Abstract

Background: The nutritional status of children under the age of five is one of the effective health achievement measures in the SDGs. Stunting is a crucial national issue that requires attention in Indonesia, given its significant impact on the potential and quality of human resources. According to data from the Prabumulih City Health Office in 2023, Pasar Health Center has the highest number of stunted children, accounting for 24 out of 90 stunted toddlers in the city. One of the Ministry of Health's strategies to combat stunting in Indonesia is to address the issue through nutritional improvement programs tailored for toddlers, focusing specifically on underweight children. Purpose: to evaluate the SFP for underweight toddlers using a systems approach (input, process, output). Method: This study was a qualitative study. The informants were selected using consecutive technique sampling, included the Health Center's Head, the Person in Charge of Nutrition, the MCH Midwife, Integrated Service Post Cadres, mothers of the targeted toddlers, and the Head of the Health Office's Public Health Division. Results: The SFP has been well-implemented, several obstacles exist in the input and process variables. These include lack of funding for cooking utensils from the central office, insufficient planning of food menus that account for toddler allergies, weather-related issues during the rainy season hindering cadres in food distribution, consumption of SFP by other family members, and tardiness of the targeted toddlers' mothers. Conclusion: The Public Health Center should include fostering open communication with parents to discuss toddler allergies, encouraging parental involvement in monitoring and recording children's weight, enhancing supervision of program implementation, fostering creativity and collaboration with available resources, and strengthening community collaboration to establish cross-sector cooperation for additional support and resources.

Abstract

Background: In response to the COVID-19 pandemic, BPJS Kesehatan has introduced an indirect contact program, but many districts and cities, including Jambi City in Jambi Province, have not achieved the target contact rate. This study aims to analyze the implementation of indirect contact services for JKN participants in Jambi City's primary healthcare facilities (PHC). Subjects and Method: This descriptive qualitative research was conducted at BPJS Kesehatan and PHC Jambi City from March to April 2022. Data were collected through in-depth interviews, observation, and document review. Informants were selected using a consecutive sampling technique based on the type of PHC, including community health centres (Puskesmas), private clinics, and individual practitioners (DPPs), representing the lowest and highest achievement levels. Thematic analysis techniques were used to analyze the data. Results: The findings revealed that the implementation of indirect contact services for JKN participants in Jambi City's PHC was influenced by various factors. These factors included challenges in increasing contact rates, dissatisfaction among JKN participants, and economic constraints faced by the participants. Contributing factors to these challenges was the absence of input guidelines, standard operating procedures (SOPs), dedicated personnel, allocated funds, and specialized equipment for indirect contact. Routine socialization efforts were also lacking. Furthermore, the understanding of officers regarding the objectives, targets, and implementation of indirect contact, as well as their competence, played significant roles in the implementation. Conclusion: Implementing indirect contact services has not been optimal, necessitating increased support from BPJS Health Jambi Branch for improving indirect contact services

Abstract

Background: The escalating number of COVID-19-positive cases and mortality within the community has prompted the government to prioritize vaccination efforts. However, the vaccination target has not yet reached optimal, particularly in Palembang. This study aims to evaluate the implementation of the COVID-19 vaccination program at the Community Health Center in Palembang through a descriptive qualitative approach. Subjects and Method: The data were obtained through in-depth interviews, observations, and document reviews conducted at the Community Health Center between November 25, 2021, and December 30, 2021. A total of ten informants were selected using consecutive sampling. Results: The findings of this study indicate consistency in various aspects of the input component, including the vaccine team, funding sources, types of vaccines, storage facilities, other support systems, and planning methods. However, due to a lack of data integration among the data management entities, the Community Health Center and Health Office face challenges in accurately determining vaccine targets. The vaccination coverage in Palembang stood at 87.08% for the first dose and 68.52% for the second dose. Unfortunately, delays in achieving vaccine coverage, particularly among specific groups such as older people, have hindered the development of expected herd immunity. There have been no reports of severe adverse events following immunization (AEFIs) within the community. Conclusion: The COVID-19 vaccination program in Palembang was implemented well. However, data management entities must prioritize data integration to fulfill vaccination targets.

Abstract

Background: The COVID-19 pandemic has led to a surge in telemedicine utilization in Indonesia, necessitating increased attention toward user satisfaction and encountering obstacles in telemedicine services. This study aims to analyze the satisfaction and barriers faced by users of telemedicine applications, considering population characteristics during the COVID-19 pandemic in Indonesia. Subjects and Method: Using a descriptive observational design with a cross-sectional approach, the study encompassed the entire population of telemedicine users during the COVID-19 pandemic, with samples adhering to predetermined inclusion and exclusion criteria. Data were collected through Google Forms questionnaires from August 2020 to November 2020. Subsequently, data were subjected to chi-square tests (alternative: Kruskal-Wallis) and logistic regression analyses, with p-value<0.05 and 95% confidence interval. User satisfaction with telemedicine was associated with factors such as domicile, settlement, provider, health insurance, and utilized internet network (p<0.05). Results: User satisfaction with telemedicine was associated with factors such as domicile, settlement, provider, health insurance, and utilized internet network (p<0.05). Conversely, barriers encountered in telemedicine exhibited associations with gender, marital status, age, regional origin, residence, education, occupation, health insurance, income, provider type, internet network, and internet quota (p<0.05). A significant correlation was observed between barriers and user satisfaction with telemedicine (p<0.001). The most influential factor affecting satisfaction was income less than Rp 1,500,000 per month (OR 30.818; 95% CI: 1.751-542.394), while pay ranging from Rp 3,500,000 to 5,500,000 per month exhibited the most substantial impact on barriers (OR 5.266; 95% CI: 2.326-11.920). Conclusion: Most respondents expressed satisfaction and encountered no obstacles when employing telemedicine. Respondents suggested enhancing telemedicine usage by emphasizing the importance of diagnostic accuracy, simplifying processes, maintaining confidentiality, optimizing application features, and improving network speed.

Abstract

Introduction: Tuberculosis (TB) remains a significant public health challenge and is included as one of the targets in the Sustainable Development Goals (SDGs). TB case finding in Lubuklinggau City decreased from 967 in 2019 to 273 (24,37%) in 2020. Aims: This study aims to analyze the TB disease management program strategy implementation in contact investigation activities by utilizing the SULI SIMULATOR application. Method: A qualitative design with a participatory action research (PAR) approach was employed, involving focus group discussions, in-depth interviews with 36 informants, and document review. Data were validated using triangulation methods and sources, followed by thematic analysis. Result: The findings revealed that, in terms of input, some PHC lacked analysts in their human resources. In terms of process, the successful implementation of the TB disease prevention program, with the application of SULI SIMULATOR for TB contact investigation, led to an increase in the case detection rate (CDR) to 255 cases (22.76%) in 2021 compared to the previous year's 237 patients (21.16%). Conclusion: The active involvement of the community and family is crucial in promoting early detection of TB cases through contact investigation activities, as well as providing support for treatment in family and community-based health efforts
