Implementation in the nursing community is implemented collaboratively between students and the community following the agreed-upon plan. Evaluating nursing practice is done with collaborative reflection of students, discussion together, and related plans, actions, and carrying on independently in public.   Conclusion: This study finds that community-based nursing practice requires student adaptation, strategic planning, and evidence-based teamwork.   Keywords: Clinical nursing education, community health nursing, photovoice, nursing students, nursing care plan

Abstract

Background: Indonesia faces a rapidly ageing population, with Bali designated as a pilot province for implementing the Case Management Program for Long-Term Care (CMP-LTC). Assessing readiness among nurses and community cadres is crucial to ensure program success and sustainability. Purpose: To explored the readiness of nurses and community cadres to implement the CMP-LTC program for older people in Bali, Indonesia. Method: A descriptive qualitative design was applied using focus group discussions involving two nurses and eight community cadres selected purposively. Data were analyzed thematically based on Braun and Clarke’s approach while ensuring credibility through triangulation and member checking. Results: Six major themes emerged: (1) enhanced knowledge and skills after training, (2) the need for continuous and advanced learning, (3) limited resources and unclear legal authority, (4) sociocultural and community challenges, (5) teamwork and collaboration, and (6) increased empathy and motivation. Conclusion: Readiness for CMP-LTC implementation is multidimensional, encompassing personal competence, structural support, and sociocultural acceptance. Strengthening continuous education, resource provision, legal clarity, and intersectoral collaboration will enhance the effectiveness and sustainability of Indonesia’s long-term care services for older people.

Abstract

Introduction: Diabetes Mellitus (DM) is one of the main causes of 1.6 million deaths every year in the world. Irregularity of blood sugar control and the absence of a healthy lifestyle are risk factors and complications of type 2 Diabetes Mellitus (DM) patients. Many factors need to be studied related to compliance to diabetes complication prevention. Objective: The objective of this research was to analyze the determinants of compliance to complication prevention in type 2 DM patients. Method: This research used a correlation design with a cross-sectional approach. The sample was 233 type 2 DM patients recruited using the convenience sampling technique. Data were collected using questionnaires and analyzed using univariate, bivariate, and multivariate analyses. Results: The results of the Chi-Square analysis showed several factors had a significant relationship with compliance with diabetes complication prevention, including knowledge (p 0.003), attitude (p 0.000), motivation (p 0.020) and family support (p 0.000). On the other hand, the results of multivariate analysis showed that attitude (AOR: 6.148), family support (AOR: 2.846) and motivation (AOR: 1.912) were the dominant factors in preventing DM complications. Conclusion: Compliance with complication prevention in type 2 DM patients is the main aspect that needs to be improved through improving several factors, namely patient attitude, motivation and family support.
