Abstract

Background: The elderly are a vulnerable age who experience various problems, especially the problem of physical dissatisfaction which results in disturbances in musculoskeletal function. Disorders that often arise are joint pain that makes the elderly experience disruption of daily activities. Osteoarthritis (OA) is a degenerative disease that causes chronic inflammation due to friction between the ends of the bones that make up the joints. Therapy that can be used to reduce joint pain is progressive muscle relaxation therapy. Progressive muscle relaxation is a relaxation technique that combines deep breathing exercises and several contraction and relaxation techniques of certain muscles. Study aims to determine the effect of progressive muscle relaxation therapy on reducing joint pain in the elderly at Wisma Dahlia UPT PSTW Jember. Method: Experimental study with a quasi-experimental research design with a pretest and posttest one group design approach where in this study compares the pretest and posttest values of the elderly pain scale after giving progressive muscle relaxation therapy at Wisma Dahlia UPT PSTW Jember. Results and Discussion: Almost all the elderly at Wisma Dahlia have complaints, namely joint pain with a percentage of 86.4% with a pain scale of 4-6 before being given therapy. After 3 sessions of progressive muscle relaxation exercise there was a decrease in the level of pain before and after progressive muscle relaxation exercise from moderate pain (4-6) from 61.5% to 15.3% pain. In addition, there was an increase in the number of mild pain (1-3) from 38.5% to 84.7%. The p-value is 0.018 <0.05, which means that there are differences in the level of pain in the elderly before and after the progressive muscle relaxation intervention. The decrease in pain scale in the elderly shows that progressive muscle relaxation therapy is able to effectively reduce pain by increasing endorphins in the body to inhibit the transmission of painful stimuli. Conclusion: The elderly at Wisma Dahlia UPT PSTW Jember after participating in the empowerment of progressive muscle relaxation exercises for 1 week, their joint pain complaints decreased. Progressive muscle relaxation needs to be included in the PSTW exercise program that can be done routinely so that it can reduce joint pain experienced by the elderly.

Abstract

Ineffective family health management is an unsatisfactory pattern of handling health problems in the family to restore the health conditions of family members. Family empowerment is one of the efforts that can be done to increase cognitive, affective and psychomotor through health education, physical exercise and traditional medicine. This scientific work aims to analyze nursing care with ineffective health management problems with family empowerment in handling hypertension so that families are expected to be able to provide effective family nursing care in overcoming hypertension problems with family empowerment. This study was conducted by conducting observations and demonstrations for 3 weeks with 8 meetings in three families with hypertensive family members who were given family empowerment interventions through hypertension health education, anti-hypertension exercises and cucumber juice therapy. Based on the results of research on three families, it was found that there was an increase in family cognition about hypertension and hypertension diet, there was a decrease in systole and diastole blood pressure after anti-hypertensive gymnastics and administration of cucumber juice. Family empowerment shows that there is an influence on the handling of hypertension in the family. Family empowerment is important to do because it has a positive influence on cognitive changes, changes in systole and diastole pressure of hypertensive patients. Family empowerment can be done consistently and continuously because hypertension is a chronic disease and needs family involvement.

Abstract

Pekerjaan pertanian mempunyai tingkat ketegangan  yang  tinggi  dan  melibatkan  banyak  gerakan  repetisi, seperti terjadinya kondisi low back pain. Program pengabdian Masyarakat ini bertujuan untuk memberikan edukasi kepada petani mengenai pentingnya menjaga kesehatan tulang belakang dengan mengadopsi kebiasaan kerja yang sehat dan postur tubuh yang benar selama bekerja. Program Petani Sehat Dan Bebas Low Back Pain (Proniat Belain) Berbasis Agronursing Di Desa Sucolor Wilayah Kerja Puskesmas Maesan ini melibatkan sasaran sebanyak 20 petani dan 3 kader Kesehatan melalui kegiatan edukasi terkait pengetahuan tentang low back pain dan pencegahannya. Hasil edukasi menunjukkan perbedaan tingkat pengetahuan  petani sebelum dan setelah diberikan pendidikan Kesehatan, dimana sebelum edukasi menunjukan pengetahuan rendah 11 orang (55%) dan sedang 9 orang (45%), sedangkan pengetahuan setelah diberikan edukasi adalah rendah 2 orang (10%), sedang 13 orang (65%) dan tinggi 5 orang (25%). Oleh karena itu, program edukasi ini memberikan aspek postif dalam peningkatan pengetahuan tentang low back pain pada petani, sehingga kedepannya dapat dijadikan sebagai Upaya Kesehatan kerja petani dalam menunjang status kesehatannya.

Abstract

Permasalahan utama dari rendahnya penemuan kasus di Puskesmas rowotengah disebabkan oleh rendahnya pengetahuan masyarakat tentang penyakit TB Paru. TB HUNTER (Together Battling Health Under Networking Towards Elimination of TB) bertujuan untuk meningkatkan penemuan kasus Tuberkulosis (TB) Paru di wilayah agraris Rowotengah melalui implementasi TB Hunter. TB Hunter merupakan sebuah platform self-assessment online yang dirancang untuk mempermudah deteksi dini TB di kalangan masyarakat. Rancangan program ini dilakukan selama kurang lebih 10 hari dalam bentuk program kemitraan dan pemberdayaan dengan nama program “TB Hunter” dalam pemanfaatan web untuk meningkatakan penemuan kasus TB dengan screening mandiri. Program ini terdiri dari beberapa aktivitas melalui 4 bentuk strategi program, yaitu pendidikan kesehatan, proses kelompok, pemberdayaan masyarakat, dan kemitraan masyarakat. Berdasarkan hasil pelaksanaan program TB Hunter di wilayah Rowotengah, dapat disimpulkan bahwa program ini memberikan kontribusi yang signifikan dalam mendeteksi dan menangani kasus tuberkulosis (TB). Melalui penggunaan teknologi berbasis web, TB Hunter berhasil meningkatkan pengetahuan masyarakat mengenai TB serta mempermudah mereka dalam melakukan screening diri, terutama bagi kelompok berisiko tinggi. Dari 12 peserta yang hadir dalam kegiatan ini, 14 masyarakat berisiko tinggi ikut melakukan self-screening, dengan 1 pasien terkonfirmasi TB setelah dilakukan tindak lanjut. Program ini juga berhasil meningkatkan jumlah penemuan kasus terduga TB, yang sebelumnya hanya sekitar 0-1 orang per hari, menjadi rata-rata 4 orang per hari setelah penerapan TB Hunter. Hal ini menunjukkan bahwa platform ini efektif dalam menjangkau lebih banyak orang, termasuk mereka yang mungkin tidak terjangkau oleh fasilitas kesehatan tradisional. Selain itu, kecepatan respons dalam memberikan hasil screening, yang hanya membutuhkan waktu 2 jam, turut mempercepat proses diagnosis dan penanganan. Secara keseluruhan, TB Hunter terbukti efektif dalam penemuan kasus dan memperkuat sistem deteksi serta penanggulangan TB di daerah dengan keterbatasan akses Kesehatan.

Abstract

Background: Nurses experience considerable psychosocial demands that may contribute to anxiety symptoms. Work settings and work schedules, particularly shift work, may be important contributing factors. However, evidence comparing anxiety symptoms across healthcare settings and examining associated demographic and occupational factors among nurses in Indonesia remains limited.Purpose: This study examined anxiety symptoms among nurses working in primary health centers and hospitals in East Java, Indonesia, and identified associated demographic and occupational factors.Methods: A descriptive cross-sectional study was conducted among 315 nurses working in primary health centers and hospitals in East Java, Indonesia. Anxiety symptoms were assessed using the Generalized Anxiety Disorder-7 (GAD-7). Descriptive statistics were used to summarize participant characteristics and the prevalence of anxiety symptoms. The Mann–Whitney U test, chi-square test, and Fisher’s exact test were used to examine differences and associations between demographic and occupational characteristics and anxiety symptoms.Results: Among 315 nurses, 72.1% worked shifts. Participant characteristics differed significantly by workplace in terms of age (p = 0.002), years of experience (p = 0.001), location (p = 0.001), marital status (p = 0.003), income (p = 0.038), and work schedule (shift vs. non-shift; p = 0.001). Anxiety symptoms (GAD-7 ≥ 7) were significantly associated with age (p = 0.001), years of experience (p = 0.008), marital status (p = 0.003), income (p = 0.001), and work schedule (p = 0.006). Hospital nurses had higher odds of experiencing anxiety symptoms than nurses working in primary health centers (OR = 4.842; p = 0.010). Shift work was also associated with higher odds of anxiety symptoms (OR ≈ 6.8) compared with non-shift work.Conclusion: Hospital work and shift work were associated with anxiety symptoms among nurses, along with demographic factors such as younger age, shorter work experience, unmarried status, and lower income. These findings support targeted anxiety screening and workplace interventions, including shift management and psychosocial support, particularly for hospital nurses.

Abstract

The family with special need children experience children's dependency behavior. This situations related to parenting stress which affected parental quality of life. The objec tive of this study was to examine the correlation characteristic of parents and parentingstress and parental quality of life among special need children's parents. A cross sec tional study design conducted among 52 parents with special need children in School of Disabled Children in Badean Bondowoso. A self-administered questionnaire in cluding the Alabama Parenting Questionnaire and the World Health Organization Quality of Life-BREF Indonesian version were used to assess parenting stress; and parental quality of life, respectively. Pearson Product Moment and Spearman Rankwas performed to answer the objective of this study. The results showed that, there were no correlation between parenting stress and parental quality of life of special need children's parents (p>0.05). However, length of disability correlated with parentingstress (r=0.339; p=0.014). Meanwhile; parent's gender (Z=-2.089; p=0.037), special need children's gender (Z=-2.102; p=0.036), and number of childrens (r=-0.297; p=0.033) were correlated with parental quality of life. This study concluded that char acteristic of parents and children with special needs correlated with parental quality of life. Therefore, adaptive parenting environment should implement to reduce parenting stress.

Abstract

In Indonesia, Confinement to PSMD is well known as "Pasung". One reason family decided to restrict the behaviour of PSMD is feeling shame. It caused PSMD hard for achieving recovery from his illness. The purpose of this study was to describe the community perceived PSMD with confinement. A quantitative descriptive study was used in this study. The sample size of this study was 150 respondents with a purposive sampling technique. The instrument of this study used the Community Attitudes questionnaire toward the Mentally Ill (CAMI). The results showed that 50.7% of the people were pro-stigmatized while 49.3% were counter-stigmatized. It can be concluded that there was still a high stigma about the PSMD who have been confined. This community stigma will cause any harms for PSMD with confinement. The PSMD will be hampered his recovery because he can't access treatment from a health care provider freely. Commonly, PSMD is often being victims of criminal acts and discriminated against. Family and community especially around them have an important role in helping recovery PSMD.

Abstract

Older people (OP) have a high risk for hypertension related to changing during older process in particularly system of cardiovascular. The purpose of this study is to identify prevalence of hypertension and related factors among OP in nursinghome of Jember, East Java, Indonesia. A cross-sectional study was conducted among 42 of OP in nursing home. A self-administered questionnaire was used to identify characteristic of participants. Data of activity daily living, cognitive function, emotional status, depression status, and spiritual activity were measured using Katz Index of Independence in Activities of Daily Living (ADL), Short Portable Mental Status Questionnaire (SPMSQ), The Mini Mental State Examination (MMSE), De pression Anxiety Stress Scale 14 (DASS-14), and Daily Spiritual Experience Scale (DSES), respectively. Blood pressure was measured using a standard protocol uti lizing a stethoscope and sphygmomanometer. A logistic regression analysis was used to determine factors related to hypertension among OP. The mean of blood pressure of systolic and diastolic among OP were 131.90 ± 16.56 mmHg and 82.02± 10.18 mmHg, respectively. The prevalence of systolic and diastolic hypertension among OP in nursing home were 38.1% and 21.4%, respectively. The factors of systolic hypertension were age (p= 0.038), activity daily living (p= 0.043), depres sion status (p= 0.012), and spiritual activity (p= 0.015). Meanwhile, factors of diastolic hypertension were age (p= 0.041) and education (p= 0.049). Increasing age of OP related to risk factor of hypertension with psychological problems like depression. It could be reduced by existence of spiritual activity of OP. Provision of health education on changes in the cardiovascular system and adaptation of lifestyle changes in the OP will be able to reduce hypertension factors in the OP

Abstract

Health care providers (HCPs) in Public Health Center (PHCs) have essential role to fulfil the various complexities of physical, psychological, economic, and social needs among people affected by leprosy (PALs) during treatment in communities. The complexity of these needs is important to prevent the stigmatization and discrimination of PALs for transmission and disability during their rehabilitation. To explore the experience of HCPs for fulfilling the PALs needs during attending community based rehabilitation in PHCs of Jember regency, Indonesia. A descriptive phenomenology was performed to HCPs dur ing July to August 2020. In depth interview was saturated to six of HCPs who were responsibility to care PALs in community. Data was analyzed using systematic coding to all of verbatim of participants. Regarding our in depth interview for sixt of HCPs, we were identified of three of themes which explained into seven of subthemes and sixteen of categories. The main themes were adherence to treatment efforts, treatment to solve social limitation, and caring for preventing disability. The experiences of HCPs in caring for PALs during community based rehabilitation were identified in three main themes, namely: efforts to maintain adherence to treatment, efforts to prevent social barriers, and basic care to prevent disability. Therefore, regular visits through home visits can be car ried out on PALs so that MDT compliance is good and reduces side effects. The forma tion of self-care groups is indispensable in teaching basic care skills in an effort to prevent future disability

Abstract

The problems of people with mental health disorders (PWMHD) include physical problems, psychological problems, and social problems. Jember Regency in 2018 had 514 new cases, and Panti village was ranked first with 114 clients (22.1%) of the total casesin the Jember Regency. Community views accompanied by cultural background often prevent families with PWMHD from coming to the health center to seek care and management of health services. Negative public views on PWMHD will reduce PWMHD visits to seek treatment so that cases of drug withdrawal will increase. Nurses have an essential role in providing nursing care to clients with mental disorders. Without knowledge of the cultural viewpoints of the community, nurses will find it difficult to find strategies to reduce the occurrence of stigma and discrimination in PWMHD This study was used an ethnography study, qualitative approach, and descriptive design to explore and to understand about the Jember community view in the treatment of mental disorders with a perspective of transcultural nursing. The technique of collecting data was used in-depth interviews with a semi-structured interview guide involving seven participants, and the data were analyzed using interpretive phenomenology analysis. This research has passed the ethical test with ethical approval No.595 / UN25.8 / KEPK / DL / 2019. This research conducted five themes. First is the family efforts to seek healing; the second theme is received trials from God; the third theme is community stigma to people with mental disorders; the fourth theme is community expectations for people with mental disorders, and the fifth theme is community acceptance of people with mental disorders. Considering the impact that will arise from this condition if it does not get proper treatment, it is needed the role of health workers in providing services to the family to be able to increase visits to health services in order to improve the recovery of people with mental disorders.
