Abstract

COVID-19 is a contagious acute respiratory disease caused by the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) virus. Changes in some laboratory markers such as absolute lymphocyte count and quantitative CRP in COVID-19 can be associated with the course and clinical phases of the patient whose results can be useful in grading and predicting the severity of COVID-19 disease. This study aims to determine the correlation between absolute lymphocyte count and quantitative CRP on the severity of COVID-19. This study was an observational correlative analytic study with a cross sectional design. The research was conducted in all COVID-19 treatment isolation wards at RSMH Palembang from March 2021 to April 2021 with the research subjects being patients with confirmed cases of COVID-19 with various degrees of disease. Routine blood tests of absolute lymphocyte counts and quantitative CRP were performed at the Clinical Pathology Laboratory of RSMH Palembang. The statistical analysis of the correlation test with Spearman for numerical data was not normally distributed, and Gamma for categorical ordinal data was followed by a regression test to determine the prognostic value of the two parameters. Samples obtained were 83 patients, namely 24 patients (28.9%) mild degree, 40 patients (48.2%) moderate degree, 10 patients (12%) severe degree and 9 patients (10.8%) critical degree. The correlation between the absolute lymphocyte count and quantitative CRP obtained the correlation coefficient value r = – 0.525. The correlation between the absolute lymphocyte count and the degree of COVID-19 disease obtained a correlation coefficient of r = – 0.605, while the correlation between the quantitative CRP value and the degree of disease was obtained r = 0.875. Based on the prognostic test, the absolute lymphocyte count (cutoff point of 1,157 cells / mm3) was obtained with a sensitivity of 68.4% and a specificity of 68.75%. Meanwhile, quantitative CRP with a cutoff point of 77.5 mg/L obtained a sensitivity of 89.4% and a specificity of 89%. The absolute lymphocyte count has a strong negative correlation with the quantitative CRP value and the disease degree in COVID-19, while the quantitative CRP value has a positive correlation with a very strong degree. Thus these two parameters can be used as predictors of disease severity, however, the quantitative CRP value can be an independent prognostic marker or a single predictor of the severity of COVID-19 disease.

Abstract

Penelitian Chronic Renal Insufficiency Cohort (CRIC) menunjukkan 86% subjek PGK terbukti menderita inflamasi. Sejumlah studi longitudinal melaporkan peningkatan inflamasi dengan mengukur interleukin-6 (IL-6) pada pasien PD baik kadarnya di sistemik dan intraperitoneal. Bawang putih dan komponennya memiliki aktivitas antiinflamasi dan antioksidan. Efek antiinflamasi dari ekstrak bawang putih, dengan menghambat aktivasi jalur pro-inflamasi serta menghambat aktivasi makrofag dan efek antioksidan dan menghambat enzim pro-oxidant. Penelitian ini merupakan studi crossover randomized placebo controlled double blind trial. Subjek penelitian 20 orang pasien PGK stadium 5 yang menggunakan CAPD, dibagi secara acak menjadi 2 kelompok, yaitu kelompok yang mendapat ekstrak bawang putih 800 mg perhari (2x400 mg) selama 8 minggu dan kelompok yang mendapatkan placebo, kemudian dilakukan periode wash out selama 2 minggu dan setelah itu kedua kelompok diberikan perlakuan bergantian. Masing masing kelompok diukur kadar IL-6 serum, IL-6 dialisat dan net ultrafiltrasi sebelum dan sesudah perlakuan. Didapati kadar IL-6 serum sebelum dan sesudah pemberian ekstrak bawang putih 7,10 ±2,30 pg/ml dan 5,00±3,00 pg/ml dengan p= 0,005. Kadar IL-6 dialisat sebelum dan sesudah pemberian ekstrak bawang putih 39,20±2,10 pg/ml dan 33,20±7,80 pg/ml dengan p = 0,000. Disimpulkan penurunan kadar IL-6 serum dan dialisat sesudah ekstrak bawang putih pada penderita PGK stadium V yang menggunakan CAPD di RSMH Palembang.

Abstract

WHO reported 1.9 million death cases of diabetes patients in 2019. Diabetes is caused by damage in the pancreas which resulted in a lack of insulin or insulin resistance.  Medication for Type 2-Diabetes Mellitus (T2DM) mainly focuses on lowering blood glucose and treating affected organs. Current medications are still lacking; thus research is needed in finding novel medications to accommodate T2DM. This paper aims to present the current research on potential plant extract in increasing GLUT4 translocation in diabetes conditions. insulin resistance state affecting GLUT4 translocation which is important in affecting glucose uptake. Some research shows that plant extract proved to be potential in increasing the translocation of GLUT4 and helping lowering blood glucose levels.

Abstract

Grade II eyelid burns are burns that reach the epidermis and part of the dermis of the palpebrae and can have impaired wound healing. TGF β1 has the broadest spectrum of action, affecting all cell types involved in all stages of wound healing. TGF β1 increases rapidly after injury and reaches peak levels three days after injury, which coincides with the peak of the inflammatory phase of wound healing. Decreased expression of TGF β1 is associated with impaired wound healing. Aloe Vera has long been known in Indonesia as a healing plant. Aloe Vera extract contains a variety of bioactive compounds that play a role in stimulating wound healing. The administration of Aloe Vera extract on thermal burns aims to reduce inflammation, increase collagen content, and increase the rate of re-epithelization. The administration of Aloe Vera gel extract allows faster healing of thermal burns and rebuilds the vascularity of the wound tissue. Objective of this study is to determine the difference between the expression of Transforming Growth Factor Beta 1 (TGF β1) in a rat model of thermal burn injury grade II of the eyelids treated with topical Aloe Vera gel extract with the concentrations of 20%, 40%, Balanced Salt Sodium (BSS) and a control group that was not given burn intervention. An experimental study with the post-test-only approach with a control group design. 24 Rats have induced grade II eyelid thermal burns, divided into four groups: Normal (without treatment), negative control (BSS), given Aloe vera 20%, 40%. Then TGF β1expression was quantified using the Image J application. The Topical application of Aloe vera gel can increase the expression of TGF along with the increase in dose. The highest rise in TGF expression was found in the group with 40% Aloe vera gel, followed by 20% Aloe vera gel compared to the negative control (BSS Balanced Salt Solution). Aloe Vera can increase TGF expression in a rat model of grade II palpebral burn injury and is dose-dependent.
