Abstract

Asthma is still become a major health problem worldwide, affected all ages from pediatrics to adults. The severity of the disease vary from mild to severe and leads to the risk of death. The uses of medicinal plants as antiasthmatic have often been conducted by the local community. The present study was conducted in vitro and in vivo to determine the activities of herbs containing of sembung leaves (Blumea balsamifera L.), kemukus seeds (Piper cubeba L.), teki rhizome (Cyperus rotundus L.), and patikan kebo herbs (Euphorbia hirta L.) with certain ratio as an antiasthmatic. The in vitro test was carried out on isolated smooth muscle cells of guinea pig. Histamine was used to stimulate the contraction of smooth muscle cells of guinea pig trachea. The in vivo test was carried out using guinea pigs which sensitized with ovalbumin and administered with the herbs formula. The trachea of guinea pigs were prepared for histopathological examination and stained on toluidine blue. The result revealed that the antiasthmatic herbs of 1, 2, and 3 showed activities as H1 receptor antagonists. The antiasthmatic herbs of 1, 2, and 3 were able to relax the smooth muscle of guinea pig’s trachea that was experiencing contraction, but in lower level compared to theophylline. The antiasthmatic herbs formula in this study did not have the activity to inhibit mast cell degranulation.

Abstract

Background: Neutropenia can increase morbidity in cancer patients because it can cause an increased risk of infection, delay or discontinue chemotherapy, and decrease the dose of cytotoxic agents which affect the patient's response and clinical outcome. The administration of G-CSF can increase ANC levels in neutropenic patients. The effectiveness of G-CSF was assessed by increasing ANC levels compared to before the administration of G-CSF.Objective: This study aims to determine the effect of G-CSF administration on ANC levels in cancer patients with neutropeniaMethods: This research was conducted at Dr. Kariadi General Hospital Semarang in October-December 2022 period with retrospective cohort method. The sample was cancer patients who experienced neutropenia and were given G-CSF.Results: Administration of G-CSF significantly increased ANC values (p-value 0.00 <0.05), increased by 921.07 ± 647.20 in the administration of filgrastim and 933.13 ± 558.89 in the administration of lenograstim and shortened the duration of ANC recovery (p-value 0.00 <0.05), recovery was 1.5 days shorter with filgrastim and 1.9 days with lenograstim.Conclusion: G-CSF Filgrastim and Lenograstim administration can increase ANC levels and shorten the duration of neutropenia treatment

Abstract

Background: Fluid overload is the common condition in the critically ill patients. This associated with the increased fluid intake and inadequate elimination. The management of fluid overload is by fluid removal with diuretic. Furosemide is a loop diuretic that frequently used as the initial therapy. However, the utilization of furosemide in the critically ill reminds a polarizing subject. The purpose of this study is to investigate the impact of furosemide on patients who are critically ill. Methods: The inclusion criteria included randomized controlled trials as well as observational cohort studies. The data sources utilized in this study were PubMed, Science Direct, ProQuest, and Cochrane.Results: We included 13 articles, of which 9 articles about generally critically ill patients with or without acute kidney injury (AKI), 2 articles about heart failure, and 2 articles about post operative. The furosemide was effective in generally critically ill patients with or without AKI, it can decrease the fluid balance, weight change, and improves the urine output. Furosemide had no harmful effect on kidney function. However, patients without oliguria were not recommended to receive high dose of furosemide. Critically ill patients with heart failure who received continuous infusion of furosemide were more susceptible to increased diuresis and greater depression of thoracic fluid content (TFC). Furthermore, it might cause the decrease of renal function. When compared to furosemide, continuous veno-venous hemodiafiltration (CVVHDF) was more successful at removing excess fluid, reducing weight, relieving symptoms, and improving hemodynamic and cardiac performance. In post operative patients, furosemide might cause metabolic alkalosis. Urinary electrolyte excretion rates were promptly altered by the use of low dose furosemide.Conclusion: Based on the patient's clinical data, furosemide use should be taken into consideration. In general, furosemide is effective to improve diuresis. Furosemide in the AKI condition with oliguria has beneficial effect. However, in heart failure condition, furosemide might affect renal function. In post operative, it might cause metabolic alkalosis. Further randomized controlled trial (RCT) is required.

Abstract

Gastroesophageal Reflux Disease (GERD) pada lazimnya disangka seperti penyakit yang ringan, namun mampu menimbulkan kekambuhan dan kematian bila tidak diatasi dengan benar. Tingkat kekambuhan terapi pada pasien GERD diperkirakan bahwa 10% sampai 40% dari pasien GERD. Studi berikut berupa narrative review dengan mengunakan pustaka acuan yang mengacu pada pangkalan data Cochrane, PubMed, dan Science Direct. Penggalian pustaka acuan memakai kata kunci GERD, PPI, keberhasilan terapi GERD.  Proton Pump Inhibitor (PPI) merupakan terapi yang sangat besar pengaruhnya akan memulihkan GERD, jika dipadankan terhadap antasida, prokinetik, dan H2 bloker receptor. Proton Pump Inhibitor (PPI) telah merevolusi penatalaksanaan pasien GERD karena efek penekanan asam yang mendalam dan berkepanjangan sehingga menjadi terapi andalan, bila dibandingkan dengan obat penekan asam lainnya, PPI menunjukkan penyembuhan mukosa dan pengendalian gejala yang lebih baik. serta mencegah mukosa dan gejala kambuh.

Abstract

Anticoagulants are prescribed very frequently, but their use is often associated with adverse drug reactions. The optimal monitoring of heparin is unknown, either by aPTT or anti-Xa levels. This paper compares which anti-Xa or aPTT most closely correlates with patients on intravenous heparin. This review also compares the achievement of target goals of unfractionated heparin (UFH) using aPTT and anti-Xa tests. Literature searching was started in June 2024 and ended in July 2024 through some online scientific databases: Scopus, PubMed, Google Scholar, Science Direct, and Willey Online Library. This review includes fifteen studies, at least involving 2,938 patients or 94,038 sets of measurements. The review demonstrates that anti-Xa outperforms aPTT in achieving the therapeutic range of unfractionated heparin, time to therapeutic goal, and dose modification.

Abstract

Salah satu penyakit degeneratif yang paling sering diderita pada pasien geriatri yaitu hipertensi. Hipertensi dikenal dengan meningkatnya tekanan darah yang melebihi batas normal yaitu 140 mmHg untuk tekanan darah sistolik dan 90 mmHg untuk tekanan darah diastolik. Tujuan dari penelitian ini adalah untuk mengetahui bagaimana gambaran pola peresepan obat antihipertensi pada pasien geriatri. Penelitian ini bersifat observasi/deskriptif dengan pengambilan data retrospektif. Data diambil dari catatan rekam medik pada pasien geriatri. Kriteria inklusi yang digunakan yaitu pasien geriatri dengan diagnosis hipertensi primer (I10 Esseential [Primary] Hypertension) yang dirawat inap di RSUP Dr. Sardjito. Hasil penelitian dari 98 pasien, 45,9% diberi resep monoterapi, 35,7%menggunakan dua kombinasi obat antihipertensi, 11,2% diberi tiga kombinasi obat antihipertensi, dan 7,1% diberi empat kombinasi obat. Pada penelitian ini yang paling banyak diresepkan pada pasien geriatri rawat inap RSUP Dr. Sardjito yaitu amlodipin sebesar 27,55%. Calcium channel blocker khususnya amlodipin merupakan obat antihipertensi yang paling sering digunakan, dan ditemukan pada pasien geriatric dianggap efektif dan dapat ditoleransi dengan baik.

Abstract

Type 2 diabetes mellitus is a chronic disease with a globally increasing prevalence. The treatment of diabetes mellitus involves oral blood glucose-lowering agents, including glibenclamide. Although effective, glibenclamide can cause severe hypoglycemia, leading people to often combine it with herbal remedies such as the root extract of Eurycoma longifolia (ELRE), commonly known in Malaysia and Indonesia as the herbal medicine Tongkat Ali. ELRE is known for its anti-diabetic effects through enhanced insulin sensitivity and antioxidant activity. However, the pharmacokinetic interaction between ELRE and glibenclamide, particularly involving CYP3A4, which metabolizes glibenclamide, has not been well studied. This study employed an in silico approach using pharmacokinetic virtual screening server, followed by molecular docking analysis to evaluate the potential interaction. The results indicated that nearly all of the investigated ELRE chemical constituents showed high predicted affinity, with two compounds, 14,15β-dihydroxyklaineanone and niloticin, exhibiting higher affinity than glibenclamide for CYP3A4. These compounds may act as competitive substrates, potentially influencing glibenclamide metabolism.

Abstract

The Brassicaceae family is recognized for its antioxidant and anti-inflammatory properties. Due to its widespread distribution, it serves as a prominent source of bioactive phytochemicals, attracting considerable research interest. Brassicaceae plants contain several secondary metabolites, including glucosinolates, which are precursors to bioactive compounds like isothiocyanates (e.g., sulforaphane) that have demonstrated anti-inflammatory effects. This study aims to investigate keywords, countries, number of publications, institutions, authors, and journals related to Brassicaceae plants as anti-inflammatory agents during the period 2003–2024 using a comprehensive bibliometric analysis method. Information was collected using the Scopus database, followed by data analysis using the Biblioshiny R Package and VOSviewer. A total of 760 articles meeting the inclusion criteria were analyzed. The results indicate that 2023 saw the largest increase in research publications, with China emerging as the leading contributor, while the USA had the highest citation count. The International Journal of Molecular Sciences was identified as the most popular publishing journal. Kyung Hee University was designated as the most productive institution, with Li Y as the author with the highest contribution. Bibliometric data also highlighted several therapeutic target molecules, including cytokines, nitric oxide, cyclooxygenase-2, and Nrf2, which play crucial roles in the action mechanisms of signature metabolites from Brassicaceae plants, including sulforaphane, isothiocyanates, glucosinolates, and flavonoids, for therapeutic purposes as anti-inflammatory, antioxidant, antimicrobial, and chemopreventive agents. These findings highlight the significant potential of Brassicaceae medicinal plants for various therapeutic mechanisms and provide recommendations for future research in the pharmacy field.

Abstract

Kesalahan pengobatan pada layanan kesehatan memberikan dampak yang tidak menguntungkan kepada pasien dan pemberi layanan kesehatan, pada pelayanan farmasi hal ini dapat terjadi saat dibuatnya resep oleh dokter sampai dengan resep diterima oleh apoteker. Penulisan resep secara elektronik diyakini memiliki tingkat kesalahan yang lebih rendah dibandingkan resep yang ditulis tangan. Meski demikian penerapan resep elektronik belum dilaksanakan secara baik dan merata di fasilitas layanan kesehatan di Indonesia, hal ini melatarbelakangi dilakukan review jurnal dengan tujuan untuk mengulas pengaruh resep elektronik terhadap medication error sehingga dapat memberikan gambaran manfaat dari penggunaan resep elektronik. Studi ini menggunakan metode tinjauan literur dan terbagi menjadi empat tahap, yakni mengidentifikasi kata kunci, penentuan kriteria jurnal yang akan dimasukan maupun dikecualikan dalam ulasan, pemilihan jurnal, dan analisis data. Pengumpulan literatur dilakukan melalui basis data elektronik seperti Science Direct, PubMed, Google Scholar, dan Springer Link. Hasil dari tinjauan ini menunjukan bahwa resep elektronik berpengaruh terhadap beberapa penurunan kesalahan pengobatan seperti kesalahan penulisan resep dan penurunan kesalahan pemberian obat, namun untuk beberapa jenis kesalahan, resep elektronik menunjukan tidak ada perbedaan secara signifikan dibandingkan dengan resep manual seperti kejadian kesalahan pemberian obat, kesalahan duplikasi obat, dan salah obat.

Abstract

Triple-negative breast cancer (TNBC) is one of subtypes of cancer with the most associated death. With more than 20% of breast cancer patients having this subtype, an effective and safe chemotherapy is needed to ensure complete pathomorphological response. Currently, therapies for TNBC patients are divided into systemic therapy regimens for operable or non-operable diseases, with non-operable diseases mostly treated with targeted therapies such as pembrolizumab in tandem with other chemotherapy agents such as paclitaxel, gemcitabine or carboplatin. TNBC patients whom tumor is operable underwent additional step such as preoperative systemic therapy, which offers several advantages than chemotherapy alone. Neoadjuvant chemotherapy (NACT), which consist of mainly drugs from therapeutic classes such as anthracycline and taxanes, is used with limited success (23%) and decreased recurrence rates. Optimal method for treating TNBC is still lacking, therefore the need for further research regarding chemotherapy for TNBC with higher success rate and better prognosis for the patients is paramount.
