Abstract

Post-stroke cognitive impairment (PSCI) is one of the post-stroke complications that is often overlooked. Quantitative electroencephalography (QEEG) is an alternative examination in patients with PSCI. QEEG results of PSCI patients shows abnormalities in spectral power, coherence, power ratio, and peak alpha frequency (PAF). Aim: Knowing the QEEG characteristic in PSCI patients and the characteristic differences between the post stroke dementia (PSD) and post stroke cognitive impairment no dementia (PSCI-ND). This research is a descriptive study of PSCI patients at the RSMH Palembang from September-November 2023, followed by a QEEG examination. There were 31 patients who completed the research. There is higher median absolute and relative power delta and theta both per region and globally, decreased interhemsiferous coherence at F3-F4, P3-P4, O1-O2, T5-T6, C3-C4, increased delta alpha ratio and delta theta alpha beta ratio in the frontal region, as well as a decrease in peak alpha frequency in all regions. Patients with post-stroke dementia have more abnormality in relative global power, frontal DAR and DTABR, and PAF than patients with PSCI-ND Stroke can cause slowed conduction along the cortical-subcortical pathways, disruption of cholinergic pathways and impaired communication in various parts of the brain. This condition can cause a decrease in alpha power as an important wave in cognitive function and an increase in delta power in various brain regions. A decrease in the fast wave spectrum and an increase in the slow wave spectrum may lead to decreased cognitive function as seen in patients with PSCI Post stroke cognitive impairement, Power ratio, Quantitative electroencephalography, spectral power.

Abstract

Insulin-like growth factor-1 (IGF-1) is a key mediator of human growth hormone (HGH), which plays an important role in promoting cell growth and differentiation in childhood and continues to exert anabolic effects in adults. IGF-1 is a small peptide that circulates in serum bound to a high affinity binding protein. IGF-1 is part of a vast network of growth factors, receptors, and binding proteins involved in mediating cell proliferation, differentiation, and apoptosis IGF-I-like growth factor has broad anabolic and insulin-sensitising effects, which are generated through endocrine (in circulation) as well as paracrine/autocrine mechanisms. Serum concentrations of IGF-1 steadily increase during childhood and peak at puberty, and continue to decline since then, as does the secretion of human growth hormone. This study aims to determine the role of physical activity in molecular impact on increasing IGF-1 levels. This article is the result of a review of various research references related to the role of physical activity in molecular impact on increasing IGF-1 levels. Physical Activity in Molecular has an impact on increasing IGF-1 levels.

Abstract

River water pollution has become a major concern, particularly in regions where human activities occur without proper regulation. This study investigates heavy metal contamination in the Batang Masumai River, Merangin Regency, Jambi Province, where local residents conduct mining near the riverbanks. Water samples were taken from upstream, midstream and downstream, then analyzed using Atomic Absorption Spectrometry (AAS) to detect concentrations of seven metals: mercury (Hg), cadmium (Cd), nickel (Ni), iron (Fe), copper (Cu), manganese (Mn), and lead (Pb). Heavy metals  are among the most dangerous pollutants due to their toxicity, persistence, and impact on human health Pollution levels were evaluated through the Heavy Metal Pollution Index (HPI) and compared with SNI 6242:2015 and WHO 2022 standards. The results showed that cadmium, nickel, iron, and mercury exceeded allowable limits. The HPI results reflect a serious level of water pollution. Based on the 2015 SNI standard, the HPI scores are 214.45 for downstream, a striking 57893.41 for midstream, and 120.61 for upstream. Using the the WHO 2022 criteria, the values are 486.03 (downstream), 4.186.55 (midstream) and 267.83 (upstream). Except for the upstream based on SNI 6242:2015, all areas show high contamination levels (HPI ≥ 200), highlighting a concerning environmental issue. Mercury and cadmium, known for their high toxicity and bioaccumulation potential, were the most concerning. These findings underscore the urgent need for remediation efforts, continuous environmental monitoring, and increased public awareness to protect aquatic ecosystems and ensure the safety of water resources for surrounding communities.

Abstract

Background: Systemic lupus erythematosus (SLE) is a multifaceted autoimmune disease where the cytokine Interleukin-10 (IL-10) exhibits a paradoxical role, functioning as both a potent anti-inflammatory mediator and a robust B-cell stimulator. The clinical significance of serum IL-10 as a biomarker of disease activity is a subject of intense debate, with conflicting reports in the literature. This study was designed to investigate this relationship within a specific Southeast Asian cohort. Methods: An observational, cross-sectional study was conducted at a tertiary referral hospital in Palembang, Indonesia, enrolling 48 adult patients with a confirmed diagnosis of SLE. Disease activity was quantitatively scored using the Mexican Systemic Lupus Erythematosus Disease Activity Index (MEX-SLEDAI). Serum IL-10 concentrations were precisely measured using a quantitative sandwich enzyme-linked immunosorbent assay (ELISA). The primary statistical analysis involved the Spearman rank correlation test. A post-hoc power analysis was performed to contextualize the statistical findings. Results: The study population was predominantly female (95.8%), with the largest subgroup (54.2%) presenting with mild disease activity. The mean serum IL-10 concentration was 9.91±1.36 pg/mL in the mild activity group, rose to a peak of 12.22±1.95 pg/mL in the moderate activity group, and was 10.65±2.34 pg/mL in the severe activity group. The Spearman correlation test identified a weak, positive association that did not achieve statistical significance (r=0.274, p=0.059). The post-hoc power analysis confirmed the study was underpowered to definitively detect a correlation of this magnitude. Conclusion: In this cohort of Indonesian SLE patients, a statistically significant correlation between serum IL-10 levels and disease activity was not established. Given the study's methodological context, including its cross-sectional design and limited statistical power, the findings are inconclusive but hypothesis-generating. The results reinforce the profound complexity of IL-10 biology in SLE and underscore the challenges in validating it as a standalone biomarker, highlighting the need for larger, longitudinal investigations.

Abstract

Purpose: This study aimed to analyze the knowledge, attitudes, and compliance with antiretroviral (ARV) antidotes among HIV patients at the Central Bangka Regional General Hospital. Research Methodology: This study used a qualitative method with a phenomenological approach. The technique for selecting respondents was a non-probability sampling technique, purposive sampling. Nine informants participated in 9 people. Data were collected using an in-depth interview guide. Results: Findings from the research show that several informants had insufficient knowledge about compliance in taking ARV antidotes. Even though the informants' attitude towards compliance with antiretroviral (ARV) antiretrovirals was relatively good, several key informants stated that they had stopped taking antiretroviral (ARV) antidotes for several months because they felt healthy. However, several months later, they experienced adverse effects that resulted in declining health. Conclusions: The study concludes that Inadequate knowledge and misconceptions about ARV therapy remain significant obstacles to sustained adherence. Strengthening education, family involvement, and consistent supervision are essential for enhancing compliance and improving health outcomes for people living with HIV. Limitations: The study was limited to a small number of participants from a single hospital, which restricts broader generalization. Contribution: This research provides practical insights for healthcare providers and policymakers to design targeted interventions, improve supervision, and foster cross-program collaboration to ensure consistent ARV adherence among patients with HIV, ultimately supporting public health goals in HIV management.

Abstract

Primary dysmenorrhea is the most frequent gynecological issue in women of reproductive age. Various therapeutic approaches have been used to treat dysmenorrhea, such as psychotherapy, administration of non-steroidal anti-inflammatory drugs, oral contraceptive pills, and others. However, these drugs often have undesirable side effects. Therefore, other alternatives, such as herbal medicine, diet, supplements, vitamins, and minerals, are important concerns. Vitamin D, also known as calciferol, is believed to regulate cell growth and apoptosis and modulate the immune system and other functions. The active type of vitamin D has the potential to decrease prostaglandin production within the endometrium and restrict its biological impact by influencing prostaglandin receptors. Moreover, it can prompt anti-inflammatory responses through diverse pathways. Vitamin D is naturally synthesized in the skin when exposed to sunlight's ultraviolet B (UVB) radiation, but it can also be acquired from dietary sources and supplements. This community service aims to educate young women about dysmenorrhea by increasing participation in counseling. The activity method used was counseling aimed at young female students at STIKES Abdurahman in Palembang, with a total of 120 participants, and this counseling activity was carried out for 150 minutes. From the results of this community service, the students understand the importance of maintaining reproductive health. They can overcome primary dysmenorrhea by adjusting a diet that contains vitamin D.
