Abstract

Corona virus disease-19 (COVID-19) is a global health crisis problem. The clinical characteristics of disease progression and outcome in children and young adults appear to be much milder than in older people. The incidence of COVID-19 in children aged 10-19 years was 549 / 72,314 or 1% of all cases, while the age group <10 years was 416 / 72,314 (0.9%) cases. Compared to adults, the number of confirmed children cases is very low and the severity and mortality is even lower. Indonesia has 450 cases of COVID-19; 38 of them died as of March 21, 2020. The incubation period for COVID-19 in children based on case reports around the world ranges from 2 to 10 days. Data suggest 13-15% of virologically positive children may be asymptomatic. The most common symptoms described in the early phase of COVID-19 in children were fever (50%) and mild cough (38%). Other clinical features that can appear are sore throat, rhinorrhea, sneezing, myalgia, fatigue, diarrhea, dizziness, skin rash and vomiting.

Abstract

Paraquat dichloride is an active ingredient of the herbicide gramoxone which is often used for weed control and crop maintenance in the agricultural sector, paraquad dichloride is a broad spectrum contact herbicide which is geneally applied post growth. This herbicide is able to kill all types of weeds in the part that provides the herbicide solution direcly, because the dose and time of application used are correct so that the herbicide is effective in controlling weeds. Paraquat herbicide has a danger to organs such as eyes, skin, respiratory system, liver, heart, kidneys, and digestive tract. A man agreed to 56 years, a farmer, came with complacency since 3 days ago. Moisture with hoarseness, recovery, difficulty in carrying, and burning sensation in the chest. 5 days before admission, the patient inadvertently swallowed poison while working in the fields. On physical examination, hyperemic mucosa appears in the oral cavity, pharynx, and tonsils, as well as erosion in the posterior tongue and uvula. In indirect laryngoscopy, hyperemic mucosa and edema appear in the larynx and epiglottis. Investigations urea 276 mg / dl and creatinine 9.72 mg / dl. Patients diagnosed with acute laryngopharyngitis suspected corrosive esophagitis and post-intoxication of herbicides with acute kidney injury. Management of patients, namely: 1) 8 lpm oxygen with NRM, 2) IVFD RL 500 ml / 8 hours, 3) ceftriaxon 1 gr / 12 hours, 4) Furosemide / 8 hours, 5) Ranitidine 1 amp / 12 hours, 6) Metilprednisolone 125 mg / 12 hours, 7) Ketorolac 1 amp / 8 hours, 8) HD cito, 9) Bicnat 3 x 1000 mg, 10) Vit B12 3x1. The prognosis for paraquat poisoning greatly affects the amount of paraquat absorbed by the body, whereas the patient in this case is considered a good prognosis because the patient drank <20mg / kg of poison which could cause mild poisoning.

Abstract

Migraine is one of the most common types of primary headache in the general population. Previous research has shown that poor sleep quality is very common among migraine sufferers and is associated with exacerbations of migraine symptoms. Information regarding the prevalence and clinical implications of poor sleep quality among individuals with migraine is also scarce. The purpose of this study was to determine the relationship between sleep quality and the incidence of migraine in medical students at the University of Lampung. This study used a cross sectional method using a questionnaire that was used to conduct a sleep and migraine survey at the Faculty of Medicine, University of Lampung. Participants with a Sleep Quality Index (Pittsburgh Sleep Quality Index) PSQI > 5 were considered to have poor sleep quality and for migraine were conducted with a questionnaire adapted by HO K-H & Ong BK-C from the International Headache Society (IHS) classification. The results of the study of 65 participants, in the univariate analysis found 13 (20%) had migraines and 36 (55%) had poor sleep quality. In the bivariate analysis, it was found that p value = 0.04 with the number of respondents experiencing poor sleep quality and migraine, there were 11 (30.6%) respondents from 36 respondents who experienced poor sleep quality. Conclusions: Poor sleep quality was prevalent among participants with migraine. It is associated with exacerbation of migraine symptoms. The results of this study indicate that there is a significant relationship between sleep quality and the incidence of migraine in students of the Faculty of Medicine, University of Lampung.

Abstract

Upper airway obstruction is a blockage in the larynx caused by inflammation, foreign bodies, trauma, tumors so that ventilation is disrupted. One of the causes is hemangioma, which is the most common soft tissue tumor in newborns. In the state of hemangiomas, sometimes also found a syndrome in the form of Kasabach-Merritt Syndrome. As many as 20% of patients with KMS exhibit unusual symptoms compared to cutaneous hemangiomas. Clinical symptoms that can occur in upper respiratory tract obstruction include hoarseness, dysphony to aphony, shortness of breath (dyspnea), stridor (breath sounds) inspiration, suprasternal, epigastric, supraclavicular and intercostal retractions on inspiration. Kasabach-Merritt Syndrome is rare because the diagnosis is often delayed. KMS is more common in boys than girls. Ultrasound examination, Computer Tomography (CT) Scan, Magnetic Resonance Imaging (MRI) can help to determine the nature and extent of the lesion and identify the involvement of organs in the body. Extensive bleeding in KMS can cause consumptive coagulopathy that occurs due to excessive use of coagulation factors such as fibrinogen. Coagulopathy will progress to Disseminated Intravascular Coagulation (DIC) and even death. From this condition, the patient also experienced bronkopneumonia which is an acute infection of the lungs affecting the lung lobules starting from the lung parenchyma which can be caused by various etiologies such as bacteria, viruses, fungi and foreign bodies. This is because patients have a susceptibility to infection due to their condition.

Abstract

The tremendous increase in infections caused by viruses and their mortality rate has demanded producing effective and safe antiviral drugs. Symptomatic treatment and immune enhancement are the most efficient methods because no specific drugs are available for every viral disease. Several naturally occurring compounds available have demonstrated antiviral potential in preventing and debilitating viral diseases. A nutrient-rich diet can help reduce many viral infections. Nutrients such as vitamins (vitamins A, D, C, E, B6, and B12) and elemental groups provide the highest nutritional value for all body systems, including bones, cardiovascular, liver, skin, and support immunity. This research provides information on dietary-based scientific recommendations, evidence, and case studies worldwide concerning a particular focus on viral infections. In this study, the use of bioactive compounds in viral mechanisms, particularly SARS and respiratory viral infections, will be discussed, which may help in facilitating preintervention either directly as supplements/fortified foods or in combination with pharmaceutical drugs. This research method reviews the immune potential in the content of dietary supplements based on laboratory studies and human research through international journals, trusted websites, and books in the last ten years. The result is that bioactive compounds such as vitamins, elements, and minerals with proven antiviral properties, in addition to pharmaceutical drugs themselves as dietary supplements, have been shown to play an important role in treating various infections caused by viruses and strengthening the immune system.

Abstract

Glaucoma still be the second most common cause of blindness in the world. Glaucoma is a group of neuropathic diseases of the optic nerve which is characterized by the extraction of the optic disc (cupping) may causes visual disturbances. Intraocular pressure (IOP) is the most influential factor of glaucoma and affected by various factors, especially body position during sleep. Sleep composed of REM sleep and NREM sleep is related to muscle tone and psychological activity that has impact for sleep posture. The increase of intraocular pressure is directly proportional to the increase of episcleral venous pressure. The increase of IOP in prone position is probably due to an increase in choroidal vascular volume and increase of episcleral venous pressure, whereas in the lateral decubitus position the increase in IOP is caused by compression of the jugular veins when using a pillow. IOP rises when move to the lateral decubitus position in the dependent eye was 1 mmHg and in the non-dependent eye was 2 mmHg. The angle of the head to the bed has an effect on increasing IOP, in the vertical head-down position there can be an increase of 70% due to the force of gravity, whereas in patients who sleep with their head at a higher angle to the mattress, the IOP will decrease compared to the horizontal sleeping position.

Abstract

Human immunodeficiency virus (HIV) is an infection that attacks the immune system, specifically white blood cells called CD4+ cells. HIV continues to be a major global public health problem, claiming 40.1 million lives so far. By 2021, 650,000 people will die from HIV-related causes and 1.5 million people will contract HIV. HIV destroys CD4+ cells, thereby overcoming a person's immunity against opportunistic infections, such as tuberculosis, yeast infections, bacterial infections, and some cancers. Case report of a 29 year old man, came with complaints of shortness of breath since 3 days before entering the hospital. Complaints are accompanied by a long cough, weakness, cold sweats at night, loss of appetite, and weight loss. The serological examination showed HIV reactive results and the chest X-ray showed the impression of active pulmonary TB. The diagnosis established was HIV grade III with clinically diagnosed pulmonary TB, after which the patient was given HIV treatment with co-infected pulmonary TB.

Abstract

Obstructive ileus or intestinal obstruction is a serious abdominal condition that requires immediate action. One of the causes of obstruction is the presence of a foreign body in the form of the nematode Ascaris lumbricoides. Ascaris lumbricoides infection is a disease that is still widely experienced, especially in children in developing countries. The prevalence of worms in Indonesia has a fairly high range, between 2.5% -62%. Complications in the form of obstructive ileus if not managed can cause death. Intestinal obstruction due to Ascaris lumbricoides can occur when there are many worms, namely more than 60 individuals. This infection is often asymptomatic, but symptoms that often arise are gastrointestinal system disorders such as abdominal pain, nausea, vomiting, and anorexia. Symptoms of obstruction may include vomiting, abdominal distention, increased bowel sounds, darm steifung, darm contour, collapse of the ampulla recti. The initial examination that can be done is in the form of a complete stool examination and abdominal ultrasound to see the presence of worms in the intestine. Management in the form of administering anthelmintic drugs, intestinal decompression using a nasogastric tube, or surgery can be performed.

Abstract

Polytrauma is trauma that occurs at several areas on the body, followed by decline in physiological function which has the potential to cause multiple organ dysfunction and death in patients. Initial assessment of polytrauma is important to identify injuries that require initial treatment, determine the management of these injuries, and assess the patient's prognosis. There are several scoring systems that can be used to assess polytrauma injuries. Abbreviated Injury Scale (AIS) can be used to assess the severity of the injury and the probability of death of the patient. The Injury Severity Score (ISS) is a scoring system that assesses the three most severe injuries from six body regions. New Injury Severity Score (NISS) is an update from ISS; assessment of the three most severe injuries to the body regardless of body region. These assessment methods can help the trauma care system to better prevent complications and death in polytrauma patients.

Abstract

Hepatitis virus infection is a global public health threat that causes substantial liver-related morbidity and mortality. Hepatitis is a liver infection that causes inflammation, causing swelling of the liver. This liver infection has various variants, all of which are detrimental to human health. In recent times, natural compounds have become popular to be developed as antiviral drugs to fight viral infections. These natural compounds can come from plant extraction or a combination of animal and plant extraction. Several of these compounds have shown significant effectiveness in their mechanism as antivirals against the hepatitis virus due to the presence of bioactive components in them. The mechanism of action of this bioactive component is demonstrated by disrupting the virus life cycle, replication, release, and interaction with the host. This literature study will explain strategies for treating hepatitis with these natural compounds. Recent research shows that bioactive components such as phenylpropanoids, flavonoids, xanthones, anthraquinones, terpenoids, alkaloids, and aromatics have antiviral activity against hepatitis. Therefore, based on a literature review of the latest research that reveals clinical trials of these bioactive components, this natural compound can be used as a potential treatment alternative in the management of hepatitis in the future.
