Abstract

Background: Intubation causes various side effects such as pain and parasympathetic stimulation, hemodynamic changes, increased intracranial pressure, and increased risk of intracranial hemorrhage. Topical lidocaine has been applied for decades via intratracheal spray, oxygen atomization, intracuff filling, or gel lubrication due to its simple operation advantages. Topical lidocaine is one of the drugs of choice to prevent post-intubation sore throat by decreasing the permeability of nerve membrane cells to sodium ions.Objective: To determine the relationship between the use of 10% lidocaine spray on the mole palate, posterior pharynx, tonsillar pillar, hypopharynx, and endotracheal tube on hemodynamics and post-intubation throat pain.Methods: The design of this study used a single-blind randomized clinical trial with 17 research subjects for each group. Group A was given xylocaine spray on the endotracheal tube, and 17 subjects belonging to group B were given xylocaine spray on the soft palate, hypopharynx, and tonsil pillars.Result: Mean arterial pressure in this study in group A was found to be 91.11 + 6.55 and in group B 91.11 + 6.55, for post-operative sore throat (POST) scores in group A 0.51 + 0.26 and for group B 1.21 + 0.13.Conclusion: There is a difference between the use of 10% lidocaine spray on the soft palate, posterior pharynx, tonsillar pillars, hypopharynx, and endotracheal tube on hemodynamics and the incidence of sore throat after endotracheal intubation in elective patients with general anesthesia at RS Adam Malik, Medan.

Abstract

Respiration is a vital process characterized by exchanging oxygen and carbon dioxide. Indicators such as respiratory rate are essential for detecting pathological conditions, such as pneumonia and heart failure. This research aims to develop a respiratory sensor system based on fiber Bragg grating (FBG) as an innovative alternative in high electromagnetic field environments. The system utilizes FBG optical fibers to detect strain changes due to respiratory activity, providing a sensitive, safe, and highly electromagnetic environment-compatible solution. The study used FBG with variations in reflectivity of 30%, 50%, 70%, and 90%. FBGs are installed inside oxygen masks at five different points to monitor wavelength changes during respiratory activity. The measurement method involves an optical system with an interrogator and an electrical method using an InGaAs photodiode converter to convert an optical signal into an electrical signal visualized in LabVIEW. Respondents were tested in three activities: stillness, walking, and running. Variations in sensor reflectivity and position in masks were evaluated to determine sensitivity to respiratory changes. The data is collected as a graph of wavelength against time. The result showed that the change in the wavelength of the FBG correlated with the intensity of respiratory activity. The reflectivity of 90% results in the highest sensitivity, allowing for more accurate detection of strain changes. The position of the sensor at the center point of the mask demonstrates the most linear results, indicating optimal sensitivity. Physical activity, such as running, produces the greatest strain on the optical fiber. This study proves the potential of FBG as a precision medical sensor for respiratory monitoring applications.

Abstract

Background: The COVID-19 pandemic affects all activities of human life, including education. Educational activities that are usually carried out face-to-face in class have now been stopped to avoid the expansion of the spread of COVID-19. The cessation of face-to-face learning activities is shifted to online-based learning (distance) by utilizing technology that is connected to the internet. This distance learning system applies to all levels of education. Learning with an online system using a computer all day makes various complaints experienced by students such as computer vision syndrome. The most common eye complaints condition is computer vision syndrome, some eye and vision problems that originate from prolonged use of computers, tablet computers, electronic readers, and mobile phones. Computer Vision Syndrome is a collection of symptoms in the eyes and neck caused by excessive use of computers/monitors. Symptoms vary but mostly involve eye strain, headache, blurred vision, dry eyes, irritation, slowed focusing of the eyes, pain in the neck, back, and sensitivity to light. Objective: The purpose of this study was to analyze the relationship between visibility, body posture, and duration of computer use with Computer Vision Syndrome (CVS) in USU Medical Faculty students Methods: This type of research is analytic with a cross-sectional study approach. The sample of this study amounted to 100 students who were randomly selected using stratified random sampling. The data used is primary data from questionnaires filled out online through the Line, WhatsApp, and Instagram applications. Data analysis using SPSS with chi-square statistical test (p <0.05). Results: The results showed that the most visibility was < 50 c, frequent bad posture, and duration of use was > 4 hours. Conclusion: The results showed that there was a relationship between visibility, body posture, and duration of using a computer with CVS for USU Medical Faculty students.

Abstract

Introduction: Stunting is a chronic malnutrition condition caused by a numeuros of factors, including inadequate maternal nutritional nutrition during pregnancy, inadequate exclusive breastfeeding, and infectious diseases. There are also some indirect factors .Objective: The objective of this study was to investigate the risk factors for stunting in children aged 6-36 months in the Sambirejo Health Center area. study. Method: This study is an observational analytic study using a cross-sectional approach that gathers data via a questionnaire. Sampling Techniques using the Non- Probability Sampling technique, and the sample size was estimated using the Lemeshow formula. The Prevalence Ratio (PR) was calculated to evaluate the strength of the link between the factors assumed to be linked with stunting. Result and Discussion: Of the 65 samples,  Four children were stunted, while 61 were not. Infants who were not exclusively breastfed, infants with a history of LBW, infants with a history of SBL, and family planning of 2 years both were risk factors for stunting in this study. Conclusions: Some variables in this study had no impact on the incidence of stunting. In this study, the risk factors for stunting were LBW, SBL, and infants without exclusive breastfeeding, and child spacing was a neutral factor for the occurrence of stunting in children aged 6-36 months.

Abstract

Background. Dengue remains an endemic disease in almost all countries worldwide, including Indonesia. One approach to its prevention and control is community empowerment through education. Education is most impactful when aligned with the culture and local wisdom of the target community. Poda na lima is a form of local wisdom practiced by the Batak Angkola-Mandailing tribe in North Sumatra Province, Indonesia. This study aims to analyze the effect of poda na lima-based education on community knowledge regarding dengue prevention. Methods. Quasi-experiment with one group pretest-posttest. This study involved 68 respondents from the community in the working area of Puskesmas Sihepeng, Mandailing Natal Regency, through cluster random sampling with inclusion criteria in October – November 2024. Data were collected using questionnaires before and after the dengue education intervention with poda na lima and analyzed using the Wilcoxon signed rank test with the SPSS software. Result. Most of the respondents were in the age range of 19 – 45 years (80.9%), with a female gender (100%) and an education level of high school graduate (39.7%). There was an increase in the average knowledge before and after dengue education with the poda na lima, with statistically significant results (p<0.05) and an effect size > 0.5, meaning that the dengue education with the poda na lima has a large effect on the improve in knowledge. Conclusion. Dengue education with local wisdom poda na lima has a significant and large effect on improving community knowledge.

Abstract

Pendahuluan: Pasien kanker merasakan nyeri yang mempengaruhi kualitas hidup terutama pasien kanker stadium lanjut. Beberapa agen yang digunakan pada nyeri kanker adalah morfin, parasetamol, dan amitriptilin. Antidepresan, anti-epileptik, dan kortikosteroid mulai dieksplorasi untuk terapi nyeri pada penyakit non-kanker, namun penggunaannya dalam nyeri kanker masih belum dipelajari dengan baik. Beberapa studi menyimpulkan manfaat penggunaan gabapentin pada nyeri neuropatik dan nosiseptif, hingga kini belum ada studi efikasi agen tersebut pada nyeri kanker, maka penelitian terfokus kepada efikasi kombinasi antara amitriptilin dan gabapentin. Metode: Penelitian ini menggunakan Randomized Clinical Trial dengan sistem blinding. Dua kelompok sampel diobservasi sebelum pemberian (T0), hari ke-1 (T1), ke-3 (T2), dan ke-7 (T3). Kelompok A menerima gabapentin dan kelompok B menerima gabapentin dengan amitriptilin. Analisis data menggunakan uji t independen atau Mann-Whitney. Hasil: Sebanyak 62 orang dibagi menjadi 2 kelompok. Kelompok A mengalami keluhan mual, muntah, penurunan nafsu makan, dan pusing terbanyak sedangkan kelompok B mengalami keluhan sulit tidur terbanyak. Secara statistik, kelompok A maupun B mampu mereduksi skala nyeri berdasarkan skala pengukuran NRS dan pain detect (p<0,001) serta data keluhan awal dijumpai perbedaan signifikan (p<0,001). Perbedaan yang bermakna secara signifikan antara kedua regimen yang dianalisis juga tidak dijumpai dengan nilai p>0,05 pada seluruh waktu, kecuali pada variabel NRS di T3 (gabapentin (4,03±0,59)) dan kombinasi (3,53±0,82); p=0,009). Simpulan: Temuan mengindikasikan perbedaan yang signifikan mungkin baru terlihat pada hari ke-7 pasca observasi, meskipun pada dasarnya regimen kombinasi lebih efektif untuk mereduksi nyeri dibandingkan gabapentin tunggal. Kelompok kombinasi mengalami penurunan derajat nyeri yang lebih tinggi namun hanya signifikan pada waktu T3-T2 menggunakan skala NRS.

Abstract

This research aims to determine the factors that influence tuberculosis infection in children with close contact with adult pulmonary tuberculosis sufferers. This research is an analytical observational study with a consecutive sampling method at the Glugur Darat Health Center in November 2024 until January 2025. Bivariate analysis using chi-square to determine whether there is a relationship between the independent and dependent variables. The type of test used is the Fisher Exact and Kruskal Wallis Test. There is no multivariate test because there are no significant variables in the bivariate test. In total, in 42 samples there were 10 children with positive Mantoux tests. Of the 23 female children there were 7 (30.4%) with positive results and of the 19 male children there were 3 (15.8%) positive results. Of the 39 children with a history of BCG immunization, there were 10 (25.6%) with tuberculosis infection. Of the 37 children with parents whose income was <UMR, 10 children (27%) had tuberculosis infection. 37 children whose parents had junior high school education, 9 people (24.3%) had tuberculosis infection. 10 children aged 1-5 years and 6-10 years, there were 2 (20%) each with tuberculosis infection. 26 children aged 11-18 years, there were 6 people (27.3%) with tuberculosis infection. There was no significant relationship between age, gender, immunization status, nutritional status, parental income and parental education level in this research.

Abstract

Background The administration of general anesthesia during surgery can trigger the body’s stress response causing changes in regular metabolic patterns that result in hyperglycemia and ketosis. The goal of this study is to investigate how preoperative oral glucose and non-glucose administration affect patients with diabetes mellitus with blood glucose levels during surgery under general anesthesia. Methods This is a single-blind, randomized controlled trial (RCT) being carried out by the Central Surgical Installation and postoperative recovery area at Haji Adam Malik General Hospital Medan. The study sample consists of 38 patients with diabetes mellitus who underwent electively scheduled laparotomies under general anesthesia and met inclusion/exclusion criteria. For treatment group participants, mineral water containing 50 grams of glucose was administered with 400 cc. Research hypotheses were assessed using the t-test, and significant findings had p-values equal to or less than 0.05 within a confidence interval of 95%. Results The study sample had a mean age of 50.1 ± 12.73 and showed a significant effect between glucose levels before (178.47) and after (163.78) administration of maltodextrin, with a P value (0.001). No side effects were observed during or after the post-operative evaluation process conducted in this study's participants. Conclusions Patients with diabetes mellitus experience a preoperative oral glucose-induced impact on blood glucose levels.

Abstract

Improve healthy and clean-living behavior starting from early childhood, this can affect the growth and development of children and also the level of intelligence of children to produce a healthy and quality generation. The North Sumatra Kahmi Amanah Playgroup where early childhood education is located in the Medan Marelan area is a potential place that should be developed for early childhood education. School Health Business Activity is an activity that can foster clean and healthy behavior from an early age in children in the surrounding area who are children around Marelan area. The community service team funded by the University of North Sumatra through the LPPM institution consisting of lecturers from the Faculty of Medicine with different departments and also lecturers from the Faculty of Public Health is engaged in providing training to improve the skills of educators for school health business activities as well as providing medical equipment and facilities. needed and not yet available in the Amanah Kahmi playgroup, such as closed trash cans, making hand washing sinks, and thermogenic so that this community service activity can produce something useful for the community around the Medan Marelan area.

Abstract

Along the decline COVID-19 cases in Indonesia, the implementation of Limited Face-to-face Learning (PTM) in education units was again carried out by observing the implementation of strict health protocols by education units. In the implementation, various problems were found, such as the lack of awareness of students in implementing health protocols. The solutions offered by the Community Service team to support preventing the spread of COVID-19 and improving healthy lifestyles include developing a health protocol ambassador program through a peer teaching program with the Montessori method. This activity was carried on August 8, 2022 with assistance and monitoring carried out until August 22, 2022 at SD Muhammadiyah 01 Medan with a total sample of 60 people. The results showed an increase before and after the activity, before the activity was carried out as many as 31 (51.7%) respondents were at a moderate level of knowledge, 37 (61.7%) positive attitudes and 32 (53.3%) good behavior respondents. After the activities were carried out, 47 (78.3%) respondents had high knowledge, 60 (100%) positive attitudes and 46 (76.7%) good behavior. Therefore, this method is effective in increasing the knowledge, attitudes and behavior of students' awareness about health protocols and prevention of COVID-19 transmission.
