Abstract

The rhizome of Alpinia zerumbet (Pers.) B.L. Burtt & R.M. Sm) contains phenolic compounds, has antihyaluronidase activity which can reduce wrinkles on the skin. This study aims to determine the physical characteristics and antihyaluronidase activity of the nanostructured lipid carrier (NLC) of the Alpinia zerumbet rhizome extract in-vitro. Alpinia zerumbet rhizome powder (4/18) was extracted by kinetic maceration method using 70% ethanol solvent, concentrated with a rotary vacuum evaporator at 100 mm Hg at a temperature of ± 50oC. The resulting extract was prepared into NLC nanoparticles using polyethyleneglycol 8-beeswax, isopropyl myristate and acrylyl glucoside with the high speed homogenization-ultrasonication method. The resulting NLC extracts were characterized by nanoparticle size and morphology, polydispersity index and zeta potential and antihyaluronidase activity test was determined using the Sigma Aldrich and Tu & Tawata methods with slight modifications. The results of the NLC characterization of Alpinia zerumbet rhizome extract were 171.3 nm; 0.37; -26.8 mV; and spherical shape. The results of the antihyaluronidase activity of Alpinia zerumbet rhizome extract loaded NLC was 552.75 µg/mL. It was concluded that the Alpinia zerumbet rhizome extract loaded NLC can be formulated into topical preparations which have the potential to be antihyaluronidase with the ability to reduce wrinkles on the skin.

Abstract

Inflammation is the response of vascular tissue to infection and damaged tissue to remove agents that cause inflammation. Macrophages are the dominant cells in the inflammatory reaction. Adhesion of leukocytes to vascular endothelium is a main feature of the inflammatory process. The presence of adhesion molecules, such as ICAM-1 and VCAM-1, can lead to adhesion of monocytes and lymphocytes to endothelial cells via bonding adhesion molecules to endothelial cells. Crude fukoidan derived from Sargassum crassifolium taken from Garut waters and extracted with dilute HCl, tested for anti-inflammatory activity through inhibition of ICAM-1 and VCAM-1 using lipopolysaccharide-induced RAW 264.7 cell models. The viability test was carried out in the concentration range of 0.49-1000 µg / mL. The pattern of inhibition of cell proliferation is influenced by concentration. The highest concentration that caused cell viability of 80% was the concentration of 86.46μg / ml. Crude fukoidan reduced ICAM-1 and VCAM-1 levels at concentrations of 50 and 25 μg / ml.. Abstract

Mesenchymal stem cells have been known to have the nature of supressing immune response. This cell have been proven to reduce the symptoms of organ transplants rejection. Clinically proofing is also being performed on an autoimmune disease caused by the complex immune such as systemic lupus erythematosus (SLE). This study aimed to know aggregated IgG-stimulating effects of mesenchymal stem cells in relation to IDO secretion, an immunosuppressant agent. Mesenchymal stem cells was isolated from liposuction waste and characterized based on its surface molecule accrording to ISCT criterias. The confirmed cell culture was used to the next culture by the addition of heat aggragated immunoglobulins gamma (HAGG) for six days. IDO secreted on supernatant culture was collected and assayed by elisa method at 450 nm. Abstract

Administration of antibiotics in the private hospital "X" adopted a concept called RonaldIrwanto Antimicrobial Steweardship Program (RASPRO). The suggestion of an empirical antibioticcombination in type III stratifi cation community pneumonia patients was meropenem-levofl oxacinor meropenem-amikasin. The aim of the study was to determine the eff ect of empirical antibioticcombination meropenem-levofl oxacin with meropenem-amikacin to RASPRO type III stratifi cationcommunity pneumonia patients towards the LOS and decreased leukocytes. The test sample calculatedusing diff erence between two population proportions formula and analysed using Chi-square method.Diabetes mellitus, immobilisation and geriatrics as confounding variables were controlled by logisticregretion multivariate analysis. The results showed that meropenem-levofl oxacin had a tendency 1.81times to experience LOS < 5 days and 0.92 times to experience decreased leukocytes ≥ 10% comparedto meropenem-amikacin, but both were not signifi cant (p 0.161 and p 0.835). The result control ofconfounding variables were found that geriatrics as a meaningful confounding variable eff ect of LOSand no confounding variables were considered to aff ect the decreased leukocytes. In conclusion, thereis no eff ect of empirical antibiotic combination meropenem-levofl oxacin with meropenem-amikacintowards the LOS and decreased leukocytes to RASPRO type III stratifi cation community pneumoniapatients using statistics after controlling the confounding variables.

Abstract

Ethanolic extract of red betel leaves (Piper crocatum Ruiz & Pav) has antihyperuricemic activity. In order to be applicable to consume, this extract need to be formulated into pharmaceutical dosage form. Granules which is packed in a tea sac bag was considered to be more practical andappropriate for person suff ering hyperuricemic, because they have to drink more water. In the granulation process, the he binder plays an important role so that this study aims to fi nd the optimal red betel leaf granule formula with variation on the extract dose and the polyvinyl pyrolidone (PVP) binder and prove granule activity in vivo in mouse hyperuricemia model. The extract was prepared by macerationthe leaves in 96% ethanol and sprayed dried by hot air. The powder was then formulated to granules form in 22 factorial design. The factors were the extract dosages (38% dan 47.51%) dan the amount of PVP (Polyvinyl Pyrrolidone) as a binder (0.5% dan 2%). The physical and chemical properties of the granules were evaluated, interaction dosage, PVP concentration and each properties was analysed by making the contour plot and superimposed contour plot to get the optimum formula. The in vivo antihyperuricemic activity of the granules was determined in hyperuricemic mice. The result showed that the optimum formula was found in granules that contained 42.5% extract and 1% PVP. At the equivalent dosage of 455 mg extract/kg, this granules formula had antihyperuricemic activity decreasing 54.2 %plasma uric acid level significantly. Thus it can be concluded that the optimum granules of ethanol extract of red betelleaf with 42.5% extra ct content and 1% PVP proved to have antihiperurisemia activity in mice induced by chicken extractand potassium oxonate.

Abstract

Low-glycemic indexed foods are increasingly in demand. The glycemic index value is influenced by macronutrient composition. Fermentation can change macronutrient levels of foods. This study aims to analize the effect of lactic acid bacteria fermentation on macronutrient content (protein, fat, carbohydrate) and glycemic index of suweg (Amorphophallus campanulatus) tuber flour. Tuber were thinly sliced, fermented with 10% lactic acid bacteria T1-2 isolated from previous studies, then incubated at room temperature for 4 days. The fermented tuber was then made into flour. The fermented suweg flour was tested to determine the glycemic index using experimental mice. Macronutrient levels of carbohydrates, fats and proteins are chemically determined. The results showed that during the lactate acid bacteria fermentation process 0-4 days there was an increase of lactat acid bacteria viability from day 0 to day 2 (3,64x108- 20,38x108 cells / ml) and decreased after day 2 (14,63x108- 7,91x108 cells / ml), the total acid count increased (0,2066% -1,2599%) as the pH decreased (5,43-4,37). The determination respectively result of protein, fat and carbohydrate contents of fermented suweg fluor was 7,41%, 0,46% and 0,38% and 6,05%, 0,38% , 82,15% in non fermented suweg fluor. The glycemic index value of fermented suweg flour was 64,6 and 69,4 in non fermented fluor. Based on statistical test, macronutrient content (fat, carbohydrate, protein) and glycemic index showed no significant difference between fermented and non fermented suweg fluor (P> 0,05). Fermentation of the lactic acid bacteria on suweg tuber for four days had no effect on the glycemic index and carbohydrate, fat and protein levels on the tubers.

Abstract

Due to the development of biomolecular science eg. overexpression genes, genes that cause diseases can be identfied, Based on this fact, researchers developed a therapeutic strategy by inhibiting the gene expression using oligonucleotide antisense. Therapy using oligonucleotide antisense was based on a natural process of gene expression. Specific artificial antisense will match complementary with DNA and mRNA. By this process the transcription will stop. The effort of therapy is relatively new but a few have been carried out in the clinical trial phase. Obstacles are encountered in reaching the target cell by the oligonucleotide antisense.

Abstract

: Brown seaweed (Sargassum polycystum) is one of the plants that produces pharmacological activity which is as anti-atherosclerotic with the potential chemical content is fucoidan. The aim of this research is to know the effectiveness of crude fucoidan in decreasing total cholesterol, LDL, TG, increasing HDL levels and atherogenic index. In the optimization phase, 5 rat test animals were used to see the effect of the composition of a high-fat diet given on lipid profiles. While in the testing phase 30 test animals were divided into 6 groups to see the effect of a high-fat and crude fucoidan diet given to lipid profiles. Examination of lipid profiles was carried out three times, namely on the 0th, 14th and 36th days, after that the atherogenic index is calculated. On the 36th day the test, rats were anesthetized and necropsy for subsequent blood collection through the heart organ.The results showed that 200 mg crude fucoidan dose could significantly decreasing total cholesterol, LDL, TG, increased HDL levels and significantly decreased the atherogenic index of rat serum compared to the dosages of 100 mg and 50 mg, but did not differ significantly from the positive control at 10 mg atorvastatin.

Abstract

A hospital "X" in Jakarta has priority obstetrics services with a high birth rate. Sectio caesarea (SC) at the hospital uses prophylactic antibiotics penicillin and cephalosporins to reducing superficial Surgical Site Infection (IDO). Complication of infection can increase prolongs stay patient (length of stay - LOS). The aim of this study is to look the influence administration of penicillin prophylactic antibiotics with cephalosporins towards the clinical signs of superficial IDO and LOS to SC patients in hospital "X". The test sample calculated using a formula of two proportions difference with retrospective data tracing from medical records which are analyzed using the Chi-Square method with the help of Microsoft Excel to determine the influence administration of penicillin prophylactic antibiotics with cephalosporins towards the clinical signs of superficial IDO and LOS. The results showed that the chi-square calculated value in bivariate analysis of prophylactic antibiotics with LOS and clinical signs of IDO are smaller than the Chi-Square table value (Chi-Square table value at degree of freedom (DF) 1 and significance of 0.05 = 3.8415) with a p value greater than 0.05. In conclusion, there is no statistically significant difference in influence between the group receiving penicillin prophylactic antibiotics with the group receiving cephalosporin prophylactic antibiotics towards the clinical signs of superficial IDO and LOS to SC patients in hospital "X".

Abstract

The data collection was conducted retrospectively, taking samples of 121 outpatients, of which 83 outpatients were evaluated for their clinical outcome. The medication profile data were collected from the medicine administration at the Hospital Pharmacy Installation. The clinical outcome data, such as fasting plasma glucose (FPG), two-hour postprandial plasma glucose (2HPP), and Hemoglobin A1c (HbA1c), were garnered from the patients’ medical records. The cost data were collected from the outpatients’ payment receipts provided by the Hospital’s Financial Department. Kolmogorov Smirnov normality test was applied in this research. The research result suggests that after Mann Whitney Test, the human and analogue insulin were not significantly different seen from the clinical outcome result showing FPG (p=0.676), 2HPP (p=0.175), HbA1c (p=0.0445) and from the cost result (p=0.795). Furthermore, the research also shows that the average cost of 30-day medication of single human insulin was Rp3,190,997.43, while the one of analogue insulin was Rp3,188,832.35. In conclusion, the research finds out that if seen from the cost (p=0.795), the human and the analogue insulin were not significantly different.
