Abstract

Kematian akibat kanker payudara masih menempati urutan pertama pada kasus baru kanker di Indonesiabahkan di dunia. Tujuan penelitian ini adalah untuk mengeksplorasi pola kehidupan klien kanker payudaradalam menjaga kualitas hidupnya. Teori Culture Care Diversity and Universality digunakan sebagai kerangkakerja dalam penelitian ini. Rancangan penelitian menggunakan pendekatan kualitatif. Teknik pengambilansampel dengan purposive sampling pada 6 orang informan klien kanker payudara yang berada di KabupatenGarut. Pengumpulan data dengan teknik wawancara dan observasi partisipasi. Transkripsi data dianalisismenggunakan metoda analisis Transcultural Nursing dari Medeleine Leininger. Hasil penelitian menunjukandomain muncul sebagai pola enkulturasi kehidupan wanita penderita kanker payudara dalam budaya Sundayaitu: 1) pengabdian wanita Sunda, 2) adaptasi klien kanker payudara dalam menjalani kehidupannya,3) makna end of life. Kesimpulan penelitian, pola budaya tersebut saling berhubungan dalam kehidupanWanita Sunda dengan kanker payudara. Disarankan perawat dapat memberikan pelayanan keperawatankepada klien kanker payudara dengan menerapkan asuhan keperawatan yang kongruen dengan nilai budaya.

Abstract

ABSTRACT One of the most common psychological problems found in patients with chronic renal failure (ESRD) undergoing hemodialysis is anxiety. Unresolved anxiety can have a negative impact on physiological and psychological conditions that can aggravate the condition of the disease. Traditional Sundanese music therapy Kacapi Suling "Ayun Ambing" has a slow tempo and soft so that it can be used as a therapy. The purpose of this study was to determine the effect of the music therapy Sunda Kacapi Suling "Ayun Ambing" to the anxiety level of patients undergoing hemodialysis.The design used was a Quasy Experimental, with a pretest-posttest control group design approach, involved 46 patients divided into control groups (23) and intervention groups (23) taken by purposive sampling. The intervention group get given Sundanese Kacapi Suling "Ayun Ambing" of music a week 2 times with time 30 minutes for 2 weeks. Anxiety levels were measured using HARS (Hamilton Anxiety Rating Scale) before and after intervention. The control group received standard interventions provided by the Hospital. Data were analyzed univariate and bivariate with Paired t test, Independent t test and Mann Whitney test.Characteristics of respondents between the intervention group and the homogeneous control did not differ significantly. Differences of average before and after in two groups with the tested paired t test showed no significant difference with p value <0.05 whereas for differences in the average change of scores between the intervention and control group with independent t test showed there was a significant difference with p value 0.05.In conclusion, Sundanese Kacapi Suling "Ayun Ambing" of music intervention has an influence on the anxiety with patient undergoing hemodialysis. An implication of this research was that Sundanese Kacapi Suling Music could be considered and applied as a complementary theraphy to decrease the anxety of client, particularly ESRD undergoing hemodialysis. Sundanese Kacapi Suling "Ayun Ambing" of music intervention can also be used as an intervention alternative in a further research in order to overcome health therapies such as anxiety and pain. Keywords: Chronic Kidney Failure, Hemodialysis, Anxiety, Music Kacapi Suling

Abstract

Keberadaan Human Immunodeficiency Virus (HIV) di dalam tubuh secara terus menerus menyebabkan gangguan pada hampir semua sistem tubuh yang berdampak pada munculnya gejala kelelahan (fatigue). Fatigue banyak dilaporkan pada penderita HIV/AIDS dengan prevalensi berkisar antara 20% sampai 60%. Penelitian ini bertujuan menguji hubungan antara fatigue dengan jumlah CD4 dan kadar Hb pada pasien HIV/AIDS. Sebanyak 77 responden direkrut secara purposif di sebuah Klinik Rawat Jalan Rumah Sakit di Kota Bandung. Fatigue diukur menggunakan kuesioner HIV Related Fatigue Score (HRFS). Data yang terkumpul dianalisis menggunakan uji pearson correlation. Hasil penelitian menunjukkan terdapat hubungan yang bermakna antara fatigue dengan jumlah CD4 dalam darah (r = -.289, p< 0.05) dan kadar Hb (r = -.349, p< 0.05). Selain itu, kadar Hb memiliki hubungan yang bermakna dengan jumlah CD4 pada pasien HIV/AIDS (r = .360, p < .01). Hasil penelitian ini mengindikasikan perlunya monitoring kadar CD4 dan Hb secara berkala dan melakukan intervensi untuk mengatasi penurunan Hb dan CD4 sesegera mungkin sehingga dapat mencegah agar fatigue tidak berkelanjutan.Kata kunci: CD4, fatigue, hemoglobin, HIV/AIDS. The Correlation of Between Fatigue, CD4 Cell Count, and Hemoglobin Level among HIV/AIDS PatientsAbstractThe existence of Human Immunodeficiency Virus (HIV) in the body continuously causes disruption in almost all body systems that impact on the emergence of symptoms of fatigue. Fatigue was widely reported in HIV/AIDS patients with prevalence ranging from 20% to 60%. This study examined the relationship between fatigue and CD4 cell count and hemoglobin levels in HIV/AIDS patients. A total of 77 respondents were recruited purposively in Outpatient Clinic, General Hospital Bandung City. Fatigue was measured using the HIV Related Fatigue Score (HRFS) questionnaire. The collected data were analyzed using pearson correlation product moment. The results showed there were significant relationship between fatigue and CD4 count in blood (r = -.289, p< 0.05) and hemoglobin level (r = -.349, p< 0.05). In addition, CD4 had significantly correlation with Hb (r = .360, p < .01). The results of this study indicated that nurses or health care providers need to periodically monitor the CD4 and Hb levels and provide early intervention to manage the hemoglobin and CD4 cell count at optimum levels to prevent prolonging fatiguel.Keywords: CD4, fatigue, hemoglobin, HIV/AIDS.

Abstract

Sleeping disorder is a symptom often suffered by breast cancer patients. To overcome it, five-finger relaxation technique is considered helpful. In previous research, this technique can be utilized to lower the level of anxiety and fatigue. This research, therefore, aims at identifying the effect of five-finger relaxation technique to the sleep quality of breast cancer patients.The research method used that of quasi experiment with pre-test and post-test control group design. The population included breast cancer patients within the service of the healthcare in Sekarwangi Hospital and R. Syamsudin SH Hospital. Sampling was done consecutively. The number of samples from control and intervention group were 30 people each. The intervention group received a standard hospital therapy and five-finger relaxation technique divided in 15 sessions with 10-15 minutes time every other day for 1 month. Meanwhile, the control group received only a standard hospital therapy. The measurement tool was PSQI. The analysis used paired sample t-test and independent sample t-test.The result of the research has shown a difference in scores during pre-test and post-test of the sleep quality on both groups due to p value < 0.05. A difference has striken on the scores of sleep quality between control and intervention group with the latter having higher scores than the former.It can be concluded that five-finger relaxation technique does affect the quality sleep of breast cancer patients. The result of this research can be an additional intervention to decrease the symptoms suffered by breast cancer patients.

Abstract

Most cases of breast cancer are found to be in an advanced stage. This is because of the patient delay in coming to health service after the emergence of early symptoms of breast cancer. It is necessary to identify factors that allegedly prevent patients from seeking health services as early as possible. The purpose of this study was to analyze factors related to the search for health services. This research used cross-sectional method. The population of this study was breast cancer patients where the sampling technique using consecutive sampling with the number of samples researched as many as 100 people. Data were obtained through a reliable questionnaire to use (Alpha Cronbach’s questionnaire = 0.92, Alpha Cronbach’s health questionnaire = 0.85). Analysis of this research data using univariate (frequency distribution), bivariate (Chi-Square) and multivariate (logistic regression).The result of bivariate analysis showed that the variables related to health seeking behavior are education (ρ value = 0.02; r = 0.3) and health belief (ρ value = 0.01; r = 0.24). While the variables that most related to health service seeking behavior was health belief (ρ value = 0.02) and OR value 3.46. This could be caused by patient’s health belief in which the patient assumes that the symptoms were not dangerous and the choice of alternative medicine as the first choice and the patient’s fear of treatment due to lack of information that obtained by the patient. It can be concluded that health beliefs are the most correlated factor with health-seeking behavior so it is necessary to consider the prevention efforts of breast cancer especially related to health belief. Community beliefs about routine health checks should be key interventions such as counseling and discussions with the community regarding the importance of routine health screening as part of early detection of disease.

Abstract

The problem of malnutrition remains overlooked in Indonesia, especially on children, caused by various factors. Indonesia is the 17th country with 3 nutrition problems, including stunting (short body), wasting (skinny body), and overweight (obesity). This research aims to analyze factors affecting nutrition status on children in the area of West Java Province, including the mother’s and the child’s socio-demographics factor, and the child’s health status. The research method was descriptive quantitative with cross-sectional approach. The number of samples is 810. The research was conducted in 6 districts that support Family Planning (KB), including Bandung District, Bandung City, West Bandung District, Subang District, Sumedang District, and Garut District. The quantitative analysis consisted of univariates using percentage and frequency distribution, as well as bivariate analysis using chi square test. The result of the research shows that nearly all toddlers have good nutrition status as much as 87.9%, and toddlers with malnutrition as much as 10.6%. The analysis factor shows that there is a relationship between the mother’s age (p = 0.048; OR = 1.583), family income (p = 0.010; OR = 1.803), delivery complications (p = 0.008; OR = 2.091), provision of exclusive breastfed milk (ASI) at the age of 0 - 6 years old (p = 0.000; OR = 2.321), provision of exclusive breast milk and complementary feeding given to babies before 6 months old (MPASI) at the age of 6 months to 2 years old (p = 0.002; OR = 2.037), and the child’s history of hospitalization (p = 0.008; OR = 2.055), while other factors are considered irrelevant. This research suggests that healthcare staff collaborate in providing knowledge to mothers on the provision of exclusive breast milk and complementary feeding as well as the prevention of illness on their children.

Abstract

The target of the 2015 Medium-Term National Development Plan is the fertility rate of 2.1 children. However, based on The Indonesian National Demographic and Health Survey 2017, the fertility rate of West Java Province is similar to the national, which is 2.4 children. West Java is a barometer of the national fertility rate since one-fifth of Indonesia's population is in West Java. This study aims to analyze the factors that influence fertility (number of children ever born). The design of this study was cross-sectional. Data derived from the 2018 Survey of Accountability Programs Performance covered 12,350 women aged 15-49 years. The sample was 9,814 woman who had been married. Data analysis used univariate, bivariate and multivariate analyses with a confidence level of 0.05. Bivariate results found that five variables that affected fertility were the age of first marriage, frequency of marriage, use of contraceptive, ideal family size, and wealth index, while the area of residence was not related to fertility. Indeed there are three most dominant factors that have been related to fertility were the age of first marriage, frequency of marriage, and the use of contraceptives. The conclusion was that women who marry at an older age and use contraception have lower fertility rates.  This study can be considered in population control policies, especially to improve health promotion programs regarding the ideal marriage age for women and the use of contraception as an effort to control the population rate.  This study can be considered in population policies.

Abstract

The high prevalence and incidence of breast cancer patients poses a threat to the life quality of Indonesian women. Beside the patient’s condition, therapy options are also the factors faced by both the patients and their families. This research aimed at analyzing socio demographic and knowledge factors relating to therapy options of breast cancer patients. A cross-sectional research was conducted directly to the people (community-based and hospital-based) from 198 cancer patients in the main region, West Java Province. Data were collected through face-to-face interviews with patients, with or without their family. The data were analyzed using the Chi-Square test to yield a correlation between socio demographic and knowledge factors on therapy options. The findings of this research showed that there was a significant relationship between socio demographic factor (physical condition (p=0.002), emotional condition (p=0.000), patient’s age (p=0.000), marital status (p=0.000), family status (p=0.000), faith (p=0.032), and income (p=0.026)) and the knowledge factor (knowledge about illness (p=0.045), the cause of breast cancer (p=0.000), indications and early symptoms of breast cancer (p=0.014), indications and symptoms during breast cancer therapy (p=0.000), therapy for breast cancer (p=0.000), and treatment on indications and symptoms (p=0.000)) with therapy options. Therapy options can be impacted by socio demographic and knowledge factors, although considerations from family also play a key role. In this context, the nurse plays the role as an advocate to raise awareness on the importance of treatment to healthcare facilities, thus the people can make a well-informed choice on their therapy. The uniqueness of the research showed the characteristic of patients, and therapies option in the multi medical system in Indonesia.

Abstract

Breast cancer is still the most common case in many countries. Breast cancer can threat a person's quality of life, especially for someone who has difficulty choosing treatment due to lack of understanding of treatment options. In choosing a treatment influenced by knowledge and income which are internal factors that must be present in determining treatment options, because itwill determine the right choice of treatment. This study aims to discover the choice of treatment in breast cancer patients based on knowledge and income.The research design used quantitative descriptive. This study used secondary data from a Leading Tertiary Study grant in 2015. Sampling in this study were 200 breast cancer patients undergoing chemotherapy. Data processing used univariate and crosstabs techniques with the correlation test using Chi-Square.In this study, there was a relationship between knowledge and choice of treatment, but there was no relationship between income and choice of treatment. In conclusion, that the choice of breast cancer treatment is influenced by the respondent's understanding of the disease. It is hoped that this research can be a source of information that can be used by health workers in providing nursing care.

Abstract

Meskipun secara global ada perubahan dalam tren epidemiologi dari penyakit menular ke penyakit kronis, prevalensi dan insiden penyakit menular serta MMR (Maternal Mortality Rate) atau AKI (Angka Kematian Ibu) dan AKB (Angka Kematian Bayi) di Indonesia masih tinggi. Tingginya angka kematian yang terjadi selama persalinan, diakibatkan faktor langsung seperti komplikasi kebidanan, perdarahan, dan eklampsia; atau faktor tidak langsung, seperti kekurangan gizi, penyakit menular dan tidak menular. Pada tahun 2000, Fakultas Keperawatan Unpad bekerja sama dengan RSHS membangun sebuah model dari ruang perawatan, yang berafiliasi dengan ruang ginekologi kebidanan. Model dibangun dengan tujuan untuk 1) meningkatkan kualitas pelayanan kesehatan; 2) untuk mengembangkan pengalaman mahasiswa dengan pasien; 3) menyediakan pendidik perawat berkualitas untuk menunjang siswa; 4) mendorong mahasiswa untuk meningkatkan hasil praktek klinis. Tujuan penelitian ini adalah untuk memberikan informasi dan gambaran tentang efektifitas dan tingkat kepuasan pasien pada pelayanan keperawatan maternitas. Hasil penelitian menunjukkan: di satu sisi, ada beberapa kekhawatiran berkaitan dengan efektivitas perawatan persalinan, yaitu: 1) rasio bidan dan pasien tidak ideal; 2) ada satu konsultan ginekolog obstetri dan satu dokter untuk setiap kamar. Delapan puluh persen lebih terapi delegasi untuk bidan dilakukan melalui komunikasi langsung, bukan melalui catatan medis. Di sisi lain, tingkat kepuasan pasien yang menerima perawatan bersalin adalah 58%, dan 42% tidak puas. Kesimpulan Penelitian ini merekomendasikan untuk mengambil perencanaan strategis yang komprehensif untuk meningkatkan pelayanan keperawatan dan kebidanan yang melibatkan semua pihak terkait di pemerintah, masyarakat sipil, pelayanan, pendidikan, dan organisasi profesiKata kunci: asuhan keperawatan maternitas, efektifitas, kepuasan pasien
