Abstract

Hyperuricemia is a pathophysiological condition associated with chronic inflammatory diseases such as rheumatoid arthritis, diabetes, and cardiovascular and renal diseases. Hyperuricemia is related to lifestyle. Elevated serum uric acid levels can be a marker for metabolic syndrome which generally occurs in adults. So far it is not known what the prevalence of hyperuremia and obesity is among members of the Kodim XXX team and adults of productive age in West Jakarta. Promotion of healthy lifestyles and prevention of disease are fundamental principles behind public health and improving public health. Measuring uric acid levels can prevent acute attacks of gout. Anthropometry is a measurement method used to assess the size, proportions and composition of the human body. The results of anthropometric measurements can be used to assess nutritional status. The target of this activity is the XXX Kodim Team and the adult community of productive age in West Jakarta. The aim of this service is to obtain an overview of uric acid and nutritional status in the Kodim XXX team and adults of productive age so that they can prevent the occurrence of metabolic syndrome in the future.

Abstract

Diabetes is common in the elderly, and routine monitoring of HbA1c and blood glucose is required to prevent complications. HOMA-IR assesses insulin resistance, while vitamin D may influence glucose metabolism by enhancing insulin sensitivity. This study examines the relationship between vitamin D levels and glycemic control, including HbA1c, fasting glucose, insulin, and HOMA-IR. This cross-sectional observational study included 91 elderly residents of Panti Wreda Bina Bhakti, excluding those with acute illness or chronic conditions affecting Vitamin D and diabetes parameters. All participants provided informed consent. The study measured serum Vitamin D-25(OH) and insulin using ELISA, while fasting blood glucose and HbA1c were assessed via blood chemistry analysis. HbA1c control was categorized as good (<6.5%), moderate (6.5–8.0%), or poor (>8%). HOMA-IR was calculated using a standard formula. Pearson correlation analysis determined the relationship between Vitamin D levels and glycemic parameters, with statistical significance at p < 0.05. The correlation analysis found a significant negative relationship between vitamin D levels and HbA1c (r: -0.211; p: 0.044). However, no significant correlation was found between vitamin D and fasting blood glucose levels (r: 0.057; p: 0.590), insulin (r: -0.083; p: 0.432), and HOMA-IR (r: -0.040; p: 0.704).  Vitamin D levels show a significant negative correlation with HbA1c in the elderly, indicating its role in long-term glycemic control. Although no significant relationship was found with other glycemic parameters, these findings suggest that monitoring vitamin D status may be considered as part of a more comprehensive approach to diabetes management in the geriatric population.

Abstract

Livedo reticularis  merupakan kelainan vaskular kulit yang ditandai dengan pola jaring berwarna kebiruan akibat perubahan aliran darah pada mikrosirkulasi dermis. Salah satu penyebab sekunder yang paling sering dilaporkan adalah penggunaan amantadin, obat antiparkinson dengan mekanisme antagonis reseptor NMDA. Efek ini penting dikenali, terutama pada populasi geriatri yang rentan terhadap efek samping obat. Tujuan laporan kasus ini untuk melaporkan kasus livedo reticularis pada pasien Parkinson usia lanjut yang menggunakan terapi amantadin, serta meninjau hubungan patofisiologi, karakteristik klinis, dan reversibilitas efek kulit tersebut. Laporan kasus observasional pada seorang wanita berusia 64 tahun dengan diagnosis Parkinson stadium 4 yang telah menggunakan amantadin 100 mg dua kali sehari selama ±4 tahun. Pasien menunjukkan lesi makular retikuler berwarna kebiruan pada kedua tungkai bawah, tanpa keluhan nyeri atau gatal. Tidak ditemukan tanda-tanda vaskulitis atau gangguan sensorik. Lesi diklasifikasikan sebagai livedo reticularis sekunder non-inflamasi yang disebabkan oleh amantadin. Berdasarkan literatur, onset livedo reticularis dapat muncul setelah beberapa minggu hingga beberapa tahun penggunaan, dan umumnya membaik setelah penghentian obat. Livedo reticularis merupakan efek samping kulit yang khas dan relatif sering terjadi akibat terapi amantadin pada pasien Parkinson. Kondisi ini bersifat jinak, reversibel, serta tidak memerlukan penghentian terapi apabila manfaat neurologis lebih besar daripada efek kosmetik yang ditimbulkan. Deteksi dini dan edukasi pasien sangat penting untuk memastikan keamanan penggunaan obat pada populasi geriatri.

Abstract

Non-communicable diseases (NCDs) such as hypertension, diabetes, and dyslipidemia are major global health challenges that contribute to high morbidity and mortality rates. Unhealthy lifestyles, including poor diet, lack of physical activity, and high-stress levels, are key risk factors for NCDs. Early detection through regular screening is essential to prevent complications and improve public health. This community service program aimed to conduct health screenings by measuring blood pressure, blood glucose levels, and cholesterol levels among adults of productive age in West Jakarta. The program also included health education sessions to raise awareness about NCD risk factors and the importance of a healthy lifestyle. Participants underwent blood pressure measurements, random blood glucose tests, and total cholesterol assessments, followed by health counseling and lifestyle modification recommendations. The results emphasized the need for routine health monitoring and preventive interventions to reduce the burden of NCDs in the community.

Abstract

Latar Belakang: Gangguan afektif bipolar remaja merupakan tantangan diagnosis dan penanganan karena gejala tumpang tindih dengan gangguan lain serta faktor psikososial seperti trauma dan konflik sosial yang mempengaruhi kondisi pasien. Penelitian ini bertujuan menggambarkan proses diagnosis dan tata laksana komprehensif pada pasien remaja dengan bipolar depresif psikotik serta menganalisis faktor neurobiologis, psikososial, dan ritme sirkadian yang memengaruhi kondisi klinisnya.Metode: Studi kasus deskriptif kualitatif melalui wawancara mendalam, observasi, dan penelusuran rekam medis, menegakkan diagnosis ICD-10, dan menganalisis respons terapi farmakologis serta intervensi non-farmakologis terhadap pasien remaja dengan gangguan bipolar.Hasil: Pasien menunjukkan perbaikan gejala depresif berat disertai psikotik setelah terapi multimodal, dengan dukungan keluarga aktif serta pendekatan holistik, menegaskan pentingnya diagnosis dini dan strategi penanganan terintegrasi pada gangguan bipolar remaja.Kesimpulan: Pendekatan holistik dan diagnosis dini krusial untuk keberhasilan penanganan gangguan bipolar pada remaja, meningkatkan peluang pemulihan dan mencegah kekambuhan melalui kolaborasi tim medis dan partisipasi aktif keluarga maupun pasien.

Abstract

Chronic kidney disease (CKD) is a growing global health concern that frequently remains undiagnosed until advanced stages. Early detection through simple laboratory screening is essential to prevent disease progression and associated cardiometabolic complications. This community service program aimed to assess kidney function using serum creatinine and estimated glomerular filtration rate (eGFR), while increasing public awareness regarding CKD prevention. A total of 59 participants were included, with a mean age of 39.15 ± 15.39 years (range 16–75 years), predominantly female (74.58%). The mean serum creatinine level was 1.0 ± 0.19 mg/dL, and the mean eGFR was 91.08 ± 20.53 mL/min/1.73 m². Most participants demonstrated normal kidney function (28.8%) or mild decline (21.6%). A progressive reduction in eGFR with increasing age was observed, reflecting the physiological decline in nephron mass and renal perfusion. The program also provided education on kidney-protective practices, including optimal blood pressure control, diabetes management, adequate hydration, and avoidance of nephrotoxic agents. This intervention improved participants’ understanding of CKD risk factors and the importance of regular screening. In conclusion, serum creatinine and eGFR evaluation offer simple, accurate, and practical tools for early CKD detection, supporting promotive–preventive strategies to slow disease progression and enhance quality of life in at-risk populations.

Abstract

Latar Belakang: Penyakit ginjal kronik merupakan masalah kesehatan global dengan prevalensi yang meningkat dan menimbulkan beban yang signifikan pada sistem kesehatan. Pelaksanaan pemeriksaan laboratorium seringkali terkendala di komunitas, sehingga pemeriksaan fisik berpotensi menjadi metode alternatif deteksi dini fungsi ginjal. Tujuan penelitian untuk menilai kemampuan parameter pemeriksaan fisik sederhana sebagai alternatif prediksi fungsi ginjal di tingkat komunitas. Metode: Penelitian potong lintang dilaksanakan di lima wilayah administrasi Jakarta dengan responden dewasa yang memenuhi kriteria inklusi. Parameter yang diukur meliputi antropometri, tekanan darah sistolik, lipatan kulit, lingkar betis, lingkar perut, indeks massa tubuh, serta handgrip strength. Fungsi ginjal diestimasi melalui eGFR (CKD-Epi formula). Analisis menggunakan korelasi Pearson, dilanjutkan regresi linear berganda. Analisis regresi menghasilkan lima model dengan nilai R 0,517–0,527 dan Adjusted R² 0,254–0,255, menunjukkan kontribusi prediksi moderat. Hasil: Pada model akhir, parameter yang signifikan terhadap fungsi ginjal adalah tekanan darah sistolik (B = −0,337; p < 0,001), lingkar betis (B = 1,187; p = 0,001), caliper suprailiac (B = −0,971; p < 0,001), serta handgrip strength (B = 0,321; p = 0,023) dengan koefisien persamaan adalah 118,339. Kesimpulan: Pemeriksaan fisik sederhana, terutama lingkar betis, caliper suprailiac, tekanan darah sistolik, dan handgrip strength, berpotensi sebagai fungsi prediktif terhadap penurunan fungsi ginjal sebagai skrining awal di komunitas dengan melibatkan kader kesehatan.

Abstract

Non-alcoholic fatty liver disease has become a health problem increasing worldwide. This disease is characterized by excessive fat accumulation in the liver cells unrelated to alcohol consumption. Education and risk factor detection are essential in identifying and preventing the development of this disease. This community service involved 52 respondents at Kalam Kudus II Jakarta High School in May 2023. All participants participated in a series of activities starting from community counseling, disease detection, and education. The results of the medical examination revealed that there were 2 (3.8%) respondents who had increased serum glutamic oxaloacetic transaminase (SGOT)/ serum glutamic pyruvic transaminase (SGPT) levels and 6 (11.5%) had hypercholesterolemia. Education about fatty liver disease can increase public awareness of this condition and promote a healthy lifestyle and good eating habits. Detection of risk factors through laboratory tests such as SGOT, SGPT, bilirubin, and cholesterol is also important in early identification of this disease. In effective fatty liver disease management, it is important to increase awareness of counseling and risk factor detection. Collaborative efforts between medical personnel, health educators, and communities can help reduce the burden of this disease and improve the quality of life of affected individuals.

Abstract

Introduction. Chronic kidney disease (CKD) is a major public health issue, particularly among the elderly, where metabolic disturbances accelerate renal decline. Diabetes mellitus contributes to kidney dysfunction through insulin resistance and altered glycemic control. This study investigates the relationship between diabetes-related parameters—fasting blood glucose, HbA1c, fasting insulin, HOMA-IR, and kidney function, as measured by estimated glomerular filtration rate (eGFR) in an elderly population. Methods. A cross-sectional analytic study was conducted among 82 elderly residents of Bina Bhakti Nursing Home. Venous blood samples were collected to assess fasting blood glucose, HbA1c, fasting insulin, and HOMA-IR. Kidney function was estimated using the CKD-EPI equation. Results. Age showed a significant negative correlation with eGFR (r = –0.266, p = 0.016), indicating age-related renal decline. HOMA-IR (r = 0.247, p = 0.025) and fasting insulin (r = 0.224, p = 0.043) demonstrated weak but significant positive correlations with eGFR. Conversely, fasting glucose (r = 0.055, p = 0.623) and HbA1c (r = 0.029, p = 0.795) had negligible, non-significant correlations with eGFR, suggesting limited predictive value for renal impairment. Insulin growth factor exhibited a weak, non-significant negative correlation (r = –0.104, p = 0.355). Conclusion. Age remains a key determinant of declining kidney function in the elderly. Insulin resistance markers showed mild but unexpected associations, warranting further longitudinal studies to confirm these findings and explore underlying mechanisms.

Abstract

Hansen’s disease (leprosy) remains a public health problem in Indonesia. Although national elimination was declared in the year 2000, the number of new cases has remained stagnant at around 17,000–20,000 cases per year, and the proportion of Grade 2 Disability (G2D) remains high. Social stigma, delayed diagnosis, and poor adherence to Multi-Drug Therapy (MDT) continue to be major challenges in achieving elimination. This study aims to evaluate MDT adherence based on three Hansen’s disease patients and to analyze the factors influencing it. This research uses a descriptive design with a three-case report approach analyzed from a public health perspective. The variables assessed include MDT adherence, family support, stigma, patient knowledge, and drug adverse effects. Two of the three patients demonstrated high adherence to MDT, while one patient exhibited non-adherence due to a severe drug reaction (dapsone hypersensitivity syndrome). Factors influencing adherence include patient knowledge, drug side effects, family support, and special conditions such as pregnancy. MDT adherence remains a challenge in leprosy elimination efforts. A multidisciplinary approach involving education, intensive monitoring, and stigma prevention is needed to improve treatment success.
