Abstract

The success of diabetes prevention is influenced by several factors, including access to information, family support, diet, activity patterns, nutritional status, and self-efficacy. These factors play a significant role in the self-care practices of individuals with diabetes. Self-care refers to actions taken by patients to improve their ability to meet their daily needs independently, enabling them to prevent and manage their illness and adhere to treatment and recommendations provided by healthcare services. This study aims to identify the self-care practices of individuals with diabetes. The research involved 40 respondents diagnosed with diabetes. The average self-care score among diabetic patients in the working area of Banjar Baru Selatan Community Health Center was 68.53, with the lowest score being 39 and the highest score 92. Establishing self-care behaviors as part of a cultural norm requires consistent repetition and routine, to the extent that these behaviors may eventually become secondary needs. One approach to cultivating such habits in individuals with diabetes is to focus on addressing their weaknesses through methods that align with their routines and learning patterns. This strategy can help foster motivation and enthusiasm in managing their condition.

Abstract

Chronic diseases such as heart failure, diabetes, and hypertension require active patient involvement in self-management. Self-efficacy is a key factor that influences the success of self-management among individuals with chronic conditions. This article aims to synthesize current empirical evidence on the effectiveness of peer-based interventions in improving self-efficacy among patients with chronic diseases. This study employed a systematic review approach based on the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA). Literature was retrieved from PubMed, ScienceDirect, ProQuest, EBSCOhost, Wiley, and Google Scholar databases. The inclusion criteria were full-text articles in English, with designs such as randomized controlled trials, quasi-experiments, and pilot studies published between 2015 and 2025. Of the 635,062 8 articles identifie the inclusion criteria and were analyzed. The included articles were critically appraised using appropriate tools and independently reviewed for quality and relevance, with data systematically extracted and narratively synthesized due to methodological heterogeneity. All reviewed studies reported a significant increase in self-efficacy following peer-based interventions. These interventions were implemented in various formats, including group training, theory-based peer coaching (eg, Health Belief Model, Health Promotion Model), digital peer support, and nurse-facilitated peer programs. Some studies also demonstrate long-term effects and additional benefits such as improved quality of life, better stress management, and reduced stigma. Peer-based interventions are proven effective in enhancing self-efficacy among patients with chronic diseases and may serve as a complementary strategy in clinical practice and primary health care services.

Abstract

Clinical judgment is a critical and multifaceted process essential for ICU nurses, integrating technical expertise and interpersonal skills to ensure safe, effective patient care. Clinical judgment, recognized as a core competency in intensive care nursing, requires further conceptual clarity. This study aimed to define and analyze the concept of clinical judgment in ICU nurses using Walker and Avant’s eight-step concept analysis method. A comprehensive literature search was conducted across databases including CINAHL, Medline, Scopus, ProQuest, SpringerLink, ClinicalKey Nursing, EMBASE, ScienceDirect, EBSCOhost, and Google Scholar, covering publications from 1994 to 2024. The analysis identified key attributes of clinical judgment: clinical knowledge, critical thinking, practical wisdom, data prioritization, intuition, scientific analysis, evidence use, and interaction. Antecedents included education, knowledge, experience, analytical ability, and the nursing process. Consequences of clinical judgment were accurate responses, reflective practice, safe nursing care, and improved patient safety. Decision-making was identified as the primary empirical referent. This concept analysis demonstrates that clinical judgment in ICU nurses is developed through a synthesis of critical thinking, clinical expertise, and situational experience, reinforcing its significance in the delivery of high-quality critical care.

Abstract

Background: The prevalence of acute coronary syndrome (ACS), which is the most critical ischemic heart disease and is the main source of morbidity and mortality worldwide, increases significantly every year in Indonesia, including Balinese young adults. Identification of modifiable risk factors, such as smoking habits, consuming alcohol, and processed food, is culturally important for making primary prevention strategies, the occurrence of attacks, and secondary prevention to reduce readmissions.Objectives: This study aims to identify modifiable factors using a cultural approach associated with the incidence of ACS in Balinese young adults.Methods: This is a quantitative cross-sectional study. A total of 150 eligible respondents were recruited consecutively at the Integrated Heart Service Centre of Prof. Dr. I.G.N.G Ngoerah General Hospital, Bali. Data were collected using validated questionnaires on socio-demographics, diet (SQ-FFQ), physical activity (GPAQ), alcohol use (AUDIT), psychological status (DASS-42), and sleep quality (PSQI). Independent variables included hypertension, diabetes, dyslipidemia, hyperuricemia, lifestyle, and psychosocial factors, with ACS incidence as the outcome. Data were analysed with descriptive statistics, chi-square, and logistic regression (p < 0.05).Results: Results showed a significant relationship between hypertension, high salt intake, and sleep quality. Hypertension demonstrated the strongest statistical association with ACS among the variables included in the model (OR = 6.785, 95% CI: 2.429 - 18.956). The Nagelkerke R² value was 0.449, indicating a moderately strong predictive model. This shows that hypertension, risky diet, and poor sleep quality contribute to 44.9% of the explained variance in the incidence of ACS among young adults. Conclusion: Risk factors contribute to the high prevalence of hypertension in Bali, including a high salt and fat diet in Balinese cuisine, poor sleep habits, and traditional activities in Bali. Recommendations for future research include exploring cardio-social factors and culturally based prevention and control strategies for ACS in young adults in Bali.

Abstract

Pendahuluan: PJK merupakan penyebab kematian kedua di Indonesia, yang berkontribusi terhadap 95,68 kematian per 100.000 penduduk Indonesia. Salah satu cara penanganan penyakit jantung koroner adalah dengan membuat jalur baru menggunakan pembuluh darah sehat dari bagian tubuh lain. Sekitar 44  per 100.000 orang menjalani operasi bypass diseluruh dunia. Pasien yang akan menjalani operasi bypass jantung umumnya mengalami kecemasan. Pada pasien bedah jantung, kecemasan praoperasi juga berhubungan dengan intensitas nyeri pascaoperasi yang lebih tinggi serta peningkatan kebutuhan akan analgesik. Pendekatan psikologis dan komplementer direkomendasikan untuk mengurangi kecemasan dan meningkatkan kenyamanan secara holistik, salah satunya dengan terapi relaksasi Benson. Metode: Penerapan ini menggunakan pendekatan quasi experiment dengan rancangan pre-test and post-test with control dengan jumlah sampel 40 responden. Hasil: Hasil penelitian menunjukkan adanya perbedaan signifikan antara sebelum dan setelah intervensi dan perbandingan antar kelompok dengan nilai p = 0,001. Diskusi: Hasil penelitian menunjukkan rata-rata usia pasien 58 tahun, mayoritas laki-laki, menikah, dan berpendidikan SMA, selaras dengan profil pasien CABG pada studi sebelumnya. Terapi relaksasi Benson terbukti meningkatkan kenyamanan secara signifikan, mendukung temuan serta studi lain yang menunjukkan penurunan kecemasan, tekanan darah, kelelahan, dan risiko delirium. Temuan ini menegaskan pentingnya intervensi non-farmakologis yang mudah diterapkan untuk mempercepat pemulihan pasien pasca operasi jantung.

Abstract

There is existing concerns from clinical education that nursing students need to improve confidence on clinical performance. Confidence is basis of nursing education and determines graduates’ level of competence to enter workforce. Confidence promotes safe and effective patient care. However, it is challenging to understand concept of confidence and how nurse educators instill confidence with innovative teaching strategies. The purpose of this review is to discuss concept of confidence, factors influencing confidence, and teaching strategies to improve students’ confidence. Science Direct, EBSCOhost, and Pubmed databases were used to identify evidence based research related to students’ confidence and teaching strategies. The key words included confidence, nursing students, teaching strategies, and clinical performance. The articles were selected based on objectives, relevance, and year published. The result suggested that simulation should be modified by other strategies such as repeated simulation, team based learning, and self-debriefing. Another strategy is multidisciplinary teaching using technological tool such as video podcasting, immersive 3d interaction, game-based learning and video-based learning. Clinical confidence is an essential element in students’ clinical performance. Innovative and creative teaching strategies are needed to instill nursing student’s confidence. Further studies are needed in assimilation of teaching strategies for nurturing students’ clinical confidence.

Abstract

Sistem jenjang karir perawat adalah perencanaan dan pelaksanaan karir yang menempatkan perawat sesuai dengan keahlian dan kompetensinya, serta memberikan peluang untuk berkembang dan melakukan perubahan yang lebih baik sesuai kemampuan. Jenjang karir profesional bagi perawat tidak hanya merupakan sistem yang bertujuan meningkatkan kinerja dan profesionalisme, tetapi juga mencakup peningkatan kompetensi yang berdampak pada kinerja profesional yang dapat dipengaruhi oleh motivasi kerja. Tujuan penelitian ini adalah untuk mengetahui hubungan antara motivasi kerja dengan pelaksanaan sistem jenjang karir perawat di Primaya Hospital Bekasi Utara. Metode penelitian menggunakan desain penelitian cross sectional. Pemilihan responden berdasarkan menggunakan total sampling. Responden pada penelitian berjumlah 174 perawat di Primaya Hospital Bekasi Utara. Nilai significancy (p-value) 0,000 atau < 0,05 yang menunjukkan bahwa korelasi antara motivasi kerja dengan pelaksanaan sistem jenjang karir perawat adalah bermakna. Nilai korelasi Spearman sebesar 0,709 menunjukkan bahwa arah korelasi positif dengan kekuatan korelasi kuat. Sistem jenjang karir perawat telah diidentifikasi sebagai strategi pengayaan pekerjaan yang berkontribusi terhadap kepuasan dan kinerja perawat, menekankan pentingnya penghargaan, pengakuan, dan peluang pengembangan profesional dalam meningkatkan motivasi kerja dalam profesi keperawatan.

Abstract

Overweight dan obesitas pada anak merupakan suatu masalah yang kompleks disebabkan multifaktor, yaitu interaksi genetik dan lingkungan. Gaya hidup perkotaan dipicu oleh asupan makanan yang berlebih pada anak overweight dan obesitas. Strategi untuk menurunkan asupan makan berlebih pada anak adalah dengan pendekatan child healthcare model dan transtheoretical model sehingga dapat mengendalikan gaya hidup anak. Penelitian ini bertujuan membuktikan pengaruh pendekatan child healthcare dan transtheoretical model terhadap asupan karbohidrat anak overweight dan obesitas. Penelitian ini dilaksanakan di wilayah kecamatan Biringkanaya dan Tamalanrea, Makassar, pada bulan Agustus 2013 sampai Maret 2014. Desain yang digunakan adalah quasy experiment yaitu pretest and posttest with control group design. Sampel dipilih secara purposive sebanyak 31 anak overweight atau obesitas pada kelompok perlakuan dan 33 kontrol pada anak sekolah dasar kelas 4 - 6. Intervensi penelitian 6 bulan dengan pemberian buku panduan gaya hidup sehat. Instrumen menggunakan kuesioner food recall. Hasil uji-t berpasangan menghasilkan asupan karbohidrat pada pre-post intervensi kelompok perlakuan dengan nilai p 0,004 ( < 0,05) sedangkan kelompok kontrol dengan nilai p 0,114. Penelitian ini menyimpulkan bahwa ada pengaruh pendekatan child healthcare model dan transtheoretical model terhadap asupan karbohidrat anak overweight dan obesitas. Overweight and obesity in children is a complex problem that is caused by a multifactorial genetic and environmental interactions. Urban lifestyle fueled by excessive food intake in overweight and obese children. Strategies to reduce excessive food intake in children is the child healthcare approach and the transtheoretical model so that the model can control the child’s lifestyle. This study aimed to prove the effect of child healthcare approach and the transtheoretical model of the food intake of overweight and obese children. This research was conducted in the district area Tamalanrea and Biringkanaya, Makassar from August 2013 to March 2014. The design used is quasy experiment pretest and posttest with control group design. Purposively selected sample of 31 children as overweight or obese in the treatment group and 33 controls on primary school children grade 4 - 6. Intervention research was conducted during a six month period by providing guide books on healthy lifestyle. The instrument used food recall questionnaire. Paired t-test results produced carbohydrate intake in the pre-post intervention treatment groups with p value 0.004 (<0.05), whereas the control group with p value of 0.114. This study concludes that there is influence of CHM and TTM approaches to the intake of carbohydrates of overweight and obese children.

Abstract

Information technology used in the health sector is known as e-health. Smartphone applications are starting to be widely used for long-term condition monitoring management in the medical and health fields. Perioperative cardiac surgery procedures require long-term monitoring. This study aimed to find out about smartphone use in perioperative cardiac surgery. A systematic review design. The reporting framework uses Featured Reporting Items for Systematic Review and Followed Meta-analysis to report findings. Data Sources, CINAHL, ProQuest, Science Direct, and Google Scholar searched for English-language articles published from January 2010 to August 2020 with the keywords"cardiac surgery," "smartphone," and "peri-operative". Six articles that met the inclusion criteria were the samples analyzed in this study. The instrument used to assess the quality of the study uses the Downs and Black checklist. In the pre-operation stage, a smartphone application has been used for surgical planning, assessment, education, training, diagnosis, telemedicine, navigation, and behavior modification interventions. In the intraoperative stage, smartphones are used for hemodynamic monitoring. In the post-operative stage, smartphones are used for monitoring surgical wounds. The benefits of smartphones in peri-operative cardiac surgery include diagnosis, telemedicine, navigation, and surgical planning, training, data collection, hemodynamic monitoring, nursing intervention media, and surgical wounds. Abstrak: Teknologi informasi yang digunakan dalam bidang kesehatan dikenal dengan istilah e-health. Aplikasi smartphone telah digunakan dalam bidang kedokteran dan kesehatan sebagai alat untuk manajemen pemantauan kondisi jangka panjang. Kondisi perioperatif pada bedah jantung memerlukan monitoring dengan jangka waktu yang panjang. Penelitian ini memiliki tujuan untuk mengetahui penggunaan smartphone pada perioperatif bedah jantung. Desain penelitian ini systematic review. Kerangka kerja dalam pelaporan menggunakan PRISMA untuk melaporkan temuan. Sumber Data ProQuest, Google Scholar, CINAHL, dan Science Direct dicari untuk mengidentifikasi bahasa Inggris asli yang diterbitkan, diterbitkan dari Januari 2010 hingga Agustus 2020 dengan kata kunci "cardiac surgery" "smartphone" dan "perioperatif". Enam artikel memenuhi kriteria inklusi menjadi sampel akhir. Instrumen untuk menilai kualitas studi dinilai menggunakan daftar periksa Downs dan Black. Smartphone digunakan dalam tahap pra operasi seperti diagnosis, telemedicine, navigasi dan perencanaan bedah, pelatihan, pengumpulan data diikuti dengan pendidikan pasien dan intervensi modifikasi perilaku. Pada tahap intraoperatif, smartphone digunakan untuk pemantauan hemodinamik. Pada tahap pasca operasi, smartphone digunakan untuk memantau luka operasi. Manfaat smartphone dalam bedah jantung  antara lain untuk diagnosis, telemedicine, navigasi dan perencanaan bedah, pelatihan, pengumpulan data, pemantauan hemodinamik, median intervensi keperawatan dan pemantauan luka bedah.

Abstract

Hypertension is a chronic disease with a prevalence that is still quite high in the world. Spirituality becomes an important element in increasing confidence in the success of hypertension therapy. This study aims to explore the spiritual experience of hypertensive patients conducting the therapy program. This study is a phenomenological qualitative research involving 6 participants of hypertensive patients at Aisyiyah Hospital of Kudus who met the criteria. The results of the study found 6 (six) themes, namely:  1) Belief in improving body condition and mildening of disease symptoms through worship, (2) Belief in worship as a means of self-improvement from bad habits and increasing goodness, (3) Belief in pain is understood as a means for self-introspection and improvement of worship, (4) Belief in synchronizing worship and effort as a means of healing, (5) Belief in doing relaxation therapy through worship,  (6) Belief in disease symptoms hinders worship. These results show that spirituality plays an important role for hypertensive patients to increase the success of therapy.
