Abstract

Background: After surgery, patients are often immobile due to possible postoperative complications and activity restrictions, so that the patient's physical capacity is at risk of decreasing. To maintain conditions that remain optimal even within limitations, patients are required to continue to be able to independently perform self-care. So that this can be realized with the nursing theory developed by Dorothea E. Orem about self-care Purpose: The aim of this study was to find out the application of Orem's self-care theory to patients after mitral valve surgery. Method: The design in this study uses the case study method. This study focuses on nursing care which emphasizes the use of Orem's self-care theory in practice. The subject in this study was one postoperative mitral valve patient who was treated in the postoperative unit with an implementation time of seven days. Results: In the results of applying Orem's self-care theory, five main nursing diagnoses were found. Most of the nursing problems are rooted in problems with decreased cardiac output due to post-surgical complications, but the complications are gradually improving. The application of Orem's self-care theory provides a systematic pattern of thinking in determining diagnoses and nursing interventions that must be carried out by nurses. Conclusion: The application of Orem's self-care model appears to have an effect on the nursing process, specifically in postoperative mitral valve cases in identifying problems in fulfilling self-care. The application of this model can provide benefits in encouraging patients to carry out activities that are in accordance with the patient's abilities and tolerance.

Abstract

Characteristic of heart failure patients is decreased activity tolerance which is defined as a decreased capacity to carry out activities accompanied by symptoms of dyspnea and/or significant fatigue. This study aimed to determine whether Inspiratory Muscle Training (IMT) can improve activity tolerance in heart failure patients. A systematic review design with the PICO (Population, Intervention, Comparation, and Outcome) approach has been arranged based on the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guideline. Articles were collected through the online database ClinicalKey Nursing, EBSCOhost, Oxford Journals, ProQuest, Sage Journals, ScienceDirect, Scopus, and SpringerLink. Articles with Randomized Controlled Trial (RCT) and quasi experimental design in English were included. Quality of the article was assessed using JBI Critical Appraisal Checklist. There were seven research articles that met the criteria in this study. Those studies show that IMT can increase activity tolerance in heart failure patients. IMT can be applied as a single intervention or in combination with other interventions. Interventions that can be combined with IMT include Cardiac Rehabilitation (CR), Peripheral Muscle Training (PMT), or Functional Electrical Stimulation (FES) programs.

Abstract

Adolescents with chronic illnesses are at high risk of psychological distress such as stress, anxiety and depression that can affect their overall quality of life. One of effective approaches to reduce these impacts is peer group support. This study was to examine the effects of peer group support intervention in reducing psychological distress in adolescents with chronic illness through scoping review. The articles were obtained from SpringerLink, Science Direct, Taylor & Francis, Proquest, Scopus, BMC, and PubMed databases. Article selection was done by the PRISMA-ScR method. Searches of 7 databases obtained 1,578 articles. Based on the inclusion criteria, 291 articles were eligible. Further screening based on title, abstract, and full text selected 10 articles for analysis. Furthermore, the quality of the article was assessed using Jonna Briggs Institute (JBI). The results showed that peer group support reduced stress, anxiety, depression, and increased social support and emotional well-being. Interventions that emphasized active communication among adolescents have a positive impact on psychological disorders. Peer group support can reduce psychological distress in adolescents with chronic illness. This approach also strengthens positive social relationships among adolescents. Further research is needed to explore the factors that influence the success of this intervention and develop more adaptive support models.
