Abstract

Based on the changing demographics of geriatrics today, it is expected that there will be an increase in demand for various surgical services, including vascular surgery and general surgery. DI Yogyakarta (14.71 percent) is the city with the largest geriatric population in Indonesia. In the geriatric age group, there are changes related to physiological functions, pharmacokinetics and pharmacodynamics. The use of opioids requires a 25-50% lower initial dose than the adult age group. The administration of opioids in geriatrics needs to consider: the type of opioid group, the level of toxicity, monitoring pharmacological effects and side effects that occur closely after the initial dose, and rotating opioids that allow the use of lower doses.

Abstract

The patient is a 16 year old male who suffered a cervical injury due to a slipping fall. Pain was felt in the neck, followed by weakness in the limbs, but did not immediately go to the hospital. The patient was referred to Sardjito Hospital about 1 month after the incident, with examination at the previous hospital the patient had a cervical injury with tetraparesis.The patient underwent corpectomy, decompression, and stabilization surgery on the second day. Postoperatively the patient was admitted to the ICU. On the second day of treatment, extubation was carried out, but was later re-intubated because the patient had respiratory distress. The next therapy was physiotherapy, nebulizer and mucolytic, ventilator weaning with pressure support mode and progressive ventilator free breathing (PVFB), Percutaneous Dilatation Tracheostomy (PDT), nutritional therapy, analgesics and antibiotics. On the way, the patient experienced pulmonary atelectasis, sepsis, and several failures in the ventilator weaning trial, so PDT was performed. The patient then experienced an improvement in his condition which was characterized by an increase in lung capacity, extremity muscle strength, and improvement in the condition of sepsis. Until finally the patient was able to be released from the use of the ventilator on the 15th day of treatment by using a T-piece through atracheostomy tube.

Abstract

Background: Glasgow Coma Scale (GCS) as one of the most important predictors and a key measure in neurological assessment after brain injury. Apart from mortality, GCS is also a predictor of quality of life that can be measured using the Extended Glasgow Outcome Scale (GOSE). GOSE is a global assessment of independent living and social reintegration that is widely used as an outcome measure in brain injury research, to analyze long-term functional outcomes. Methods: This study used a retrospective cohort observational study design that aimed to predict mortality and quality of life using the Glasgow Coma Scale. This research has been conducted at the Medical Record Installation of Dr. Sardjito Hospital Yogyakarta using data from patients whom are treated with a diagnosis of traumatic brain injury at Dr. Sardjito Hospital from January 1, 2020 to December 31, 2020 retrospectively. Quality of life was calculated using the GOSE interview questionnaire. The relationship of GCS variables to mortality and quality of life was tested by the unpaired correlative hypothesis test followed by multivariate analysis with logistic regression methods. Results: There were 174 research subjects with a mean age of 37.51 (± 14.17) years, 124 male (71,3%) and 50 female (28.7%). Subjects GCS score < 8 had a higher risk of death than those with GCS score 13-15 (p<0.001, RR=18.3). Subjects with GCS score 9-12 had a higher risk of death than those with GCS score 13-15 (p<0.001, RR=9.71). Subjects with GCS score < 8 had a higher risk of unfavourable outcome than those with GCS score 13-15 (p<0.001, RR=9.49). Subjects with GCS score 9-12 had a higher risk of unfavourable outcome than those with GCS score 13-15 (p=0.001, RR=4.93). Conclusion: GCS on admission can be a predictor of mortality and quality of life for traumatic brain injury patients

Abstract

Background: Anesthesia service is a complex part of health care and has the potential to be dangerous. Anesthesia services can be improved by knowing the death rate related to anesthesia. This study aims to determine the death rate related to anesthesia in RSUP Dr. Sardjito. Methods: A retrospective review of the patient database under anesthesia from January 2017 to May 2021 was carried out by taking data from the medical record installation of RSUP Dr. Sardjito. Data on patients who died related to anesthesia were determined by 3 anesthesiologists. We assessed the patient's preoperative, intraoperative, and postoperative variables such as age, sex, ASA physical status, anesthetic technique, type of surgery, size of surgery, and patient comorbidities associated with anesthesia-related death and place of death. Results: There were 39,965 patients undergoing anesthesia. Thirty patients died related to anesthesia. Characteristics of patients who died related to anesthesia in January 2017 to May 2021 at Dr. Sardjito was a patient with male gender (n=14 (46.6%)), ASA III physical status (n=20 (66.7%)), type of emergency surgery (n=12 (40%)), general anesthetic technique (n=25 (83.3%)), and underwent major surgery (n=25 (83.3%)). The mean age of patients who died related to anesthesia was 46.53 with a standard deviation SD of ±26.31. There were 9 patients (30%) who died with comorbid hypertension. There were twenty six patients (86.6%) died outside the operating room. Conclusion: The death rate related to anesthesia within 24 hours at RSUP Dr. Sardjito is 7.5 per 10,000 anesthetic actions.

Abstract

Guillain-Barré syndrome (GBS) is an autoimmune disorder that affects the peripheral nervous system. Most patients with GBS are clinically characterized by tetraplegia with or without sensory disturbances. In GBS this is due to a hyperreactive immune response including release of antiganglioside antibodies, formation of antibody-dependent immune complexes, and increased macrophages leading to axonal demyelination and degeneration. Severe cases of GBS may present with progressive clinical signs involving respiratory muscle and requiring mechanical ventilation and treatment in an intensive care unit (ICU). We reported a 26-year-old woman with type 2 respiratory failure with suspicion of GBS who was being treated in the ICU of RSUP Dr. Sardjito and undergone plasma exchange as immunomodulator therapy. The patient was treated for 9 days in the ICU of RSUP Dr. Sardjito with the outcome of the patient discharged from ICU to the ward.

Abstract

Epidural Volume Extension (EVE) technique injects normal saline into the epidural chamber immediately after the injection of local anesthesia into the intrathecal space. Normal saline injection will increase the epidural space thus increasing the spreading of local anesthesia in previous intrathecal injections and making the distribution of cerebrospinal fluid to be higher. It can be said that EVE technique utilizes the volume effect of normal saline that decreases the volume of intrathecal space. The EVE technique may be useful for reducing the dose of local anesthesia injected intrathecally by still reaching a stronger block height for surgery, as well as for faster recovery of motor blockades. Unfortunately, the EVE technique does not significantly reduce the risks of hypotension. Hypotension persists even though the hemodynamics change is more stable.

Abstract

Pregnant woman with cardiac abnormality was associated with substantial fetomaternal risk. Uncorrected cardiac abnormality associated with R to L shunting (such as ventricle septum defect) may cause chronic over volume which may cause progressive hypoxemia and cardiac failure which is known as Eisenmenger Syndrome. We reported a 27 years old woman with G1P0A0, Eisenmengersyndrome planned for emergency caesarean section post failed termination with induction. There was failed epidural block, so we used a low dose subarachnoid block as an alternative. There is no ideal anesthesia technique for Eisenmenger syndrome, thus every technique carried its own risk. In regional anesthesia, the risks that may happen include decreased SVR, increased R to L shunt, and hypoxemia exacerbation. According to the literature, low dose SAB as used in this case was already followed the principle of failed epidural anesthesia.

Abstract

A thyroid storm is one of endocrinology emergency. Its incidence is very rare but has a very high mortality. Early diagnosis and aggressive treatment must be made in this case so that the patient has a good prognosis. Thyroid storm is often occurring in patients with hyperthyroidism who are not given adequate treatment and are triggered by infection, trauma, thyroid surgery, or uncontrolled diabetes mellitus. Initial treatment in Emergency Department includes assessment and management of the ABCDSs, appropriate intravenous (IV) fluid resuscitation, electrolyte replacement, precipitate factors management, and thyroid-specific therapy (include Anti Thyroid Drugs [ATDs], inorganic iodine, corticosteroid, betaAdrenergic Agonists [beta-AAs]). After getting proper treatment in Emergency Department, the patient must continue his care in the Intensive Care Unit (ICU) for clinical and treatment close monitoring. Therapeutic plasmapheresis (TPE) should be considered if clinical improvement is not noted within 24–48 hours of initial treatment.

Abstract

Background: COVID-19 is a disease caused by the SARS-CoV-2 virus with the main clinical manifestations of respiratory disorders. Patients with severe and critical symptoms require oxygenation therapy during treatment. Interleukin-6 (IL-6) plays a role in the course of the disease and is associated with the clinical severity of COVID-19. IL-6 has the potential as one of the test parameters to estimate the outcome of COVID-19 patients. Objective: To know the relationship between IL-6 levels and the outcome of mortality, length of stay (LOS) and positive pressure oxygenation days of COVID-19 patients during hospitalization at Dr. Sardjito hospital. Methods: The study design was a retrospective observational cohort by taking secondary data from the medical records of hospitalized COVID-19 confirmed patients who were examined for serum IL-6 during April 2020- March 2021 at dr.Sardjito Hospital. Data collection was carried out at the Medical Records Instalation of Dr. Sardjito Hospital. The relationship of the IL-6 variable to the outcome was tested by logistic regression method and further analyzed with Kaplan-Meier and Cox regression analysis of survival. Results: There were 302 research subjects with a mean age of 55.45 (+14.79) years, 183 male (60.6%) and 119 female (39.4%). Subjects with IL-6 >80 pg/mL had a higher risk of death than those with IL-6 <80 (p=0.000, HR=4.68). As many as 87.4% of the subject required oxygenation therapies during hospitalization. Group of subjects with an IL-6 value >80 was significantly required more positive pressure oxygenation therapy (p=0.000) and require longer (p=0.005) duration of positive pressure oxygenation (9.36(+5,9) days) compared to the group of subjects with IL-6 <80 (6(+2,98) days). The difference in LOS was significantly different between groups of subjects, with the median LOS for subjects with IL-6 >80 was 25 days, while for subjects with IL-6 <80 was 13 days (p= 0.000). Conclusion: There was a significant relationship of IL-6 levels >80 pg/mL with increased risk of mortality, LOS and the requirement of positive pressure oxygenation therapies in COVID-19 patients.

Abstract

The objective of this study is to examine the influence of Classroom Action Research and Innovation on Incentives, with Competence serving as the mediating variable. The variables investigated in the research are Classroom Action Research, Innovation, Competence, and Teacher Professionalism. The data collection method employed was a survey conducted through the distribution of questionnaires. The analysis was conducted using Smart PLS 3.2.8 software, specifically utilizing Partial Least Squares (PLS) analysis. The research findings is a) Classroom Action Research has a positive and significant impact on teacher professionalism, b) Innovation has a positive and significant effect on teacher professionalism, c) Competence shows a positive and significant association with teacher professionalism, d) Classroom Action Research has a positive and significant effect on teacher professionalism through Competence, e) Innovation has a positive and significant effect on teacher professionalism through Competence. Based on these results, it is recommended that teachers at Mandailing Natal Regency Elementary School prioritize and enhance the influence of Classroom Action Research, Innovation, and Competence on Teacher Professionalism. This will contribute to the overall improvement of teacher professionalism in the school. By emphasizing the importance of Classroom Action Research and Innovation, teachers can enhance their professional development and contribute to the advancement of education quality.
