Abstract

The complexity of sexuality and sexual trauma experiences, including harassment, unwanted touching, sexual urges, and forced sexual intercourse, should not be underestimated because of the tendency to initiate adverse psychosocial impacts. Sexual harassment can increase mental health risks, which may influence sexual orientation decisions. This quantitative study, using a cross-sectional design, aimed to determine a relationship between sexual trauma experiences and sexual orientation in men who have sex with men (MSM). This study population was MSM living in Palembang and Prabumulih City, with a sample size of 216 respondents. The proportion of respondents with homosexual orientation was 57.1%, and the proportion having sexual trauma experiences was 42.3%. The multiple logistic regression identified a significant relationship between sexual trauma experiences and sexual orientation in MSM with a pvalue of 0.02, an adjusted odds ratio of 2.71, and a 95% CI of 1.17-6.26. These results suggested a higher risk of a different sexual orientation among traumatized MSM compared to MSM without one after controlling access to pornographic content and correcting parenting patterns. Sexual trauma has been shown to affect individual sexual orientation; therefore, special attention is needed due to the impact on future life behavior.

Abstract

Background: Sarcopenia is a syndrome characterized by decreased muscle mass and decreased muscle strength and muscle function. Oxidative stress and inflammatory processes are known to contribute to sarcopenia by releasing the catabolic stimuli interleukin-6 (IL-6) and insulin-like growth factor (IGF). Methods: The method used is a literature review by collecting appropriate articles from the PubMed and Google Scholar databases. Results: Low IL-6 can trigger satellite cell activity and myotube regeneration, whereas if IL-6 production increases chronically, it triggers muscle wasting. Changes in muscle size are the result of changes in the synthesis and degradation of muscle protein. IGF-1, where signal changes are strongly related to muscle size. IGF-1 binding to the receptor causes intracellular phosphorylation of the adapter protein Shc or insulin receptor substrate (IRS-1). Conclusion: The role of IGF-1 and IL-6 can be helpful as diagnostic and prognostic parameters for sarcopenia.

Abstract

Tumor necrosis factor-alpha (TNF-α) is one of the most crucial proinflammatory mediators critically involved in the development of insulin resistance and the pathogenesis of type 2 diabetes mellitus (T2DM). Elevated levels of TNF-α induce insulin resistance in adipocytes and peripheral tissues. A literature review was conducted using databases such as PubMed and Science direct to explore articles discussing TNF-Alpha in type 2 diabetes over the past 5–10 years. Keywords were searched individually or in combination with others, and articles in English were screened. The literature reveals a connection between TNF-alpha and type 2 diabetes mellitus. The pathogenesis of type 2 diabetes mellitus (T2DM) and the development of insulin resistance are characterized by various stimulating factors, particularly the formation of reactive oxygen species (ROS) and an increase in various pro-inflammatory cytokines. Tumor necrosis factor-alpha (TNF-α) is identified as one of the key proinflammatory mediators involved in insulin resistance development and T2DM pathogenesis. TNF-α is implicated in various human pathologies, including autoimmune diseases, obesity-related insulin resistance, cardiovascular complications, cancer, and plays a role in the occurrence of chronic inflammation in the kidneys, a form of complication in type 2 diabetes. Other studies support the notion that the effects of pro-inflammatory and anti-inflammatory cytokines (TNF-α) can disrupt insulin signaling pathways, leading to insulin resistance in pancreatic B cells. In conclusion, this research indicates that Tumor necrosis factor-alpha (TNF-α) can induce insulin resistance, contributing to the onset of type 2 diabetes.

Abstract

Background: Carpal tunnel syndrome (CTS) is a prevalent entrapment neuropathy. While nerve conduction studies (NCS) remain the gold standard for diagnosis, ultrasound (USG) offers a non-invasive alternative. This study aimed to compare the diagnostic accuracy of USG and NCS in CTS at Dr. Mohammad Hoesin General Hospital Palembang, Indonesia. Methods: A cross-sectional study was conducted on patients presenting with CTS symptoms. Demographic and clinical data were collected. NCS and USG assessments were performed, blinded to each other's results. USG measurements included the cross-sectional area at the carpal tunnel inlet (CSAc), proximal to the carpal tunnel (CSAp), and the difference between them (∆CSA). Diagnostic accuracy was calculated, and agreement was assessed using Cohen's kappa. Results: A total of 86 wrists from 49 patients were included. The mean age was 52 ± 11 years, with a female predominance (86%). The majority had mild CTS based on NCS (55.8%). USG measurements showed mean CSAc of 13.1 ± 3.5 mm², CSAp of 10.6 ± 3.0 mm², and ∆CSA of 2.5 ± 0.9 mm². ∆CSA had the highest sensitivity (92.2%), specificity (88.9%), and accuracy (91.9%), with substantial agreement with NCS (Kappa = 0.65). Conclusion: USG, particularly using ∆CSA, demonstrates high diagnostic accuracy in CTS, comparable to NCS. It can serve as a valuable tool, especially in settings with limited NCS availability.

Abstract

Background: Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by widespread inflammation and diverse clinical manifestations. The systemic inflammatory immune index (SII), calculated as platelet count * neutrophil count/lymphocyte count, has emerged as a potential marker of systemic inflammation in various conditions. This study aimed to investigate the relationship between SII and disease activity in SLE patients. Methods: We conducted a cross-sectional study involving 60 SLE patients diagnosed according to the 2019 EULAR/ACR classification criteria. Demographic and clinical data were collected, and disease activity was assessed using the Mexican Systemic Lupus Erythematosus Disease Activity Index (MEX-SLEDAI). Blood samples were analyzed to determine SII values. Statistical analysis included Spearman's correlation to assess the relationship between SII and MEX-SLEDAI scores. Results: The study population predominantly consisted of women (98.3%), with a median age of 29 years. A strong positive correlation was observed between SII and MEX-SLEDAI scores (r = 0.931, p < 0.001). Patients with higher SII values exhibited significantly greater disease activity. Conclusion: Our findings suggest that SII is a promising predictor of disease activity in SLE patients. This readily available index may aid clinicians in assessing disease severity and tailoring treatment strategies. Further research is warranted to validate these findings and explore the utility of SII in monitoring disease progression and treatment response.

Abstract

Research on ergonomic risk factors for neck pain among Islamic boarding school students remains scarce, despite their prolonged study and Qur’an memorization activities in static sitting postures. Non-neutral body posture during learning may increase the risk of musculoskeletal disorders (MSDs), particularly neck pain, which can impair concentration and quality of life. This analytic cross-sectional study aimed to assess students’ body posture during study and memorization activities and to examine its association with neck pain. A total of 72 boarding and non-boarding students participated. Data were collected using questionnaires and the Rapid Upper Limb Assessment (RULA), and analyzed with chi-square tests and logistic regression to obtain prevalence ratios (PR) and p-values. Neck pain was reported by 63.9% of respondents; 72.2% exhibited poor sitting posture, and 80.6% reported moderate-to-severe stress. Bivariate analysis showed a significant association between stress level and neck pain (p = 0.033; PR = 2.19; 95% CI: 1.25–3.83). In multivariate logistic regression, no variable reached conventional statistical significance; however, students with moderate-to-severe stress had a 3.81-fold higher risk of neck pain (p = 0.057). These findings underscore the need for integrated ergonomic and psychosocial interventions in Islamic boarding school settings to reduce neck pain risk among students.
