Abstract

Psychosocial health is one of the most important factors in achieving optimal growth and development in children. The increase in psychosocial problems among children has been accompanied by a decrease in the rate of exclusive breastfeeding over the past two years. Another important aspect is the history of natural disasters such as tsunamis, military operations, and the implementation of Islamic law in Aceh. This research aims to explore the relationship between exclusive breastfeeding and psychosocial problems in children in the city of Banda Aceh. The research method employed in this study is an analytical observational design with a cross-sectional approach. The population of this research comprises all children aged 4-6 years old. Samples were obtained using a cluster sampling technique, involving 296 children from 5 villages around one of the public health centers in Banda Aceh. Data analysis in this study is conducted using a logistic regression test. The results of the study indicate that the percentage of children aged 4-6 years in Banda Aceh who have received exclusive breastfeeding is 261 children (88.18%), while the number of children with psychosocial problems based on the Pediatric Symptom Checklist 17 is 27 children (9.12%). There is a significant relationship between exclusive breastfeeding and psychosocial problems in children aged 4-6 years in the working area of the Baiturrahman Public Health Center in Banda Aceh. Children who did not receive exclusive breastfeeding have a 5 times higher risk of experiencing psychosocial disorders compared to children who received exclusive breastfeeding during the first 6 months of life

Abstract

Background: The prognostic nutritional index (PNI) parameter has been widely used in estimating the severity of COVID-19. In contrast, investigating the role of the systemic immune-inflammation index (SII) in determining the COVID-19 severity is prospective. This study aimed to investigate the potential of PNI and SII parameters to distinguish the severity of symptoms of COVID-19.Methods: A retrospective observational study was conducted among 209 hospitalized patients with COVID-19. Data were collected from August 2021 to February 2022 in Indonesia’s general COVID-19 referral hospital. Demographic and laboratory data, including PNI and SII, were analyzed and compared between the severe and non-severe symptoms of COVID-19 patients. The statistical analysis of the receiver operating characteristic curve (ROC) and area under curve (AUC) was conducted to predict the potential of these parameters in distinguishing the COVID-19 severity. Results: More than half of this study’s patients (54.55%) were non-severe COVID-19. The SII values in patients with severe symptoms were significantly higher than in those with non-severe symptoms (2,445.24 vs. 1,423.28, p=0.005). In contrast, the PNI value in patients with severe COVID-19 symptoms was significantly lower than those with non-severe symptoms (38.04 vs. 33.93, p<0.001). The area under the curve (AUC) value of PNI was 0.694, meanwhile the SII was 0.635. The optimum cut-off for the PNI was <35.407, whereas the SII was >2,212.787. PNI and SII were the potential new diagnostic parameters for COVID-19 severity. Conclusion: PNI and SII parameters can potentially distinguish the severity of symptoms of COVID-19.

Abstract

Chronic kidney disease (CKD) patients, especially those with hemodialysis, frequently struggle with mental health issues like anxiety and depression. Psychotherapy has been known to treat psychological problems, but its effectiveness in managing CKD patients is still rarely scientifically proven. The aim of this study was to analyze the role of psychological treatments in improving the mental health of CKD patients with hemodialysis. We comprehensively reviewed the related studies published in PubMed, Google Scholar, ScienceDirect, and Clinical Key over the last ten years, up to June 7, 2023. A keyword combination was used in the search engine strategies, and all articles about CKD patients receiving hemodialysis and psychotherapy were included. Based on the eligibility criteria, 716 patients were included in 13 out of 18,830 studies in the final analysis. Psychological problem was complained by 399 CKD patients. The psychotherapy included cognitive behavioral therapy (reported in four studies, n=4), diaphragmatic breathing relaxation (n=1), meditation (n=1), hypnotherapy (n=1), Kidney Optimal Health Program (KOHP) (n=1), psychological intervention (n=1), murottal Al-Qur'an therapy (n=3), and spiritual therapy (n=1). These interventions were performed once to four times a week, for ten minutes to five hours during hemodialysis for two to ten weeks. Meditation and KOHP showed no significant improvement in anxiety and depression. The remaining psychotherapies significantly improved the quality of life by reducing anxiety and depression in hemodialysis patients and enhancing sleep quality, self-esteem, hopefulness, medication adherence, and physical condition. In conclusion, psychotherapy should be considered in an interdisciplinary team to treat CKD patients comprehensively. Further studies are still necessary to determine the efficacy of each psychological intervention in CKD patients with psychiatric problems.

Abstract

Background: Malignancy and inflammation are strongly connected. The inflammatory processes play a significant part in the development of lymphoma. Neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), and platelet-to-lymphocyte ratio (PLR) can be used as potential biomarkers of inflammation in lymphoma. This study aimed to discover the differences between NLR, MLR, and PLR in patients diagnosed with non-Hodgkin lymphoma (NHL), and Hodgkin lymphoma (HL). Methods: This study employed a retrospective design using data from the lymphoma registry at Dr. Hasan Sadikin General Hospital, Bandung, Indonesia from 2020 to 2023. Sampling was carried out consecutively. Hematological data of patients with NHL and HL before chemotherapy were collected. The variance between the two groups was examined utilizing the Mann-Whitney U test. Results: In total, 122 data of patients were included, consisting of 75% NHL patients and 25% HL patients with a median age of 54 years (IQR 43–62). The overall NLR, MLR, and PLR tended to be lower in nHL than in HL patients although the differences were not statistically significant; with NLR 2.7 (0.7 – 12.2) vs. 3.2 (1.1 – 10.8)  p=0.287, MLR 0.36 (0.04 – 1.86) vs. 0.46 (0.09 – 1.78) p=0.150, and PLR was 160.6 (20.2 – 1533.3) vs. 211.2 (50.6–1156.3) p=0.189, for NHL and HL, respectively. Conclusion: The lower values of NLR, MLR, and PLR in NHL indicate lower systemic inflammatory status in NHL than HL patients. Further studies are needed to evaluate dynamic changes of these biomarkers during treatment.

Abstract

Background: The duration of untreated psychosis (DUP) is a crucial factor influencing the prognosis of schizophrenia, affecting long-term outcomes and treatment efficacy. Despite its importance, there is a paucity of research on the determinants of DUP in Indonesian patients with schizophrenia, highlighting the need for a focused investigation within this specific context. This study aims to identify the determinants of DUP among patients with schizophrenia in Indonesia. Methods: A cross-sectional study was conducted involving patients with schizophrenia admitted to a referral psychiatric hospital in Indonesia. Data were collected using guided interviews and observations through structured questionnaires. Additionally, socio-demographic information was gathered. Results: A multiple linear regression analysis revealed that previous consultations with traditional healers (β = 21, 95% CI = 11.4 - 30.6, p = 0.001) and religious healers (β = 16.7, 95% CI = 8.2 - 25.2, p = 0.0001) were independently predicting the longer DUP. Conclusions:Treatment-seeking behavior is significantly influencing the DUP in patients with schizophrenia. Efforts should focus on improving health literacy to encourage earlier mental health treatment-seeking.
