Abstract

BACKGROUND Local tissue immunity plays a significant role in anogenital warts’ (AGW) pathomechanism and persistence. Assessing biomarkers from lesions instead of serum is recommended to evaluate therapeutic response. Since biopsy is invasive, it is necessary to find less invasive and more comfortable methods. This study aimed to assess the reliability of cotton swabs and tape stripping for evaluating AGW’s lesions biomarkers. METHODS We compared cotton swab versus tape stripping method to quantify nuclear factor-κappaB (NF-ĸB), interferon-gamma (IFN-γ), and FOXP3+regulatory T cell (FOXP3+Treg) from 3 patients with AGW in the preliminary study. The method was selected based on contamination possibility, side effects, and a simpler approach. The main study examined 48 patients with AGW for reliability and reproducibility using the best sampling method from preliminary result and Spearman’s Rho analysis, while considering the HIV status and CD4+ counts. RESULTS Both cotton swabs and tape stripping obtained adequate protein content for biomarkers examination. However, the tape stripping method was causing serum contamination and painful for patients due to the stripping. The total lesion volume in cotton swab method was positively correlated with all patients’ NF-ĸB (p = 0.001). IFN-γ had a negative correlation in all reactive HIV patients (p = 0.012). FOXP3+Treg and CD4+ counts were negatively correlated with total volume in reactive HIV patients (p = 0.046 and 0.017, respectively). CONCLUSIONS The cotton swab method was reliable in examining NF-ĸB, IFN-γ, and FOXP3+Treg due to its convenience and lack of serum contamination from AGW lesions, potentially improving patient comfort and practical benefits.

Abstract

COVID-19 (penyakit virus korona 2019), wabah yang disebabkan oleh SARS-CoV-2 baru (sindrom pernapasan akut berat virus korona-2) pertama kali muncul di Wuhan, Tiongkok pada akhir tahun 2019. Pada beberapa kasus, infeksi SARS-CoV-2 dapat mengakibatkan aktivasi sistem kekebalan tubuh yang tidak terkendali, berpuncak pada kejadian badai sitokin, bahkan berujung pada sindrom gangguan pernapasan akut (ARDS). Badai sitokin terjadi sebagai respon berlebihan tubuh dalam upaya menghalangi transmisi virus, namun demikian hal ini juga memicu kerusakan jaringan yang menginduksi pelepasan berbagai faktor inflamasi. Faktor-faktor ini dapat berkontribusi pada penyakit dengan kondisi kritis termasuk sepsis, keganasan, bahkan kematian. Hingga saat ini, belum terdapat pengobatan yang secara mutlak dapat digunakan untuk mengobati COVID-19. Beberapa penelitian menunjukkan bahwa SPM (sel punca mesenkimal) dapat mengendalikan badai sitokin melalui sifat imunomodulasinya. Sayangnya, pengobatan berbasis SPM masih terhambat oleh permasalahan seperti keefektivan, keamanan penggunaan, skala produksi, dan biaya yang mahal. Efek parakrin SPM diperankan oleh sekretom melalui berbagai zat yang disekresikannya. Sekretom digambarkan sebagai kumpulan faktor bioaktif yang disekresikan oleh SPM, menunjukkan efek terapeutik yang serupa dengan SPM, tetapi dapat menghindari efek samping yang ditimbulkan pada terapi berbasis SPM. Sekretom yang berasal dari SPM diketahui dapat memberikan sinyal antiapoptotik dan pro-survival pada sel target yang memungkinkan perbaikan pada sel target dan regenerasi jaringan melalui aktivasi mekanisme antiapoptotik. Dalam ulasan ini, kami akan membahas tentang COVID-19, mekanismenya dalam menyebabkan badai sitokin, dan potensi sekretom sebagai terapi COVID-19.

Abstract

UPT. Perpustakaan Universitas Malahayati mempunyai tanggung jawab dan fungsi salah satunya memberikan pelayanan kepada pengunjung. Peran pengunjung sangat penting dalam mengukur kualitas pelayanan yang diberikan. Tujuan penelitian ini untuk mengetahui tingkat kualitas pelayanan UPT. Perpustakaan Universitas Malahayati serta mengetahui tingkat kesenjangan kinerja dan atribut yang menjadi prioritas bagi perpustakaan. Metode yang digunakan yaitu metode Service Quality dan Importance Performance Analysis. Berdasarkan hasil penelitian menunjukkan kualitas pelayanan UPT. Perpustakaan Universitas Malahayati tergolong baik yaitu diperoleh angka Customer Statisfaction Index Service Quality sebesar 80,82%. Namun, berdasarkan tingkat kesenjangan kinerja UPT. Perpustakaan masih dikatakan rendah dibuktikan dengan skor gap atribut pelayanan bernilai negatif. Untuk kesenjangan Importance Performance Analysis tingkat kesesuaian menunjukkan belum memuaskan diperoleh nilai rata-rata 90% dari 24 atribut kepuasan layanan yang diberikan. Terdapat 6 atribut yang masuk kedalam kuadran A. 6 atribut tersebut merupakan prioritas utama untuk dilakukan perbaikan yaitu (Tangible) atribut ke 2,3,8, (Reliability) atribut ke 12,13 dan (Assurance) atribut ke 19. Kata kunci: UPT. Perpustakaan Universitas Malahayati, Service Quality, Importance Performance Analysis.

Abstract

The coronavirus disease 2019 (COVID-19) pandemic has encouraged global vaccine research, yet vaccine effectiveness in the elderly remains a concern due to immunosenescence. The aim of this study was to compare the cytokine response elicited by an inactivated virus-based COVID-19 vaccine between elderly and young adults, focusing on key cytokines involved in cellular and humoral immunity: tumor necrosis factor-alpha (TNF-α), interleukin (IL)-2, IL-6, IL-10, and interferon-gamma (IFN-γ). A cross-sectional study was conducted in the Jakarta-Bogor region of Indonesia from January 2023 to December 2023. The study population was divided into two age cohorts: elderly (60−85 years) and younger adults (30−40 years). Blood samples were collected twice, after the first booster dose and four weeks after the second booster dose. Serum cytokine concentrations were measured using Luminex assays with microparticles conjugated to monoclonal antibodies against TNF-α, IL-2, IL-6, IL-10, and IFN-γ. Comparisons of the cytokine levels were conducted using Student’s t-tests or Mann-Whitney U tests as appropriate. A total of 74 individuals were included, with 37 each in the elderly and young adult groups. The results showed significant differences in cytokine responses between the two age groups. After the first booster, the levels of IL-6 and IFN-γ were significantly higher in young adults compared to the elderly. After the second booster, the levels of IL-6 were still significantly higher in the young adult group compared to the elderly group (p=0.001). Data indicated that after the second booster dose, the levels of TNF-α increased significantly in the young adult group only (p=0.004), while the levels of IL-2 (p=0.040) and IFN-γ (p=0.006) increased in the elderly group only compared to after the first dose. IL-10 levels increased in both groups (both had p=0.020). This study highlights that young adults had stronger pro-inflammatory responses, while the elderly relied more on IL-2 and IFN-γ for T-cell immunity, suggesting the need for vaccination strategies for the elderly to optimize immune responses.

Abstract

Burnout syndrome (BOS) is an occupational phenomenon highly prevalent among healthcare workers, particularly physicians and nurses. Despite its prevalence, no single therapy universally addresses all cases of BOS. The aim of this study was to develop a novel approach to managing BOS through traditional music therapy, evaluated from psychosomatic, neurological, immunological, and endocrine perspectives. The study involved 80 participants who were randomly assigned to either the intervention or control group. The intervention group received traditional music therapy for 10–15 minutes, three times a week, over four weeks. Key outcomes were assessed at weeks 2 and 4. Measurements included the Maslach Burnout Inventory (MBI), heart rate variability (HRV), saliva cortisol, saliva β-endorphin, saliva immunoglobulin A (IgA), and serum FOXP3. Instruments included the MBI-HSS questionnaire and HRV measuring devices. Over four weeks, significant improvements were observed in the MBI scores (p=0.001), HRV (p=0.001), and FOXP3 delta (p=0.035) in the intervention group compared to the control group. However, no significant differences were found for cortisol, β-endorphin, or IgA. These findings suggest that traditional music therapy positively impacts the psychological, neurological, and immunological aspects of BOS and potentially influences immunological and endocrine responses. Future research should explore the effects of longer intervention durations, test varying doses, and examine the combination of music therapy with other non-pharmacological treatments to enhance its therapeutic potential.

Abstract

COVID-19 exhibits a wide range of clinical manifestations which severity of the disease is linked to uncontrolled escalation of inflammatory mediators. Ongoing research has identified the immunomodulatory effects of the MSC-derived secretome as a potential therapy for COVID-19. However, the precise mechanism by which the secretome exerts its therapeutic effect on COVID-19 remains unclear. This study aims to investigate whether the components of the UC-MSC-derived secretome can alter the inflammatory characteristics of immune cells. To achieve this, an in vitro study will be conducted involving co-incubation of whole blood with secretomes, followed by LPS stimulation. A total of 12 blood samples from severe COVID-19 and healthy subjects were cultured into three groups (RPMI control, 3μl and 9μl secretome group) incubated for 24 hours. Then, the cultures were exposed to LPS for 48 hours. The levels of sIL-6R, sgp130, IL-1RA, IL-6, TNF-α, IFN-γ, and IL-10 were measured. Results showed that LPS increased IL-6, TNF-α, and IL-10 production, while reducing sIL-6R, and sgp130, but no changes seen in IFN-γ in secretome-incubated blood cultures. The post-LPS/pre-LPS ratio analysis was conducted to investigate the anti-inflammatory potential of secretome. It was found that the secretome provides its anti-inflammatory effects through the role of IL-1RA.

Abstract

Background: The cause of chordee in hypospadias is not fully understood. Dartos fascia resection has been shown to correct penile curvature, suggesting its potential involvement. This study aimed to evaluate the histopathological properties of dartos tissue and their association with hypospadias severity and chordee.Methods and Materials: Dartos tissue samples were collected from 37 children with hypospadias and ten controls undergoing circumcision between March and October 2024. Histopathological analysis using hematoxylin-eosin and Masson’s trichrome staining assessed for collagen, smooth muscle, and vascular density. Assessment and quantification of all histopathological components were blinded. Results: The hypospadias group showed lower collagen density (60.85±6.75% vs. 67.31±4.61%; p=0.003) and higher smooth muscle density (7.34±4.92% vs. 2.51±2.24%; p<0.001) compared to controls, with no significant difference in vessel density. Sub-analysis showed severe chordee was linked to lower collagen density compared to mild (Mean Difference [MD] -6.24±2.41%; p=0.014) and moderate chordee (MD -5.73±2.54%; p=0.031). Conclusions: The dartos tissue in hypospadias patients exhibited lower collagen density and higher smooth muscle density, with chordee severity specifically linked to decreased collagen density.

Abstract

Asthma is a disease marked by chronic inflammation of the airways, with indications including wheezing, shortness of breath, and/or cough. As reported by GINA, the global prevalence of asthma is 1-18%. One type of asthma is Allergic asthma which is triggered by allergens, like Der p 1 and Der f 1 which upregulate type II immune response to secret IL-4 and IL-13 to upregulate IgE secretion. IgE binds to FcεRI in mast cells to release mediators that cause hypercontraction, this condition leads to asthma. Meanwhile, anti-inflammatory IL-10 manages IgE production by reducing inflammatory cytokine production and changing isotype switching. IgE Production is a crucial step. Therefore, it is essential to explore the cellular and molecular factors that trigger the regulated isotype-switching IgE and its suppressed mechanism, so this review will explore the biological activity of Der p 1 and Der f I and their contribution to inflammation and the role of anti-inflammation in allergic asthma especially isotype switching IgE We used PRISMA approaches, and tools (RSCB PDB, Uniprot, and SMART). The result appeared Der p 1, and Der f 1 activity have impacts on IL-4, IL-13, IL-10, and IgE secretion.

Abstract

Introduction: Monocytes are very sensitive to changes in the metabolic environment, including hyperglycemia, such as type 2 diabetes mellitus (T2DM). T2DM is characterized by chronic low-grade inflammation and becomes one COVID-19 comorbid. TNF-α and IFN-γ are cytokines often linked to inflammation, the severity of COVID-19, and long COVID-19. This study aims to analyze the relationship between COVID-19 severity and TNF-α and IFN-γ in T2DM one month post-infection.Methods: This research is an experimental study at the Integrated Laboratory Faculty of Medicine, Universitas Indonesia, for four months. The total samples are 44 cryotubes of PBMC (18 T2DM and 26 Non-T2DM) from the CARAMEL study (COVID-19, Aging, and Cardiometabolic Risk Factors). PBMCs were stimulated with inactivated whole virions SARS-CoV-2 and incubated for 24 hours. Monocyte subsets and intracellular cytokines are detected by flow cytometry.Results: Research showed that IFN-γ in the T2DM group was higher in all subsets. There was no significant difference between the T2DM and non-T2DM groups. Based on the history of the severity of COVID-19, MedFI IFN-γ classic and intermediate monocytes differed significantly between the COVID-19 severity groups (p = 0.049 and p = 0.022). Further research is needed to analyze the risk factors involved.Conclusion: MedFI of classic and intermediate monocyte IFN-γ differed significantly between the severity of COVID-19. There were no significant differences between the T2DM and non-T2DM groups. The history of the severity of COVID-19 is significantly related to long COVID-19.

Abstract

Filariasis is a contagious disease caused by worms of the genus Filaria which transmitted through the bite of various species of mosquitoes. In Indonesia, the existence of filariasis cases are still very concerning. Jati Sampurna and Jati Karya villages in Pondokgede Sub-district, Bekasi District, West Java have been known as filariasis endemic area. Length of stay is presumed as one of many factors that affects filariasis occurrence in those villages. This study aimed to determine the distribution of anti-filarial IgG4 on the region and its comparison with the length of stay and residence status. This study was based on secondary data. Secondary data were obtained from primary research data done by cross-sectional method. These data were used to assess the correlation of filarial infection risk factors in pregnant women living in endemic areas based on the distribution of anti-filarial IgG4 in Pondok Gede, Bekasi district, West Java. The study showed that there was an increase in anti-filarial IgG4 against residence status (p=0.017) and a positive correlation between the number of anti-filarial IgG4 with length of stay in years (p=0.003).
