Abstract

Kekerasan terhadap anak merupakan isu yang mengalami peningkatan dari waktu ke waktu, dengan Kabupaten Bener Meriah dilaporkan sebagai salah satu wilayah dengan angka kasus yang tinggi. Penelitian ini bertujuan untuk menganalisis faktor-faktor determinan kekerasan terhadap anak di Kabupaten Bener Meriah. Desain penelitian yang digunakan adalah observasional analitik dengan pendekatan cross-sectional. Penelitian dilaksanakan pada bulan Februari–Mei 2024. Sampel terdiri atas 78 anak berusia 6–18 tahun yang menjadi korban kekerasan fisik, psikis, dan seksual, dengan teknik pengambilan sampel total sampling. Analisis data dilakukan menggunakan uji regresi logistik. Hasil menunjukkan bahwa mayoritas korban adalah perempuan (51,3%) dengan rata-rata usia 13 tahun, sementara pelaku kekerasan sebagian besar adalah laki-laki (92,3%) dengan rata-rata usia 21 tahun. Sekolah diidentifikasi sebagai lokasi dengan risiko kekerasan 5,5 kali lebih tinggi dibandingkan rumah, dan anak perempuan memiliki risiko 3,8 kali lebih besar mengalami kekerasan dibandingkan anak laki-laki. Temuan ini mengindikasikan perlunya kebijakan pencegahan kekerasan anak berbasis institusi pendidikan dan sensitif gender di tingkat lokal.

Abstract

Background: Mental health is an important aspect of human life. Traumatic experiences in childhood are associated with increased mental health morbidity. Factors such as lack of parental support, low self-esteem, and social difficulties play a role in increasing the risk of risky behavior in adolescents. Purpose: To determine the determinant factors that influence the incidence of depression in adolescents living in orphanages in Central Aceh Regency. Method: Used a Cross Sectional design in four orphanages in August 2023. The sample consisted of 302 adolescents aged 8-18 years, selected by purposive sampling. The analysis used logistic regression with 95% confidence. Results: A total of 121 respondents (40.07%) experienced depression. Related factors were education, region of origin, family visits, social support, and social discrimination. Social support was the most dominant factor (AOR=5.64; p=0.000). Conclusion: Social support plays a major role in preventing depression in adolescents in orphanages, so interventions that increase social support need to be prioritized. Suggestion: Explore other psychosocial factors such as quality of relationship with caregivers, trauma experiences, and adolescent coping strategies.

Abstract

Background: Pregnancy is a crucial period that significantly influences both the physical and psychological well-being of mothers. One of the most common health concerns during pregnancy is the mother's mental health, which can influence both her weight gain and the estimated weight of the fetus. Purpose: To identify the factors that influence prenatal stress levels and the impact of preparation on labour and delivery. Method: This cross-sectional study involved 120 third-trimester pregnant women, recruited using a total sampling technique. Data were obtained through questionnaire-based interviews and analyzed using ordinal logistic regression. Results: Mild stress levels was experienced by 63.33% of participants. Variables associated with prenatal stress included maternal age, education, income, occupation, social support, pregnancy complications, pregnancy status, antenatal care (ANC) visits, and parity. Prenatal stress was found to influence both maternal weight gain and estimated fetal weight. Conclusion: Maternal age and parity were identified as protective factors against prenatal stress. The multivariate model accounted for 63% of the variation in mental health observed.

Abstract

Hipertensi merupakan salah satu penyakit tidak menular dengan prevalensi tinggi dan menjadi tantangan serius dalam sistem pelayanan kesehatan. RSU Sundari Kota Medan dalam tiga tahun terakhir mengalami tren peningkatan jumlah pasien hipertensi yang signifikan setiap tahunnya. Penelitian ini bertujuan untuk menilai efektivitas pelaksanaan program pengendalian hipertensi di RSU Sundari dengan menggunakan pendekatan deskriptif melalui metode campuran kuantitatif dan kualitatif. Data dikumpulkan melalui wawancara mendalam, observasi lapangan, serta telaah dokumen program dan rekam medis pasien. Hasil penelitian menunjukkan bahwa cakupan layanan program hipertensi mencapai lebih dari 80% pada tahun 2022 dan 2023, namun terjadi penurunan signifikan menjadi 64,6% pada tahun 2024. Penurunan ini dipengaruhi oleh keterbatasan sumber daya, belum optimalnya kegiatan edukasi, serta kurangnya konsistensi dalam pemantauan pasien. Meskipun demikian, program hipertensi terbukti memberikan kontribusi positif terhadap peningkatan deteksi dini, kesadaran pasien, serta perluasan cakupan populasi berisiko melalui kolaborasi dengan Prolanis BPJS dan kegiatan skrining di instalasi rawat jalan. Tantangan utama program meliputi keterbatasan media edukasi interaktif, ketidakteraturan pelatihan tenaga kesehatan, serta kendala logistik dalam distribusi obat antihipertensi. Secara keseluruhan, program dinilai cukup efektif, namun memerlukan penguatan dalam aspek edukasi, standar operasional prosedur layanan, serta konsistensi monitoring pasien. Untuk menjamin keberlanjutan program, disarankan pengembangan media edukasi berbasis digital, peningkatan pelatihan berkala bagi SDM, serta integrasi program dengan sistem informasi manajemen rumah sakit.

Abstract

The 2023 Indonesian Health Survey Report issued by the Ministry of Health revealed that there was an increase in the prevalence of diabetes mellitus (DM) in the population aged over 15 years based on the results of blood sugar level measurements. In the 2018 Basic Health Research, it was recorded that the prevalence of diabetes in Indonesia reached 10.9%. Now, the prevalence has reached 11.7% in 2023. The Aceh Health Office recorded that sufferers of Diabetes Mellitus until early 2023 reached 154,889 cases. The purpose of this study is to find out the difference in blood sugar levels in people with diabetes mellitus who consume rice after cooking with the rice cooker method and with the steaming method. This quantitative analytical research uses a cross-sectional study design. The population of the study was all people with diabetes mellitus at the Calang Health Center, Aceh Jaya Regency, totaling 50 people. Data collection was carried out from January 15 to March 15, 2024 using nonprobability sampling techniques. This study also took direct data by looking at the sugar content of the respondents by eating rice cooked by the rice cooker method and 1 month later by measuring the sugar content of the respondents consuming rice by cooking the rice by steaming. Data were analyzed using univariate, bivariate and multivariate statistical tests. The results of the study showed that factors related to blood sugar levels in DM patients were gender, age, education, occupation, income, physical activity, nutritional status, smoking behavior, and family history of diabetes. The factors most related to increased blood sugar levels in diabetes mellitus patients were steaming with OR=7.47, use of a rice cooker of 11.2 and family history of diabetes mellitus of 7.50. It is expected that respondents will control their blood sugar levels, so that DM sufferers and their families will cook rice using the steaming method, and control other risk factors that can cause high blood sugar levels.

Abstract

Melinjo cracker home industry workers must pay attention to several factors that can cause ISPA incidents, such as using personal protective equipment by wearing a mask because it can protect their breathing from dust and smoke resulting from heating melinjo fruit. The purpose of this study is to determine the relationship between the work environment, mask use and the duration of smoke exposure to the incidence of ispa in emping melinjo artisans in Kembang Tanjong District, Pidie Regency in 2023. Workers must also pay attention to the length of work each day, at least taking time to rest. This type of research is descriptive-analytic using a cross-sectional design. The sample taken in this research was the entire population in the Kembang Tanjong sub-district, 435 people who were emping melinjo craftsmen in the Kembang Tanjong sub-district, Pidie Regency. Data analysis in this study was univariate, bivariate, and multivariate. The results of the research show that there are three variables that have a relationship with the incidence of ISPA in emping melinjo craftsmen, namely: Length of exposure to smoke, p-value 0.000, use of masks, p-value 0.000, working place conditions, p-value 0.035 and roof condition, p-value 0.000, while for Multivariate test of risk factors The most associated with the incidence of ISPA is the use of masks with a p-value of 0.000, length of exposure to smoke p-value of 0.000 and roof condition p-value of 0.004.

Abstract

Blood supplementation tablets through the nutritional action program have been implemented, but coverage is still 74.29% of the 100% target. Peer support is thought to be an important factor influencing adherence to Fe tablet consumption. This study aims to determine the effectiveness of class motivators in increasing compliance with the consumption of blood supplement tablets among young women in Pidie Regency. This research is a quantitative research designas if the experiment were not equivalent use pretest and posttest control group design. Implemented in Pidie Regency from October 2023 to January 2024. The population consists of all female students at MTsN 5 Pidie (567) as the intervention group and SMP N 2 Delima (262) as the control group. The total sample was 628 female students who met the inclusion criteria, with 368 in the intervention group and 260 in the control group. Data collection was carried out through questionnaires, and the formation of class motivators in the intervention group. Data analysis used Mann Whitney and Wilcoxon test statistics using the Stata 14 program. The results showed that there were differences in changes in adherence to consuming blood supplement tablets, knowledge, HB levels, side effects of tablets, consumption of enhancer foods, consumption of inhibitor foods and belief in myths between the intervention groups. and the control group in increasing the consumption of blood supplement tablets, and there was no difference in changes in attitudes (p=0.308) between the intervention group and the control group in increasing the consumption of blood supplement tablets. Compliance with consuming blood supplement tablets among female students has increased in the group given class motivator intervention, which significantly increased compliance with consuming blood supplement tablets, knowledge, HB levels, side effects of tablets, consumption of enhancer foods, consumption of inhibitor foods and belief in myths. However, there was no difference in the attitudes of the intervention and control groups, this was because the respondents' attitudes were good from the start.

Abstract

Pulmonary tuberculosis (TB) cases in Nagan Raya have increased every year. Efforts to prevent tuberculosis transmission with healthy lifestyle behaviors have been carried out. However, the prevalence of pulmonary tuberculosis is still high and many factors are associated with the incidence of pulmonary tuberculosis—analytical study with a case-control approach to determine risk factors for tuberculosis in the Nagan Raya district. The sample in this study was 290 people consisting of 58 cases and 232 controls. The sampling technique used purposive sampling. Data was collected by interviewing respondents, observing, and reporting TB case records. Multivariate analysis using multiple logistic regression tests with a 95% confidence level. The results showed that the characteristic factors associated with the incidence of pulmonary TB were primary education (p=0.002), not working (p=0.000), and income <REGIONAL MINIMUM WAGE (p=0.001). Environmental factors associated with the incidence of pulmonary TB were the number of household members >4 people (p=0.036) and no ventilation in each room (p=0.000). Health factors associated with the incidence of pulmonary TB were underweight nutritional status (p=0.019), obese nutritional status (p=0.042), and not containing BCG (p=0.014). The most dominant factor associated with the incidence of pulmonary TB is not working (OR = 4.17; 95% CI = 1.36-12.73). Respondents who did not work had a 4.17 times higher risk of experiencing pulmonary TB than those who did not work after controlling for environmental and health factors. For this reason, it is expected that respondents need to increase their income to prevent transmission of pulmonary TB and maintain cleanliness, improve nutritional status and limit contact with TB sufferers

Abstract

Tuberculosis (TB) is a contagious infectious disease that causes major health problems globally. Indonesia is a country with a high burden of TB cases, with an incidence of 312 per 100,000 population. The number of suspected tuberculosis cases in Aceh in 2022 is 85,945 cases. Aceh Besar has the second highest number of tuberculosis cases, namely 3644 people. DM sufferers have uncontrolled glucose levels so they are at risk of developing TB. This research was conducted to analyze the risk factors for the incidence of tuberculosis in DM sufferers at the Aceh Besar. This research is a quantitative research approach case control study. The sample in this study was DM sufferers with a total sample of 104 people with a sample number of 52 people in each group. Sampling in research uses the technique of purposive sampling. Sample selection for the control group was carried out by selecting DM sufferers of the same gender as the case group. Data collection was carried out using questionnaires and data from examination results from community health centers. Data analysis used multiple logistic regression tests with a confidence level of 95%. The results of the study showed that factors associated with the incidence of pulmonary tuberculosis in diabetes mellitus sufferers in Aceh Besar were secondary education (OR=8.84; 95%CI=1.67–46.69), not working (OR=3.12; 95%CI=1.16–8.35), poor lifestyle (OR=4.97; 95%CI=1.68–14.71), underweight nutritional status (OR=4.38; 95%CI=1.14–16.77), smoking (OR=3.19; 95%CI=1.35–7,55), unhealthy house (OR=2.36; 95%CI=1.07 – 5.18), poor quality of health services (OR=0.03; 95%CI=0.01–0.09), family history of TB (OR=9.40; 95%CI=2.57–34.34) and contact directly with TB sufferers (OR=4.71; 95%CI=1.79–12.40). The most dominant factor associated with the incidence of tuberculosis in diabetes mellitus sufferers in Aceh Besar is smoking (OR=12.71; 95%CI=2.07–77.68). It is hoped that the public, especially DM sufferers, will stop smoking to engage in physical activity or consider seeking support or health consultation to help stop smoking. Additionally, it is hoped to not have direct contact with TB sufferers, to and use a mask when interacting with TB sufferers.

Abstract

COVID-19 is a global pandemic that threatens everyone's health. The increase in confirmed COVID-19 cases continues to occur, including in Aceh Besar Regency. Public health intervention efforts have proven effective in reducing the incidence of COVID-19 and community leaders can be involved in these public health intervention efforts. Public health interventions are expected to cause changes in health behavior, but these behavioral changes require health beliefs. This study is a qualitative study with an exploratory approach to determine the beliefs of community leaders towards COVID-19. The number of informants in this study was 8 community leaders who were selected by purposive sampling. Data collection was carried out by conducting in-depth interviews. Data analysis in this study was carried out before entering the field, while in the field and after finishing in the field. The results of the interviews showed that the health beliefs of community leaders towards COVID-19 were influenced by the sources of information obtained, beliefs about COVID-19, the attitudes of community leaders towards people around them who were infected with COVID-19 and knowledge about the dangerous impacts of COVID-19. Good knowledge will increase the trust of community leaders and improve the attitudes of community leaders to participate in the COVID-19 vaccination which is one of the preventions of COVID-19. Community leaders are also involved in helping health workers provide information about efforts to prevent and handle COVID-19. It is hoped that community leaders will play an active role in efforts to prevent and handle COVID-19 by increasing knowledge and attitudes about COVID-19 which can be obtained through online and print information media. In addition, community leaders are expected to work together with health workers in preventing COVID-19.
