Abstract

Dengue virus is a human pathogen that has as an increasingly important public health threat. These virus have four serotypes (DEN-1, DEN-2, DEN-3, DEN-4). There is no specific treatment for any pathogenic dengue virus and there are few vaccines available. This study was carry out to E protein characterization from dengue virus infected C5/36 cells for vaccine candidate. We found that E protein induced antibody response. The data of the reactivity on the nitrocellulose membrane between E protein against polyclonal and monoclonal antibodies on western blot analysis, shown that the E protein whisc was once of dengue virus protein has immunogenic. The Indonesia isolate had no difference with the Phillipine isolate. The result suggested that all of the E protein dengue virus strain have peptides overlapping. Also promising for he production of immunogenic particulate proteins, especially DEN protein for the four strains.

Abstract

Depression is an important mental health problem in developed societies, and as many as 50%-90% of patients with depression complain of sleep disorders. In the elderly, sleep disorders comorbid depressive disorders are getting worse. Late-life Depression (LLD) is often not detected because the symptoms are less specific than younger age. It even provides an independent risk factor impact on suicide ideas and behaviour. Causative risk factors of sleep disorders in Late-life Depression are biological and non-biological factors that form the basic of pharmacological and non- pharmacological management.

Abstract

Background: Some species of the Mycobacterium tuberculosis complex (MTBC) which can cause tuberculosis, particularly M. tuberculosis and M. bovis, may have different virulence property and therefore associated with various clinical severity in tuberculosis (TB) patients. The aim of this study was to determine the differences in clinical and demographic characteristics between M. tuberculosis and M. bovis infection among Indonesian TB patients. Methods: Thirty-one new and active TB patients were confirmed to have acid fast bacilli (AFB) sputum positive and/or Xpert MTB/RIF positive for M. tuberculosis from Dr. Soewandhie Hospital, Surabaya, Indonesia. Interviews were conducted to record the clinical and demographics required. The MTBC were isolated from bronchoalveolar lavage fluid (BALF) and determined by primer-specific PCR targeting TbD1 and RD9 region gene. The degree of lung tissue damage was classified using NICE Scoring System. Results: The MTBC were detected in all patients on whom 19 and 12 isolates were classified as M. tuberculosis and M. bovis respectively. There was a different on 74.2% of productive age subjects (21-50 years) with M. tuberculosis infection (P

Abstract

Human Immunodeficiency Virus type 1 (HIV-1) has been known to cause Acquired Immune Deficiency Syndrome (AIDS) disease and has been alaso divided into several subtypes (A, B, C, D, F, G, H, J, K) and Circulating Recombinant Form (CRF). Different characteristics of subtype of the virus and its interaction with host can affect the severity of the disease. This study was aimed to analyze HIV-1 genotypes circulating in HIV/AIDS patients from the East Java region descriptively. Information from this research was expected to complement the data of mocular epidemiology of HIV in Indonesia.  This study used blood plasma from patients who had been tested to be HIV positive who were seeking treatment or are reffered to the Intermediate Care Unit of Infectious Disease (UPIPI) Dr. Soetomo Hospital Surabaya from various area representing the East Java regions. Plasma was separated from blood samples by centrifugation for use in the the molecular biology examination including RNA extraction, nested PCR using specific primer for HIV gp120 env gene region, DNA purifying, DNA sequencing, and homology and phylogenetic analysis. Based on the nucleotide sequence of the HIV gp120 env gene, it was found that the most dominant genotypes in East Java belonged to one group of Circulating Recombinant Form (CRF), namely CRF01_AE and CRF3x_01B, which has been also found in Southeast Asia. In the phylogenetic tree, most of HIV samples (30 samples) were in the same branch with CRF01_AE and CRF3x_01B, except one sample (HIV40) was in the same branch with subtype B.

Abstract

Polymorphisms in TNF-α gene promoter region are known of its role in the production of TNF-α which may influences the pathogenesis of liver disease. SNPs in positions 238 and 308 of TNF-α gene promoters may affect the production of these cytokines. This study was aimed to detect Single Nucleotide Polymorphism (SNP) on -238 and -308 positions in the TNF-α gene promoter among liver cirrhosis patients with HBV infection in Surabaya, Indonesia. This was descriptive exploratory research with cross sectional study design using serum liver cirrhosis patients with HBV infection in Endoscopy Outpatient Clinic Dr. Soetomo General Hospital, Surabaya from April-May 2017. SNPs at -238 and -308 on TNF-α gene promoter (rs361525 and rs1800629 respectively) were detected using Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP) with primers specific for the TNF-α promoter region and restriction enzymes NcoI and MspI. The genotypes of TNF-α gene promoter were assessed according to the length of the fragments produced in RFLP. Serum TNF-α levels was measured by commercial ELISA. In this study, as much as 149 positive HBsAg patients was found in Endoscopy Outpatient Clinic, Dr. Soetomo General Hospital, Surabaya. From those amount, as much as 30 liver cirrhosis patients with positive HBsAg were obtained. From 2/30 (6.7%) patients showed the GA heterozygote SNP either position -238 or -308. No patient had the AA genotype. Median blood TNF-α level in women (38 ng / L) was higher than in men (33 ng / L). TNF-α levels in patients with GA heterozygote genotype at -238 and -308 in this research was not different than wild-type (GG genotype). Among patients with liver cirrhosis due to chronic HBV infection in Surabaya, Indonesia, Surabaya, we found GA polymorphisms the TNF-α promoter gene at positions -238 and -308 in 6.7% patients, and did not find homozygous AA polymorphisms. Further studies including larger numbers of patients from various ethnic backgrounds in Indonesia are needed to provide robust data on TNF-α gene promoter polymorphisms and their role in the pathogenesis of liver cirrhosis with HBV infection in this country.

Abstract

AbstractIntroductions: Pregnancy is a stressor to women, in terms of biological as well as psychological. For individuals who previously received medication for schizophrenia in the past, it can be a triggering factor for the relapse of the illness. Here we describe a case of a pregnant woman who experiences a relapse in pregnancy. Case: The patient was a 40-year-old pregnant woman, brought to the emergency room after attacking her father by stabbing her father's head with a knife. She was suspicious of her dad's gaze at her. She was hostile towards healthcare workers when being examined. For her pregnancy, consultation with the Obstetric and Gynaecology department had been done to ensure that she and her baby were taken care of while being treated in the psychiatry ward. Discussions: The patient was diagnosed as having paranoid schizophrenia recurrent episodes. There was no complication found in her pregnancy, and she was scheduled for Sectio Caesarean as she was not mentally fit to do vaginal delivery. The psychiatric approach to the patient was atypical antipsychotic along with supportive psychotherapy. Psychoeducation and family therapy were also done to ensure that the patient's family understand the illness and the importance of compliance with the medication. Conclusions: Pregnancy is one of the factors that lead to the relapse of schizophrenia in patients with a history of mental illness. Holistic treatment is needed to treat the patient to prevent the relapse of the illness.

Abstract

Background: Ramadan fasting is one religious challenge for children with type 1 diabetes mellitus (T1DM) which required specific management and strict monitoring. Fructosamine, which reflects glycemic control within 2-3 weeks, is an ideal parameter of glycemic control in Ramadan fasting. This study aimed to analyzed the effect of Ramadan fasting on fructosamine level of children with T1DM.Subjects and Method: This prospective cohort study included children aged under 18 years old diagnosed wit T1DM for >6 months at Dr. Soetomo Hospital, Surabaya, Indonesia in Ramadan 1438 H (May-June 2017). The independent variable from this study was Ramadhan fasting and the dependent variables were fructosamine and HbA1c. The sampling technique was total sampling. Forty-two children were included and devided into 27 children in the fasting and 15 children in the non-fasting group. Fructosamine and HbA1c levels were measured before, mid- and after Ramadan and the comparison between groups were analyzed using the independent T-test.Results: The mean age of all the subjects was 8.95 (SD=3.2) years old, 47.6% were males. The mean (SD) HbA1c level of all the subjects was 10.42 (1.16) %. There was a mild reduction of mean fructosamine level after Ramadan in the fasting group. There was no significat differece I fructsamie level before (Mean= 556.8; SD= 180.39; p= 0.620), mid (Mean= 568.07; SD= 175.37; p= 0.520), and after Ramadhan between groups (Mean= 539.41; SD= 173.04; p= 0.410) respectively. The average fructosamine levels after Ramadan was positively and strongly related to HbA1c (r=0.69; p<0.001). There was no case of severe hypoglycemia and diabetic ketoacidosis in both groups. Conclusion: There is a reduction of mean fructosamine level after Ramadan fasting in children with T1DM although it isn’t statistically significant. Keywords: type 1 diabetes, children, fasting, Ramadan, fructosamineCorrespondence: Nur Rochmah.
