Abstract

Multiple primary cancer (MPC) is multiple tumors with synchronous or metachronous occurrence in the same individual. Metachronous tumors are defined as those that occur more than six months after the first tumor is diagnosed. It has been reported two serial cases with metachronous multiple primary cancers. One of the cases was triple metachronous, a combination that has never been previously reported. First case: A 58-year-old woman presented with a lump in her right breast and axilla. Histopathology found an invasive micropapillary carcinoma with many lymph node metastases. Two years later, the patient got a rectal mass biopsy with an adenocarcinoma diagnosis. In one month, the patient got a nodule on the right toe, and the result revealed squamous cell carcinoma with moderate to poorly differentiated. Second case: A 76-year-old woman presented a lump on her left cheek with keratinizing squamous cell carcinoma. The patient had a left breast nodule eight months later, and the diagnosis was invasive carcinoma of no special type. Both patients died within 1 year of the last diagnosis. We revealed two cases of primary cancers with different locations and histopathology. The causes of MPCs are unclear, but they may be related to various factors. Multiple primary cancer patients should be closely followed up for possible growth of other cancers. Interdisciplinary collaboration can guarantee the best possible diagnosis and treatment for the patient. Keywords:  metachronous, multiple, primary cancer

Abstract

Latar Belakang: Kanker tiroid merupakan proses keganasan dari sel kelenjar tiroid yang penyebabnya belum diketahui secara pasti. Kanker tiroid termasuk jarang ditemukan dibandingkan jenis kanker lainnya, tetapi kanker tiroid merupakan keganasan endokrin yang tersering ditemukan. Objektif: Untuk mengetahui gambaran klinikopatologi kanker tiroid di RSUP Dr. M. Djamil Padang periode Januari 2018 – Desember 2020 Metode: Penelitian ini bersifat deskriptif dengan desain cross-sectional yang dilakukan di RSUP Dr. M. Djamil Padang pada tahun 2023. Sampel menggunakan rekam medis pasien kanker tiroid di RSUP Dr. M. Djamil Padang periode Januari 2018 – Desember 2020 yaitu sebanyak 35 pasien. Pengambilan data menggunakan teknik total sampling. Hasil: Penderita kanker tiroid lebih banyak ditemukan pada kelompok usia 55 – 64 tahun (34,3%), berjenis kelamin perempuan (60%), memiliki rentang IMT normal (48,6%), dan hampir semua penderita tidak memiliki riwayat keluarga yang menderita kanker (97,1%). Mayoritas merupakan single nodul (60%), dengan nodul terbanyak berukuran >4 cm (48,6%), dan sebagian besar konsistensi nodul adalah solid (74,3%). Jenis histopatologi didominasi kanker tiroid tipe papiler (80%), dan tatalaksana pembedahan yang paling banyak dilakukan yaitu total tiroidektomi (65,7%). Kesimpulan : Gambaran klinikopatologi kanker tiroid terbanyak ditemukan pada usia 55 – 64 tahun, berjenis kelamin perempuan, tidak memiliki riwayat keluarga dengan kanker, rentang IMT normal, single nodul, ukuran nodul >4 cm, solid, papiler, dan sebagian besar pembedahan berupa total tiroidektomi.

Abstract

Colorectal cancer is the third most common malignancy worldwide. Chronic inflammation, including intestinal protozoan infection, is linked to its risk. This systematic review examined the association between protozoan infection and colorectal cancer. Literature was searched in five databases (2014–2024), screened with PRISMA, and assessed using the JBI checklist. Thirty-two studies were reviewed. Protozoan infection incidence was generally higher in colorectal cancer than in non-cancer or other cancer groups. Blastocystis sp. was the most common species. Eighteen of 22 studies showed significant associations, particularly with Blastocystis sp., Cryptosporidium sp., Pentatrichomonas hominis, and Entamoeba histolytica. Factors involved included demographics, biology, environment, lifestyle, clinical history, and cancer stage. Most studies support a significant link. Opportunistic protozoan infections may worsen colorectal cancer progression, and chronic Cryptosporidium infection may induce preneoplastic lesions.

Abstract

ABSTRACT Background Adenoid cystic carcinoma is a malignant salivary gland tumor with unique features, including slow growth, progressive, poor prognosis, recurrence propensity, and perineural invasion tendency. SOX10 is a transcription factor expressed in the majority of tumors. SOX10 overexpression was hypothesized to play an important role in tumor-stroma interactions, especially perineural invasion and histopathological patterns. Method A cross-sectional study was performed using 30 blocks of formalin-fixed embedded specimens previously diagnosed as salivary gland adenoid cystic carcinomas. Perineural invasion and histopathological patterns were evaluated followed by immunohistochemical staining to evaluate SOX10 expression. The staining intensity and proportion of positively stained cells in both tumor and stroma cells were grouped into high and low expression levels. Chi-square tests were used for statistical analysis. Result The cribriform pattern was the most common histopathological pattern in both high and low SOX10 expression. The majority of tumors with high SOX10 expression (66.67% in stroma cells and 73.33% in tumor cells) were found to have more perineural invasion. Conclusion There was a tendency for perineural invasion in tumors with high SOX10 expression, although this was not statistically significant. There was no significant association between SOX10 expression and histopathological pattern.

Abstract

Background: The concern surrounding the physical and mental health-related quality of life (HRQoL) among patients with breast cancer emerged during COVID-19. However, there is a lack of sufficient studies that have specifically addressed this issue. It is crucial to emphasize the evaluation of this aspect as it significantly impacts the enhancement of future physical and mental HRQoL for these patients.Purpose: This study investigated the determinants of physical and mental HRQoL among patients with breast cancer during COVID-19.Methods: In a cross-sectional study conducted between September and October 2022, participation involved 260 patients diagnosed with breast cancer, selected through convenience sampling. Our observation focused on assessing the outcomes of physical and mental HRQoL using the Short Form (SF-12) questionnaire for data collection. Additionally, the data underwent analysis via binary logistic regression.Results: Respondents aged <50 years (p=0.010; aOR=2.08; 95% CI=1.18–3.67), unemployed (p=0.022; aOR=2.29; 95% CI=1.16–4.53), and had high depression (p=0.026; aOR=2.41; 95% CI=1.13–5.12) have a more likelihood to have low physical HRQoL. In addition, unemployed respondents (p=<0.001; aOR=4.15; 95% CI=2.14–8.04) and had high anxiety (p=0.004; aOR=2.71; 95% CI=1.38–5.33) have more likelihood to have low mental HRQoL.Conclusion: In this study, it was found that anxiety, depression, and unemployed respondents were associated with a higher likelihood of experiencing lower physical and mental HRQoL. Nurses and healthcare workers should prioritize addressing psychological issues such as anxiety and depression among breast cancer patients, emphasizing the necessity for interventions aimed at alleviating these concerns.

Abstract

Background: The BRCA1/2 mutation rate was found to be higher in triple-negative breast cancer (TNBC) patients depending on young age, and the presence of a family history of breast cancer and ovarian cancer. Research on BRCA1/2 germline mutations that focused on premenopausal TNBC patients in Indonesia has never been carried out. The objective of this research is to determine the BRCA1/2 germline gene mutation status in premenopausal TNBC patients in West Sumatra.Methods: This is a descriptive study conducted at the Surgical Oncology Division of DR. M. Djamil Hospital, Padang from April to July 2023. This study included patients aged 50 years who were diagnosed with the TNBC subtype at Dr. M. Djamil Hospital, Padang from January 2016 to July 2023. Secondary data was collected through the patient’s medical records to observe the TNBC patient’s characteristics. A blood test of TNBC patients was carried out to determine germline mutation of BRCA1/2.Results: 22 patients with TNBC participated in the study. In this study, 4/22 (18.2%) had BRCA2 pathogenic mutations and no BRCA1 pathogenic mutation was found. All patients with BRCA2 pathogenic mutation have the same mutation characteristics, i.e exon 3, genetic variation (c.262_263delCT), amino acids (p.Leu88AlafsTer12), and type of mutation (Frameshift Deletion). Conclusions: TNBC patients in West Sumatra have BRCA2 pathogenic mutations with the same characteristics as the mutations

Abstract

Latar Belakang: Lebih dari 90% kematian terkait kanker payudara disebabkan oleh tumor sekunder di organ yang jauh akibat metastasis. Kanker payudara tripel negatif atau triple negative breast cancer (TNBC) merupakan subtipe paling agresif diantara subtipe lainnya. Secara klinis TNBC memiliki ukuran tumor yang lebih besar, melibatkan kelenjar getah bening, dan memiliki derajat keganasan yang lebih buruk. Pasien TNBC lebih berisiko mengalami metastasis dan kematian dibandingkan dengan jenis subtipe lainnya sehingga prognosisnya juga lebih buruk. Objektif: Mengetahui hubungan kanker payudara tripel negatif dengan kejadian metastasis aksila dan metastasis jauh di RSUP Dr. M. Djamil Padang. Metode: Penelitian analitik observasional dengan desain penelitian cross-sectional bersifat retrospektif. Jumlah sampel penelitian sebanyak 11 pasien TNBC dan 99 pasien non-TNBC di RSUP Dr. M. Djamil Padang tahun 2016 – 2021. Digunakan analisis uji Chi Square. Hasil: Kejadian metastasis aksila pada pasien TNBC sebesar 45,5% dan kejadian metastasis jauh sebesar 90,9% dengan lokasi metastasis terbanyak pada organ paru. Tidak terdapat hubungan antara kanker payudara tripel negatif dengan kejadian metastasis aksila (p=0,071) dan terdapat hubungan bermakna antara kanker payudara tripel negatif dengan kejadian metastasis jauh (p=0,018). Kesimpulan: Penelitian ini dapat disimpulkan bahwa tidak terdapat hubungan antara kanker payudara tripel negatif dengan kejadian metastasis aksila dan terdapat hubungan antara kanker payudara tripel negatif dengan kejadian metastasis jauh. Abstract

Antibiotics have been proven to be beneficial for human life since their discovery. This type of drug is the most widely used in infections caused by bacteria. the function is to inhibit growth or kill microorganisms. Therefore, more than 30-80% of patients in developed countries who are hospitalized receive antibiotics, either alone or in combination. Evaluating the implementation of PPRA at RSUP. Dr. M. Djamil by comparing the evaluation of antibiotic use before and after the application of PPRA in inpatients in the Obgyn.We conducted this research including a type of correlational analytical research with a retrospective approach. The research sample was taken by the Medical Record room of the RSUP. Dr. M. Djamil Padang. The research was carried out for 1 month, from May 1 to May 31, 2020. The population in this research was all inpatients of Obstetrics and Gynecology in Dr. M. Djamil Padang.There was an increase in the percentage of antibiotic use in 2019 in line with the higher percentage of surgery in 2019 compared to 2018. As regulated by the guidelines for the use of antibiotics in Dr. M. Djamil, for surgery, prophylactic antibiotics are given 30 minutes before the procedure to prevent infection due to the invasive action. The use of prophylactic antibiotics whether to continue is determined by the type of operation, the length of the operation, the amount of bleeding during the operation, the clinical patient, the laboratory value.There were no significant changes in the implementation of PPRA's work on the qualitative analysis of antibiotic use.Keywords: Antibiotics, PPRA, Evaluation

Abstract

Catechin and their derivatives have been studied to have antiviral potential against COVID-19 through in silico method pharmacokinetics screening and molecular docking. Pharmacokinetics and toxicity profiles were obtained through the ADMETSAR server and SwissADME server. Then proceed with the prediction of affinity through the method molecular docking using the software application MOE 2007.09. The testmaterial is in the form of a 3D catechin structure and its derivatives as well as several control ligands downloaded via Pubmed. While templatethe Receptor M pro protein and the Spike glycoprotein are downloaded from pdb.org (PDB ID: 6LU7 and 6LXT). The pharmacokinetic profile of catechins is relatively better than all control ligands with the lowest toxicity. Molecular docking results also show that catechins and theirderivatives have a stronger affinity than control ligands. This research proves that catechin has antiviral potential through inhibition of Mpro protein and Spike glycoprotein COVID-19 virus.

Abstract

Kanker payudara merupakan kanker dengan prevalensi tertinggi pada perempuan dan berdampak pada aspek psikososial. Selain beban fisik akibat pengobatan, pasien juga menghadapi stigma yang memengaruhi citra diri, hubungan sosial, dan pemulihan.Penelitian ini bertujuan untuk menggambarkan tingkat stigma yang dialami oleh pasien kanker payudara di RSUP Dr. M. Djamil Padang. Desain penelitian yang digunakan adalah deskriptif kuantitatif dengan teknik purposive sampling. Sebanyak 280 responden yang memenuhi kriteria inklusi berpartisipasi dalam penelitian ini, yaitu perempuan berusia ≥18 tahun yang telah didiagnosis kanker payudara dan sedang atau telah menjalani pengobatan. Instrumen yang digunakan adalah Breast Cancer Stigma Assessment Scale (BCSAS) yang terdiri atas 28 item dengan tujuh domain. Analisis data dilakukan secara deskriptif menggunakan SPSS versi 26. Hasil penelitian menunjukkan bahwa sebagian besar pasien mengalami tingkat stigma tinggi (39,6%), sedangkan 25,4% dengan tingkat sedang dan 35% pada tingkat rendah. Domain dengan skor tertinggi adalah dapat disembunyikan dan perubahan citra diri, sedangkan skor terendah terdapat pada gangguan keluarga dan asal penyakit. Hasil ini menunjukkan bahwa stigma masih menjadi beban psikososial signifikan bagi pasien kanker payudara. Oleh karena itu, diperlukan intervensi psikososial yang berkelanjutan, seperti pembentukan peer support group, untuk membantu pasien saling berbagi pengalaman, meningkatkan penerimaan diri, dan mengurangi dampak stigma terhadap kesejahteraan emosional.
