Abstract

Introduction : That was a difficulty to diagnose the early stage of HIV while the symptoms was only enlargement of the lymph nodes, so that the pattern A of HIV lymphadenitis that presented on histopathological examination is expected to be a determinant of early-stage. Although the staining of p24 antibody has already be used to detect HIV infection in the tissue, but the positivity of p24 in pattern A HIV lymphadenitis has not been investigated. Aim of study : To identify pattern A of HIV lymphadenitis on histopathological examination and to identify p24 antibody to be a determinant of early-stage HIV infection. Methods : This is a retrospective descriptive study. We collected 64 cases of non spesific lymphadenitis from the Anatomical Pathology archive, Cipto Mangunkusumo hospital and examined these cases by using the criteria of pattern A HIV lymphadenitis in the tissue specimen. Then we performed the staining of p24 antibody by immunohistochemistry. Results : There were twelve by sixty four (12/64) cases of that have pattern A HIV lymphadenitis on microscopic evaluation. Only three cases was clinically noted as Acquired Immunodeficiency Syndrome (AIDS) by clinician, while 11 of 12 cases showed immunohistochemistry positive for p24 antibody. Conclusion : Most of the patten A HIV lymphadenitis (11/12) showed the positive staining for p24 antibody. In patients who had received Anti Retro Viral (ARV) therapy, the staining of p24 antibody was negative. This study still needs more samples, the serologic data of the patient to be more reliable.

Abstract

Breast cancer is the most common type of cancer in women worldwide. One of the factors that are believed to have a role in the development and growth of breast cancer is nutritional status which can be associated with the body mass index of adult patients. It is estimated that overweight and obesity are risk factors for the spread of breast cancer to distant metastases. Mechanism of spread through two ways, estrogen and leptin. Aromatase is an enzyme in estrogen biosynthesis that promotes tumor growth through its expression in breast adipose stromal cells. Meanwhile, leptin is produced by adipocytes. Adipocytes that experience hypertrophy and hyperplasia cause pathophysiological changes that affect the development of breast cancer. Various case studies show that a diet high in fat and calories is associated with breast cancer. The high risk of obesity for breast cancer metastases. This is important to be studied further because of the large number of cases of breast cancer both globally, in Indonesia and in Lampung. This review article is important to know the relationship between obesity and breast cancer metastases, as scientific information and knowledge, and can be used as material for dietary intervention in obese breast cancer patients.

Abstract

Tuberculosis (TB) is one of the most common causes of health problems in the world. The prevalence of TB in the world is estimated at 10.6 million, and 1.6 million of them died from TB, in Indonesia the incidence of TB reached 503,712. Tuberculosis is a chronic infectious disease caused by Mycobacterium tuberculosis, which is known to cause complex hematological disorders. The hematopoietic system is thought to be affected during TB infection, so that Hb levels tend to decrease along with bacterial increase, which eventually causes anemia. The article aims to determine the incidence of anemia in TB patients and the mechanism of anemia in TB patients. Based on the results of a literature review conducted on references found from 2012–2022, it shows that anemia is a common condition in TB. The mechanism that causes anemia in TB patients is due to its pathogenesis as a chronic inflammatory disease. Anemia is an indicator in providing clues to diagnosis, prognosis, complications and response to TB therapy. Nutritional intake and adherence to TB therapy need to be done properly to prevent the development of severe disease and improve the patient's quality of life.

Abstract

Surgical site infection (SSI) is a fairly common complication that can arise after surgery, both minor surgical procedures and more complex surgical procedures. This infection can occur due to the entry of pathogenic microorganisms into the surgical wound, causing an inflammatory reaction. This infection can prolong recovery time, increase mortality, require additional medical intervention, and affect the patient's quality of life. This impacts patient clinical outcomes as well as additional care costs. Factors such as surgical technique, cleanliness of the operating environment, use of implants, and the patient's health condition can influence and also contribute to increasing the risk of infection. Symptoms of surgical site infection include pain, swelling, redness, or the presence of pus in the surgical wound. When this infection occurs, immediate medical action is needed to prevent the spread of infection which can cause advanced conditions such as sepsis. Prevention of surgical site infections involves sterile measures before and during surgical procedures, the use of prophylactic antibiotics, and thorough post-operative monitoring. These efforts aim to reduce the risk of surgical site infections, ensure optimal recovery, and improve the overall results of the surgical procedure. An in-depth understanding of the risk factors and prevention of surgical site infections can help medical personnel provide quality care and ensure the success of the operation. There is also the importance of early detection and treatment of infections in patients undergoing surgery.

Abstract

The kidneys are retroperitoneal organs located at the top on both sides of the III lumbar vertebra and attached directly to the abdominal or ventral wall of the M. psoas major and M. quadratus lumborum. The kidneys function to regulate the amount and concentration of ECF electrolytes and maintain nerve and muscle excitability, activating vitamin D which is important for calcium absorption in the digestive tract and producing erythropoietin which is a hormone to stimulate the bone marrow to form red blood cells. The main function of the kidneys is to filter and excrete metabolic waste products in the urine so that administration of toxic compounds or irritating compounds can cause degenerative changes such as fatty degeneration to necrosis of the kidneys. Disorders of the kidneys can disrupt kidney function. Chronic Kidney Disease (CKD) sufferers have several complications such as overhydration, hyperkalemia, metabolic acidosis, mineral and bone disorders, hypertension, anemia and dyslipidemia. The most common etiologies of CKD in adult patients are diabetes mellitus and hypertension. In children, the most common primary cause of PGTA is congenital abnormalities of the kidneys and urinary tract (such as obstructive uropathy, vesicoureteral reflux, renal dysplasia). Indonesian Nephrology Association (PERNEFRI) in 2006, which found that the prevalence of Chronic Kidney Disease (CKD) was 12.5%. Meanwhile, data according to the 2019 Health Insurance Administration (BPJS) shows that there were 1.93 million cases of kidney failure and 1.79 million cases in 2020. With this data, the author is interested in discussing the kidneys in more depth.

Abstract

Diabetes Mellitus (DM) is a chronic disease characterized by blood sugar levels that exceed normal limits. Chronic DM can cause liver complications in the form of inflammation. Streptozotocin can induce mice to become a DM model and can also directly damage the liver. Garlic (Allium sativum) is a tuber that is often found in Indonesia. The various compounds contained in it have anti-diabetic and antioxidant effects which have the potential to prevent inflammation in the liver. This research was conducted to determine the effect of garlic extract (Allium sativum) on histopathological features in the form of lymphocyte cell infiltration in the portal and lobular tracts of the liver in white rats (Rattus norvegicus) of the Sprague-dawley strain induced by streptozotocin. The method used was a randomized controlled design with a pre-clinical Post-test Only Control Group Design pattern. This study used 24 mice divided into 4 groups with 6 mice in each group. The appearance of lymphocyte cell infiltration will be assessed based on the Omenetti scoring. The mean scores for lymphocyte cell infiltration in the portal tract obtained were KN=0.22, K+=2.10, P1=0.60, and P2=0.56. From the analysis using the One-Way ANOVA test followed by the Post Hoc Benferroni test, the results showed a significant difference in scores between K+ and P1 and P2. Meanwhile, the mean scores for lymphocyte cell infiltration in the lobular area obtained were KN=0.50, K+=1.16, P1=0.50, and P2=0.33. From the analysis using the Kruskal-Wallis test, no significant differences in scores were found between all groups. There was an effect of garlic extract (Allium sativum) on the infiltration of lymphocyte cells in the portal tract and no effect was found on the lobular area of the liver in white rats (Rattus norvegicus) of the Sprague-dawley strain which were induced by streptozotocin.
