Abstract

Latar belakang. Obesitas pada anak berhubungan dengan meningkatnya risiko sindrom metabolik, seperti resistensi insulin. HOMA-IR merupakan marker yang sering digunakan untuk menilai resistensi insulin. Chemerin merupakan protein 18 kDa yang dihasilkan jaringan adiposa, berfungsi sebagai chemoatractant memegang peran penting berkontribusi terhadap perkembangan inflamasi dan resistensi insulin. Tujuan. Untuk mengetahui hubungan kadar chemerin dengan HOMA-IR pada remaja obesitas.Metode. Penelitian ini merupakan penelitian cross sectional yang dilakukan pada 3 sekolah menengah umum (SMU) di kota Padang. Jumlah sampel berjumlah 28 remaja obesitas dan 28 remaja dengan IMT normal. Obesitas ditentukan berdasarkan nilai IMT, HOMA-IR dihitung berdasarkan rumus yang menggunakan nilai glukosa dan insulin puasa. Glukosa diukur dengan metode glucose hexokinase fotometrik, insulin diperiksa dengan metode chemiluminessence immunoassay dan kadar plasma chemerin dengan metode ELISA. Data dianalisis dengan sistem komputerisasi dengan uji korelasi.Hasil. Kadar plasma chemerin lebih tinggi pada kelompok obesitas dibandingkan kontrol 121,52 (SD 2,09) ng/ml vs 97,23(SD 2,41) ng/ml, p: 0,001 dan pada kelompok obesitas dengan resistensi insulin dibandingkan non resistensi insulin 133,1(SD 19,24) vs 115,09 (SD 19,52), p=0,001. Terdapat hubungan lemah kadar chemerin dengan nilai HOMA-IR pada obesitas(r=0,382;p=0,045) dan hubungan lemah kadar chemerin dengan nilai HOMA-IR pada obesitas resistensi insulin (r=0,297;p=0,405).Kesimpulan. Terdapat hubungan lemah kadar chemerin dengan nilai HOMA-IR pada remaja obesitas, dan hubungan lemah kadar chemerin dengan nilai HOMA-IR pada obesitas resistensi insulin.

Abstract

Latar belakang. Gangguan ginjal akut (GgGA) berhubungan dengan mortalitas dan morbiditas yang tinggi di antara anak sakit kritis. Cystatin C adalah protease inhibitor yang menurut beberapa penelitian merupakan biomarker yang baik untuk mendeteksi gangguan ginjal akut pada anak sakit kritis.Tujuan. Mengetahui sensitivitas, spesifisitas, nilai prediksi positif, dan nilai prediksi negatif cystatin C serum dan kreatinin serum dalam mendiagnosis gangguan ginjal akut pada anak sakit kritis.Metode. Penelitian potong lintang pada 70 subjek di HCU dan PICU RSUP. Dr. M. Djamil Padang dari Mei 2017 – Juni 2017. Subjek penelitian laki-laki 55,71%, median usia 16,50 bulan. Subjek dipilih dengan teknik konsekutif. Dilakukan pemeriksaan cystatin C serum dengan ELISA dan kreatinin dengan kolorimetrik. Baku emas menggunakan estimasi laju filtrasi glomerulus berdasarkan formula Schwartz. Gangguan ginjal akut terjadi bila terjadi penurunan laju filtrasi glomerulus minimal 25% berdasarkan kriteria pRIFLE. Kurva receiver operating characteristic (ROC) digunakan untuk menilai cystatin C dan kreatinin dalam mendiagnosis GgGA.Hasil. Rerata cystatin C dan kreatinin serum pada GgGA 0,88±0,14 mg/L, 1,13±0,59 mg/dL berturut-turut. Tiga puluh tujuh pasien didiagnosis GgGA. Cut off point cystatin C serum 0,56 mg/L, sensitivitas 85,19%, spesifisitas 60,47%, nilai prediksi positif 57,50%, nilai prediksi negatif 13,33%. Cut off point kreatinin serum 0,95 mg/dL, sensitivitas 51,85%, spesifisitas 100%, nilai prediksi positif 100%, nilai prediksi negatif 23,21%.Kesimpulan. Cystatin C serum sensitif untuk mendiagnosis GgGA tetapi kurang spesifik.

Abstract

Background: The pathophysiological mechanism associated with spontaneous preterm delivery is oxidative stress through the increased formation of reactive oxygen species (ROS) due to lipid peroxidation. Malondialdehyde (MDA) is one of the biomarkers of oxidative stress produced through the lipid peroxidation process. Objective: The aim of this study is to observe the difference in MDA levels among preterm labor compared to full-term labor. Methods: Observational research was conducted with a comparative cross-sectional design. Maternal venous blood samples were taken from private hospitals and midwives in Padang city and Aro Suka Hospital Solok Regency. Samples were selected by consecutive sampling and divided into two groups with a total of 40 samples. MDA level was measured using the spectrophotometry method. Results: MDA levels in preterm delivery were 3,6±0.42 nmol/mL and in full-term delivery were 2.9±0.33 nmol/mL. Conclusion: There was a significant difference in MDA levels between preterm labor and full-term delivery. MDA levels in preterm childbirth were higher than MDA levels in full-term delivery.

Abstract

Background: Breast cancer is one of the most common cancers as well as one of the leading causes of cancer mortality in women worldwide. Cancer risk potentially continues to increase because of the many sources of exposure to carcinogenic chemical compounds. Carcinogenic compounds can contribute to free radical formation which might further interact and damage biomolecules such as lipids. Lipid peroxidation will increase malondialdehyde (MDA) levels, triggering gene mutations that leads to cancer. Objective: The purpose of this research was to measure and compare MDA levels between breast cancer patients and control. Methods: This research was observational research using a cross-sectional comparative design of 30 breast cancer patients and 30 healthy controls. The place of this research is in Ropanasuri specialized surgery hospital and biochemical laboratory, Faculty of Medicine, Andalas University, Padang. This research was conducted from August to September 2019. The MDA was measured using a spectrophotometer and an independent T-test was done. Results: The result of this research showed the mean MDA level of breast cancer patients was 3.98 ± 0.35 nmol/ml, higher than controls was 3.04 ± 0.36 nmol/ml with p-value = 0.001. Conclusion: There were significant differences in MDA levels among breast cancer patients and control in Ropanasuri specialized surgery hospital, Padang.

Abstract

Pelayanan kesehatan terhadap lanjut usia (lansia) perlu mendapatkan perhatian khusus, karena lansia rentan terhadap penyakit Tujuan: Pengabdian masyarakat ini dilakukan dengan tujuan untuk mengidentifikasi faktor risiko penyakit dan memberikan penyuluhan pada kelompok lanjut usia di Kenagarian Sumanik. Metode: Kegiatan yang dilakukan adalah pemeriksaan kesehatan lanjut usia dan pengkajian psikologis dengan menggunakan kuisioner DASS 42. Selanjutnya dilakukan penyuluhan kesehatan dengan harapan meningkatkan pengetahuan para lansia sehingga mampu mengidentifikasi tanda bahaya dan mendapat pertolongan lebih awal. Hasil: Kegiatan dihadiri oleh 63 orang warga, 31 orang diantaranya di atas usia 60 tahun. Hasil pemeriksaan ditemukan 22 orang mengalami hipertensi stadium 2, dan 5 orang menderita diabetes yang sebelumnya sudah diketahui akan tetapi kontrol tidak teratur. Pada pemeriksaan mata terdapat 25 orang menderita gangguan refraksi, 5 orang ekstensi lensa dan dirujuk ke RSUD melalui Puskesmas. Dari kajian psikologis didapatkan 5 orang memiliki gejala depresi, 15 orang dengan ansietas dan 48 orang mengalami stress-psikologis. Kesimpulan: Penyuluhan kesehatan sangat diperlukan terutama kelompok lanjut usia supaya lansia dapat mengenal tanda-tanda penyakit sehingga dapat pelayanan kesehatan lebih dini. Aktivitas fisik seperti berolahraga diperlukan namun lansia perlu mengetahui jenis olah raga yang tepat sesuai dengan usia.

Abstract

Elementary school students are consumers who are among those who consume the most food and beverages. Food and drinks sold around the school contain many hazardous substances such as dyes that have been banned from circulating (Rhodamine B in iced drinks, jelly drinks, and so on) and other additives such as formalin which is used as a chewy substance in meatballs sold in carts and restaurants that can cause acute poisonings such as allergies, coughs and chronic poisonings such as cancer of the liver, kidneys and other organs. The purpose of this activity is to increase knowledge about substances in food and an introduction to blood group examination. This activity was carried out at SD Negeri 13 Tungkal Selatan Village in North Pariaman District. The method of activity is in the form of counseling and demonstration. According to the survey before the blood group examination, it turned out that most of the elementary school students did not know their blood type. Before the blood group examination, the students were also given knowledge about determining a person's blood type. After this counseling was carried out, respondents became more aware of the impact of food/snacks on health, and respondents knew their respective blood types. Of the 64 students, there are 22 A, 17 B, 12 AB, 13 O. To avoid problems related to food and blood type. There is a need for continuous health education for elementary school students.

Abstract

Background Obesity is a global problem. Even in poor and developing countries, obesity has reached alarming levels. In childhood, obesity may lead to insulin resistance. Retinol binding protein (RBP4), secreted primarily by liver and adipose tissues, was recently proposed as a link between obesity and insulin resistance. The role of RBP4 in pediatric obesity and its relationship with insulin resistance have not been well elucidated.Objective To compare RBP4 levels in obese and lean adolescents and to assess for a relationship between RBP4 levels and insulin resistance. Method This cross-sectional study was conducted in three senior high schools in Padang, West Sumatera, Indonesia. Subjects were adolescents aged 14-18 years, who were obese or normal weight (n=56). We measured subjects’ body mass index (BMI) and serum RBP4 concentrations. Insulin resistance was assessed using the homeostasis model assessment of insulin resistance (HOMA-IR) index.Results Similar RBP4 levels were found in the obese and normoweight groups (P>0.05). Higher RBP4 levels were found in the insulin resistant compared to the non-insulin resistant group, but the difference was not significant (P > 0.05).Conclusion There is no significant difference in mean RBP4 levels in obese adolescents compared to normoweight adolescents. Nor are mean RBP4 levels significantly different between obese adolescents with and without insulin resistance.

Abstract

 Latar belakang: Infeksi kronis pada tonsil palatina disertai pembesaran tonsil palatina, dapat menyebabkan sumbatan pada jalan nafas bagian atas. Gangguan pertumbuhan banyak ditemukan pada kelompok pasien ini. Tujuan: Mengetahui apakah ada pengaruh tindakan tonsiloadenoidektomi terhadap kadar serum insulin-like growth factor-1 (IGF-1) dan nilai body mass index (BMI) pada anak yang menderita hipertrofi tonsil palatina dan adenoid, dengan gejala obstructive sleep apnoea syndrome (OSAS). Metode: Desain penelitian adalah eksperimental dengan studi pre- dan post-trial, dengan membandingkan kadar IGF-1 dan nilai BMI dalam serum anak dengan gejala OSAS, sebelum dan tiga bulan setelah dilakukan tonsiloadenoidektomi. Hasil: Sesuai protokol penelitian terdapat 14 subjek penelitian, menunjukkan peningkatan nilai kadar IGF-1 dan nilai BMI. Tindakan tonsiloadenoidektomi terbukti dapat meningkatkan kadar hormon pertumbuhan, yang tergambar dari peningkatan kadar IGF-1 dan nilai BMI. Kesimpulan: Tindakan tonsiloadenoidektomi berpengaruh terhadap peningkatan kadar hormon pertumbuhan, yang disimpulkan melalui pengukuran kadar IGF-1 dan nilai BMI dalam serum anak dengan gejala OSAS. Kata kunci: tonsiloadenoidektomi, insulin-like growth factor-1, obstructive sleep apnoea syndromeABSTRACT Background: Chronic infection with the enlargement of the palatine tonsil, could cause blockage of the upper airway. Growth disorders are common in this patient group. Purpose: To determine the effect of tonsilloadenoidectomy on the level of serum IGF-1 in children with palatine tonsil and adenoid hypertrophy and obstructive sleep apnoea syndrome (OSAS) symptoms. Methods: The study design was experimental, pre-and post-study, by comparing the serum levels of IGF-1 and the value of Body Mass Index (BMI) in children with OSAS symptoms, before and three months after tonsiloadenoidectomy. Result: According to the study protocol, there were 14 subjects showed increased levels of IGF-1 and BMI values. Tonsilloadenoidectomy was shown to increase the growth hormone levels, which was reflected from the increased levels of IGF-1 and BMI. Conclusion: Tonsiloadenoidectomy affected the increased levels of growth hormone, which was concluded through the measurement of the levels of IGF-1 and the value of BMI in the serum of children with symptoms of OSAS. Abstract

AbstrakPenelitian yang menyimpulkan bahwa stres oksidatif meningkat pada penyakit diabetes mellitus telah cukup banyak. Stres oksidatif tersebut dapat diartikan sebagai suatu ketidakseimbangan antara prooksidan (radikal bebas) dengan antioksidan, karena antioksidan tidak mampu meredam peningkatan prooksidan. Penelitian ini bertujuan mengkaji pengaruh asupan ekstrak mengkudu (Morinda citrifolia linn) yang berfungsi sebagai antioksidan terhadap kadar malondialdehid (penanda prooksidan) dan enzim katalase (penanda antioksidan) pada tikus diabetes karena induksi aloksan.Penelitian eksperimental ini dilakukan pada 12 ekor tikus putih dengan berat badan 200-250 gram yang dibagi menjadi tiga kelompok, yaitu kelompok kontrol negatif (hanya diberi makan dan minum), kelompok kontrol positif (makan dan minum,serta diinduksi aloksan), kelompok perlakuan (makan dan minum, diinduksi aloksan, dan diberi ekstrak mengkudu (500 mg/kg BB/hari) selama 12 hari. Data yang diperoleh dianalisa secara statistik dengan uji Anova dengan derajat kepercayaan 95%.Hasil penelitian menunjukkan terdapat perbedaan bermakna antar kelompok (p<0,05), dimana rerata kadar MDA darah kelompok kontrol negatif (4,2610,427 nmol/ml), kelompok kontrol positif (5,6050,391 nmol/ml), kelompok perlakuan (4,2610,427 nmol/ml) dan rerata aktivitas katalase kelompok kontrol negatif (6,5800,277 unit/mg), kelompok kontrol positif (4,9540,485 unit/mg), kelompok perlakuan (6,3140,651 unit/mg).Kesimpulan dari penelitian ini adalah pemberian ekstrak mengkudu dapat menu-runkan kadar MDA darah dan meningkatkan aktivitas enzim katalase tikus DM yang diinduksi aloksan.Kata kunci: ekstrak mengkudu – kadar MDA – aktivitas katalase – aloksan – tikus diabetesAbstractThere were so many study concluded that oxidative stress increased in diabetes mellitus. The result was analyzed by using one way Anova with confidence interval 95%.There were significantly different in groups (p<0,05). The MDA level in negative control group is 4,2610,427 nmol/ml, positive control group is 5,6050,391 nmol/ml, and the group was treated by alloxan induced which followed by mengkudu extract oral is 4,2610,427 nmol/ml. Catalase activity in negative control group is 6,5800,277 unit/mg, positive control group is 4,9540,485 unit/mg, and the last group is 6,3140,651 unit/mg.The conclusion is mengkudu extract has capability to quench MDA level and increasing catalase activity in alloxan induced diabetic rats.Keywords : mengkudu extract – MDA level – catalase activity – aloksan – diabetic rats

Abstract

AbstrakStress oksidatif yang terjadi pada diabetes melitus (DM) yang tidak terkontrol dapat menyebabkan peningkatan peroksidasi lipid yang menghasilkan malondialdehid (MDA). Untuk menekan stress oksidatif diperlukan antioksidan tambahan dari ekstrak mahkota dewa. Penelitian ini bertujuan untuk mempelajari efek ekstrak mahkota dewa terhadap kadar malondealdehide serum pada mencit DM akibat induksi aloksan.Penelitian ini dilakukan di laboratorium Biokimia Fakultas Kedokteran Universitas Andalas dengan menggunakan binatang percobaan 12 ekor mencit yang berumur 3 bulan. Binatang percobaan dibagi dalam 3 kelompok yaitu kelompok kontrol negatif, kelompok kontrol positif (175 mg aloksan/kg BB) dan kelompok perlakuan (175 mg aloksan/kg BB dan 500 mg ekstrak mahkota dewa extract/kg BB). Data yang didapat dianalisa secara statistik dengan uji One Way Anova.Hasil penelitian menunjukan kadar MDA serum kelompok kontrol negatif 4,43 + 0,02 nmol/ml, kelompok kontrol positif 5,32 + 0,74 nmol/ml dan kelompok perlakuan 3,98 + 0,38 nmol/ml. Terdapat perbedaan yang bermakna (p˂0,05) antara kelompok kontrol negatif, kelompok kontrol positif dan kelompok perlakuan. Dapat disimpulkan bahwa ekstrak mahkota dewa bisa menurunkan kadar MDA serum pada mencit DM akibat induksi aloksan.Kata kunci : mahkota dewa, aloksan, diabetes melitus, MDAAbstractMalondialdehide (MDA) is the important marker of lipid peroxidation and showed that progression of diabetic evidence is corelated with oxidative stress and can be folowed up by MDA measurement. This research was conducted to study the effect of mahkota dewa extract on the MDA serum level on diabetes mellitus aloxan-induced rats.This research was held at Biochemistry Laboratory Medical Faculty of Andalas University Padang. Twelve Wistar rats of 3 months age were used. The rats wereARTIKEL PENELITIAN66grouped into 3 treatment i.e. 1) negative control, 2) positive control (175 mg aloxan/kg BW) and 3) treated group (175 mg aloxan/kg BW and 500 mg mahkota dewa extract/kg BW). The data was analyzed by one way anova test.The results showed that MDA serum level was 4.43 + 0.02 nmol/ml for the negative control group, 5.32 + 0.74 nmol/ml for positive control group and 3.98 + 0.38 nmol/ml for treated group. There were significant differences (p; 0.05) between negative control, positive control and treated groups.
