Abstract

This study aimed to determine the effect of counseling and training on the manufacture of probiotic-based health drinks as well as the self-efficacy ability of members of the community group waste management in the city of Depok on the interest in the probiotic beverage business. The 68 respondents included members of the Dahlia Pirus waste bank group (Pondokjaya Village, Cipayung District), Mekar Peduli waste bank (Krukut Sub-district, Limo District) and the Villa Tanah Baru waste bank (Tanah Baru, Beji). This activity involves students. The research method uses multiple regression analysis. The research results show that training has a positive and significant effect on business interest, but self-efficacy has a negative and insignificant effect on business interest.

Abstract

LATAR BELAKANGStandar Nasional Akreditasi Rumah Sakit (SNARS) edisi 1/2018 menjadikan kejadian Infeksi Daerah Operasi (IDO) sebagai salah satu indikator mutu untuk menilai kinerja pengendalian infeksi di rumah sakit. Pemberian antibiotik lanjutan pascaoperasi sectio caesaria (SC) menjadi isu yang penting untuk dikaji, mengingat operasi ini pada dasarnya tidak membutuhkan pemberian antibiotik lanjut pascaoperasinya. Penelitian ini bertujuan untuk melihat apakah pemberian antibiotik lanjutan pascaoperasi SC berpengaruh terhadap penurunan IDO superfisial dan Length of Stay (LOS) pasien. METODEPasien dibagi menjadi 2 kelompok dengan jumlah masing-masing sebesar 49 subjek. Kelompok pertama adalah kelompok subjek yang diberikan antibiotik lanjut pascaoperasi SC dan kelompok kedua adalah kelompok subjek yang tidak diberikan antibiotik lanjutan pascaoperasi SC, kemudian dari masing-masing kelompok dikaji kemungkinan munculnya tanda IDO superfisial dan LOS pasien. Desain penelitian ini adalah kohort retrospektif dengan data sekunder dari rekam medis pasien Januari 2019-Desember 2019. Uji analisa dengan Chi-square. HASILSetelah mengontrol variabel perancu, pemberian antibiotik lanjut pascaoperasi SC tidak signifikan berpengaruh menurunkan kemungkinan munculnya tanda IDO superfisial (OR=0.157;p=0.098; 0.02-1.41 IK 95%), juga tidak memiliki pengaruh terhadap LOS pasien (OR=1.73; p=0.562; 0.27-10.85 IK 95%). KESIMPULANTidak terdapat pengaruh dari pemberian antibiotik lanjutan terhadap tanda kejadian IDO superfisial dan LOS pada pasien post SC. Pemberian antibiotik lanjutan pascaoperasi SC merupakan pemberian antibiotik yang tidak bijak.

Abstract

The use of plants as natural medicine is used because Indonesia has a wealth of natural resources. One of the plants used is water clover (Marsilea crenata). This plant is used as food and medicine. This study aims to identify the potential of natural medicine from water clover (Marsilea crenata). The research method in the literature study is to apply journal searches to the Google Scholar, Science Direct, and Pubmed databases with the keywords water clover (Marsilea crenata) and pharmacological activity. The journals used are national and international journals published from 2012 to 2025. The journals used are research on the potential of water clover as a natural medicine in silico, in vitro and in vivo. From the results of the literature study, 9 journals were obtained. After obtaining research data from journals, they were then combined and studied, then explained and explained systematically. The results of the study showed that water clover leaves in both extract and fraction forms have pharmacological activities including anticholesterolemia, antihyperglycemia, antioxidant, anti-inflammatory, HMG-CoA reductase enzyme inhibitory activity, sperm quality improvement activity in hyperglycemic mice, anti-neuroinflammatory serum. The content of secondary metabolites found in Marsilea crenata is a bioactive compound that is responsible for providing pharmacological activity.

Abstract

Indonesia is a source of raw materials for tropical medicines that treat various diseases. One of the places to plant soursop leaves in West Java is Jayasari Village, Pangandaran District. Annona muricata Linn (Soursop) leaves is a medicinal plant widely used as an anti-diabetic, anti-inflammatory, insecticide, antimalarial, anticancer, antibacterial, and antioxidant. Soursop leaves have many benefits because they contain phytochemical compounds. This study aims to determine the phytochemical content, total flavonoid content, and antioxidant activity of A. muricata leaves. This research was carried out in several steps, including extraction, evaporation, phytochemical testing, total flavonoid content, and antioxidant activity. Phytochemicals were extracted with 96% ethanol by maceration. Measurement of flavonoids in extracts was determined using standard quercetin and UV-Vis spectrophotometry method. Antioxidant activity was carried out through a 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical removal test. The results showed that the ethanol extract of soursop leaves contained alkaloids, flavonoids, phenolics, saponins, and tannins. The results of measurements of flavonoids from the ethanol extract of soursop leaves showed that 1 g of the extract contained 8.32 mg of quercetin equivalent. The ethanol extract of soursop leaves has antioxidant activity, as indicated by the scavenging of DPPH radicals with an IC50 of 56.73 ppm.

Abstract

Glucomannan is a polysaccharide of the hemicellulose type, composed of glucose and mannose chains. It has the ability to form solutions that expand and thicken. The bioactivity of carbohydrates is influenced by the length of their saccharide chains. Hydrolysis can alter the structural dimensions of polysaccharides, including molecular weight, type, number, and position of substituent groups, which in turn impacts their biological activity. Various methods can modify carbohydrate structures to enhance their value, with degradation being one such method. Enzymatic degradation is particularly advantageous due to its high specificity, efficiency, and minimal side effects. Numerous studies have identified glucomannan as having immunomodulatory effects on macrophage cells. When stimulated with lipopolysaccharides, macrophages release various cytokines and inflammatory mediators, such as TNF-α, iNOS, and NO. This study explored the potential anti-inflammatory effects of glucomannan (GM) and its enzymatically hydrolyzed form (Enz-GM) on RAW 264.7 cells. The research involved degrading glucomannan using the β-mannanase enzyme, followed by in vitro testing of the degradation product (Enz-GM) for anti-inflammatory activity. This was achieved using an ELISA kit to measure the expression of IL-6, TNF-α, and iNOS. The results showed that Enz-GM at pH 9 (Mw 1.24 kDa) significantly reduced the pro-inflammatory cytokines IL-6 and iNOS (p ≤ 0.05) compared to GM, while TNF-α expression did not show a significant difference between Enz-GM and GM.

Abstract

Chlorogenic acid, the primary constituent found in green coffee, is believed to possess anti-obesity properties. Numerous studies have indicated that the etiology of obesity is predominantly influenced by genetic factors. This study employs molecular docking techniques to estimate the effectiveness and toxicity of chlorogenic acid as an anti-obesity agent, focusing on its interaction with PLANTS. Initially, the validation process involved confirming the target cell or receptor (PDB code) which included PPAR-? (3NOA, 2ATH), pancreatic lipase (5ZUN), ghrelin (6ZYF), leptin (3V6O), and melanocortin (6W25, 7F58) through the utilization of YASARA software. In addition, the process of docking chlorogenic acid compounds and a positive control (for comparative purposes) was conducted on target cells utilizing the PLANTS program. The toxicity test and prediction of lethal dose (LD 50) were conducted on active substances and positive controls using the ProTox-II program. Chlorogenic acid exhibits anti-obesity properties by acting as an inhibitor of the ghrelin hormone, as seen by its activity at PDB code 6ZYF with a docking score of -19.7099 higher than the positive controls bupropion -18.5269 and naltrexone -18.5871. Furthermore, it has been determined to possess a generally safe profile, with an LD50 value of 5000 mg/kg body weight. The docking studies indicate that chlorogenic acid exhibits anti-obesity activity specifically at PDB code 6ZYF, where it functions as an inhibitor of the ghrelin hormone. Chlorogenic acid typically exhibits modest efficacy as an anti-obesity agent, hence presenting potential avenues for enhancing its effectiveness by structural modifications

Abstract

Porang tuber is rich in fiber and low in calories of carbohydrates. Moringa oleifera leaf has a high protein and a low content of lipids. Obesity and diabetes mellitus are the risk factors of cardiovascular disease, also known as cardiovascular disease risk factors. Combining of porang and moringa as feed can increase nutrition without causing weight gain. This study aims to determine these two materials as antidiabetic and antiobesity agents. The samples are porang tuber, a combination of porang tuber and M. oleifera leaves, a combination of M. oleifera leaves and wheat flour, wheat flour, and standard feed of mice. The parameters tested in this study are analysis of macronutrients of feed, loss of weight of mice, average consuming feed in 28 days, and reduction of blood glucose with a glucometer. The results obtained are a combination of porang tuber and moringa had a low contain carbohydrates and a high contain lipids and protein, can reduction in body weight of 21%, the lowest consumption of feed, and the lowest fasting and postprandial blood glucose levels compared to the other feeds. A combination of porang and moringa can be an antidiabetic and antiobesity agent.

Abstract

Coronavirus Disease 2019 (COVID-19) merupakan penyakit menular yang disebabkan oleh Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Kasus COVID-19 diperparah oleh beragam komorbid, menyebabkan biaya per pasien menjadi bervariasi. Pemerintah mengalokasikan dana yang tinggi sebagai strategi dalam upaya percepatan penanggulangan wabah COVID-19 yang teknis pengajuan klaimnya merujuk pada Keputusan Menteri Kesehatan 446/2020 dan 4344/2021. Penelitian ini bertujuan untuk mengetahui selisih biaya yang diterima oleh rumah sakit dari Kementerian Kesehatan (Kemenkes) dengan biaya riil rumah sakit. Pada penelitian observasional ini, sampel (236 responden) diambil secara acak dari populasi pasien rawat inap COVID-19 derajat sedang periode Januari–Juni 2021 (597 pasien) menggunakan tabel Krejcie dan Morgan. Data restrospektif diambil dari rekam medis, casemix, dan penagihan (billing), secara potong-lintang. Hasil analisis deskriptif yang dilakukan menunjukkan bahwa pasien COVID-19 dalam penelitian ini sebagian besar adalah perempuan (139 orang, 58,9%), rentang usia 18–82 tahun dan sebagian besar dari kelompok usia 26–45 tahun (102 orang, 43,2%), tidak memiliki komorbiditas (121 orang, 51,3%), dengan rerata lama rawat-inap (LoS) 10 hari. Terkait biaya, total realisasi klaim pembiayaan periode Januari–Juni 2021 yang diterima dari Kemenkes mencapai Rp18.659.815, atau lebih tinggi secara signifikan dibanding total biaya riil rumah sakit (Mann-Whitney, p <0,05). Dari biaya riil rumah sakit Rp2.286.282.430, komponen terbesar adalah obat Rp557.193.006 (24,37%). Rerata selisih antara klaim dan biaya riil yang cukup tinggi, Rp57.860.630 -Rp76.870.587 per pasien tersebut belum memperhitungkan biaya investasi yang dikeluarkan rumah sakit dalam bentuk berbagai peralatan (seperti ventilator) dan fasilitas lainnya (ruang isolasi bertekanan negatif, penambahan ruangan ICU dan HCU) yang belum tentu produktif setelah pandemi berlalu, sehingga dapat dikatakan wajar. Selisih positif tersebut menurun setelah diberlakukannya Keputusan Menteri Kesehatan Nomer 4344/2021 untuk periode 20 April 2021 sampai 30 Juni 2021 sebagai bentuk evaluasi secara berkesinambungan dari pemerintah terhadap pembiayaan COVID-19.Kata Kunci: COVID-19; analisis biaya; realisasi klaim; biaya riil; selisih biaya AbstractCoronavirus Disease 2019 (COVID-19) is an infectious disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Cases of COVID-19 are exacerbated by various comorbidities which lead to various per patient costs. The government allocates high funds as a strategy to accelerate the management of the COVID-19 outbreak, wherein the technical claims refer to the Decree of the Minister of Health number 446/2020 and 4344/2021. This study aims to determine the difference between costs received by hospitals from the Ministry of Health and the real costs incurred by the hospitals. This was an observational study which involved 236 respondents who were randomly selected from the population of moderate-degree COVID-19 inpatients for the period January–June 2021 (597 patients) by using Krejcie and Morgan table. Retrospective data were taken from medical records, casemix, and billing, cross-sectionally. The results of the descriptive analysis conducted showed that most of COVID-19 patients in this study were women (139 respondents or 58.9%). Age of respondents ranged from 18–82 years old and most of respondents were involved in the age group of 26–45 years (102 respondents or 43, 2%). Furthermore, most of respondents had no comorbidities (121 respondents or 51.3%), with the mean length of stay (LoS) of 10 days. Regarding costs, the total realization of financing claims for the period January–June 2021 from the Ministry of Health was IDR 18,659,815, or significantly higher than the total real hospital costs (Mann-Whitney, p < 0.05). Of the real hospital costs of IDR 2,286,282,430, the largest component was medicine by IDR 57,193,006 (24.37%). The mean difference between claims and real costs was quite high by IDR 57,860,630 – IDR 76,870,587 per patient, by not considering the investment costs incurred by the hospital in the form of various equipment (such as ventilators) and other facilities (negative pressure isolation rooms, as well as additional ICU and HCU wards) which are not necessarily productive after the pandemic has passed. Thus, it can be said that such difference was natural. The positive difference decreased after the enactment of Decree of the Minister of Health No. 4344/2021 for the period April 20, 2021 to June 30, 2021 as a form of continuous evaluation provided by the government towards COVID-19 financing.Keywords: COVID-19; cost analysis; claim realization; real cost; difference in cost
